We have a big announcement we’re excited to share with our friends and family! We are officially rolling out our new parent company Trinity Senior Services come July. Trinity Senior Services builds on the century-long legacy of Milwaukee Catholic Home. While we’re best known for being the place people go for an outstanding senior living experience, we’re much more than apartments for older adults. Our mission to connect people to their purpose spans across generations and programs. Our 500 team members fulfill this mission every day in the most amazing ways – from taking exceptional care of residents to managing the 18 acres of sustainable farmland that provides produce for our communities. 2023 Community Impact Report

Milwaukee Catholic Home began its journey over a century ago, with a group of visionary women from the Marquette Women’s League. As our founders were mission based, we are inspired by their work to continue what they’ve started and continue to give back to this demographic and the community.

We’re so proud of the work we’ve done over the past century to bring our community together to address our toughest issues and most urgent needs. But we couldn’t do this important work without you!

But even as we celebrate these successes, we know there’s so much still to be done. By working with our nonprofit and community partners, we aim to be a catalyst for impactful and lasting change in Southeast Wisconsin.

Charitable Care

Our philosophy of service, which focuses on whole-person wellness, ensures our residents have a variety of opportunities for engagement which support all aspects of one’s being. We believe that an engaged life is a full life, and that the more aspects of your life that you engage, the happier and healthier you age.

In addition to benevolent assistance, our communities are proud to participate in the Medicaid program, accepting reimbursement rates far below the actual costs of care, for enrolled residents living in skilled nursing. In 2023, at Milwaukee Catholic Home, we provided over 17,045 Medicaid bed days, or, on average, 47 people a day.

Community Outreach

Partnership with Cristo Rey Work Study Program

Part of Cristo Rey Jesuit High School’s academic program involves having all students work to learn about different jobs in the community and gain valuable onthe-job experience. All students work and learn skills that will serve

As a Life Plan Community, we at Milwaukee Catholic Home pride ourselves on being a place where health, wellness, and choice come to life. This past year we rolled out two new programs, the Navigator Program and our brandnew memory care program and services, Clare Garden Suites.

The Navigator Program provides individualized support, guidance and advocacy for older adults and their families during periods of transitions, with a special emphasis on the needs of those facing changes in their home environment or in one’s physical or cognitive functioning. The Navigator Program utilizes a dedicated team of social workers and the Milwaukee Catholic Home multi-disciplinary team to fully assist residents, and in 2023, 70 residents were successfully navigated!

Implemented as a direct response to our customers, Clare Garden Suites offers two different memory care specific neighborhoods that enable residents to live comfortably and according to their needs and abilities. Inspired by our philosophy of service for dementia care, Everyday Amazing, Clare Garden Suites ensures that those we serve are safe and living lives enriched by social, spiritual, and recreational experiences.

We have a lot to be proud of as we celebrate our present-day successes, and so much to look forward to in the future.

Who We re Trinity Woods

An Update from:

In 2023, Trinity Woods hosted many exciting events that our residents and their families loved.

The summer season was bustling with activity and excitement; while the Rising Star tutoring program took place for four weeks, we also had the honor of hosting a farm-totable dinner! Created in partnership with the Mount Mary University dietary program, students served a four-course dinner that included fresh produce from Clare Gardens. Some of the finest performers in the Milwaukee area also entertained our residents, their families, and members of the greater community during our summer concert series, Backyard Beats.

The holiday season at Trinity Woods was especially merry in 2023. Our Christmas Giving Tree, proudly displayed in the Trinity Woods Town Center, displayed creative gift ideas for those who needed some inspiration on what to purchase for those on their holiday lists. We also hosted our first annual Holiday Stroll and bake sale. Residents had the opportunity to purchase a plate of holiday cookies and get their picture taken with Santa!

At Milwaukee Catholic Home, Trinity Woods, and Our Lady of the Angels Convent, Cristo Rey students helped with crucial tasks in the A Life Engaged™ and Pastoral Care departments in 2023. They especially enjoyed relationship building with residents when assisting with events and programs.

