Dear Friends and Supporters,
As we reflect on another year at Triad Adult and Pediatric Medicine, Inc (TAPM), we are reminded of our commitment to making a profound TAPM Impact in our community. This year’s theme, "TAPM Impact –Mission Driven, Vision Focused," perfectly encapsulates our dedication to serving our patients and fostering their health and well-being.
At TAPM, our mission has always been to provide equitable and comprehensive healthcare with compassion and respect. Every day, our dedicated team exemplifies this Mission Driven approach, working to ensure that all individuals, regardless of their circumstances, have access to the quality care they deserve. This mission guides our initiatives, shapes our programs, and inspires our interactions with patients and families alike.
In 2024, we are proud to have launched several innovative programs aimed at addressing our community’s healthcare needs. Our Pilot Self-Monitoring Blood Pressure Program has empowered patients to take control of their health, while our expanded Diabetes Management services have provided critical care to those managing chronic conditions. Our Period Poverty Project has addressed the essential need for menstrual products, distributing over 1,500 products to women in need, further promoting health equity.
Recognizing the national challenges of recruitment and retention, TAPM has proactively launched the Pathways to Success program and the Medical Assistant Apprenticeship program in partnership with the North Carolina Community Health Center Association (NCCHCA). These initiatives are essential in developing and retaining the next generation of healthcare professionals. Additionally, we are excited to continue our Vision Focused strategy, particularly as we look forward to the opening of our inaugural dental clinic in High Point, NC, as part of a collaboration with High Point University
We are also proud to report a successful Operational Site Visit (OSV) conducted by the Health Resources and Services Administration (HRSA), where TAPM’s Pathways to Success program was recognized as a promising practice that could serve as a model for other health centers.
Together, we have achieved remarkable milestones, but there is still much work to be done. Your support plays a vital role in helping us realize our vision and amplify our TAPM Impact. Dr. Santiya Bell is a dedicated dental professional with over 20 years of experience in private practice, community care, and dental education. She earned her Doctor of Dental Medicine from Nova Southeastern University in 2001. Before joining TAPM, Dr. Bell served as Director of Quality Improvement/Quality Assurance at High Point University’s Workman School of Dental Medicine, and she has also practiced with University Dental Associates and DentalWorks.
Dr. Bell's career highlights include founding her own private practice and later providing care for Guilford Adult Health Dental Inspired by her parents’ commitment to public service, she is passionate about enhancing community health At TAPM, her guiding principle is to ask, “What can I do to set this practice and its people up for success?” Dr. Bell is excited to lead the establishment of TAPM’s first dental clinic.

CALL CENTER
YOU ASKED, AND WE LISTENED!
This year, we're excited to highlight the success of our new call center team, created in response to patient feedback about long wait times at our Wendover location Staffing shortages had caused delays, so we took action by introducing virtual call center specialists to better serve our patients.
With two specialists now fielding around 2,100 calls per month, we've drastically reduced wait times and improved patient satisfaction. This has also allowed our in-office staff to focus more on delivering exceptional face-to-face care.
We remain dedicated to being a trusted resource for our patients and look forward to continuing to enhance our call center services in the year ahead.

BEHAVIORAL HEALTH
Behavioral Health and Social Drivers of Health (SDOH) play a crucial role in shaping an individual’s overall well-being. Behavioral health refers to the emotional, psychological, and social factors that impact how we manage stress, maintain relationships, and navigate mental health challenges. These aspects are deeply connected to physical health, as mental health conditions often have a direct effect on physical wellness.
Social Drivers of Health, including factors like income, education, and living conditions, significantly influence an individual's ability to access care and maintain good health. By addressing both behavioral health and these external influences, we can better tackle health disparities, improve overall mental and physical health outcomes, and move toward a healthier, more equitable community.

