极乐寺慈善医院在过去三年 有何发展和变化?

与槟城其他医院相比,极乐寺慈善医院在今年 7月迎来了成立三周年的里程碑,可说是医院界的“ 年轻一辈”。从我的个人观点来看,我们在这三年里 已经完成了许多重要的计划和目标。虽然我们的表 现尚算合格,但仍然有改进的空间。总体而言,我 对医院的发展感到满意。

2024年,医院即将进入巩固并提升医疗服务水 平的全新阶段。今年我们将推出一系列新服务和设 施,旨在满足患者的多样化需求:

三维乳房X线检查 (Mammogram) : 这项最新技术将为妇女提供更全面和准确的乳腺健 康检查服务。

心脏冠状动脉造影 (CT Cardiac Angiogram) : 这一非侵入性诊断工具将提供更准确的心血管评 估,有助于早期发现和治疗心脏疾病。

运动心电图检查/心脏负荷测试 (Exercise Stress Test) : 通过评估患者在运动负荷下的心脏功能和反应,我 们能够更早地发现和干预心血管问题。

What developments and changes have the Kek Lok Si Charitable Hospital (KLSCH) seen in the last three years?

KLSCH will be celebrating its third anniversary in July this year. Compared to other hospitals in Penang, it is a noteworthy newcomer in the healthcare sector. From my personal viewpoint, we have achieved

several important objectives over the last three years. While our performance has been commendable, there are still areas in which we can improve. Overall, I am satisfied with the hospital’s progress.

In 2024, the hospital will enter a new phase of consolidation and upgrade. We are set to unveil new services and facilities that will meet the diverse needs of our patients.

Mammogram: This cutting-edge technology will offer women a more comprehensive and precise assessment of their breast health.

CT Cardiac Angiogram: This non-invasive diagnostic tool will provide highly accurate evaluations of cardiovascular health, and will go a long way towards the early detection and treatment of heart conditions.

Exercise Stress Test: This test will enable earlier detection and intervention for cardiovascular issues by evaluating heart function and responses to physical exertion.

High Dependency Units: These advanced care facilities will cater to patients’ specialised needs during critical phases, and provide a safer and more comfortable environment for recovery.

The successful launch and operation of these projects will bolster the hospital’s medical services,

自今年2月起,我们启动了病黎家访计划 (Home Visit Welfare Patient),这一举措旨在深化对 曾获得医疗福利援助的受惠病人的关怀。我们不仅 关注他们在医院的治疗情况,更关心他们在康复期 间的生活质量。无论他们仍在医院接受治疗或已出 院,我们都会定期前往他们的住家。除了提供符合 医疗需求的药物和补品之外,我们还致力于建立与 他们的紧密联系,了解他们的需求和挑战,并尽力 提供支持和帮助。

我也会亲自进行家访,与病人和家属直接交 流,了解他们的后续发展和需求,并提供额外的支 持和鼓励。我深信这个计划将成为我们医院特有的 文化。

过去三年,医院面临了哪些 挑战,又是如何克服这些挑 战的?

2024年对我们来说是一个充满挑战的一年,为 了应对患者数量急剧增加的挑战,我们已经着手扩 建医院,新建的三层医院大楼将能容纳更多的专科 诊所。动土典礼已于2023年12月举行,预计在一年 内完成建设。

在医院扩建的过程中,我们深知硬体设施和软 体支持的重要性,同时也认识到拥有不同专业背景的 人才是医院运作的真正关键,这是我们的医院扩展计 划核心,以确保我们能够有效地满足患者需求并提供 卓越的医疗服务。我们计划邀请数位专科医生加入团 队,以拓展医疗服务范围,进一步提升医疗水平。

然而,和其他正处于扩建阶段的医院一样,我 们也面临着护士短缺的问题。随着国内就医需求的 不断增加,部分护士不堪超负荷的工作压力而选择 离职,导致护士职业的流动性非常大。马来西亚卫 生部对设立医疗机构和人才招聘的要求也较为严 苛,使得医院一时之间也难以聘请专业护士填补空 缺。

确保医院的最大使用率也是一项挑战。随着医 院扩建项目正如火如荼地展开,我们必须精心规划 整个建设过程,包括人员配置、设备采购等。通过 合理的规划和管理,我们能够确保资源得到最佳分 配,同时满足患者和医务人员的需求,让医院可持 续发展。

医院如何与其他组织合作来 增强服务或影响范围?

“我一直强调,我们必须获得公众和社会集体力 量的认可和协助,极乐寺慈善医院的运作才能事半 功倍。”

过去两年里,我们与各大非政府组织保持密切 合作,比如定期与槟岛各大社团合作举办捐血活 动,旨在为中央医院的血库筹集血液,同时也借 此机会接触年轻一代,让他们了解极乐寺慈善医 院在社会中的重要角色。3月在立达学院(DISTED College)的捐血运动已圆满落幕,我们计划在6月 份前往理科大学(Universiti Sains Malaysia)举办下 一次活动。

我们也与发林集团及马来西亚国家癌症协会合 作,推行免费检验癌症的活动,这一合作旨在协助 有经济困难并且可能已有症状的人士做进一步的癌 症检验,让他们能够及时接受健康问题的检测和治 疗。

借此机会,我要感激媒体朋友对极乐寺慈善医 院的持续关注与报道,已使我们逐渐被大众认识。

一所医院,我当时以 “无底洞” 三字 形

容办医院的经费。谁曾

想,28年过去,我们 现在已经准备要兴建 第二栋医院 大楼。 In 1996, Venerable Seck Jit Heng invited me to assist him in establishing a hospital upon my retirement. I described the funding for setting up the hospital as a ‘bottomless pit.’ Little did I know that 28 years later, we would be gearing up to construct the second building for the hospital.

We launched our Patient Welfare Home Visit programme in February this year. It was designed to offer more thorough care for patients who had been recipients of medical welfare assistance. This program extends our care beyond monitoring hospital treatment to improving patients’ quality of life during their recovery period. Whether in hospital or at home, we will visit the patient regularly. Along with providing medications and supplements, we also hope to build stronger relationships, better understand their needs and challenges, and provide additional support and assistance as needed.