Partnership with Milwaukee Public Schools

In 2023, Milwaukee Catholic Home welcomed five students during the spring semester and seven students during the fall semester as part of the School-to-Work Transition program (STWTP). With a history of more than thirty years in Milwaukee Public Schools (MPS), the STWTP provides a continuum of transition services to MPS students to develop their skills and help them achieve their highest level of independence. By providing social and vocational opportunities in a real-world setting, STWTP focus on abilities and on promoting and supporting the highest capacities of MPS students in their personal and professional development.

Book Buddies

“Book Buddies,” is an impactful intergenerational program pairing Milwaukee Catholic Home residents with first grade students at Maryland Montessori School to support literacy development. Once a week the children walk across the street with their book and spend 30 minutes reading to their “buddy”. Relationships develop over the year and in some cases that relationship has been maintained for years!

“It is heartwarming to see the interactions between the residents of Milwaukee Catholic Home and the children of Maryland Montessori School,” says Tori Smith, Director of A Life Engaged™ at The Health and Rehabilitation Center. “Watching the relationships grow each week has been inspiring and shows how we can easily bridge the gap between generations if given the opportunities.”

A similar program, “Book Nook” takes place at Trinity Woods. Trinity Woods resident volunteers come together every other Thursday to read to the children at the Wauwatosa Daycare, which is onsite at Trinity Woods. The children, from ages two to four, are paired up with one of our volunteers to read for half an hour.

Both “Book Buddies” and “Book Nook” are just some of the ways that intergenerational relationships thrive across our organization.

Partnerships with Learning Institutions in Southeastern Wisconsin

Our organization was proud to partner with the following learning institutions in 2023 in the areas of RN and CNA Clinical Practicums, Social Work, Art and Recreational Therapy, Health Care Administration, and Dietetics:

• Mount Mary Universitynursing clinicals took place in Fall 2023 as well as dietetic internships and dietetic integrated learning classes in both the spring and fall semesters.

• Alverno College - brought nursing students to the MCH Health and Rehabiliation Center for their geriatric clinical classes.

• The University of WisconsinMilwaukee - In 2023, the School of Social Welfare had three social work students from UWM, and we ended up hiring one of them to be our social worker for The Health and Rehabilitation Center.

• Marquette University - In 2023, Marquette University collaborated with Milwaukee Catholic Home for a number of initiatives, including Physical Therapy practicums, practicums and wellness initiatives in Exercise Science. The Marquette University Communications Department hopes to work with Milwaukee Catholic Home on wellness initiatives and communications efforts in the future.

• Milwaukee Area Technical College - nursing clinicals for CNAs held throughout 2023 on both the Milwaukee and Mequon campuses.

• Herzing University - LPN clinicals held throughout 2023 and individually precepted four nurses for their last semester. One nurse was hired full time at Milwaukee Catholic Home after graduation.

Who We re Clare Gardens

This past year, our management company began providing backoffice finance and record-keeping support for The Catholic Ecology Center, as well as all payroll services for their employees.

In 2023, Clare Gardens completed its second full growing season at the Catholic Ecology Center! There were over 26,120 pounds of produced delivered, 700 pounds of food gleaned by local partners to serve local food pantries, and over 30,600 seedlings starting in the onsite greenhouse, despite the drought! The fall farmstand allowed for this amazing produce to be available to all visitors.

We enjoyed offering educational workshops to the public about organic gardening practices and how to bring these practices home, thanks to our A Life Engaged™ programming and our partnered curriculum for Mt. Mary University dietetics students. In 2023 alone, we offered 10 tours of Clare Gardens with a total of 200 people participating in these intimate and informative experiences.

Educational and social experiences for residents of Milwaukee Catholic Home, Trinity Woods, and members of the greater community truly came alive at Clare Gardens in 2023. An Update from:

An Update from: Who We re OurLady of theAngels

Special weekly visits from cuddly therapy pets, monthly poetry readings, musical performances from local bands and choirs, and dancing demonstrations were just a few of the many exciting activities the sisters enjoyed this past year at Our Lady of the Angels (OLA).

