Other Insurance Coverage

Upon joining NVA your hospital will immediately be covered under our umbrella policy. See proof of coverage on following page. NVA's insurance covers: Professional Liability (PLIT), Disability, Worker's Compensation, General Liability, Property, Employment Practices Liability Insurance (EPLI). *See exceptions below

Worker's Compensation

Your hospital will receive a laminated sheet (also available on the following page) with quick reference information. Please post this in your breakroom. If a Worker's Comp. claim needs to be filed, Hospital Managers should call the Toll-Free number to report the claim over the phone. The "Employer's First Report of Injury" will be completed over the phone. Questions should be directed to Adam Weiss at ABWeiss@Lockton.com.

A few anomalies: • Alaska is to contact Republic Indemnity--please see individual form on next pages • California has an independent policy number--please see individual form on next pages • Ohio & Washington must go through your state's required Worker's Compensation • Canada note covered under NVA umbrella--please see individual form on next pages

Disability

Most hospitals will move under NVA's umbrella policy for disability insurance upon acquisition. The exceptions are: • California • Hawaii • New Jersey • New York • Rhode Island • Puerto Rico.

Property

Property Coverage covers the structure and contents up to the $100,000,000 limit. Loss of rent for the building owner is also included in coverage.

Professional Liability/Malpractice

NVA's coverage is up to $2 million and will cover up to $100,000 in license defense. All DVMs practicing at an NVA hospital (relief included) fall under this umbrella. Thus, they do not need to have supplemental coverage through AVMA or another company.