I have participated in some of these home visits personally. They have enabled me to connect with patients and their families, gain insights into their needs and concerns, and provide customized support and encouragement. I believe our Patient Welfare Home Visit program will distinguish our unique culture here at KLSCH.

What challenges has the hospital encountered over the past three years, and what strategies have been implemented to overcome them?

2024 has been challenging for us because of the rapid increase in patient numbers. To meet this rising demand, we are expanding our hospital by constructing a new three-storey building, which will house additional specialty clinics. The groundbreaking ceremony took place in December 2023, and we expect construction to be completed within a year.

在新的扩展规划中,我们将重点考虑增设以 下 几个方面:

洗肾中心: 为慢性肾脏病患者提供洗肾服务及相关 医疗支持。

骨质密度扫描 (DEXA Scan): 用于评估骨质密度, 帮助诊断骨质疏松和骨骼相关疾病。

你对医院的未来几年有什么 期望?

目前我们拥有约160名成员,每位成员都是不 可或缺的重要存在。虽说已相当稳定,我们仍需进 一步营造更加美好的氛围。我们致力于打造一种积 极向上的工作文化,鼓励团队成员相互尊重、合 作,积极分享知识和经验,确保他们在工作中拥有 自信和满足感。我们希望每个家庭成员都能感受到 医院温馨的氛围,更积极为患者提供优质服务。

一旦稳定下来,我们就可以考虑如何拓展我们 的服务范围。在这个过程中,管理层必须以身作 则,承担共同的责任,将每一笔笔资金都用在最需 要的地方,努力实现医院的可持续发展。我们希望 团队成员真正理解并认同医院的核心价值——“慈 爱、承诺、关怀”,在他们的日常工作中贯彻这些价 值观。

As we expand, we are mindful of the significance of both physical infrastructure and the work environment created by our staff. Having a diverse range of expertise in house is a priority in our expansion plan this year. Our goal is clear: to effectively meet patients’ needs and deliver outstanding medical care. To achieve this, we will be inviting several specialists to join our team

However, as with other expanding hospitals, we are also facing a shortage of nurses. The rising demand for medical services nationwide has increased the workload for nurses. Those unable to cope are resigning from their positions, resulting in a high turnover within the profession. The situation is further exacerbated by the Ministry of Health’s stringent requirements for establishing medical facilities and recruiting staff. Because of these factors, we have to recruit actively to fill the vacancies.

Another challenge for us is ensuring that the hospital operates efficiently. With the expansion project in full swing, we have to be meticulous in our planning, covering everything from staff allocation to equipment procurement to ensure optimal utilisation of our new facilities. Strategic planning and effective management will allow us to allocate our resources in the most optimal manner to meet the needs of both patients and staff and ensure the sustainability of KLSCH.

How can hospitals collaborate with other organisations to improve their services or extend their reach?

To achieve this, we’ve forged strong partnerships with prominent non-governmental organisations over the past two years. For example, we’ve collaborated with various community groups in Penang to host regular blood donation events. These events serve a dual purpose: replenishing the supply at Penang General Hospital’s blood bank and engaging with the younger generation to raise awareness of KLSCH’s societal role. Our blood donation event at DISTED College in March was a success, and we are planning the next one at Universiti Sains Malaysia in June.

We also collaborate with Farlim Group and National Cancer Society Malaysi to launch a free cancer screening initiative. This collaboration aims to help financially disadvantaged individuals who may already show symptoms by providing further cancer screening, allowing them to receive timely detection and treatment for their health issues.

To the media, I want to express our appreciation for their interest and ongoing coverage of KLSCH, which has boosted our visibility. It remains a goal for us to ensure that all segments of society understand our hospital’s unique role and mission. Therefore, we have prioritized transparency in our management practices and procurement processes, which has contributed to gaining the trust and cooperation of the community.

Which aspects of hospital expansion are you focusing on?

I have consistently stressed the importance of gaining support from both the public and society for the efficient functioning of KLSCH.

The hospital’s expansion is a multifaceted project that requires thorough consideration and meticulous planning in many areas, such as facility construction, departmental configuration, and outpatient services. At its core, the goal is to increase

bed capacity and construct a new three-story building to accommodate additional specialty clinics, thereby offering a broader range of services.

The following are some of our key considerations for the new expansion project:

An Outpatient Department that includes a urology clinic focused on diagnosis and treatment.

A Dialysis Centre that provides essential dialysis services and comprehensive medical support for patients suffering from chronic kidney conditions.

A Blood Bank for the storage and provision of blood and blood products.

A Magnetic Resonance Imaging (MRI) facility that employs advanced imaging technology to diagnose and monitor various medical conditions.

A Bone Density Scan (DEXA) facility to diagnose osteoporosis and other bone-related conditions.

A Catheterisation Laboratory to conduct cardiac catheterisation and treatments as well as provide diagnostic and therapeutic services for cardiovascular conditions.

The establishment of a Nursing College to nurture and raise the expertise of nurses and keep up with the latest developments in medical technology.

Additionally, we plan to expand our team of specialists this year with the following new specialists and services:

What are your expectations for the hospital in the next few years?

Our current staff strength is around 160, and each one is indispensable. While the situation is relatively stable, we aim to create an even more conducive environment characterised by a strong culture of positivity, where team members respect and collaborate with one another, actively share knowledge and experiences, and feel confident and fulfilled in their roles. We want every member of

our hospital family to experience the warmth of our environment, so that they are motivated to provide the best possible care to our patients. This will in turn ensure that KLSCH’s service quality remains high.

As the situation stabilises, we will explore ways to expand the scope of our services. Our management team must also lead by example, play their part, ensure that every dollar is spent judiciously, and work hard to ensure KLSCH’s sustainability. I hope our team members will embrace KLSCH’s core values of “compassion, commitment, and care,” and personify these values in their daily work.

