Contact lens wearers who wear cosmetics on a daily basis may be especially vulnerable to eye problems. Misuse of products and adverse reactions to ingredients used in cosmetic formulas cause lens deposits, eye irritation, allergy, dryness, injury and infection. Knowing which products to use and how to use them is important for long-term, problem-free contact lens wear.
Before handling lenses, wash your hands with a mild soap such as Neutrogena, Ivory or a clear glycerin soap. Or, use one of the specialty soaps for contact lens wearers such as AOSoap or Optisoap. Avoid soaps containing cream, deodorant, antiseptics, or heavy fragrances.
Contact lenses should be inserted before any cosmetics are applied to prevent contaminating the lens by makeup and disrupting makeup by tears. Mascara should be used sparingly and only on the outer half of the lashes. Besides being a potential irritant, mascara is frequently a source of infection. Even with the best of care, mascara and eyeliner should be
replaced every three months. Use a light touch with eyeliners and shadows, as they may cause blepharitis, an infection of the eyelid that can lead to styes and chalazion. Don’t use eye liner pencils inside the lower eyelid. Color pigments can cause irritation, damage contact lenses, or lodge underneath the contact lens and scratch the cornea.
Prevent contamination of your makeup by keeping it dry and avoiding contact with fingers. Keep applicators clean and replace them after approximately three months. Hair spray,
deodorant, cologne, mousse, nail polish and nail polish remover should be used before inserting your lenses. If one of these products gets into your eye it can cause permanent damage to the contact lens surface. If you must use hair spray while wearing contacts, close your eyes tightly while spraying and then leave the area quickly. Aerosol mist lingers in the air for some time after spraying.
Never wear contacts when using hair dyes, permanent wave lotions, or medicated shampoos.
Use cosmetics labeled “hypoallergenic,” “for contact lens wearers,” or “for sensitive eyes.” Approximately one in ten women have either a respiratory or skin allergy to perfume. Hypoallergenic brands are designed to be free of irritants such as perfumes and lanolin. Lanolin may be used in cosmetics and soaps and is one of the most common allergens, causing redness, itching, and blotchy skin spots.
Wash your hands and remove contact lenses before removing makeup. Your fingers are less likely to be contaminated by pigments, creams and oils from cosmetic products when the lenses are removed first.

Orthodontic treatment is commonly associated with teenagers, but early intervention can be crucial in addressing dental issues before they become more severe. Phase I orthodontic treatment, also known as interceptive treatment, typically occurs when children are between the ages of 8 and 10. This early intervention can significantly influence a child’s oral health and overall development, providing benefits that extend far beyond the immediate correction of dental issues. It can improve airway and breathing, alleviate oral habits, and avoid the need for future extraction of adult teeth, a practice that was very predominant in the past decades.
Phase I orthodontic treatment is designed to address specific dental problems that are best corrected while a child’s jaw is still growing. This phase often involves the use of appliances such as expanders, partial braces, or functional appliances. The primary goal is to guide the growth and development of the jaw and teeth to prevent more complex issues in the future. Additionally, Invisalign therapy for young children serves the same purpose as traditional braces and expanders, offering a more discreet and comfortable option for early intervention.

The age range of 8 to 10 is critical for several reasons:
Jaw Growth: At this stage, a child’s jaw is still growing and malleable, making it easier to correct skeletal discrepancies. For instance, a narrow upper jaw can be expanded to prevent crowding and improve bite alignment.
This is the period when the first permanent molars and incisors have typically erupted, and significant orthodontic problems can be identified. Early detection allows orthodontists to address issues before they worsen.
For Phase I treatment, having stable primary molars is crucial as they aid with jaw expansion. As children grow older, these primary molars become loose, and the expansion or tooth movement will not take place as intended.
Habit Correction: Habits such as thumb sucking, mouth breathing, or tongue thrusting can lead to dental problems. Early intervention can help correct these habits, preventing damage.