Diabetes is a significant public health issue, and at TAPM, we are dedicated to reducing barriers to care for our patients living with this condition As a patient-centered medical home, our goal is to provide comprehensive care that helps prevent serious complications such as heart disease, stroke, kidney disease, blindness, and more
In partnership with Prevent Blindness North Carolina, TAPM offers free retinopathy screenings for diabetic patients 18 years and older, regardless of their ability to pay. Patients with abnormal findings are referred by their primary care provider to specialists for further treatment. Beyond retinal screenings, we also assess patients for a range of risk factors, including family history of glaucoma and vision changes.
Through this program, we’re able to provide essential preventive services to uninsured and underinsured patients who might otherwise face lengthy wait times for specialist care.

SELF-MONITORING BLOODPRESSURE
At Triad Adult and Pediatric Medicine, we are committed to meeting our patients where they are and addressing their unique needs. Many of our patients face chronic conditions like hypertension, often compounded by various social drivers of health. As their patient-centered medical home, we prioritize empowering our patients on their wellness journey.
Hypertension, commonly referred to as the "silent killer," is a prevalent issue in our patient population. To address this, we launched the Self-Monitoring Blood Pressure Program—a 12-week initiative designed to educate patients on managing their blood pressure, recognizing symptoms of heart attack and stroke, and adopting healthy lifestyle habits. Through weekly collaboration between Health Coaches and Primary Care Providers, we closely monitor patients’ progress, medication adherence, and help overcome any barriers they face
This program has not only led to significant improvements in blood pressure control but also empowered patients to take charge of their health, working hand-in-hand with their healthcare team to reach their personal health goals.

NUTRITION +DIABETES
TAPM Nutrition Service provides individualized nutrition counseling to promote wellness and manage medical conditions such as diabetes, cardiovascular diseases, gastrointestinal disorders, and more. Our registered dietitians work with patients to assess eating habits, establish goals, and improve overall health through tailored dietary plans and physical activity
The Diabetes Self-Management Education and Support (DSMES) program empowers patients to manage their diabetes effectively through personalized assessments, educational classes, and follow-up support. The program helps improve blood sugar levels, prevent complications, and lower healthcare costs while also offering emotional support for those living with diabetes.
A key highlight of our Nutrition Service in 2023-2024 is the partnership with the Guilford County Public Health Department. Since November 2023, TAPM’s Registered Dietitian, Ying Marshall, was training and collaborating with Deshawn McNeil, a nutritionist from Guilford County Public Health. Deshawn leads diabetes education for preconception and interconception health services to reduce infant mortality and provided nutrition services for TAPM patients two days a week. This collaboration significantly extended our outreach and impact in the community.

PERIOD POVERTY
During Women’s History Month, TAPM focused on addressing the critical issue of period poverty a growing health concern that affects millions of women worldwide. In the U.S., nearly 22 million women live in poverty, many of whom struggle to afford menstrual products. This lack of access poses significant health risks, including increased chances of urinary tract infections, especially when inadequate menstrual supplies are used for extended periods.
TAPM is committed to advancing health equity through various community-focused initiatives, such as our food pantry and clothing closet. This year, we expanded our efforts by launching a feminine product drive to support our female patients. We partnered with employees and the local community to collect essential items that were distributed to patients in need.
This initiative addressed a vital health concern and empowered women within our community by ensuring access to necessary products that promote both dignity and well-being.

Patient surveys are sent out each night to patient cell phones. Patients are given 3 days to complete the survey. Questions asked in the survey include topics such as service received, facility maintenance, provider approval, etc. Patients have the ability to request feedback from a TAPM team member as well.