在外科手术中,麻醉是非常重要的其中一环。麻醉科是一门通过使用药物或麻醉 剂来暂时性阻断患者痛觉的学科,麻醉师则是受过专业培训的医生。麻醉及重症 学专科柯彩虹医生指出,“我负责的工作包括了手术前的评估、与病患及家属讨 论并制定合适的麻醉方式、确保病患在麻醉过程中的安全,以及手术后的疼痛管 理。”

麻醉方式可略分为三大类:全身麻醉 (General anesthesia)、区域性麻醉 (Regional anesthesia) 和局部麻醉 (Local anesthesia)。

In all surgeries, anaesthesia is key. Anaesthesiology – the field of medicine concerned with temporarily numbing a patient’s sensation with pharmaceuticals or anaesthetics – relies on the expertise of anaesthesiologists, who are trained specialists of the field. Dr. Quah Chai Hoong, an esteemed anaesthesiologist and intensive care specialist at Kek Lok Si Charitable Hospital (KLSCH), explains her responsibilities as follows: “I conduct comprehensive pre-operative assessments, consult with patients and their families to devise tailored anaesthesia plans, ensure patient safety during the administration of anaesthesia, and manage postoperative pain effectively.”

There are three main categories of anaesthesia: general anaesthesia, regional anaesthesia, and local anaesthesia.

柯医生表示:“一般上,我会在手术前会见病患 和家属,做术前评估,了解患者的病史,接着会向 病患和家属详细解释可使用的麻醉方式以及可能发 生的副作用。例如一位病患即将进行腿部手术,他 可以选择进行全身麻醉,或是半身麻醉。我会确保 病患在最佳的身体状态下进行手术。”

她指出,若病患清楚自己即将面对的各种过 程,在一定程度上会减低他们的术前焦虑。此外, 一些高风险群,例如高龄、有心脏问题、中风病史 等等的病患,柯医生会与专业团队如心脏专科等一 起协商,以期将风险减至最低。她重申:

Prioritising Patient Safety and Comfort

For Dr. Quah, standard procedure involves conducting a thorough pre-operative assessment with patients and their families to gather the patient’s medical history and evaluate various anaesthesia options along with their potential side effects. For example, a patient undergoing leg surgery can opt for either general or regional anaesthesia. The decision depends on which option offers the greatest safety and ensures the patient remains in optimal physical condition throughout the procedure.

understands all the procedures involved in his or her surgery, as this can help alleviate pre-operative anxiety. For high-risk groups such as the elderly or those with cardiac issues or a history of stroke, she will collaborate with the relevant specialists to minimise risk. Extra care must be taken when making decisions on anaesthesia method, agent, and dosage, as there are inherent risks in all of them.

Beyond surgical procedures, anaesthetics are also used in critical care and pain management. Following surgery, patients whose vital signs – heart rate and blood pressure, for example – are unstable may require intensive care in the ICU. In such instances, Dr. Quah’s post-operative role will include monitoring the patient’s recovery and managing any post-operative pain.

Dr. Quah recently recounted the case of a critically ill patient admitted to Kek Lok Si Charitable Hospital (KLSCH) requiring urgent intensive care. The patient presented with dangerously low blood pressure and a severe infection, and her prognosis was poor. However, thanks to the dedicated efforts and expert care of the medical team, the patient regained consciousness and her condition began to improve. By the time of discharge, she had made a full recovery.

“My greatest wish is for patients and their families to fully understand their upcoming surgeries, including the procedures and anaesthesia methods involved. I will also reassure them that the anaesthesia we use will be safe so that they can go into surgery with peace of mind.”

The Path to Anaesthesiology

柯医生认为,一名医生的责任不仅是照顾病人 的健康,也需要关注病人的身心灵健康。除了处理 生理疾病外,也要关注病人的心理状态和情绪。她

最初选择成为医生,是因为相信医生可以帮助他 人。她回忆起自己家人生病时的场景,希望自己能 够成为那个能够沉着应对的人。她认为,若没有相 关知识,会不知所措,不知要如何帮助病人。

尽管医学系学习艰难,但对柯医生来说,求学 生涯充满乐趣。真正的挑战来自于在槟城中央医院 的实习经历。柯医生当时实习时压力非常大,尤其 是长时间的繁忙工作,有时候甚至需要连续上班一 整天。在医院实习期间,也需要经常轮值当班,从 早上开始工作,一直持续到隔天傍晚,完成当天所 有任务后,才能休息。

麻醉科是柯医生最后实习的部门,也是她决定 进修麻醉及重症学专科的转折点。她表示,一起共 事的麻醉师让她深受启发,因此选择成为一名麻醉 师。进入麻醉科实习前,她对插管技术、急救复苏 等技能并不熟悉,在麻醉师顾问的耐心指导下逐渐 进步,在麻醉科的学习也为她后来的专科修读奠定 了坚实的基础。

抗击疫情前线

柯医生分享了她在新冠疫情(COVID-19)期 间的经历,这段时期对她来说是最难忘的。她提到 疫情肆虐期间,许多年轻生命因不敌病毒而不幸离 世,站在医疗前线的他们虽然已经尽力抢救但也无 力回天。

“我们医疗人员帮病患拨打视频通话给他们的家 人,看着他们哭泣,我们也感到非常伤心。”

Dr. Quah believes that a physician’s duty is more than just tending to a patient’s physical health. He or she should also consider the patient’s overall well-being, such as his or her mental and emotional states. Dr. Quah’s motivation to pursue medicine sprang from her belief that doctors can play a major role in transforming and improving peoples’ lives. She recalled the time when she wished she had been more supportive and knowledgeable when illness befell her family. It made her realised that some understanding of the factors that affect a person’s overall well-being is important too.

Dr. Quah said that although her pursuit of medical knowledge was challenging, it was also deeply fulfilling. She remembers her internship at Penang General Hospital well. There was immense pressure; the shifts were long and demanding, sometimes extending into a full day’s work. The roster often required her to be on duty from early morning until late evening of the next day, and rest breaks were granted only when all assigned tasks had been completed.

Her internship concluded with anaesthesiology posting. It was during this time, inspired by the anaesthesiologists she was working with, that she decided to specialize in anaesthesia and intensive care. Initially unfamiliar with procedures such as intubation and resuscitation, she made good progress under the guidance of her consultant anaesthesiologist. This experience laid a solid foundation for Dr. Quah to specialize in anaesthesia.

