SWEET POTATOES

Have you ever heard that sweet potatoes are healthier than white potatoes? The truth is one type of potato is not healthier than the other but each type gives our bodies different nutrients. Thanks to the brilliant orange color, we know sweet potatoes are a rich source of beta-carotene. Beta-carotene is a phytochemical (plant chemical) that is responsible for giving fruits and vegetables their yellow and orange pigments. In our bodies, beta-carotene is converted into vitamin A Protecting cells from damage, promoting vision health, bone development, and supporting the immune system are all vital functions that vitamin A assists with Along with vitamin A, sweet potatoes also contain vitamin C, fiber, potassium, and B6
Try this Scalloped Sweet Potatoes and Apples recipe, by Healthy Family Project, for a cozy side or festive holiday dish
SCALLOPED SWEET POTATOES AND APPLES
2 poundssweet potatoespeeled, thinly sliced
½ teaspoonsalt
1/2teaspoonpepper
1/4teaspoonnutmeg
1/4teaspooncinnamon
3medium applescored, chopped
1large sweet onionchopped
2tablespoonsflour
1teaspoonlemon juice
1cupheavy cream
1cupunsalted chicken stock
DIRECTIONS:
Preheat oven to 350F Lightly grease a 13×9-inch baking dish
Toss sweet potatoes with salt, pepper, nutmeg and cinnamon in large bowl
In separate bowl, toss apples, onion, flour and lemon juice
Spread half of sweet potatoes in single layer in baking dish Top with apples and onions Arrange remaining sweet potatoes on top in overlapping pattern
Mix cream and stock Pour over sweet potatoes
Cover with foil and bake 30 minutes Increase heat to 425F and remove foil
Bake 20-25 minutes, or until tender and lightly browned on top

WHY ARE WE RECOMENDING THE INFLUENZA VACCINE FOR ALL TEAM MEMBERS THIS YEAR?
As a health care organization, our patients rely on us to keep them safe It is our responsibility to protect those we care for and work with, as well as lead by example in the communities we serve Doing everything we can to keep people safe and healthy is key to ensuring we have the appropriate resources to support our teams and care for our communities
HOW TO SUBMIT PROOF OF VACCINATION
Team members need to submit their proof of influenza vaccination to Employee Health by November 1st, 2024, if received from Public Health or another provider.
To submit information through email, send to kims@humboldthospital.org.
Documentation for proof of influenza vaccine needs to show the following:
Team members first and last name
Date of vaccination
Indication of the vaccination type (eg, Influenza)
If you receive your vaccine at your primary care provider, most offices will provide you a visit summary that will include this information If you do not receive a summary, you may need to ask
Any team member who utilizes MyChart can take a screen shot of their visit summary To do this: Open MyChart
Select 'Visits'
Find the date of the event and select 'View After Visit Summary' Take a screenshot. Make sure your name is on it.
If you receive your vaccine at a Hy-Vee, CVS, or Walgreens, most pharmacy receipts will indicate this information.
HOW TO SUBMIT AN EXEMPTION REQUEST
We recognize there is a small number of team members who have a need for medical or religious exemptions. Exemption requests must be made by November 1st, 2024. The exemption process for the influenza vaccination is separate from the COVID-19 vaccination exemption.
INFORMATION ON NON-COMPLIANCE
Team members who do not receive the influenza vaccine and have not signed a refusal form will be subject to termination

VACCINE INFORMATION STATEMENT
Influenza (Flu) Vaccine (Inactivated or Recombinant):What you need to know
1. Why get vaccinated?
Influenza vaccine can prevent influenza (flu).
Flu is a contagious disease that spreads around the United States every year, usually between October and May Anyone can get the flu, but it is more dangerous for some people Infants and young children, people 65 years and older, pregnant people, and people with certain health conditions or a weakened immune system are at greatest risk of flu complications
Pneumonia, bronchitis, sinus infections, and ear infections are examples of flu-related complications. If you have a medical condition, such as heart disease, cancer, or diabetes, flu can make it worse. Flu can cause fever and chills, sore throat, muscle aches, fatigue, cough, headache, and runny or stuffy nose. Some people may have vomiting and diarrhea, though this is more common in children than adults.
In an average year, thousands of people in the United States die from flu, and many more are hospitalized. Flu vaccine prevents millions of illnesses and flu-related visits to the doctor each year.
2. Influenza vaccines
CDC recommends everyone 6 months and older get vaccinated every flu season Children 6 months through 8 years of age may need 2 doses during a single flu season Everyone else needs only 1 dose each flu season
It takes about 2 weeks for protection to develop after vaccination.
There are many flu viruses, and they are always changing. Each year a new flu vaccine is made to protect against the influenza viruses believed to be likely to cause disease in the upcoming flu season.