Anesthesia is one of the top concerns that surgery patients have. Thankfully, more and more patients can undergo procedures using local anesthesia. Patients can eat before their procedure and may take medication to decrease anxiety. Patients are often surprised by how easy
the process is and feel completely in control. Here are five common plastic surgery procedures that can be performed without general anesthesia.
We call the upper eyelid lift, a blepharoplasty, our Friday special. Think about it: if someone accidently falls and cuts their eyelid, they go to the ER for numbing medication
and stitches. A blepharoplasty is just the same process in reverse, in a controlled fashion. First numbing medication is placed into the eyelid. The skin is pinched off and removed like a tailor removes excess fabric. The resulting skin is then stitched up. Patients go home immediately afterwards and can shower the next day. Stitches come out in a couple of days.
Torn or stretched ear lobes are a common problem. We now have patients email a picture to the office and if they are appropriate candidates, our office schedules the procedure. This is like going to the dentist, but nobody is in your mouth. The ear is numbed, and the torn skin or stretched skin is removed. Patients can re-pierce their ears in 3 months.
Otherwise called otoplasty or ear lobe setback, an earlobe pinning is done by removing skin behind the ear and pulling the ear cartilage back. The advantage of doing this in the office is the patient can provide feedback on how much the ears are set back. Patients appreciate the control they have from being awake.
Neck liposuction is performed using a thin suction tube to remove fat between the skin and deeper compartments. First numbing medication is introduced into the soft tissue under the skin to loosen the fat. This is called tumescent technique. Then a cannula, or blunt hollow tube is used to remove fat using negative suction. Patients report minimal pain and return to activities as soon as the next day.
The first facelift was performed in Berlin around 1908 when a lady had a piece of skin cancer in front of her ear removed. This resulted in her skin being pulled back. She returned to the doctor remarking how much she loved the aesthetic result and asked for the same thing to be done on the other side!
A mini facelift can be performed using the same medication that is used to numb the skin to close a laceration. Skin is excised in front of the ear and the facial skin is pulled back, lifting the cheeks and neck. Patients appreciate the natural results and quick recovery of a mini in-office facelift.

A smile is the first impression that communicates a woman’s happiness and confidence; if you wish to keep your winning smile out in front, you will want to take special care of what’s behind it – your periodontal (gum) health.
Periodontal health is connected to a woman’s overall health. As your health care needs change throughout your life, during puberty, pregnancy, and menopause, your oral care needs may change.
Hormonal fluctuations during these times may affect your gum tissue and the underlying bone that support your teeth. These changes may increase your susceptibility to periodontal disease and require you to take special care of your oral health.
Gum Care During Puberty
When a young woman enters puberty, the production of sex hormones, such as progesterone and estrogen increase. Studies show that these elevated hormone levels may cause gum sensitivity and lead to a greater reaction to any irritation, including food particles and plaque. During this time, the gums can become swollen, turn red, and may feel tender.
Menstruation and Gingivitis
During menstruation, some women may experience gingivitis. This condition may cause gingival (gum) bleeding, redness or swelling of the gums, sores on the inside of the lip and cheek, a slight burning sensation or gum discomfort. It is important to maintain good oral health during these hormonal fluctuations. However, in some cases, periodontal treatment and antimicrobial agents may be recommended to ensure that your periodontal health is at its best.
Pregnancy and Gum Disease
If you are pregnant, or planning to become pregnant, you need to know that your periodontal health can affect your pregnancy and ultimately the health of your baby. Studies have shown that any infection, including gum infection during pregnancy, may put you at a significant risk of delivering a preterm, low birth weight baby.
If you are already pregnant and have been diagnosed with periodontal disease by your general dentist, a simple non-surgical procedure called scaling and root planning performed by an experienced periodontist may significantly reduce your chances of complications.

