“Laser” Cataract Surgery

A cataract is a clouding of the lens inside the eye, causing vision loss that cannot be corrected with glasses, contact lenses or corneal refractive surgery like LASIK.

In cataract surgery, the lens inside your eye that has become cloudy is removed and replaced with an artificial lens (called an intraocular lens, or IOL) to restore clear vision.

The procedure typically is performed on an outpatient basis and does not require an overnight stay in a hospital or other care facility. Most modern cataract procedures involve the use of a high-frequency ultrasound device that breaks up the cloudy lens into small pieces, which are then gently removed from the eye with suction.

What Is the Femtosecond Laser?

Recently, a number of femtosecond lasers – similar to the lasers used for LASIK – have been approved by the FDA for use in cataract surgery to do the following steps:

• Creating corneal incisions to allow the surgeon access to the lens

• Removing the anterior capsule of the lens

• Fragmenting the cataract (so less phaco energy is required to break it up and remove it)

• Creating peripheral corneal incisions to reduce astigmatism (when needed)

A femtosecond laser can also be used to perform some of the steps of surgery to remove a clear lens or refractive lens exchange (RLE), and to make arcuate incisions in the cornea (AK) to reduce astigmatism. There are benefits and risks associated with the use of the laser, and there may be additional costs.

Lasers vs. Traditional Surgery To Remove the Lens

What Are the Possible Benefits?

Traditionally, the eye surgeon uses blades to create the incisions in the cornea (the front window of the

eye), and other special instruments to create the capsulotomy (the circular incision in the outer layer of the cataract or clear lens). The surgeon also uses a phacoemulsification device that utilizes ultrasound power to break up the lens and remove it from the eye. The femtosecond laser can be used to perform some or all of these steps.

The possible benefits of the laser include the ability to make more precise and consistent incisions in the cornea, a more circular and centered capsulotomy, and to pre-soften the cataract so less ultrasound energy is necessary with the phacoemulsification device.

How Is the Laser Used To Treat Astigmatism?

Patients with astigmatism have several choices for the reduction of astigmatism. Nonsurgical options for astigmatism correction include glasses and contact lenses. Surgical correction of astigmatism can be achieved through a toric intraocular lens, a limbal relaxing incision (LRI) made manually with a blade, or an arcuate incision made with the femtosecond laser (AK). Refractive surgery such as LASIK or PRK can also treat astigmatism. The shape and size of incisions made with the laser may be more precise.

Understanding Sleep Apnea, Adenoids, and Tonsils In Kids

Proper breathing is essential for overall health, especially in children. What many parents may not realize is that breathing issues can significantly impact a child’s dental and oral health. Conditions such as sleep apnea, enlarged adenoids, and swollen tonsils can disrupt both breathing and development, making early diagnosis and treatment crucial.

The Connection Between Breathing and Dental Health

When children experience difficulty breathing, especially while sleeping, it can affect the growth and development of their jaw and facial structure. Nasal breathing is ideal because it filters and humidifies the air. However, when children struggle to breathe through their noses—often due to enlarged adenoids or tonsils or nasal obstructive issues—they compensate by breathing through their mouths. Chronic mouth breathing can lead to issues such as dry mouth, an increased risk of cavities, and changes in jaw structure, which may cause crowded or misaligned teeth.

Sleep Apnea In Children

Sleep apnea is a condition where breathing repeatedly stops and starts during sleep. In children, this can be caused by enlarged adenoids and tonsils obstructing the airway. Symptoms of pediatric sleep apnea include snoring, restless sleep, excessive daytime fatigue, and difficulty concentrating in school. Left untreated, it can lead to behavioral issues, delayed growth, and dental complications.

The adenoids and tonsils are part of the immune system and help fight infections. However, chronic infections or inflammation can cause them to enlarge, leading to airway obstruction. Enlarged adenoids and tonsils are a common cause of mouth breathing and sleep apnea in children. dental issues, such as a high-arched palate or a narrow jaw.

Laser Genesis: A Popular Option For the Laser Curious

Laser Genesis is a non-surgical treatment for enlarged pores, dark spots, wrinkles and scars. Laser genesis works using a near-infrared (NIR) ND: YAG laser with a wavelength of 1064 nanometers.