Even when the vaccine doesn’t exactly match these viruses, it may still provide some protection
Influenza vaccine does not cause flu.
Influenza vaccine may be given at the same time as other vaccines
3. Talk with your health care provider
Tell your vaccination provider if the person getting the vaccine:
Has had an allergic reaction after a previous dose of influenza vaccine, or has any severe, lifethreatening allergies
Has ever had Guillain-Barré Syndrome (also called “GBS”)
In some cases, your health care provider may decide to postpone influenza vaccination until a future visit. Influenza vaccine can be administered at any time during pregnancy. People who are or will be pregnant during influenza season should receive inactivated influenza vaccine.
People with minor illnesses, such as a cold, may be vaccinated. People who are moderately or severely ill should usually wait until they recover before getting influenza vaccine.

4. Risks of a vaccine reaction
Soreness, redness, and swelling where the shot is given, fever, muscle aches, and headache can happen after influenza vaccination. There may be a very small increased risk of Guillain-Barré Syndrome (GBS) after inactivated influenza vaccine (the flu shot).
Young children who get the flu shot along with pneumococcal vaccine (PCV13) and/or DTaP vaccine at the same time might be slightly more likely to have a seizure caused by fever. Tell your health care provider if a child who is getting flu vaccine has ever had a seizure.
People sometimes faint after medical procedures, including vaccination Tell your provider if you feel dizzy or have vision changes or ringing in the ears
As with any medicine, there is a very remote chance of a vaccine causing a severe allergic reaction, other serious injury, or death.
5 What if there is a serious problem?
An allergic reaction could occur after the vaccinated person leaves the clinic. If you see signs of a severe allergic reaction (hives, swelling of the face and throat, difficulty breathing, a fast heartbeat, dizziness, or weakness), call 9-1-1 and get the person to the nearest hospital
For other signs that concern you, call your health care provider.
Adverse reactions should be reported to the Vaccine Adverse Event Reporting System (VAERS). Your health care provider will usually file this report, or you can do it yourself. Visit the VAERS website at www.vaers.hhs.gov or call 1-800-822-7967. VAERS is only for reporting reactions, and VAERS staff members do not give medical advice.
6. The National Vaccine Injury Compensation Program
The National Vaccine Injury Compensation Program (VICP) is a federal program that was created to compensate people who may have been injured by certain vaccines. Claims regarding alleged injury or death due to vaccination have a time limit for filing, which may be as short as two years. Visit the VICP website at www.hrsa.gov/vaccinecompensation or call 1-800-338-2382 to learn about the program and about filing a claim.
7. How can I learn more?
Ask your health care provider. Call your local or state health department. Visit the website of the Food and Drug Administration (FDA) for vaccine package inserts and additional information at www.fda.gov/vaccines-blood-biologics/vaccines. Contact the Centers for Disease Control and Prevention (CDC):
- Call 1-800-232-4636 (1-800-CDC-INFO) or
- Visit CDC’s website at www.cdc.gov/flu.
COVID VACCINE update
WE WANTED TO SHARE A FEW ITEMS REGARDING COVID AND THE COVID VACCINE HCMH does not require employees to get their COVID-19 vaccine However, HCMH is required to keep employees educated about COVID-19 and the COVID-19 vaccination HCMH is also required to submit employee vaccinations via the CDC National Healthcare Safety Network (NHSN).
WHAT TO KNOW
Everyone ages 6 months and older should get a 2024–2025 COVID-19 vaccine. The COVID-19 vaccine helps protect you from severe disease, hospitalization, and death It is especially important to get your 2024–2025 COVID-19 vaccine if you are ages 65 and older, are at high risk for severe COVID-19, or have never received a COVID-19 vaccine Vaccine protection decreases over time, so it is important to stay up to date with your COVID-19 vaccine.
If you are interested in getting the COVID-19 vaccination, HCMH has 5 doses remaining on a first come first serve basis in the pharmacy. You must have either hospital insurance or a commercial insurance plan to receive these vaccinations
Otherwise COVID-19 vaccinations remain available at Hy-Vee or Larry’s Pharmacy