Believe it or not, most of the research out there shows that women are more prone to TMD (Temporomandibular Disorders) issues than men. There are a few theories on this that might be interesting to investigate. First of all, women are regarded as generally being more sensitive than men and even more nurturing. This
might explain why they would feel more pain or be more aware if there is a problem with the body. And being the nurturer in the family means being more aware of not only their own pain, but the pain of others. Maybe this is why the new patients that we see are mostly women.
Another factor with women experiencing more TMD is that the disease process known as EDS (Ehler Danlos
Syndrome) is far more common in women than men. In fact, the most common subtype of EDS (hEDS- hypermobile EDS) shows that 90% of the patients are women. This number is staggering. And with the more common forms of EDS, it turns out that the prevalence is roughly 70% women and only 30% men have it. TMD. With EDS the ligaments and joints are all looser than they should be so the discs on top of the jaw bones (the condyles) are far more likely to slip off or become displaced. So along with joint laxity, the EDS patient has to contend with a much higher risk of developing TMD problems. According to many of the doctors who treat EDS, joint laxity is a much more common disorder (disease) than previously thought and is far more prevalent amongst our people than anyone really realizes. What this means for the TMD patient is that they have another problem to contend with potentially and a higher need for treatment exists. Maybe this is why so many people are hurting, have displaced discs, and are not all that aware of the problem? Many people show up at the doctor’s office not really sure why they are aching, have headaches, jaw pain, neck pain, or even tremors when all along it’s a TMD problem that can and should be dealt with.
Women also have the added problem as they age, their estrogen levels begin to drop. During menopause the lower levels of estrogen predispose the jaw joints to degenerate and this can increase alveolar bone loss. The bone loss means the structure of the jaw joint is breaking down which will mean the disc can slip off the condyle more readily and TMD issues begin to take place: headaches, neck pain, etc. Regardless of the situation, it does seem that women are more prone to TMD problems and should have a proper professional diagnose their condition and treat it before it becomes more complicated and even more painful. Children with healthy teeth are more likely to grow into adults with healthy smiles. Your child’s diet not only affects their overall health, but
their oral health as well.
Foods and drinks that contain sugars of any type can contribute to tooth decay. Most items found in your local grocery store will contain nutrition facts and sugar contents on the label. Many of your child’s favorite items may have a low sugar alternative.
Introducing yogurt, cheese or other milk products with meals or as a snack can be a great alternative to carbohydrates.
Food and drink items containing dairy can help increase pH levels in the oral cavity, which can help decrease the acids produced by cavity causing bacteria. The Calcium and vitamin D available in dairy products can also aid in enamel protection.
Fruits and vegetables, such as strawberries and apples, that contain vitamin C can strengthen gum tissue and can act as a natural plaque removing abrasive when extensive chewing is required.
If possible, regulate the amount of sticky sugary foods and candies your child consumes such as taffies, caramels, raisins and gummies. These items get stuck in the grooves of your teeth and may sit there until brushed out.
Sugary food items are better served with meals than as a snack in between meals. This allows for the increased saliva needed for digestion to help rinse these sugars away.
The action of chewing gum containing xylitol can also increase salivary flow and can be a natural abrasive to naturally rinse away food particles.
Juices with high sugar contents should also be limited for four to six ounces per day. Try diluting your child’s juice to decrease the sugar content.
Although sodas and juices that are high in sugar should be limited, introducing water in the oral cavity can help to neutralize the pH and decrease the harmful effects. Water is also the liquid of choice for babies at bedtime instead of milk, formula, juice, or soda.
It is important for infants and children to start an oral care routine twice a day, as well as attend their regular six-month dental checkups. These habits, along with a healthy diet, can help lead your child to a lifetime of good oral health.

Edentulous means lacking teeth. The complete loss of teeth (edentulism) due to gum disease and tooth decay can have severe consequences on your health. Complete dentures are an unacceptable substitute for natural teeth due to slippage, discomfort and inefficient chewing. Dental implants for endetulous patients are typically the best solution.
Edentulism is a complex condition due to the interaction between the disease and aspects that do not depend on the disease itself, such as biologic and patient factors. Edentulism presents socioeconomic problems to the worldwide health care systems.
Numerous studies have proved that the absence of teeth can impact the systemic health of patients in the following ways:
• Patients may reduce consumption of fruits and vegetables, fiber and carotene and they may increase the consumption of foods rich in cholesterol and saturated fats. This may lead to cardiovascular diseases such as hypertension, stroke, aortic valve sclerosis and coronary problems.
• There may be an increase in gastric and duodenal ulcers, pancreatic tumors, upper gastrointestinal tract tumors and kidney problems due to the lack of antioxidants that control the inflammatory cascade.
• There is an increased risk of insulin resistance and type-2 diabetes, due to the increases fat and long chain carbohydrate consumption.
• People may decrease daily physical and social activity.
• People may develop sleep disorders, such as sleep apnea.
Diet appears to play a decisive role in the relationship between
edentulism and general health. There are studies that report a significant decrease in blood concentrations in vitamins A, C, folate and beta carotene in edentulous versus those with teeth.
It is abundantly clear that a compromised or absent dentition has consequences, such as the restriction on the choice of foods, choking on food, poor digestion, alteration of taste, change in chewing efficiency, discomfort and premature facial aging. The self-
image issues associated with edentulism adversely impact on ones socialization.
Studies show that 39% of the elderly edentulous patients cannot eat the food they want, 29% report a decrease in satisfaction from food and 14% avoid eating with other people.

