Fiscal Year 2021 was no surprise! Another year in the midst of a global pandemic and the ever-challenging world crisis, yet Triad Adult & Pediatric Medicine (TAPM) remained fearless and strong This is the story of TAPM!

In the midst of the second peak of a deadly pandemic, Triad Adult and Pediatric Medicine, Inc. (TAPM) continues to thrive. Because of the resiliency and dedication of our talented staff, we have become successful in navigating through uncertain times in healthcare and our world
This time last year, we hoped to be at the end of the COVID-19 crisis, yet we find ourselves battling a more dangerous and contagious variant strain while striving to provide continuous quality care to our patients TAPM has been on the front lines of the COVID-19 testing and vaccination efforts When the vaccine became available, TAPM quickly adapted and began providing the much-needed service to our patients, staff, and community. TAPM was a part of the first cohort of FQHC’s (Federally Qualified Health Centers) to participate in HRSA’s (Health Resources Services Administration) federal vaccine program. TAPM has hosted numerous outreach efforts, Q&A sessions, and lunch talks to help stymie vaccine hesitancy and dispel myths regarding the pandemic.
Amid the COVID-19 crisis, our clinical and quality teams continued to flourish in many areas such as hypertension management, cholesterol management, children and adolescent weight management, adult weight management, and our new measures, cervical cancer screenings, BMI screenings, and early entry to prenatal care Diabetic and nutrition management continue to be monitored despite decreased face-to-face patient visits. Telemedicine for primary care, behavioral health, and nutrition visits became the new normal.
In the works is TAPM’s inaugural Office-based Opioid Therapy program in collaboration with the Mountain Area Health Education Center and the NC Formerly Incarcerated Transitions program. We will offer opioid use disorder treatment to patients who live with opioid addiction This is such an important service for our marginalized community We are grateful for our awesome community partners, and are eager to get started in this endeavor! Other strategic quality initiatives on the horizon are a teen wellness program, a self-monitoring program for patients with hypertension, and a more robust EHR (Electronic Health Record) system to enhance the patient and provider experience

TAPM has partnered with CHS to provide HealthySteps services since early February 2018 It started at TAPM Commerce and quickly expanded to TAPM Wendover There are now 3 HealthySteps Specialists on site who partner with staff to support the families they serve.
As part of the Guilford County Ready for School, Ready for Life initiative, the Children’s Home Society of North Carolina (CHS) has partnered with Zero to Three to provide the HealthySteps program In order to improve outcomes for children in Guilford County, families receive parent education and connections to community resources at their pediatrician’s office
HealthySteps is an evidence-based program that promotes the health, well-being, and school readiness of babies and toddlers. The program recognizes parents as experts on their children. We also know that babies’ brains grow faster from ages 0-3 than any point in life and that pediatric primary care is a trusted source of information for parents during these critical years. HealthySteps Specialists partner with pediatric providers in the primary care setting, where they meet with families during well-child visits to offer free familycentered parenting information and support to parents with children from birth to age 3. Additionally, our Specialists are just a call away for when parenting support is needed between family visits at the pediatrician’s office
HealthySteps Specialists are parent educators who are trained in issues that can impact families with young children They connect families with a wide variety of community resources and they provide tailored support for common and complex concerns, such as: behavior, sleep, feeding, attachment, parental depression, social determinants of health, and adapting to life with a baby or toddler. The entire practice works together to implement the HealthySteps model.
Last year, HealthySteps supported 6,092 families in 8 different pediatric sites across Guilford County.
Since 2018, HealthySteps Specialists have helped serve over 1,400 children at TAPM.

The Social Determinants of Health are the conditions in which we are born, we grow, age and which we live. Residents who are homeless or live in public housing are more likely to experience more than one Social Determinant of Health.
The FIT Program has experienced growth and has continued to serve more men and women in Guilford County FIT is continually building relationships with community agencies in the local area In working with the NCDPS, FIT has been a part of assisting in welcoming home individuals who have received early release due to the pandemic by connecting them to a medical home FIT Providers, community health workers (CHW), and case managers immerse themselves in various trainings so they can better serve the FIT patients of TAPM CHW’s are keenly focused on patient care and services that can better assist patients with reintegration -Eugene Wilson, FIT Community Health Worker Supervisor

TAPM's nutrition program provides personalized counseling for adults and children with food allergies, pre-diabetes, diabetes, hypertension, hyperlipidemia, obesity, underweight, and other diagnoses. Medicaid Transformation and Care Management are well underway at TAPM While this is not a new concept, TAPM has hired one Care Coordinator and is contracted with CMHN (Carolina Medical Home Network) to support our Medicaid patients North Carolina DHHS defines this program as an avenue for all Medicaid patients to gain appropriate access to care management and coordination support across multiple settings of care, including a strong basis in primary care and connections to specialty care and community-based resources. We enhanced our total rewards package thus implementing a $15.00 minimum wage, increasing the number of paid annual leave days, and the introduction of 401K participation after 90 days instead of 1 year We also enhanced our employee touch bases by introducing “Stay Interviews” and robust training during the Performance Management cycle Key deliverables to be introduced in the near future include, digitizing the performance appraisal form from paper to electronic, launching a comprehensive employee opinion survey, and imploring a robust people strategy to recruit and retain top talent. -Nakeisha Thorpe, HR Director

2021 fiscal year was brand awareness and social media presence. All of TAPM's marketing materials got a fresh new look from business cards, to brochures, newsletters, stationery, and graphics Adding to the existing Facebook and Twitter accounts, TAPM created a new Instagram page, LinkedIn business profile, and a Youtube channel Since beginning social media advertising, TAPM has increased followers on its Facebook page by over 100% Additionally, with the creation of review cards, patients leave their appointments with step-by-step instructions on how to post a review on Google.