当时还在槟城中央医院服务的柯医生,有时一 整天七、八个小时都是待在重症监护室里治疗和照 料新冠患者,她们不惜付出长时间的努力,尽力抢 救每一个患者。她回忆,当时他们身穿个人防护装 备(PPE)和呼吸防护具(PAPR),轮班进入重症监护室 看顾病危的新冠患者,进行各种评估和测试,包括 检查病患的呼吸系统和血流动力学(haemodynamic) 以稳定患者的状况,并经常更换所有的导管来降低 感染风险。

COVID-19: Reflections from the Frontline

The COVID-19 pandemic was profoundly impactful for Dr. Quah. She recalled the heartwrenching loss of many young lives to the virus, despite the tireless efforts of frontline medical workers like herself.

“During the pandemic, we often helped patients connect with their families through video calls because they had to be isolated. When we see them cry, we too were overcome with sadness,” said Dr. Quah.

At the time, Dr. Quah was with the ICU of Penang General Hospital, where she spent her entire work day – often seven to eight hours at a stretch – attending to COVID-19 patients. The hospital staffs spared no effort in saving every patient. She remembers donning personal protective equipment (PPE) and powered air-purifying respirators (PAPRs), entering the ICU in shifts to care for critically ill COVID-19 patients. She conducted various assessments and tests too, such as evaluating a patient’s respiratory system and haemodynamic to stabilise their condition, along with frequent tube changes to mitigate infection risks.

Performing surgeries on COVID-19 patients presented great challenges for the medical staff. Everyone had to wear tightly sealed PPE and respirators to prevent infection. These included protective suits, face shields, gloves, goggles, and more. Adhering to this protocol was imperative due to the virus’s high level of contagion.

Dr. Quah’s appointment at Kek Lok Si Charitable Hospital marked a milestone in her professional journey.

“I’m thrilled to join this expansive family. The exemplary professionalism displayed by the doctors and nurses fosters a strong sense of team spirit and collaboration. Towards the end of our interview, Dr. Quah reiterated her aspirations: “My greatest wish is for my patients and their families to fully understand their upcoming surgeries, including the procedures and anaesthesia methods involved. I will also reassure them that the anaesthesia we use will be safe and that they can go into surgery with peace of mind. As a physician, I will do my very best to give every patient the best medical care. Their health and wellbeing is my greatest concern and responsibility.”

12岁时因盲肠炎入院的经历让黄诗幈医生心里 萌生了对医学的向往:“当时接触到的医生给了我很 深的印象,我觉得医生是真正可以帮助人的,而且 相较其他行业,医生只需要全神贯注把病人的病给 治好。”

她回忆,实习期间,医生们在病房里总是忙得 不可开交,作为实习生的他们经常需要与放射科医 生交涉,跟进病人的检查报告。每次踏入放射科部 门,黄医生看到放射科医生们有条不紊地进行超声 波和阅片工作。这种平静的工作氛围给了她很好的 印象,于是她下定决心要进入放射科领域。

经历了一番努力后,黄医生最终成功进入了放 射科领域。她开始学习超声波操作等技术,并逐渐 发现自己对放射学专科越来越感兴趣。

“我觉得放射科虽然没有直接接触和问诊病人, 但在诊断和治疗方面扮演着举足轻重的角色,因此 我决定去考取放射学专科资格。”

黄医生曾被派往多家政府医院服务,虽然需要 重新适应新的环境和认识新的同事,但这些经历也 让她受益匪浅。她提及在士拉央医院时曾参与不少 台肝脏和肾脏移植手术,由于手术非常耗时,放射 科医生有时甚至需要待命到凌晨才能前往协助进行 超声波检查。 医学梦想的萌芽

The Beginning of a Medical Dream

Dr. Wong See Peng first discovered an interest in medicine when at the age of 12 she was hospitalised for appendicitis.

“The doctors really made an impression on me. I saw them as true agents of healing, completely dedicated to curing a patient’s illness,” she recalled.

During her internship, she observed that doctors often work at a hectic pace and interns like herself often worked closely with radiologists to keep track of a patient’s diagnostic reports. Dr. Wong vividly remembers the methodical and efficient way radiologists worked whenever she stepped into the radiology department, whether it was conducting ultrasound scans or reviewing images. Impressed by the serene yet purposeful atmosphere, she resolved to pursue a career in radiology.

With hard work and determination, Dr. Wong managed to secure a place to study radiology. As she gained knowledge on imaging techniques like ultrasound, she found herself increasingly attracted to the subspecialty of radiology.

“While radiology may not involve direct interaction with patients, its role in diagnosis and treatment is paramount. That was reason enough for me to want to specialise in radiology,” she said. During her stints at various government hospitals, Dr. Wong found herself having to continuously adapt to new environments and work with new colleagues. This was challenging but the experiences proved invaluable. Especially eye-opening were

黄医生指出,计算机断层扫描的好处是能提供 更准确的诊断结果,坏处则是辐射会比普通X-光检 查来得高,只有病患有需要才会做这个扫描。她进 而举例:“例如有一位长期抽烟的病人有症状,做了 X-光检查发现有阴影,临床医生就可能会要求再做 计算机断层扫描来确定阴影的正确位置,以获得更 清晰的肿块影像。”

黄医生强调,定期做乳房超声波和乳房X-光检 查可提早发现完全没有任何症状的乳癌病兆,及早 发现问题并采取治疗措施,帮助在疾病进展到更严 重阶段之前进行干预。她回忆一名六十多岁妇女想 要更新保单,保险公司要求做乳房超声波检查,那 时才发现胸部有一个1公分不到的小阴影,后来被 证实是初期乳腺癌。由于及时发现,她能够接受手 术治疗,目前身体状况良好。

Evolution and Innovations in Imaging Diagnostics

With advancing technology, physicians increasingly rely on new equipment and techniques for diagnostic imaging. Radiology, which integrates examination, diagnosis, and treatment, plays a vital role in diagnosing various diseases in all clinical disciplines. At Kek Lok Si Charitable Hospital (KLSCH), the Radiology Department offers a range of services, including X-ray examinations, ultrasound scans, and computed tomography (CT) scans.