Heart attacks, medically known as myocardial infarctions, are often depicted as sudden, severe chest pain gripping a person’s left side. However, this classical portrayal predominantly reflects the male experience. Women, on the other hand, often
exhibit symptoms that can be subtler and easily overlooked, leading to delayed treatment and potentially fatal consequences.
It’s crucial to recognize that heart disease is the leading cause of death in women globally. Despite this, there persists a misconception that heart attacks primarily affect men. Women are just as susceptible, albeit with differences in how symptoms manifest.
Understanding these distinctions is paramount for timely intervention and improved outcomes.
One of the key differences lies in the variety and subtlety of symptoms experienced by women. While chest pain or discomfort can occur, it might not always be the predominant symptom. Women are more likely to present with atypical signs such as:
• Shortness of Breath: This could
manifest as difficulty breathing, even without exertion, or feeling winded during routine activities.
• Nausea or Vomiting: Women may experience gastrointestinal symptoms like nausea, vomiting, indigestion, or abdominal discomfort, often mistaken for food poisoning or gastrointestinal issues.
• Back or Jaw Pain: Unexplained pain in the back, jaw, or neck, which may feel like pressure rather than sharp pain, is common among women experiencing heart attacks.
• Fatigue: Overwhelming fatigue, especially if it’s sudden and unrelated to physical or emotional exertion, can be a warning sign.

She entered active duty in 2014 and completed residency training in Otolaryngology –Head & Neck Surgery at Walter Reed National Military Medical Center in Bethesda, MD. She graduated from Residency in 2019. MAJ Will completed active duty service after 10 years in June of 2024. She served as Chief of Otolaryngology from 2021 through 2023 and was an Associate Professor at Uniformed Services University. A fungus is a common mold that thrives in dark, warm, moist areas. On the feet, it can grow on and between toes, as well as on soles and toenails. Fungal problems can be a result of environment (socks, shoes, heat and humidity) or weakened immunity (diseases such as diabetes). If fungal infections are untreated, they can develop into serious secondary bacterial infections,
which may require antibiotics to treat. The fungal problems seen most often are athletes foot and fungal nails.
Athletes foot is a chronic infection caused by various types of fungi. It is often spread in places where people go barefoot, such as public showers or swimming pools. The condition ranges from mild scaling and itching to painful inflammation and blisters. It usually starts between the toes or on the arch and may spread to the bottom and sides of the foot.
Athletes foot can often spread and cause fungal nails. Artificial nails, nail polish or injury to the nail can also lead to fungal nails. Fungal nails may become thickened, inflamed or sensitive, and turn unnatural colors.
Treatment Options

All relationships affect our wellbeing, and when a relationship ends, people often experience grief, anger, or shame even if they were the one who initiated the split. You may replay arguments in your mind or find yourself reliving painful moments from past relationships, which can lead to sleepless nights and induce a fight-or-flight response.
Over time, traumatic memories will naturally fade. However, if not properly addressed and stored in the mind, the feelings associated with past relationships create an emotional imprint. Like a timestamp on a video, this imprint remains in our mind so that we can reference it in the future. The formation of this imprint is a normal brain function intended to keep you safe. However, maintaining this imprint will not always serve your current goals. For example, you may notice yourself becoming more aware of your old emotional imprints when you enter into a new relationship. Your reactions to events and social interactions may be influenced more by your past experiences than your current reality. We live in a society that places great emphasis on first impressions. A smile filled with beautifully aligned teeth will make a better impression than one that has crooked teeth. Recently, a lot of people have used Invisalign as a means to achieve this goal. Invisalign is manufactured and marketed by Align Technology, Inc, which was founded in 1997 and received FDA clearance in 1998. Since then, this technology has been widely used as an alternative to traditional braces. Usually the treatment is cov-
ered by insurance the same way as traditional orthodontic treatment.
The treatment consists of a series of clear, removable aligners or trays. These aligners need to be worn 20-22 hours per day. The success of Invisalign treatment is largely dependent on patient commitment. Like braces, these aligners put gentle pressure on the teeth to move them into a new position.
Each set is worn for 2-4 weeks before proceeding to the next set. Besides the obvious cosmetic advantage, they are more comfortable than traditional braces and there are no wires or metal to rub against your cheeks.
While eating, trays can be removed and you need not worry about food being stuck between the front teeth. These trays can also be removed for brushing, flossing and special occasions.

