The prostate, a male organ located below the bladder and wrapping 360 degrees around the urethra, can enlarge with age, restricting urinary flow. This condition, known as benign prostatic hyperplasia (BPH), often leads to symptoms such as frequent urination, weak urinary stream, and urinary tract infections. Treatment focuses on reducing prostate size and improving urinary flow.
Symptoms range from mild to severe, often developing gradually over years but potentially worsening after surgery or other triggers. Common symptoms include:
• Weak urine stream
• Difficulty starting urination
• Incomplete bladder emptying
• Frequent urination, especially at night
• Sudden urge to urinate
• Dribbling or leaking urine
• Straining to urinate
• Recurrent urinary tract infections Evaluation Methods
Diagnosing an enlarged prostate involves assessing symptoms and determining the severity of urinary blockage. Common tests include:
• Physician evaluation and symptom questionnaires (e.g., IPSS)
• Rectal exam
• PSA blood test
• Cystoscopy (camera-based urethra and bladder examination)
• UroCuff test (measures blockage severity)
• Trans-rectal ultrasound (prostate size assessment)
Treatment Options
Treatment is tailored to individual needs and symptoms, often progressing from lifestyle changes to medical or surgical interventions.
Behavioral Modifications
Simple changes, like reducing evening fluid intake and limiting caffeine or alcohol, can alleviate symptoms.
Alpha-blockers (e.g., Flomax, Rapaflo): Relax prostate muscles to improve urine flow but may cause side effects like sexual dysfunction or blood pressure changes.
5-alpha-reductase inhibitors (e.g., Proscar, Avodart): Shrink the prostate, potentially enhancing urinary flow, with side effects including erectile dysfunction and breast tenderness.

If you’re a baby boomer, born between 1946 and 1964, there are so many reasons to focus on and improve your oral health. Periodontal (gum) disease risks rise with age and can lead to a host of physical problems. There is no better day than today to get a checkup by a dentist specially trained in knowing the links between your mouth and body health.
Boomers grew up in a time of prosperity, education, and opportunity. This generation has a greater need for oral treatment than any other. The Centers for Disease Control reports that 70% of Americans 65 and older have chronic gum disease. Surveys suggest that growing up in a time of plenty may have led to a taste for a diet rich in carbohydrates and sugar. This has caused enamel erosion and gingivitis.
Research also shows that boomers can do better to protect their oral health. One survey shows only 40% of older people use mouthwash, 34% brush their teeth only once a day, and 47% say they often skip flossing.
When age-related conditions like arthritis or joint pain make proper brushing and flossing more difficult, it exacerbates the problem further.
Another reason to take care of your mouth as you age is the growing connection between gum disease and Alzheimer’s disease. More than 6 million Americans over 65 have been diagnosed with this form of dementia, a number projected to more than double by 2050.
A 2019 National Institutes of Health study published in the Journal of Alzheimer’s Disease shows the bacteria that cause gingivitis, the earliest form of periodontal disease, are also connected to several forms of dementia. One species of bacteria is believed to migrate from gum infections through the bloodstream and into the brain. There, they release enzymes that can destroy nerve cells, leading to memory loss and Alzheimer’s.
The relationship between de-
mentia and gum disease goes the other way, too: People with memory loss may neglect their oral health and a mild problem may turn into something much more severe.
Research is underway to develop a drug that can block bad bacterial enzymes and slow the progression of dementia. Until then, it is imperative to take extra care of your teeth and gums as you age.
• Brush twice a day, floss once or twice a day, and visit your perio-
dontist for a checkup.
• Cut out sugary foods and beverages. Use the artificial sweetener xylitol, which is shown to starve harmful bacteria and reduce plaque buildup.
• Dry mouth lowers oral pH, increasing the risk of tooth decay and gum disease. Talk to us about special alcohol-free oral rinses, lozenges, and other preventive measures.
• Swish with water after eating and consuming sweet and acidic drinks.
• Resolve to adopt a healthier diet and lifestyle for overall good health. Studies show this alone may lower your risk for Alzheimer’s disease by up to 60%.