“X-rays are most commonly used for chest imaging, followed by orthopaedic conditions such as fractures and joint issues. Ultrasound, on the other hand, is used for soft-tissue organs such as the breasts, liver, kidneys, gallbladder, and bladder. There is no radiation exposure in ultrasound. CT scans involve layering X-rays to create detailed images with clearer results,” Dr. Wong said.

一般人提及放射科,可能都会联想到辐射,也 难免会对这些医疗设备的辐射有些误解。黄医生笑 说对辐射有误解的人可大略分为两大类,一是非常 怕辐射的,反之则是完全不在乎的。她解释:“其实 在我们生活的周遭环境里也会有辐射,这些医学设 备都是经过非常严谨的科学研究与实验后,才会全 面运用在病人身上。医生也会根据病人状况来做合 适的检查。普通X-光和乳房X线检查的辐射是很低 的,例如拍一个胸片的辐射是相当于正常生活中十 天所累积的辐射。”

黄医生表示,计算机断层扫描的辐射水平相对 较高,通常比普通X-光胸片高出10至100倍,但她 也强调了情况视具体情况而定。医生会根据病人的 症状和诊断需求来决定是否进行检查,通过这些检 查可以得到准确的诊断,从而采取相应的治疗措 施。总体而言,这些检查的利大于弊。

黄医生也指出,有些病人可能会根据自身症状 主动要求进行某些检查,比如计算机断层扫描,他 们误以为这些检查可以揭示身体所有的问题。然 而,实际上这些检查应该是在临床医生的问诊和其 他辅助检查(如血液检查等)的基础上,根据具体 情况来决定是否需要进行。而且这些检查通常只针 对特定的部位,以避免不必要的辐射暴露。举例来 而三维乳房X线检查(Mammography / 3D Breast Tomosynthesis)设备也预计在今年内全面投入运 作。此外,磁共振成像扫描(MRI)和骨密度扫描 (DEXA scan)也被纳入未来将增添的设备里。她相信 这些新设备和技术将可以为病人提供更完善准确的 检查结果,有助于临床医生给予更有效的治疗。

Dr. Wong pointed out that while CT scans are more precise, they expose a patient to higher levels of radiation compared to conventional X-rays. That’s why CT scans are used only when it is necessary. “For example, if a long-term smoker shows symptoms and his X-ray examination reveals a shadow, clinicians may request a CT scan to pinpoint the exact location of the shadow and obtain a clearer image of any potential masses,” she said.

For women, Dr. Wong stressed the importance of regular breast ultrasound and mammography screenings for early detection of asymptomatic breast cancer. This is so that treatment can begin before the cancer progresses. She recounted a case where a woman in her sixties, seeking to renew her insurance policy, underwent a breast ultrasound examination and discovered a small – less than 1 cm – shadow in her breast. This was later confirmed to be earlystage breast cancer. Thanks to timely detection, she soon underwent surgery and has been well since.

According to Dr. Wong, KLSCH recently introduced CT Coronary Angiography. By the end of this year, its Mammography/3D Breast Tomosynthesis equipment will be also operational. The hospital also plans to introduce MRI and DEXA scan devices in the near future. Dr Wong believes that these new technologies will offer more accurate results and help clinicians to make better treatment decisions.

Demystifying Radiology

Many people associate radiology with radiation, which can lead to misconceptions about the radiation emitted by medical equipment. Dr. Wong humorously pointed out that those with such misconceptions generally fall into two categories: those who are excessively fearful and those who are completely indifferent.

She clarified: “In reality, radiation is present in our everyday environment. The medical devices we use have undergone extensive scientific research and testing to ensure they are safe. Imaging tests are customised for each patient to minimise exposed radiation. Furthermore, the radiation exposure from imaging tests such as standard X-ray and mammogram are minimal. For instance, the radiation from a chest X-ray is equivalent to about ten days’ worth of typical background radiation from daily activities.”

Dr. Wong said that CT scans generally involve higher levels of radiation, typically 10 to 100 times higher than those from standard chest X-rays. The decision to opt for CT scans depends on the patient’s particular circumstances, his symptoms, and diagnostic requirements. The ultimate goal is to obtain accurate diagnoses to formulate an appropriate treatment plan.

Dr. Wong noted that some patients, wrongly believing that some tests are able reveal all their health issues, may specifically ask for tests such as CT scans. In reality, all tests should be conducted only after consultations with doctors and considered together with other tests, such as blood investigation. The choice of tests should always be tailored to the

patient’s condition. If scan is ordered, they typically target only specific areas of concern so that there is no unnecessary radiation exposure. For instance, for a patient who suffered head injuries after a car accident, doctors may only order a CT scan of the brain area rather than whole body to assess for possible bleeding.

Children and pregnant women are particularly sensitive to radiation. So doctors will choose a nonradiation-based examination for them whenever possible, such as ultrasound and magnetic resonance imaging (MRI) scans. Healthcare providers will typically confirm the pregnancy status of women of childbearing age before proceeding with any radiation-based tests.

Dr. Wong decided to join KLSCH because the hospital’s core values resonated with hers – that the hospital is dedicated to philanthropy and the needs of the underprivileged. 吴明音护士长出生在经济条件一般的家庭,她 在家中八个兄弟姐妹中排行第七。本来想报读设计 的她,考虑到姐姐要上大学,弟弟还在读中学,家 里的经济情况无法负担她的学费,只好放弃报读设 计。迷茫之际,她看到了护理专业的招生广告。

“我告诉自己,努力学习是改变未来的唯一途 径,无论如何我都必须继续学习。”

她坦言学习护理的初衷是为了更好地照顾妈 妈。那时妈妈生病了,家里没有人有医学背景,也 不清楚妈妈究竟患了什么病。她认为选择护理专业 就能更深入地了解妈妈的病情,于是毅然决然选择 了这条路。

未接触护理专业之前,她对护士工作的初步印 象主要是进行基本的照顾工作,比如喂饭、擦身 等。然而,当她真正接触护理专业后,才深刻明白 护士需要具备丰富的专业知识和技能,以应对各种 医疗场景和患者需求。