How Laser Genesis Works

Laser Genesis is thought to reduce

large pores (sebaceous gland openings) through several mechanisms. The 1064 nanometer laser wavelength heats blood vessels in the dermis – the deep layer of the skin. This is believed to result in dermal (skin) collagen deposition and remodeling in the perifollicular area (around the pore). The remodeling around the pore results in reduced sebum (oil) production and smaller pore size. Smaller sebaceous

gland means smaller pore size.

Treatment Details

Treatment with Laser Genesis takes 30 minutes or less. No numbing is required because of the gentle nature of the treatment. Laser safety shields are used, and the skin of concern is targeted. Because of Laser Genesis’s gentle nature, it can be used for all ages and skin tones. Some skin tones will benefit from more conservative settings to ensure safety and efficacy.

Post-Treatment Benefits

After treatment, patients notice several improvements. Due to increased collagen production, skin tone and texture is improved. Collagen provides structural support for the skin. More collagen means plumper skin. Elastin stimulation results in improved skin tightness and elasticity. Since the 1064 wavelength of the Laser Genesis targets blood vessels, the treatment can also help with rosacea, dark spots (hyperpigmentation), spider veins, and broken capillaries.

Recovery and Downtime

Patients experience minimal to no downtime. Some patients may note mild erythema (redness) and swelling on the treated skin, but this is generally mild and resolves within 12-48 hours after treatment. If redness and swelling occur, this is generally mild, and patients do not experience any limitations in their activities.

Popular locations for treatment include the face, neck, decolletage and hands. Multiple treatments are recommended for safety and efficacy.

As everybody’s skin is different, we “start slow and go for the goal.” A lower initial setting helps us understand how the patient’s skin reacts to the treatment in terms of effect and downtime. Subsequent treatments adjust the laser fluency (intensity), time (in milliseconds), and number of passes for the optimal rejuvenation effect. Furthermore, the skin on different parts of the body is different in thickness and dermis to epidermis ratio. Consequently, the same settings cannot be used on the face and a patient’s hands.

The Connection Between Sleep Apnea and TMJ Disorder

Obstructive Sleep Apnea (OSA), and Temporomandibular Disorder (TMD) are conditions that can severely disrupt daily life. OSA occurs when the airway is blocked during sleep, leading to pauses in breathing. This not only disrupts sleep but also poses significant health risks, such as cardiovascular issues, diabetes, and even cancer.

In the case of Sleep Apnea, sleep studies are commonly used to measure breathing patterns, while TMD diagnosis may involve imaging techniques like X-rays or MRIs to examine the jaw joint and surrounding tissues. Recognizing the signs and seeking professional help are key steps in managing these disorders. If you look for studies, there are clear links between OSA and TMD.

Both Sleep Apnea and TMD share several risk factors that can exacerbate symptoms. If the discs in the jaw joints are displaced, this will greatly contribute to TMD as well as OSA. Oftentimes, genetics are involved – a family history of OSA and/or TMD is common.

Another important factor is age; the likelihood of developing Sleep Apnea and TMD increases as one gets older. Being overweight is another significant risk factor for Sleep Apnea, as excess fat can obstruct the airway. This excess weight can also put additional stress on the jaw muscles, contributing to TMD symptoms.

Dental Implants FAQs

You can’t grow new teeth, but dental implants offer the next best thing if you’ve lost a tooth. A dental implant looks like a tiny screw and replaces the roots of a tooth. During a minor surgical procedure, your dentist creates a small opening in your jawbone and places the implant inside. Over the course of several months, the implant fuses to the bone, providing a secure foundation for the crown that will be placed on top of it. In some cases, you may receive a temporary crown the same day you receive your implant.

Replace Multiple Teeth – When

you have more than one tooth missing, implants provide an ideal replacement mechanism. You don’t even need one implant for every missing tooth. Instead, implant teeth can act as supports for fixed bridgework. For example, if you are missing three teeth in a row, we can place two implants, one on either side of the gap, and a crown in between that has no implant underneath. That way, you won’t need to use any of your remaining natural teeth as bridge supports, which could weaken them and make them more susceptible to decay.