Proper breathing during sleep is crucial for the overall well-being of children, yet many parents may not be fully aware of potential issues related to Sleep-Related Breathing Disorders (SRBD), which includes pediatric obstructive sleep apnea (OSA).
SRBD refer to disturbances in normal breathing patterns during sleep, ranging from common snoring to more severe pauses in breathing known as
apnea events. These interruptions can lead to sleep deprivation and a variety of health complications. In the case of children, untreated SRDB can result in developmental, behavioral, and emotional challenges, potentially leading to conditions like allergies, ADD/ADHD, and depression.
Pediatric OSA, a condition where the airway is partially or completely blocked during sleep, is often caused by enlarged tonsils or adenoids and a lack of tongue space. It differs from adult OSA, as it may manifest alongside behavioral issues and daytime sleepiness in children.

Menopause (twelve consecutive months without a menstrual cycle) usually occurs at age 40 to 55. A significant reduction of estrogen and progesterone occurs, which can cause symptoms such as: hot flashes, night sweats, mood changes, loss of libido, anxiety, depression, weight gain, and painful intercourse.
There are three different types of estrogen: Estrone, Estradiol, Estriol. Estradiol is the strongest and can cause the most adverse side effects. Estriol is the weakest and causes the least side effects, and Estrone is somewhere in the middle.
There are different cell receptor sites for estrogen. ER-Alpha receptors are “growth promoting”and excess stimulation of ER-Alpha can cause abnormal cell growth . ER-Beta receptors cause the reduction of inflammation, control abnormal cell growth. Balancing ER-Alpha and ER-Beta is
optimal. Estrone is mainly attracted to ER-Alpha sites, Estradiol is attracted equally to ER-Alpha and ER-Beta sites, and Estriol is mainly attracted to ER-Beta sites. Balancing ER Alpha and ER Beta stimulation is an important aspect of estrogen use.
Hormone function can be disrupted by excess cortisol (a stress hormone) or environmental pollutants. Either one can occupy estrogen receptor sites, and prevent estrogen from binding, causing low estrogen symptoms. Managing cortisol levels and stress or toxic exposure are important aspects of estrogen use.
The use of synthetic versus bioidentical hormones is very controversial. They act very differently in your cells and how they are excreted by your liver. Bio-identical hormones are plant based and have the same structure as the hormones our bodies make. Synthetic hormones have a somewhat different structure than hormones our bodies make.
The use of hormones must be evaluated in terms of risk. High risk is associated with: positive BRCA genes, a history of cancer, smoking, use of birth control pills and obesity. Co-morbidity such as diabetes, cardiovascular disease, blood circulation disease can also increase the risk of hormone use.

During an IEMT session, a practitioner asks a client to recall an unhappy memory or unwanted emotion while moving their eyes. These combined actions allow a client to reprocess the memory and the emotion attached to it, enabling them to resolve that trigger. Your podiatrist may take a culture from the infected area. This test is helpful in determining the type of fungus you have and can also determine the cause of your fungal problem.
Depending on the type of infection you have, various kinds of medication may be used in treating your fungal problem. Successful treatment usually involves a combination of medication and self-care.
If your athletes foot is not serious, over-the-counter and prescription powders, lotions or ointments can often help treat scaling, itching and inflammation. Foot soaks may help dry excessive perspiration, but talk with your podiatrist first. If your athletes foot doesn’t improve, your podiatrist may prescribe stronger medication.
Since fungal nails are usually more resistant and more difficult to treat than athletes foot, topical or oral antifungal medications may be prescribed. Secondary bacterial infections may require antibiotic treatment. Debridement may be suggested and involves trimming, filing or grinding away the diseased part of the nail. Permanent nail removal is another form of treatment.
Prevention
Your daily habits play a big part in helping to prevent fungal problems. Follow these suggestions to help avoid fungal problems:
• Always dry your feet carefully after you bathe or shower.
• Avoid fungus-causing perspiration, alternate your shoes regularly.
• Try to wear shoes made of breathable materials like leather or canvas.
• Change your socks often.
• Wear sandals or thongs in public areas such as pools, showers or locker rooms.