她强调,护理专业的前景实际上非常广泛。学 习护理并不局限于在医院从事基本护理工作,还有 许多其他的职业选择。例如,在新加坡医院,营销 部门需要护士来协助进行媒体营销工作,为护士提 供另一种发展机会。此外,若是经济条件允许,还 可以考虑开设疗养院或培训中心等其他职业路径, 这些都是护理专业毕业生可以探索的新领域,让他 们在职业发展上有更广阔的空间。

在急诊部门的八年

每个从南华护士学院刚毕业的护士无权选择自 己向往的部门,吴护士长也不例外,毕业后直接被 派到急诊部门。

Sister Goh Beng Imm’s journey in professional nursing began in 2005 when she graduated with a Diploma in Nursing from the Lam Wah Ee Nursing College. She then opted to further her studies with a Post Basic Nursing Course. For eight years, she dedicated herself to the Accident & Emergency (A&E) department at Lam Wah Ee Hospital, which, at the time, was the hospital with the most number of beds in Penang. In 2022, Sr. Goh began a new chapter in her career when she joined Kek Lok Si Charitable Hospital (KLSCH) as Head of Nursing in the Training Department.

Nursing: A Life-changing Choice

Sr. Goh’s journey in nursing is an unusual one. Growing up in an economically disadvantaged family, she is the seventh of eight children. Her initial ambition was to be a designer. However, with her elder sister in university and her younger brother still in high school at the time, she was forced to set aside her aspiration as her family didn’t have the financial resources. While contemplating her uncertain future, a serendipitous encounter with a nursing recruitment advertisement sparked her interest. Recognising an opportunity to reshape her future, Sr. Goh saw nursing as a viable alternative.

A moment of revelation occurred when her mother fell ill and she realised no one in her family had the medical knowledge to comprehend her condition. Sr. Goh recognised the potential of a nursing career – that with knowledge, not only is she able to understand her mother’s illness but also care for her more effectively. This was enough motivation for her to want to become a nurse.

一年的时间里,她在急诊部门学到了很多,也 在不知不觉中成长。直到有一天,当时的护士长告 诉她,明显看到了她的进步和改变,这番话让她决 心留在急诊部门。就这样吴护士长在急诊部门一待 就待了八年,虽然每天在快节奏、紧张氛围下工 作,但每个人员彼此互助合作的团队精神让她深受 感动,也是她爱上这里并坚持留下来的原因。

“就算现在我在培训部门工作,我还是会和刚毕 业的护士说:不要抵触你们目前的部门,能的话在 这个环境里找到自己喜欢的事。”

Reflecting on her journey, Sr. Goh highlighted that nowadays, beyond traditional caregiving roles, there are many career opportunities open to a nurse. He or she can venture into various departments, like how nurses are contributing to marketing efforts in Singapore hospitals, or even set up their own businesses like a nursing home or training centre. Sr. Goh said nursing careers can be very multifaceted, and encouraged aspiring nurses to consider the many possibilities.

Dedicating Eight Years to Emergency Nursing

Sr. Goh recalled that, like her peers, when she graduated from Lam Wah Ee Nursing College, she did not get to choose the department she had wanted to work in. Instead, she was assigned to the fastpaced Accident & Emergency (A&E) Department. She struggled initially because the environment was demanding and she felt outpaced by her colleagues and ill-suited for the job. After a year of self-doubt, she unexpectedly saw the possibility for growth amid the challenges. Her charge nurse, who recognised her progress and encouraged her to persevere, ultimately convinced Sr. Goh to remain in A&E. She eventually settled in and went on to thrive in the dynamic and demanding realm of emergency nursing.

Remarkably, Sr. Goh remained in the A&E department for eight years. What sustained her despite the perpetual frenzy in the department’s workflow was the deep sense of camaraderie and belonging she found among her colleagues. The support and teamwork made the difference for her. “很多人认为我当护士的

吴护士长的工作不仅限于传授基础护理技能, 她还负责教授基础生命支援(Basic Life Support)和进 阶生命支援(Advanced Life Support)等急救技能。

Even now, as a staff of the Training Department at KLSCH, Sr. Goh strives to impart her hard-earned wisdom to newly graduated nurses. She encourages them to embrace their assignments and seek fulfilment in the value of their work. She likes to use her own experience to illustrate the importance of resilience, adaptability, and camaraderie amid the rigors of a fast-paced healthcare environment.

Exploring Overseas Horizons

Earlier in her career, despite having contributed significantly at Lam Wah Ee Hospital, Sr. Goh found herself longing for a broader experience and began looking overseas. She found an opportunity in Saudi Arabia and signed on. There, in the vibrant cultural landscape of Saudi Arabia, Sr. Goh not only interacted with individuals from diverse backgrounds but also familiarised herself with many advanced medical technologies. Her original intent was to work in Saudi Arabia for several years but family obligations prompted her to return to Malaysia after only one year.

At KLSCH, the training and observation phase for nurses typically spans three to six months, During this time, Sr. Goh would stress the importance of not only acquiring foundational knowledge but also cultivating adaptability and resilience. This means nurses need to be effective communicators too as they gain confidence and experience, so that their contribution can be recognised as an indispensable asset in a healthcare setting. Sr. Goh is therefore committed to nurturing well-rounded nursing professionals who are capable of providing seamless and optimal care to patients.

Championing Nursing and Community Welfare

At KLSCH, Sr. Goh has two aspirations that concern the nursing profession and the community at large. First, she wants to dispel misconceptions about nursing and envisions a future where the profession will attract more individuals for its noble calling. For this to happen, she believes a better system is needed to train more nurses, so that there is steady pool of nursing talent to serve a growing patient population. Second, Sr. Goh cites KLSCH’s longstanding dedication to delivering quality medical services to the community, particularly those from marginalised groups. She hopes KLSCH will continue to embody the altruism of its founding mission as it grows. 投身护理行业13年的李欣霖护士长,中学时期 就已在槟城圣约翰救伤机构服务。询及为何选择跻 身护理行业时,她表示中学毕业时,母亲提及在医 院工作的风险相对较低:“母亲觉得待在医院的话比 较有保障,鼓励我报读护理专业。毕业以后,她也 相信我能够胜任护士这份工作。”

