Termination / Separation Just as hiring, reviews and raises are overseen by local leadership, so is terminating, and separating with employees. You have NVA's support through the process and as a resource. For any employees you are looking to terminate, please contact your HR Business Partner prior to so they may assist with the proper steps. Check when the final paycheck is due according to your local state law. For California, Colorado, Massachusetts, Minnesota, Missouri, and Nevada, final pay is due at the time of termination. Many other states have laws that require final paychecks for terminated employees be issued within 24, 48, or 72 hours and we encourage you to familiarize yourselves with all applicable state laws for your hospital.

FINAL PAY is due at time of termination in CALIFORNIA & ALASKA & MASSACHUSETTS COMPANY ASSETS (keys, IDs, etc.) should be collected prior to departure EMAIL/SYSTEMS: If employee was in a secure position (with an nvanet.com or nva.com email) send email notification to offboard@nvanet.com so all systems and access can be turned off SEPARATION FORM: Complete and submit to liza.parodi@equifax.com make sure to include resignation/write-ups/disciplinary notes STATE SPECIFIC FORMS: *If applicable, check ‘Termination’ folder in NVA Share (AZ, CA, CT, GA, IA, IL, LA, MA, MI, NJ, NV, NY, PA, TN) PAYROLL: Be sure to change their status to “terminated” in ADP – be sure to include last day of employment, reason for leaving and if they’re eligible for rehire or not. COBRA: COBRA paperwork is automatic for NVA medical participating locations and the notice is automatically sent to the eligible employee once you set their status to ‘Terminated’ in ADP. (If your location is not on the NVA consolidated plan, please follow your COBRA process typically the broker initiates the COBRA notice).

Receive the resignation in writing (email is fine) Notice is usually two (2) weeks. If not enough notice is given, do not withhold pay Make arrangements for their departure o Communicate with staff and make scheduling arrangements o Initiate recruiting efforts to find a replacement (if applicable) o Exit interview (if appropriate)
