Telecommunications
NVA has a Telecommunications Team that does a lot for our hospitals… • Audits phone billing records to ensure you are getting the best price • Circuit moves/adds/changes/disconnects with carriers • Contract negotiations on your behalf • Troubleshooting phone lines for you • Coordinates replacement/install of phone systems o Note: New phone systems are not included in the conversion process; if your phone system is more than 10 years old, faulty or doesn't have caller ID please let your Transition Manager know and we can look into getting you a new system! • Emergency preparedness • Mobile phone support • You will receive a Telecom Questionnaire from your TM to be completed within about 2 months from the time you join.
Cell Phones
• Want a cell phone for the hospital? An emergency phone or on-call phone? Or just a phone to take photos of patients and text their owners with? Let your Transition Manager know! • As a Hospital Manager are you using your personal phone for lots of work data? Let your
Transition Manager know and we have some options to move to NVA's coverage. o See the following pages for mobile phone options! o NVA will reimburse a portion of the Seller's and HM’s personal mobile phone but not family members.
NVA Mobile Phone Device Policy for Hospitals
This policy may be updated at any time (without notice) to ensure changes to NVA’s structure and/or business practices are properly reflected in the policy.
NVA Hospitals:
• HOSPITAL o Emergency mobile phone for Hospital ($50) Either VZ or ATT Hospitals can choose to opt in our out.
TWO Options for MDVM & Hospital Manager:
1) NVA will purchase and setup a new iPhone (Business Specifications) with NVA provided phone number. (See below for details). The phone is equipped for all NVA business applications. NVA Approved service provider is Verizon. 2) Bring your own device (BYOD): Monthly $50 reimbursement – Processed via IPS.
Attach copy of bill (the first page).
All Mobile Devices Must use a Screen Lock Password for Security.
If for any reason you leave or are terminated from work then you can either:
1) Keep the NVA provided iPhone, and reimburse NVA with a onetime fee (amount remaining on 2yr contract), By signing the acknowledgement below, employee specifically understands and agrees that NVA will withhold from employee’s final paycheck remaining dollar amount on phone issued if employee does not return to NVA pursuant to this agreement to the extent permitted by law and employee specifically consents to such withholding.
OR
2) Send the Phone (Charger & Accessories) back to NVA Support Center with no cost to you.
If your phone is lost:
• Notify the helpdesk immediately @ helpdesk@nva.com or 888 777-7804 • Replacement fee will apply • Make sure to back up your data via ITunes or ICloud.com (recommended) using NVA email address
If your phone is broken:
• Notify the helpdesk immediately @ helpdesk@nva.com or 888 777-7804 • Make sure to back up your data via ITunes or ICloud.com (recommended) w/ NVA email • Turn in old phone (with accessories) and new phone will be activated
The Purpose
Usage: It is National Veterinary Associates policy that representatives of our organization who are issued a mobile phone understand the phones are issued for business use. Mobile phone bills are reviewed on a monthly basis.
Driving: National Veterinary Associates has a zero-tolerance policy regarding using a mobile phone while driving. For the safety of our employees and others it is imperative that you pull over and stop at a safe location to dial, receive or converse on the mobile phone in any way. National Veterinary Associates reserves the right to amend or alter the terms of thispolicy.