Crohn’s disease is a chronic inflammatory bowel disease (IBD) that affects the gastrointestinal (GI) tract. While often thought of as an adult condition, Crohn’s disease can begin in childhood, causing significant symptoms and challenges during critical stages of growth and development.
Most children present with symptoms in the early teen years, although there are many cases seen in early childhood as young as three years old. The disease can affect any part of the GI tract—from the mouth to the anus—but most commonly involves the end of the small intestine and the beginning of the colon.
Children with Crohn’s disease may experience:
• Persistent diarrhea
• Abdominal pain or cramping
• Fatigue and low energy
• Weight loss or failure to gain weight
• Delayed growth or puberty
• Blood in the stool
Because symptoms can be mild at first or confused with other common childhood illnesses, diagnosis is often delayed.
A thorough evaluation is needed to confirm Crohn’s disease and rule out other conditions. Diagnosis may include:
• Blood tests to check for anemia, inflammation, or poor nutrition
• Stool tests to rule out infections
• Imaging studies like MRI or CT enterography to view inflammation
• Endoscopy and colonoscopy with biopsies to examine the GI tract and identify signs of chronic inflammation
Early diagnosis is important to avoid complications and support proper growth.
There is no cure for Crohn’s disease, but it can be managed effec-
tively. Treatment focuses on reducing inflammation, relieving symptoms, and promoting healing and growth. Common treatments include:
• Nutritional therapy, especially in children, using special liquid diets (known as exclusive enteral nutrition)
• Anti-inflammatory medications, such as corticosteroids for flare-ups
• Immunomodulators or biologic therapies, such as infliximab or adalimumab, for moderate to severe cases
• Surgery, sometimes necessary to
remove damaged sections of the intestine, though typically reserved for specific complications
Children with Crohn’s disease benefit from care by a pediatric gastroenterologist, along with support from dietitians and other specialists. Regular monitoring helps adjust treatment, ensure proper growth, and support a good quality of life.

It is almost impossible to maintain a nice smile when you are hurting, and especially when that pain comes from TMD (Temporomandibular Disorder). When the discs in the jaw joints are out of place, they can and will pinch nerves and blood vessels in the jaw joints. This is a concept that few doctors understand as it was
never taught in school. Such training often requires extensive post-graduate education, and oftentimes that can be very hard to find!
As a patient, you should be aware that there are many ways to treat the TMJ disorder. To figure out which one(s) are right for you, the first step is diagnosing. A proper diagnosis is where it all starts and that means a very good MRI (soft tissue) and CBCT x-ray of the joints. From there, your
course of treatment might include orthotic (non-surgical dental oral appliance treatment), physical therapy, osteopathy, dry needling, acupuncture, massage, and even laser therapy.
At our office, we most frequently use appliance therapy to make space for the displaced discs to return into the socket. Along with your appliances, we oftentimes do massage or laser therapy. This is a specialty treatment. Our massage therapist incorporates the Fotona laser to achieve good energy deep into the tissues to maximize healing. It quite honestly feels “wonderful” according to our patients.

When a patient wears an orthotic appliance on their lower teeth, this allows the displaced discs the chance to get back into proper position within the socket. This takes time. Most cases the displacements occurred over the course of years and it often takes a year for those discs to go back into place, if they will at all. In the event that they do not go back where they belong, a minor surgery can be done to put them back into the joints. This is called plication.
The other appliance that is used is the ALF. This little wire has the ability to open up the palate which makes it easier to breathe. It can also lower a high palatal vault which allows better nasal breathing. When a person breathes through their nose more, this allows for better production of nitric oxide which helps the body to heal better. It also helps level the cranial bones to some extent and thus reduce asymmetry of the skull. This little device really is amazing. Admittedly, it takes years of training to get a handle on it, but it is well worth it.
Ultimately, when a TMD case has healed or improved enough to not need surgery, it is time to finish up so you do not need to wear appliances any longer. This can be accomplished either by orthodontics or prosthodontics. The purpose is to support your jaw joints and the discs in the correct anatomical position. It is important that you understand the mechanism of how this works so choose your provider carefully and do your research! Your smile is worth it.