OLA was also blessed with dedicated volunteers and impactful donations in 2023. Beautiful flowers were often delivered by Locker’s Florists in West Allis, bringing some cheer and sunshine to the sisters’ dining experience. Aggie’s Bakery in West Allis was also a generous provider of birthday cakes to celebrate each sisters’ special day and also is kind enough to donate bakery items for daily teatime.

The year 2023 was also filled with exciting internal changes. The daily nursing and caregiving services are now entirely provided and managed by the Milwaukee Catholic Home team, marking a significant difference in the daily care and wellbeing of the sisters.

In addition, I assumed the role of new the Administrator at Our Lady of the Angels, taking over the role from Taylor Smith, who is the new Director of Sales at Milwaukee Catholic Home. I am looking forward to continuing the peaceful, calm, and fun atmosphere that OLA is known for amongst its residents and visitors.

• Divine Savior Holy Angels High School -in 2023, students served at Milwaukee Catholic Home as part of their annual “Vocare” program. This service immersion experience, which is the capstone of a student’s four years of study, allows a senior to serve for two weeks in the Greater Milwaukee community.

• Marquette University High School - in 2023, students served at Milwaukee Catholic Home as part of their annual “Senior Shared Life” project. This service immersion experience, which is the capstone of a student’s four years of study, allows a senior to serve for two weeks in the Greater Milwaukee community.

The CNA Nurse Aide Training Program

First established in 2022, the CNA Nurse Aide Training Program is letting people start their healthcare career now! This 80-hour course over five weeks includes 40 hours of online, self-paced learning and 40 hours of in-person skills lab and hands-on training. Scholarships which pay for the Nurse Aide Training Program are available to students who wish to work at one of our communities for a minimum of

one year. In 2023, while a total of 65 students participated, 43 of those students passed the Nurse Aide Training Program.

Nurses Affecting Change

In 2011, Bonnie Anderson, a nurse at Milwaukee Catholic Home, helped create Nurses Affecting Change. The group travels around the greater Milwaukee area providing free breast exams to women. Community members can ask Nurses Affecting Change to host pop-up breast examination clinics. At the pop-ups, examinees will be brought into a room with a nurse practitioner and nurse clinician who will talk them through the process of an examination and explain how to conduct self-exams at home.

In 2023, 96 sites were visited, including churches, women’s shelters homeless clinics, and

housing authority apartment complexes. Of the 1,984 clinical breast exams performed, 11 women were diagnosed with early-stage breast cancer. Nurses Affecting Change continues to make an impact and save lives throughout our community.

Lenten Almsgiving

The Catholic Church considers almsgiving "a witness to fraternal charity" and "a work of justice pleasing to God" (Catechism of the Catholic Church, No. 2462). Almsgiving is the act of donating money or goods to the poor or performing other acts of charity. Giving alms is essential to fostering a deep spiritual life because it allows us to step outside of ourselves and focus on the needs of others. More than simply giving money, almsgiving is an act of love that can deepen our prayer life.

So, in Lent 2023, residents of Milwaukee Catholic home gave alms, both physically through financial donations, and spiritually through prayer and devotion, to Ukrainian Humanitarian Aid through St. Michael’s Ukrainian Catholic Church in Milwaukee. May God listen to our prayers and bless us with a peaceful end to this conflict.

Earth Day Clean Up

Several team members across our organization worked through sun, rain, sleet, hail and snow in Estabrook Park while participating in Milwaukee Riverkeeper’s Earth Day Cleanup, making our world a little more beautiful. This annual earth day celebration is a great and impactful way to learn about ecofriendly opportunities and support the community in its environmental efforts. Our team was part of nearly 4,000 volunteers who joined together across the Milwaukee River Basin to pick up trash, get outdoors, and help achieve swimmable, fishable rivers.

Capuchin Community Services’ Holiday Food Program

Residents, staff, and friends of our organization participated in the annual Capuchin Community Services’ Holiday Food Box Program. The annual Holiday Food

The retired Franciscan brothers residing at Queen of Peace Friary are thriving thanks to the hard work and dedication of our management team. That management team now includes Christine Smith, a registered nurse who began serving the friars in 2023.

