OUR HISTORY!

CELEBRATING 25 YEARS

In November 2010, six medical practices in Guilford County merged to become Triad Adult and Pediatric Medicine, Inc (TAPM) Over the years each of these practices has worked to improve the health and well-being of the patients they served Collectively, these medical practices have provided decades of quality service to the community. Formerly these practices were known as Guilford Child Health, Inc., HealthServe Community Health Clinic, and High Point Adult Health

The practices formerly known as Guilford Child Health, Inc were established in 1997 to serve as providers of primary and specialty healthcare services for children in Guilford County. Prior to that date, the Guilford County Department of Public Health was the primary provider for mandated public health services Eventually, the demand for health services became so large that Guilford County found that it could not continue to meet the growing needs of the community The answer to the challenge was for County Commissioners, Board of Health members, Moses Cone Health System (now Cone Health), and Wake Forest Baptist Health (now Atrium) to join together to create Guilford Child Health, Inc

The practice formerly known as HealthServe Community Health Clinic was established in the late 1980s by a physician and nurse (husband and wife team) to provide basic medical care services to those individuals staying at Greensboro Urban Ministry’s Pathways Shelter With financial support from Leadership Greensboro, a storefront clinic was opened in 1989 at 414 Battleground Avenue and the clinic provided care to 1,700 individuals that first year In 1993, the organization was incorporated as a 501(c) 3 nonprofit, and a new permanent facility was built at the corner of Gate City Blvd. and Eugene Street In addition, the Greensboro Pharmacy opened and began providing prescription medications HealthServe Northeast opened in 1998 at 1439 East Cone Boulevard Due to funding both HealthServe offices and the pharmacy have closed Family Medicine at Eugene currently occupies the space on the corner of Gate City Blvd , and Eugene Street adjacent to the Greensboro Urban Ministry providing integrated healthcare and outreach services

The practice formerly known as High Point Adult Health was a collaborative effort between Guilford County and High Point Regional Health System, (now Atrium) with an aim to meet the needs of the uninsured and underinsured in the City of High Point, NC This practice merged with our pediatric location and is now Family Medicine at Elm

FQHC – Federally Qualified Health Center Designation

On Sept 13, 2013, Triad Adult and Pediatric Medicine, Inc received notification it had been designated as an FQHC – Federally Qualified Health Center Currently, TAPM is the only FQHC (Federally Qualified Health Center) in Guilford County.

New Sites and Collaborations (these sites were made possible by NAP (New Access Point) funds from HRSA – Health Resources and Services Administration)

In 2016, the organization opened Family Medicine at Arlington in Greensboro, NC; a collaboration with Triad Adult and Pediatric Medicine and Guilford Child Development (now Children and Families First) In 2017, the organization opened Family Medicine at Brentwood in High Point, NC, a collaboration with Triad Adult and Pediatric Medicine and Children and Families First.

TAPM Building Ownership

In November 2021, TAPM purchased its first building – Family Medicine at Elm located at 606 N. Elm St. in High Point, North Carolina. Soon after, TAPM purchased a building – 300 W Northwood St Greensboro, North Carolina and has since opened Family Medicine at Northwood in June, 2022 TAPM then purchased a building 2210 E Cone Blvd Greensboro, NC in an effort to expand primary care services to the citizens of Northeast Greensboro This location is scheduled for opening mid-late 2024

In late 2024 TAPM will open its first dental practice.

NC-FIT PROGRAM

NC FIT connects formerly incarcerated individuals who have chronic disease, mental illness and/or substance use disorder with appropriate healthcare services and other reentry resources.

The North Carolina Formerly Incarcerated Transition (NC FIT) Program experienced many transitions this program year, ranging from dealing with the fallout of a global pandemic to staffing changes and expanding the program Staff has embraced that the only thing that is constant is change

While navigating program transitions, the FIT Program has brought on additional staff to address the community's growing need for experts with lived experience to assist them on their journey. Outreach and community involvement have been earmarked as top priorities for the FIT Program.

OUR MISSION:

Improve the well-being and health of people who have been recently released and improve their chances of successful reentry into the community

OUR GOALS: Connect people being released from incarceration with needed health services; to include physical health, mental health and substance use disorder treatment. Assist FIT Program clients with all aspects of reentry; to include housing, job training, legal needs, education, transportation and life skills.

The Family Medicine at Northwood location of Triad Adult & Pediatric Medicine is our newest facility in the Greensboro area, situated at 300 W Northwood St, Greensboro, NC 27401. This location offers comprehensive integrated care. The providers at this location specialize in integrated family medicine, and they are trained to diagnose and treat a wide range of conditions.

CARE: TAPM’S EPIC JOURNEY

TAPM Achieves Healthcare Milestone with EPIC Launch in 2023

In August 2022, our journey began as TAPM transitioned to EPIC, a company with over two decades of experience in electronic health record transformation This marked the dawn of a new era in patient care for TAPM. We recognized that this undertaking would be incredibly complex, surpassing any previous EHR endeavors, and it required an unprecedented level of commitment and collaboration from various teams.

The EPIC/IT Team, consisting of Maurice McGhee, Michelle Cook, Randa Rumley, Jonita Duron, and Burley Baker, forged ahead in this new era of healthcare technology, streamlining workflows to deliver better patient care. For our staff, it represented a profound shift in workflow. With the adoption of the EPIC EHR system, we gained deeper integration with major hospitals and healthcare clinics.