Tooth loss is extremely common among adults, especially as we age. Rather than living with the discomfort and hassles of dentures, many people are opting for what is called “all-onfour” dental implant restoration.
An implant is a small titanium

A bunion is an enlargement of the joint at the base of the big toe the metatarsophalangeal (MTP) joint that forms when the bone or tissue at the big toe joint moves out of place. This forces the toe to bend toward the others, causing an often painful lump of bone on the foot.
Symptoms
• Development of a firm bump on the outside edge of the foot, at the base of the big toe.
• Redness, swelling or pain at or near the MTP joint.
• Corns or other irritations caused by the overlap of the first and second
toes.
• Restricted or painful motion of the big toe.
Podiatric medical attention should be sought at the first indication of pain or discomfort because, left untreated, bunions tend to get larger and more painful, making nonsurgical treatment less of an option.
The primary goal of most early treatment options is to relieve pressure on the bunion and halt the progression of the joint deformity. A podiatric physician may recommend these treatments

His 3-year surgical residency was at Phoenixville Hospital (now Tower Health) in the Main Line Area in PA and then completed a one-year Fellowship in Naples, FL in Reconstructive Foot and Ankle Surgery + Minimally Invasive Surgery.

Dr. Horowitz arrived in the Washington metro area in 1986 and established Bowie Foot and Ankle later that year. He quickly joined the staff at the newly opened Bowie Health Center Surgery Center. Dr. Horowitz enjoys the general nature of his practice and sees patients of all ages with a variety of foot problems. He especially enjoys dealing with the orthopedic nature of many podiatric problems, whether they be of recent onset or long standing. He has found that chronic problems affecting the ankle, knee, hip or back, can often be traced to recent symptoms within the foot, or vice versa.

leading to loneliness and a decreased quality of life. Over time, this isolation can contribute to depression and anxiety, compounding the emotional toll of hearing impairment.
Cognitive decline is another major concern. Multiple studies have found a strong link between untreated hearing loss and increased risk of dementia. When the brain receives less auditory input, it must work harder to interpret sounds, diverting resources away from memory and thinking processes. Over time, this increased cognitive load can accelerate mental decline.
Balance and physical safety are also impacted. The inner ear plays a crucial role in maintaining balance, and hearing loss can make it harder to detect environmental cues, like approaching cars or warning signals. Older adults with untreated hearing loss are more likely to experience falls and other accidents, which can lead to hospitalization or long-term disability.
Additionally, untreated hearing loss often leads to communica-
tion breakdowns with loved ones and healthcare providers. Misunderstandings and frustration can strain relationships and create barriers to effective medical care. Seniors may mishear instructions or fail to report symptoms properly, leading to medication errors or delayed diagnoses.

As we grow older, many aspects of our health evolve — and sleep is no exception. While people of all ages need quality rest, older adults often face unique challenges that make it harder to get the deep, restorative sleep essential for overall well-being.
It’s a common myth that older adults need less sleep. In reality, most seniors still require 7–8 hours per night. However, changes in the body’s internal clock (circadian rhythm), reduced melatonin production, and increased health issues can all interfere with sleep quality and consistency.
Age-related changes in sleep architecture often mean lighter, more fragmented sleep. Older adults may wake up more frequently throughout the night, have trouble falling back asleep, or experience earlier wake
times in the morning. These shifts can leave individuals feeling fatigued, unfocused, or irritable during the day.
Medical conditions common in older adults — such as arthritis, acid reflux, and heart or lung disease — can also disturb sleep. In particular, sleep apnea is significantly underdiagnosed in seniors. Loud snoring, pauses in breathing, or excessive daytime sleepiness should never be dismissed as simply “getting older.” Untreated sleep apnea can contribute to high blood pressure, heart disease, insulin resistance, and cognitive decline. Sleep problems in older adults are also closely linked to mental health. Chronic insomnia has been associated with depression and anxiety, and poor sleep may accelerate memory issues or dementia-related symptoms. Fortunately, there are steps that can help. As with any medical profession, there are many different types of nurses – surgical, medical office, emergency department, pediatric– the list goes on and on. Saying that hospice nurses are different might sound cliché. But hospice nurses are different and provide a unique type of care. Here are three ways that set us apart from other nurses.
1) Hospice nurses honor the sanctity of end of life. This is the definition of a hospice nurse. People living with acute illness often come to the realization that trying to fix the unfixable may result in exhausting, painful attempts to recover. The patients and families we care for have made the decision to no longer seek curative treatment for their illness and that means a shift in the type of care they need. Hospice nurses help with that shift by focusing on comfort, peace and quality of life. We do that by listening to the patients’ wants and needs. We work directly with the patients to determine their goals of care and to help them share those goals with their loved ones and those providing their care.
2) Hospice nurses educate patients and families to support them as they make their goals of care clear. The knowledge and experience a person has about death comes from having someone in their life who has died. How can one make important decisions about their end of life without understanding what that looks like? Hospice nurses have joined hundreds of families on the journey and they use that experience to help patients and families consider what their end of life should look like. There’s still a lot of fear of hospice because there’s a misunderstanding of what we do. Hospice professionals have conversations about end-of-life care throughout the community. Advance directives are now more frequently part of a primary care physician’s discussion about goals and medical history.
3) Hospice nurses have the hospice heart. We are often asked if our job is sad. Most hospice nurses say that helping families connect to bring the best possible final days to someone they love is a very touching experience. When you have played a role in helping someone say a final goodbye in their own home surrounded by loved ones and as comfortable as possible, how can that be sad? That is what we call having the hospice heart.