Ms. Smith has been in the healthcare industry for over 43 years, starting out her journey as a certified nursing assistant before becoming a licensed practical nurse and finally, a registered nurse. She prides herself in her many years experience working with the geriatric population, serving on numerous management teams in the areas of skilled nursing, assisted living, home health care, hospice, and mental health.

“I really enjoy my role at Queen of Peace,” says Christine Smith. “I feel welcomed and supported not only by my fellow team members, but most importantly, by The Guardian of the Friary. The caregiving team at Queen of Peace is compassionate, honest, and dedicated, committed to working together to serve the friars most efficiently and effectively.”

Ms. Smith’s past experience and her current work with the friars has truly been the answer to a prayer; her dedication and commitment will make an impactful and meaningful contribution to the lives of the dear friars who live at Queen of Peace Friary.

ALife Engaged

We believe that an engaged life is a full life, and that the more aspects of your life that you engage, the happier and healthier you age.

In 2023, A Life Engaged™ offered over 60 programs to the larger community. From musical performances to a wide variety of lectures and experiential programs, health and wellness opportunities to celebrations and socials, book club discussions to opportunities for spiritual growth and development, we had over 500 people from the community participate in these experiences.

Highlights include the Health Fair at Milwaukee Catholic Home and Trinity Woods (which featured over 20 health and wellness vendors that were available for screenings, consultations, massages, reiki, and healthy food and beverage samples), and the annual Fall Harvest Festival at Milwaukee Catholic Home and Trinity Woods (which celebrated the harvest of Clare Gardens and allowed residents and members of the greater community to purchase Clare Gardens produce, homemade resident crafts and specialty items, and participate in fun activities).

All of these and countless more programs prove that thanks to A Life Engaged™ programming, residents and community members enjoy the opportunity to pursue their passions and continue living their best life.

Box Program provides central city families in need with food baskets that contain all the fixings for a festive meal during both the Thanksgiving and Christmas holidays. While the initial goal of our residents and staff was to fill and donate 50 food boxes, 100 boxes were filled and returned to the Capuchin’s for distribution.

Ladies of Charity Winter Clothing Drive

The Milwaukee Ladies of Charity is a group of 120 women volunteers who work to clothe children in need in our city. In 2023, our Pastoral Care Team hosted two unique drives to call special attention to this local organization and the great work they do; while a “Mitten Tree” was on display at Milwaukee Catholic Home’s The Residence at 2462, a “Stuff the Chimney”

fundraiser took place at The Health and Rehabilitation Center. These festive appeals filled 5 large boxes with hats, mittens, gloves, and winter coats.

Annual Retreat

“Searching for God and Letting God Find Us: A Meditation on Francis Thompson’s Poem ‘The Hound of Heaven’” took place over two days in September 2023. Led by Fr. Tim Kitzke, retreat participants from our communities reflected on this inspirational work centering on the pursuit of a sinner by a loving God.

Sunday Spirit Series

The 2023 theme for the Sunday Spirit Series was “The Hands and Feet of Christ”. This quarterly speaker series, which took place at both Milwaukee Catholic Home and Trinity Woods for both residents and members of the greater community, examined the inspiring lives of four individuals who live the prayer of St. Teresa of Avila. Our speakers throughout the year ask the question: How does your faith inform your life? How do you endeavor to be the hands and feet of Christ to the broader community?

Residents Support Guest House of Milwaukee

Guest House of Milwaukee provides shelter, housing, education, and services to Milwaukee's homeless who seek to transform their lives with dignity and purpose. Located on North 13th Street between Juneau and McKinley Avenues in the King Park neighborhood, Guest House of Milwaukee is the largest publicly funded homeless shelter in the city and has been leading our community in ending the cycle of poverty in our clients' lives for nearly forty years. In July 2023, residents of Milwaukee Catholic Home made nearly 500 sandwiches for the Guest House of Milwaukee as part of their Faith in Our Lives program! All who participated were honored and humbled to support such a meaningful cause.