OPEN ENROLLMENT
2025 Benefit Highlights
HEALTH INSURANCE
2025 will mark the fifth year of the 8 hospital MEWA and we will continue to offer two health plans with Wellmark Blue Cross and Blue Shield – the POS and High Deductible Plan (HDHP).
Both plans will now be Point-of-Service (POS) plans – POS networks offer flexibility by providing in-network coverage from local doctors and hospitals, while allowing out-of-network coverage from participating doctors at a higher cost share POS Network includes 100% of Iowa hospitals and 29,000+ Iowa doctors.
HCMH will continue to pay most of the premium cost for medical coverage. The estimated total cost of the health plan for 2025 is $1,818,021.
Alliance Select $2,500 PPO Plan – The Employee will cover 220% of the cost of the plan and HCMH will cover 78% of the cost. There is a slight increase in cost for the employee for 2025.
Out-of-Pocket maximums remain the same: $5000 Single/$10,000 Family
Calendar Year Domestic In-Network is $500 Single/$1000 Family
Calendar Year Deductible In-Network is $2,500 Single/$5000 Family

Alliance Select $2,500 HDHP Plan – The Employee will cover 155% of the cost of the plan and HCMH will cover 845% of the cost
Out-of-Pocket maximums remain the same: $5000 Single/$9,100 Family
Calendar Year Domestic-In-Network increased to $1650 Single/$3,300 Family
Calendar Year Deductible In-Network is $2,500 Single/$5,000 Family

YOUR MEDICAL ID CARD WILL COME FROM WELLMARK
Employees don’t throw your current card away until you receive your new card!
Level 2 (In-Network) Deductible and Out-of-Pocket Maximum will be displayed on your card for showing both In-and Non-Network for Individual and Family
Wellmark’s Customer Service number is on the back of your card
ID cards will only be issued for all participants and will be delivered to your home address prior to January 1, 2025.
Please make sure you show your new ID card to your medical practitioner on or after January 1, 2025