MEDICALASSISTANT APPRENTICESHIP
In 2024, TAPM, in partnership with the North Carolina Community Health Center Association (NCCHCA), launched its Medical Assistant Apprenticeship Program to provide a clear pathway into the healthcare field for individuals seeking hands-on experience and professional development. This one-year program offers participants the unique opportunity to earn while they learn, combining paid work with formal education. It’s a stepping stone for those with a high school diploma or equivalent who are eager to begin a career in healthcare.
This year, we welcomed two dedicated individuals into the program. As they balance classroom learning with practical experience in our clinical setting, they are gaining valuable skills and knowledge. By the end of the program, these apprentices will be well-prepared to enter the workforce as qualified medical assistants, equipped with the training needed to support TAPM’s mission of providing equitable, compassionate healthcare to our community

COMMUNITY OUTREACH
Community outreach is a vital component of TAPM’s mission, as it extends beyond clinical care to support the broader Guilford County community. Our work is powered by dedicated Community Health Workers (CHWs), who use best practices in community care to help individuals build a sense of belonging, connection, and collective support. These efforts play a critical role in improving recovery, overall well-being, and access to essential health services.
As part of TAPM’s Strategic Plan, we are committed to intentional outreach and building partnerships with local stakeholders. These collaborations allow us to better serve the community and ensure that residents have access to resources that enhance their health and quality of life. One such initiative includes our partnership with local Enrollment Assisters, who are onsite to help individuals apply for Medicaid following its expansion on December 1, 2023.
Additionally, TAPM’s mobile unit has bolstered our visibility and brought healthcare services directly to underserved areas From facilitating health clinics to engaging with community initiatives, we continue to make an impact A highlight this year was our Outreach team being invited to the Governor's Mansion for Governor Roy Cooper's Proclamation for Reentry 2030. In the coming year, we are excited to explore and expand our services even further, reaching more individuals in need.
NC-FIT PROGRAM
The North Carolina Formerly Incarcerated Transition (NC FIT) Program has seen growth and change throughout the past year. As the program has expanded, our team has focused on adapting to meet the evolving needs of the community.
To better support those we serve, NC FIT has brought on additional staff, particularly individuals with lived experience, who provide invaluable guidance to those reentering society. Outreach and community engagement remain top priorities, helping us further our mission.
NC FIT connects formerly incarcerated individuals with chronic diseases, mental health conditions, and/or substance use disorders to critical healthcare services and reentry resources, aiding their transition back into the community
OUR MISSION:
Improve the well-being and health of people who have been recently released and improve their chances of successful reentry into the community.
OUR GOAL:
Connect individuals being released from incarceration with necessary health services, encompassing physical health, mental health, and substance use disorder treatment. Assist FIT Program clients with all aspects of reentry, including housing, job training, legal needs, education, transportation, and life skills.
Mental Health First Aid equips individuals with the skills to recognize and respond to signs of mental illness and substance use disorders. It empowers participants to offer initial support and timely intervention to those in need. The training also emphasizes empathy, nonjudgmental support, and self-care for those providing assistance, helping to ensure their own mental well-being while supporting others.

EQUITY ALLIES GIVING CIRCLE
Donor Spotlight: Equity Allies Giving Circle
This year, Triad Adult and Pediatric Medicine was honored to receive a generous grant from the Equity Allies Giving Circle, a group within the Community Foundation of Greater Greensboro dedicated to addressing systemic inequities in our community. This giving circle, which emerged from the Equity Allies affinity group founded in 2019, brings together a committed group of approximately 20 donors who pool their resources to make impactful grants targeting social and racial inequities in Greensboro
Led by Robin Lane, the Equity Allies Giving Circle meets regularly to engage in discussions about local issues, with a particular focus on the causes and effects of medical debt and its disproportionate impact on communities of color. Their work emphasizes a commitment to viewing these challenges through a justice lens, advocating for and funding initiatives that go beyond traditional charitable approaches.
The mission of the Equity Allies Giving Circle aligns closely with TAPM's commitment to providing equitable and comprehensive healthcare. We are immensely grateful for their support, which will help us further our mission to serve the community with compassion and respect. This partnership inspires us as we strive to address the health disparities facing our community and work towards creating a more equitable healthcare system for all.