This comprehensive system included features such as the North Carolina Immunization Record Interface (NCIR), Care Message (a patient reminder platform to reduce no-shows), integrated notes dictation via Dragon, telehealth capabilities through Zoom Virtual Visits, and seamless receipt of medical records from other EPIC facilities.

Throughout the project, we had specific milestones to meet, and the TAPM team met them head-on. This new healthcare technology brought forth new leaders, including superusers, provider leads, and trainers. On May 9th, 2023, EPIC officially launched for TAPM, marking a significant milestone in our journey!

SCHOOL-BASED TELEHEALTH

TAPM LAUNCHES SCHOOL-BASED TELEHEALTH PROGRAM

TAPM entered into a partnership with Greensboro Area AHEC, Cone Health, and Union Hill Elementary School in High Point, North Carolina to provide school-based telehealth This collaboration represents one of the first partnerships with a federally qualified health center and a school in North Carolina to establish virtual healthcare within a school system Union Hill Elementary has been identified as a school in Guilford County, North Carolina, with low attendance rates and high absenteeism The goal of the program and collaboration was to enable student visits to occur virtually while the student remained at school In general, students who were deemed well enough to stay in school but required a low level of medical attention could benefit from the services.

There were Two types of school-based telehealth visits:

1) Live visit with the student/patient The medical assistant serves as a tele presenter to the provider and enables the examination of the patient with state-of-the-art equipment During this examination, the child's parent is also conferenced in or is in person at the school

2) The other type of visit is a store-forward or asynchronous visit

The medical assistant would obtain vital signs and information about the patient and their chief complaint and course of illness This information is presented to the provider, and they can offer medical advice and provide orders for over-the-counter medicines if needed If further evaluation is required, a telehealth visit is requested This has been the primary visit type that has occurred during this past school year.

There are great benefits and potential in this program. We are able to provide the service to patients of TAPM who attend Union Hill Elementary but also to others in the community. It helps us to ensure that patients, whether they are TAPM's or not, have a medical home and have appropriate medical follow-up It also, as intended in its original purpose, helps to keep children in school who are able to be there instead of being sent home for minor issues that can be handled during the school day Thank you to Greensboro Area AHEC, Dr. John Jenkins, Cone

Year one was challenging and rewarding TAPM is committed to continuing this program while taking steps to increase access at Union Hill, and potentially expand services to other schools

TEEN WELLNESS

TEEN WELLNESS PROGRAM

In October 2021, TAPM explored the idea of a teen wellness program to assist teenagers during the COVID-19 pandemic. TAPM recognized the need for teenagers who were overweight, pre-diabetic, or diabetic to engage in meaningful exercise and instruction In November 2021, the first virtual Teen Wellness program was launched, with three students registered and two students completing the class. Since the start, we've had four sessions, and a total of 19 students have completed the program. The program fully launched in January 2022

The program consists of seven one-hour sessions over five weeks, and we break for the summer. All the sessions are virtual. The sessions focus on teaching teenagers how to be healthy in an educational, interactive, and engaging way Most youths are overweight because of a poor diet and little to no exercise I was an overweight teenager, and I share my own teen health journey with the students. I’m now an internist, and I see the issues that occur because of obesity and an unhealthy lifestyle. It is expected that 1.3 billion people worldwide will have diabetes by 2050

Many youths don’t understand how to fully take care of their bodies. During the classes, information is given in short bursts called 'Hacks.' In fact, another name for the class is called 'Body Hacking: Hacking your body to achieve health and your ideal body size

With these hacks, students learn to work smarter and not harder in achieving a healthy body. An example of a hack is that it takes 20 minutes for the message to travel from your stomach to your brain to let you know you are full Set a timer for 25 minutes (5 extra minutes), serve the recommended serving size for a healthy meal, and take the entire 25 minutes to eat the meal It sounds easy, but for most, it is very hard, as most tend to eat too fast, not allowing enough time for your brain to recognize that you are full, which can lead to overeating

The class consists of four segments, each having 8-12 hacks:

1. Your Mind: how your mental state affects your wellness

2 Your Mouth: talking about what to eat, when to eat, and how much to eat.

3. Your Move: the best way to exercise and move your body.

4 Your Rest: Sleep is important

Each student picks a hack they want to work on each week. During class, there is a two-week Gut reset to help the students understand how food affects them. For two weeks, they avoid all foods with gluten, processed sugar, and dairy products These foods are then reintroduced one at a time, and the students journal how their bodies respond. Surprisingly, most kids love the Gut Reset, and one noted how their allergies improved when they stopped gluten

On April 12, 2023, TAPM was honored to host Congresswoman Kathy Manning, Xavier Becerra, Secretary for the Department of Health and Human Services, and Governor Roy Cooper at the Family Medicine at Northwood location. The purpose of the visit was to obtain the community health perspective on rising prescription drug costs, Medicaid expansion, and other health issues facing marginalized populations.

In 2023, we collaborated with Spectrum Reach to create five distinct spot commercials (branding, Medicaid, Pediatric, Family Medicine, recruiting) From April 5th, 2023, to November 5th, 2023, we successfully delivered TV and streaming TV ads, totaling 17,652 TV airings Our TV streaming efforts resulted in 1,084,703 impressions, achieving an impressive completion percentage of 98 64%

Mental Health First Aid is a comprehensive program that equips individuals with the knowledge and skills to recognize and respond to signs of mental illnesses and substance use disorders. It empowers participants to offer initial assistance and support to those facing these challenges or experiencing a crisis.