Health Savings Account (HSA) through WEX Health Inc.
HCMH will continue to invest in HSAs through an annual employer contribution of $500 for single coverage and $750 for family coverage at the beginning of the calendar year.
Annual contribution limits will increase from $4,150 to $4,300 for single coverage and from $8,300 to $8,550 for family coverage The additional catch-up contribution (age 55 or older) remains at $1,000
The annual contribution limits, which include team member and HCMH contributions, are your responsibility to ensure you do not exceed. You can track what you have in your HSA account on your WEX Personal Account.
A new Debit card will be issued only if it has expired – check the dates on your current card.
Flexible Spending Accounts (FSA) through WEX Health Inc.
HCMH offers health care, limited use, and dependent care accounts for Flex accounts
HCMH will increase flexible spending account to the new IRS rates - annual maximum to $3,300 for 2025 and the maximum allowed for carryover to $660.
IRS regulations require that every transaction from an FSA be substantiated. This means there needs to be proof that the funds were for eligible medical, prescription, dental, or vision expenses
Dental Insurance
HCMH will be offering two dental plans through Delta Dental this year: The Base Plan which is the same as the 2024 Plan – slight change in premiums
Dental Buy-Up Plan offers coverage on Orthodontia up to $1500 per child
New cards will not be sent unless you make a change
Vision Insurance with Avesis
HCMH will continue to offer Vision Insurance with Avesis Avesis is Accepted by Wolfe Eye Clinic
Two Plans will be offered
Plan A is a full-service plan covering the exam and materials
Plan B covers materials only
No Increase in cost from last year
Voluntary Life and Accidental Death & Dismemberment (AD&D) Insurance
To help protect you and your family:
HCMH continues to offer a $20,000 Basic Life Insurance policy that includes accidental death and dismemberment (AD&D) coverage, for all part-time and full-time employees
You may increase your voluntary life and AD&D coverage and/or your spouse/domestic partner’s life and AD&D coverage without Evidence of Insurability (EOI) – meaning no medical questions asked for up to $50,000 coverage.
Long Term Disability is paid by HCMH. All benefit-eligible team members will receive 60% of their annual salary up to $10,000 per month
NO Changes to Voluntary Accident Insurance, Critical Illness/Cancer Insurance:
Accident, critical illness/cancer insurance do not replace your medical coverage. Instead, they complement it for added protection.
Symetra will remain the carrier for Accident, Critical Illness/Cancer Insurance
Iowa Public Employees Retirement System HCMH is a county hospital and is therefore qualified to participate in IPERS Being a member of IPERS is similar to having an insurance policy for retirement. Since July 1, 2018, employees’ contribution rate remains at 6.29% and the hospital as the employer matches with 9.44% for a total of 15.73% that will go towards the employee’s retirement. This deduction is on a pre-tax basis.
HCMH also offers Employees the opportunity to invest in ROTH IRA’s and Deferred Comp through RIC – Retirement Investors’ Club through Edward Jones
ROTH IRA’S – Contributions to Roth IRA’s are taxed but the gain on investments is not taxed according to law. HCMH does not contribute towards employee’s Roth IRA’S but does the paperwork for the employee.
DEFERRED COMP – Employee’s contributions toward retirement are taken out of your check pre-tax and taxed at retirement or when money is withdrawn HCMH does not contribute towards this but does the paperwork for the employee
2025 PTO Cash-Out Requests
The election period is October 28 – November 12. 2024.
Benefit-eligible employees can buy back up to 160 hours in 2025 Payouts will occur on the April 3, 2025 paycheck and/or the November 13, 2025, paycheck Per policy, you must have a minimum of 80 PTO hours in reserve at the time of the buyback
FREQUENTLY ASKED QUESTIONS
When is open enrollment for 2025 benefits?
Open enrollment will occur within ADP from October 28, through Tuesday, November 12, 2024. Is anything changing with benefits this year?
NO Medical Insurance will remain Point-of Service Coverage for both PPO and High Deductible plans
Are medical plan premiums increasing for all team members?
Yes, the premiums for the PPO Plan and the HDHP will increase slightly for employees this year. Is the employer coverage amount increasing too?
Yes As overall healthcare coverage premiums rise across the country, we remain committed to taking on the majority of the impact
How do I know that I’m in the best plan for me?
We strongly encourage you to assess your healthcare needs to determine which plan best suits your situation. Please meet with Mary in HR, to discuss which plan best suits you and your family. Do I need to sign up again this year, if I am not making any changes?
Yes. Please go through the open enrollment process so you are aware of any possible changes in your costs for benefits If you are choosing not to enroll in the health insurance because you are still on your parents plan (under age 26) or on your spouse’s plan you need to waive coverage

PAID TIME OFF (PTO) CASH-OUT REQUEST
PTO BUY BACK - per IRS Code 409A
A. During benefits election period, eligible employees can elect to sell PTO hours for the following year.
B. Employees may elect to sell Paid Time Off (PTO) hours which will be accrued in the upcoming calendar year by completing a PTO Sell Back Election Form
C The employee must have a minimum of 80 PTO hours in reserve at the time they make the election and at the time of each buyback.
D. Buyback dates will be the first pay date in April and November. Employees can choose to sell PTO one or both of the dates.
E. Eligible employees may request to sell back up to 160 hours per calendar year with a minimum of 8 hours at each payout date
F The form must be completed during the annual employee benefits election period.
G. Employees will receive 100% of their PTO’s value when cashing it in, less applicable taxes and other standard deductions.
H. Elections for PTO Buyback are irrevocable once made and no new elections can be made after the open election period.
INSTRUCTIONS: Please indicate the number of hours that you want to cash out from your PTO bank for one or both of the cash-out periods below. You will not have any IPERS taken out of the PTO buyout. The payment will be included in your regular payroll check and will be subject to applicable taxes, as required by law.
REMEMBER that you must leave a minimum of 80 hours in your bank after cashout.