对未来感到困惑的她听从母亲的建议,考取护 理文凭并开始在槟榔医院工作。积累了足够的临床 经验后,为了提升专业知识水平,她决定接受专科 护理(Post Basic)培训,并成功获得临床护理专科 证书。母亲的支持和信任激励着她不断提升在这个 领域的专业能力,坚定地走上了护理事业的道路。

此外,她在圣约翰救护机构担任救护车队员的经验 也为她日后从事护理工作打下稳定基础。在这段时 间里,她有机会深入接触医疗领域,并且通过协助 急需救治的病人获得了极大的满足感,促使她对医 疗保健和照顾他人产生浓厚的兴趣。

从护理系毕业到槟榔医院任职后,李护士长被 分派到急诊部门工作,之后也在好几个不同部门任 职。由于本身性格与急诊部更为契合,来到极乐寺 慈善医院(KLSCH)后,她选择留在急诊部门,成为 该院第一位驾驶救护车的女护士。

KLSCH备有两辆救护车,每辆救护车配备一名 司机和两名救护人员。司机需持有手动档驾驶执 照,并具备基本急救知识,包括心肺复苏等技能。

With 13 years of professional nursing experience, Sister Lee Shin Ling’s journey began with joining the Penang St. John Ambulance during high school. Upon graduation, she heeded her mother’s advice about the relatively secured option of a career in healthcare and pursued a nursing diploma. She then spent several years serving in the emergency department of Island Hospital Penang. After gaining enough clinical experience, she decided to further her study with Post Basic Certificate in nursing. When that was completed, Sr. Lee returned to Island Hospital Penang and served as a nurse in the operating theatre for three years. To broaden her horizons, she then moved to Georgetown Specialist Hospital, before returning to Island Hospital Penang again. In June 2022, she joined Kek Lok Si Charitable Hospital (KLSCH).

When asked about what inspired her to choose nursing as a career, Sr. Lee cited her time with St. John Ambulance when she first assisted critically ill patients. The experience deepened her interest in healthcare and caregiving. Her mother, too, played a role. Her encouragement and faith motivated Sr. Lee to pursue nursing professionally and to continuously upskill herself.

KLSCH has two ambulances, each staffed with a driver and two medical attendants. The driver is required to hold a manual transmission driver’s license and possess basic first aid skills, including CPR. During an emergency, the team works closely to ensure prompt and effective medical care during patient transit. The driver focuses on safe ambulance operation and timely arrival, while the medical attendants deliver onboard emergency aid to ensure patient safety and comfort.

Recognising that Ayer Itam has a large number of patients who require medical attention, KLSCH has initiated a volunteer patient transport service on behalf of Penang General Hospital. This service was launched in September 2022 and is now available from Monday to Friday, 8:00 am to 6:00 pm.

Sr. Lee’s typical day begins at 8:00 am with an inspection of the ambulance’s equipment. The inspection includes checking the automatic external defibrillator (AED), wheelchair, stretcher, spinal board, suction machine, aspirator, and the first aid kit with its stock of medications. Once she confirms that everything is in good order, she will be on standby for a distress call to come in. When that happens, she will verify the need for ambulance service and other details such as patient address before promptly setting off on her assignment.

Adapting to Adversity

Ambulances are indispensable in an emergency response. In Malaysia, most ambulances operate with manual transmission, which can pose additional challenges for drivers. Besides having to navigate traffic and road conditions, drivers must also ensure the safety of both patients and staff throughout the journey. Therefore, they must maintain constant vigilance and the ability to make quick, accurate decisions.

Sometimes, Sr. Lee receives simultaneous ambulance requests from both KLSCH and Penang General Hospital. When that happens, she will have to be extra thorough in prioritising dispatch, taking into account the availability of ambulances and urgency of a patient’s condition.

Despite her experience, Sr. Lee admitted that she still faces challenges in emergency medical services and pre-hospital care. While there are more female ambulance drivers nowadays, they are often not as efficient as their male counterparts when it comes to more physically demanding tasks, such as lifting stretchers.

Emergency teams also need to be prepared for unforeseen circumstances, such as when it is not possible to quickly assess a patient’s condition, which will in turn delay the preparation of medication for treatment. In such scenarios, Sr. Lee will have to rely on experience and critical thinking, and make do with whatever resources available and assistance from others.

Prompt decisions are crucial in an emergency, whether it’s providing on-site medical care or transporting the patient to a medical facility. Decision-making during an emergency is a complex process, influenced by factors such as the patient’s

身为一名专业的救护人员,李护士长的首要责任是 确保患者安全送到医院,为顾及当前患者的健康, 她们一般不被允许转运额外的病患。然而当时的情 况异常严峻,一场意外导致交通严重拥堵,延误了 另一辆救护车的到达时间。如果错过了治疗的黄金 时间,摩托车骑士的状况随时可能会恶化。

Acts of Kindness Amid Chaos

Emergency department staff categorise patients based on the urgency of their medical needs. After assessing their vital signs, symptoms, and the nature of their injuries or illnesses, patients are categorised into three zones: red zone for critical conditions requiring immediate resuscitation; yellow zone for high risk or semi-critical patients; and green zone for patients with less severe conditions. This is so that the emergency team can prepare and respond accordingly.

Sr. Lee recalled an incident where an unforeseen event required her to act decisively. Once, while transporting a patient to Penang General Hospital, she encountered a traffic accident involving a car and a motorcycle. The female motorcyclist was critically injured and was lying unconscious on the ground. As a professional paramedic, her primary responsibility is to ensure the safe transportation of a patient to hospital. She is not authorised to transport additional patients as that may heighten the risk for the first patient. However, the situation at the scene of the accident was exceptionally dire. The accident had caused severe traffic congestion, delaying the arrival of another ambulance. Any delay in treatment for the unconscious motorcyclist could be a matter of life and death. After careful consideration, Sr. Lee approached her patient and asked if she would consent to accommodate the injured motorcyclist in their ambulance. Much to her surprise, the patient agreed without hesitation. Sr. Lee was touched by the selflessness of her original patient, who, despite

It made her realise that while they may not be able to save every patient’s life, healthcare providers can still provide crucial support and comfort, ensuring that the patient feel cared for and understood during their most trying moments. The episode underscored how important compassion and collaboration are when navigating the many challenges within the healthcare profession.