Replace All Teeth Permanently – Implants can support an entire arch of upper or lower replacement teeth that are fixed into the mouth and are never removed. Sometimes the new teeth can be supported by as few as four implants. It’s comparable to the structure of a table, which only needs four legs to hold it up. In cases where jawbone density and volume have deteriorated, five or six implants might be needed to support a row of 10-12 teeth. There are many moving stories about how a hospice social worker helped a patient get married or take that one last trip to the ocean. These are impactful, memory-making moments that go a long way to honor the patient’s final wishes while helping a

family heal after their gone.

But I’m not going to share those kinds of moments.

I’m going to share moments like helping the patient make a phone call to an estranged family member -- or giving them permission to NOT make that phone call. Counseling a daughter whose mother no longer knows who she is due to dementia. These are moments that matter, everyday moments, that are nurtured by a hospice social worker.

Families can be messy. For many, making moments that matter means reconciling grudges or misunderstandings. A social worker might start some conversations with the patient to explore the history of the rift and what the difficulties are. Then, there can be a family meeting where the social worker is a third-party intermediary to guide a talk about giving or asking forgiveness.

The moment that matters here is to give all involved healing before the patient dies and they’ve lost the opportunity. For the patient, this unfinished business could cause extra stress at the end of their life, leaving behind unresolved issues.

Sometimes a patient is not able or willing to reconcile. Then social workers become a support for the family to help them find in some other way the peace they hoped would happen before their person died.

Memory making moments like going to an Orioles game or making a big birthday party happen can be a challenge for social workers that leads to an important moment for everyone. But I’m also helping to make moments that matter each time that I am with a patient who shares how great it is to watch Jeopardy with their family or asks to help them call a long lost relative to say, “I’m sorry.”

Some moments that matter are even simpler. A family asked that I visit their father every week, which I did for three months while he was in our care. On a few visits I had to complete about five minutes of paperwork. The rest of the time was just being there. To listen. To talk. Each week I knew I made a difference. Each week I knew I had made moments that mattered.

Mouth Breathing and Dentistry

Entire books have been written about the negative effects mouth breathing has on our health. This article will help the reader understand just one of these negative effects: jaw growth and development.

When we breath through our mouth, whether because of nasal obstruction or out of habit, we are creating a situation which causes our upper jaw to become more narrow than normal. This is especially true if mouth breathing

occurs during growth and development. As we grow, the tongue is supposed to act as a natural palate expander, counteracting the equal and opposite forces of the lips and cheeks pressing down and in. If the tongue is forced down so we can get air, which is our most immediate need, then the roof of the mouth still grows but in a distorted shape. It becomes narrower than genetically programmed and the palate grows higher than normal. This eventually becomes a selfperpetuating problem because the roof of the mouth is also the floor of the nose. So a high, narrow palate leaves less room in the nose for air to pass and eventually mouth breathing becomes the only way to get air to our lungs.

High narrow palates result in less room for the teeth, creating crowding and misaligned teeth. A narrow palate usually results in an exaggerated overbite, because

the growth of the lower jaw is not as affected by tongue posture as the upper jaw. If the upper jaw isn’t somewhat larger than the lower jaw to allow the lower jaw to fit directly underneath it, then the lower jaw tends to be forced further back.

When the lower jaw sits further back, this can lead to jaw joint problems know as TMJD (temporo-mandibular joint dysfunction.) TMJD symptoms include clicking and popping of the jaws, headaches, neckaches, backaches, jaw pain, and forward head posture. Also, with the lower jaw further back, the tongue is forced into the throat making the airway smaller and often leading to sleep apnea later in life, an unhealthy condition where the patient stops breathing due to tongue obstruction during sleep.

Traditionally, dentists and orthodontists have just accepted the

size of the upper jaw as-is and dealt with the consequences by extracting teeth. Physicians have dealt with sleep apnea caused by small palates by using CPAP (continuous positive airway pressure) machines to keep people breathing.