Here are eight things you can do to protect your child from getting injuries:
• Supervise children at all times and intervene when there is inappropriate behavior such as pushing or shoving.
• Make sure playground equipment is age appropriate. Maximum fall height should be five feet for preschool-aged children and seven feet for school aged children.
• Swing seats should be made of plastic or rubber. Metal or wood should be avoided.
• Avoid any equipment that has openings that could entrap a child’s head.
• Make sure you can clearly see your children on the playground.
• Remove tripping hazards such as tree stumps, or rocks.
• Avoid playgrounds with asphalt, concrete, grass, and solid surfaces under the equipment. Look for surfaces of hardwood fiber, mulch chips, pea gravel, fine sand or rubber to a depth of at least nine inches.
• Wear appropriate protective gear during sports activities (for example, shin guards for soccer, a hard-shell helmet for baseball or football, body padding for ice hockey, and helmet when on a bike or skateboarding).

Schizophrenia is a serious, but treatable illness which affects the brain. One person in 100 has this illness. It is a disease that makes it difficult for an individual to tell the difference between real and unreal experiences, to think logically or to have a normal emotional response. Sleeping and remembering may be difficult.
Symptoms usually appear slowly over months, though they may occur suddenly. At times symptoms may be confused with those of other conditions. A person may feel tense, have
difficulty concentrating or sleeping. Often there is a tendency to withdraw from activity.
As the illness progresses, symptoms of psychosis appear. These can vary. A person may begin to act or speak strangely. Hallucinations may develop. A person sees, feels, smells, and hears things that are not really there. There may be physical symptoms, unusual movements or the person may sit or stand in strange positions. Some people become almost motionless, while others may move constantly. Without proper diagnosis and treatment, symptoms tend to come and go. Getting appropriate treatment can break this cycle. Proper treatment usually can control symptoms just as insulin controls diabetes. Most people can lead productive, satisfying lives. Like many illnesses, primary treatment is medication and learning to manage the illness. The major medications used are called neuropletic medicines. They work by changing the balance of the chemicals in the brain.

What Is Osteoporosis?
Bone loss is a natural part of aging in all people. But some people develop a disease called osteoporosis, and for them, bone loss is severe. Bones become fragile and break easily. Fractures can happen almost anywhere in the body, including the spine, hip, leg, pelvis, and wrist.
More women than men develop osteoporosis, but bone health should be a priority for both men and women.
• Are you Caucasian or Asian? These two ethnic groups are more likely to develop osteoporosis than are other ethnic groups.
• Are you thin and petite? Because small people, women in particular, have less bone mass to begin with, they are at greater risk for the disease.
• Do you have older relatives who have curvature of the spine (sometimes called “dowager’s hump”), loss of
height, fractures, or chronic back pain? If so, you may have inherited a tendency toward osteoporosis.
Examine your health history:
• Have you passed menopause? In women, estrogen aids in the production of bone mass. At menopause, the amount of estrogen their bodies produce declines sharply.
• Have your menstrual periods stopped? Women who have had a hysterectomy, women who are high-performance athletes, and women who diet excessively or who have an eating disorder may stop having periods. These women have had a hormonal change that can cause bone loss.
• Are you taking medicines that increase the risk of osteoporosis? The most common culprits are steroid medications and high doses of thyroid hormone. Long-term use of these medicines can cause bone loss.