Different codes happen throughout the hospital on any given day, these are unplanned events so knowing what to do “in the moment” is vital.
In the event of a Code Alert
Push the button on your phone for OVERHEAD PAGE 1
Announce the code three times with “Code Alert” only at the beginning 2
Say
“Code Alert” > [the code that is needed] > [location of the code]
Examples
“Code Alert” > Clinical Assistance Needed > 2nd Floor Therapy

E3 Training
You (or your leader) should have signed you up for an E3 training. E3 is a desktop application (and phone app) we will be using moving forward inconjunction with doing an overhead page
This will be used to notify people of current codes that are going on. Everyone will get a notification either on their phone or desktop alerting them when a code has been activated.

A CULTURE OF SAFETY
Enhancing a culture of safety is a priority for HCMH. This new program will highlight opportunities for improvement in the delivery of patient care and place a positive connotation to reporting and investigating “near misses”.
A “Good Catch” is the recognition of an event that could have been harmful to a patient, but was prevented.

This team member was reviewing a patient’s medications during IDT rounding on IPC and noticed that while the patient was hospitalized at a different hospital, he was on a blood thinner. When he was admitted to HCMH the blood thinner was not ordered on admission.
Lindy then further discussed this with the provider and the medication was ordered.

See One, Report One.
Everyone has responsibility for keeping patients and staff safe. Be part of improving HCMH’s culture through proactive “Near Miss” reporting. Know that it is OK to report “Near Misses” before a patient is put at risk or harmed. Make patient safety your NUMBER ONE PRIORITY

If you see something that has the potential to cause harm to patients, report the “near miss/good catch” in QSTATIM. Include steps leading up to the “near miss/good catch” and what may have caused it. Communicate what should happen to prevent it in the future and keep patients safe. ashleys@humboldthospital.org

Who can use the HCMH pharmacy?
Current employees of HCMH and their immediate family (people who live with you such as spouses and children)
Employees who are retired from HCMH in good standing
Do you take all insurance plans?
No. We accept most insurances, but not all! We are unable to process Iowa Medicaid and Tricare plans
Can I pay with my HSA card?
Yes, for prescription items (items that are processed and labeled by us).
Usually, for OTC items. However, there are caveats with using HSA for OTC HCMH pharmacy will not provide you an itemized receipt If you get audited by the HSA company and are requested to provide an itemized receipt, we are unable to help you with that. It is at your own risk that you use your HSA card for OTC items

Humboldt County has high rates of alcohol misuse.
Humboldt County consumes more gallons of alcohol per person (3.2) than the State of Iowa (2.6)
Alcohol-related deaths are higher in Humboldt County 13 per 10,000 people) than the State (8 per 10,000 people).

All maintenance requests need to be submitted to your leader. Each leader has access to our new maintenance request program called, “MaintainX” If you notice something like a leaky faucet, broken door, etc please inform your leader so they can submit a request

Are you looking to help out your fellow employees in times of need and hardship? This is an easy way to help out those people in need. The REACH Program has granted over $6,000 in employee aid over the last 5 years, but we cannot do that without your help.
What is REACH and how does it work?
The REACH Program is a financial assistance program sponsored by the HCMH Foundation. The purpose of the REACH Program is to make funds available to HCMH employees in times of unusual need
To make it work, we ask employees to consider a small gift (suggested amount $2/pay period or $52/year) which is placed into a fund, strictly for employee aid
Confidential applications will be accepted from eligible employees (full or part-time employees who have been employed for at least 12 months) who find themselves in unexpected circumstances and in need of financial assistance A committee reviews the applications for appropriateness of need and grants are made based on that information
Criteria for application and approval
Donations are purely voluntary and making, or not making, a contribution does not affect one’s chances of receiving a grant
Requests to the REACH Program may be submitted as the situation or need warrants Maximum allowable award is $500 with a lifetime cap at $5,000
Criteria considered in evaluating includes:
Unexpected occurrence or specific situation that significantly impacts the personal life of the employee, as well as seriously affecting the ability of the employee to work effectively
Health problems, which result in a depletion of personal PTO and excessive medical bills
Loss of family income
An occurrence that could have been anticipated or avoided by good advance planning or appropriate money management.
Frequency of application.
How are funds distributed?
The HCMH Foundation will make payments directly to the appropriate provider of service, not to the applicant
For example, if help was needed by paying the utility bill the Foundation would write a check directly to the utility company.
How can I contribute?
Contributions can be made in a number of ways:
A one-time check or cash
$2 (suggested) per pay period payroll deduction
One-time payroll deduction gift