Fortunately, small narrow palates can be prevented if treated during the growing years with retainer-like devices to counteract the lip and cheek pressure. Small narrow palates in adults can also be treated with oral devices like the DNA appliance, which can encourage jaw growth even in fully grown adults. I can personally relate to this topic because I grew up breathing through my mouth due to severe allergies as a child. This led to orthodontic problems treated by extracting teeth, TMJ problems later in life, and eventually sleep apnea, which I have now managed to eliminate using the DNA appliance.

Mouth-Body Connection

People with non-insulin dependent diabetes (diabetes type 2) are three times more prone to develop destructive periodontal disease.

There is a direct correlation with patients with periodontal disease and rheumatoid arthritis. Periodontal disease precedes rheumatoid arthritis.

There is a clear relationship between obesity (upper body obesity) and periodontitis. Recent studies show how people with normal weight that exercise have a lower incidence of periodontitis and better blood glucose control.

There is a correlation between advanced Alzheimer’s disease and poor oral health. Scientist at the School of Medicine and Dentistry, University of Central Lancashire in the UK, have discovered the presence of a substance (lipopolysaccharides) from the bacterium Porphyromonas gingivalis in the brains of those with Alzheimer’s disease. This bacteria is present in people that have gum or periodontal disease.

He has a special interest in treating colon and rectal cancer using advanced surgical minimally invasive techniques including robotic, laparoscopic, and transanal minimally invasive surgery. Dr. Makhoul is Board Certified in both General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons education committee.

As part of his commitment to surgical training and education, Dr. Makhoul serves as a Clinical Instructor of Surgery at the George Washington University. I knew I wanted to be a veterinarian at age five and started working in clinics at 14. I earned my DVM from The Ohio State University in 1977 and spent a decade in clinical practice before discovering holistic medicine. Seeking alternative ways to improve pet health, I became certified in veterinary acupuncture, natural health, and homeopathy, training with Dr. Richard Pitcairn.

In 1993, I sold my practice and moved to Sedona, AZ, embarking on a spiritual journey. I became a Tibetan Buddhist nun from 2002 to 2023, dedicating myself to relieving suffering. I also studied energy healing and volunteered in disaster relief efforts, including Katrina Relief and Tara’s Babies Animal Sanctuary.

Returning to Maryland in 2006, I worked at Veterinary Holistic Care before co-founding Holistic Veterinary Healing in 2013 with Dr. Kitty Raichura. I later pursued a Master’s in Human Acupuncture and practiced on both humans and animals until Dr. Raichura’s passing in 2016. Growing up with a background in Traditional Chinese Medicine, I discovered my passion for integrative veterinary medicine early on. My father introduced me to Gua Sha and acupressure massage, sparking my fascination with healing modalities. Initially pursuing human medicine, I worked in acupuncture clinics, volunteered at Walter Reed, and conducted research at UMD and FDA.

In 2015, I encountered Traditional Chinese Veterinary Medicine (TCVM) through Dr. Allison Marshall, a veterinarian certified in acupuncture. Inspired, I pursued veterinary medicine, earning a Bachelor’s in Biomedical Engineering at VCU in 2017 and completing my DVM at Virginia-Maryland College of Veterinary Medicine in 2021. I specialize in integrative medicine, blending holistic and conventional approaches to provide optimal care. Certified in veterinary acupuncture (CVA) through Chi University, I also have experience in pathology, surgery, and exotics.

Hair loss can be a very emotional part of the natural aging process, and seeking ways to curb this process is a challenge. There are many products on the market that claim to re-grow hair, but the results are mixed. Some work for some people, and for others, not so well.

When Should My Child See an Orthodontist?

Many parents assume that they must wait until their child has all of his or her permanent teeth before they can see an orthodontist. The American Association of Orthodontics recommends that all children get a check-up with an orthodontist no later than seven years of age.

Early consultation allows the orthodontist to identify any subtle problems associated with jaw growth

and/or incoming permanent teeth.

As orthodontic treatment not only involves the teeth but also the surrounding boney structures, it is highly recommended to start treatment before the face and jaws have finished growing.

By age seven, the six-year molars (adult first molars) and incisors have usually erupted. After the first molars erupt the space available for the front teeth does not increase as a child grows. To the contrary, in most people after the permanent molars erupt, the space available for the front teeth decreases with age.