Compassionate Care and Personal Growth

Sr. Lee, who is approaching her second anniversary at KLSCH, believes that as a charity hospital, the hospital stands out for its commitment to providing compassionate medical care regardless of the patient’s financial circumstances. This emphasis on charity and empathy has helped foster an atmosphere of dedication and selflessness among staff, enabling the entire medical team to fully devote themselves to the care of patients. The hospital radiates compassion, which is a tremendous support for both patients and healthcare professionals.

KLSCH also provides its staff with opportunities for personal growth. Sr. Lee is not confined to specific roles; she has the flexibility to apply her knowledge and take on various responsibilities. She is now a nurse as well as an ambulance driver. Her expanded

As the interview drew to a close, Sr. Lee, who maintained a gentle smile throughout, shared her aspirations for the future. She reaffirmed her commitment to the core values of KLSCH –compassion, commitment, and care, vowing to continuously improve her skills and knowledge and work closely with her colleagues to raise the quality of nursing care at the hospital. With a strong, cohesive team, she hoped KLSCH will continue to offer a supportive environment for both patients and staff.

Due to lack of resources and manpower, some medical services at KLSCH have yet to be fully implemented. Sr. Lee hoped that as the hospital expands, it can also change the public’s perception of a hospital by offering more opportunities for professional and personal growth for its medical team.

来自浮罗山背的83岁蔡沐汉,是慢性阻塞性肺 疾病 (COPD) 病患。早年他长期吸烟的习惯引发肺部 疾病,在蔡老先生戒掉烟瘾及进行治疗后,他的健康 状况有了显著的改善。不过,2年前蔡老先生意外失 足,伤及腰部与脊椎骨,也因此诱发肺病复发。

“我们考虑到父亲当时的病情危急,而且他年事 已高,若选择附近的浮罗政府医院就医,等待时间太 长,必须再转入中央医院作进一步检查。” 后来在姑 姑的推荐下,他们决定将父亲送至极乐寺慈善医院。

“当时送到极乐寺慈善医院,医生告知父亲情况 不乐观,加上伤及脊椎骨,需要我们有心理准备。”

蔡老先生在慈善医院前后治疗差不多2-3个星期,曾 因呼吸困难被送进重症监护室,后来在院方适当的治 疗与细心照料下,蔡老先生的情况渐好,也从重症监 护室回到普通病房,不过需要长期佩戴鼻导管持续接 受氧疗。

出院之后,蔡老先生因为呕吐和无法进食而第二 次入院,在梁家强医生的建议下为他导入鼻胃管协助 进食,喂以牛奶、营养补充剂和药,以此摄取营养。

目前父亲的病情已经稳定下来,虽然因为病情只能长 期卧床,但他的精神状态保持良好,蔡先生言语中满 是对医生成功救治父亲的感激。

83-year-old Mr. Chuah Bak Hun resides in Balik Pulau, Penang. Several years ago, he had struggled with Chronic Obstructive Pulmonary Disease (COPD) as a result of his lifelong habit of smoking. However, through a series of advice and treatments, along with his own initiative to stop smoking, his health improved remarkably with a notable reduction in COPD exacerbation. Two years ago, he accidentally slipped and fell, injuring his waist and spine. The pain from the fall triggered his COPD attack.

“Considering my father’s critical condition and advanced age at the time, we realised that seeking treatment at the nearby government hospital would result in a prolonged waiting period. Additionally, my father would need to be transferred to Penang General Hospital for further examinations.” Later, following his aunt’s recommendation, Poh Hin brought his father to the Kek Lok Si Charitable Hospital.

Upon arriving at KLSCH, the doctors promptly briefed Poh Hin on the seriousness of his father’s lung disease and spinal injury, and advised him to be mentally prepared. “My father received treatment

尽管医院位于亚依淡,距离他们浮罗山背的家 比较远,蔡先生仍每个月前去医院为父亲取药。他表 示,不只是极乐寺慈善医院的药物价钱比较低,并且 可以定期咨询医生,随时更新父亲的病况。

蔡先生赞赏极乐寺慈善医院以患者为中心的医 疗理念,院长拿督丁福南也曾远赴他们家探访他的父 亲,让他倍感感动。他表示极乐寺慈善医院的效率、 富有同情心的工作人员和对患者的用心,与他在其他 医院的经历形成鲜明对比。蔡先生希望极乐寺慈善医 院有一天能扩展到西南区,让那里的患者也能享受此 等优质服务。

Fortunately, after several days of treatment, his condition improved, and he was transferred to the general ward,” recounted Poh Hin. To alleviate his COPD symptoms, he now had to rely on continuous oxygen therapy via a nasal cannula.

After being discharged, Mr. Chuah was readmitted to the hospital due to vomiting and unable to eat. On Dr. Neoh Kar Keong’s advice, a nasogastric tube was inserted to assist with feeding. To maintain basic nutrition and sustain life, he was fed milk, nutritional supplements, and medications. His condition has now stabilised. Despite Mr. Chuah’s immobility and confinement to bed due to his illness, his spirit remains high.

Poh Hin expressed heartfelt gratitude towards the doctors at KLSCH for their exceptional dedication in treating and caring for his father. “They spared no effort, and I truly appreciate their help,” he said.

After discharge, we rented it at a monthly rate of RM 350, with an additional charge of RM 200 every six months for service and maintenance. Over time, these expenses accumulated substantially.” Later, the hospital suggested that they seek assistance from other charitable organisations to acquire an oxygen concentrator to alleviate the financial strain.

Despite the hospital’s location in Ayer Itam, which is significantly far from their residence in Balik Pulau, Poh Hin diligently visits the hospital each month to collect his father’s medication. He valued the competitive pricing KLSCH offered and the opportunity to consult with the doctors regularly. Even after his father’s discharge, Poh Hin maintained regular communication with the doctor. 走访混沌暗影

从北海医院调职往山打根,是一段由海至海的 冒险旅程。虽然同在马来西亚,山打根草根面对的 问题,因为经济架构、种族比例、历史渊源、地理 位置等条件,自然与槟城州有所差异。身边亲友鼓 励我在异地发掘新的写作素材,但我常有所保留, 深怕一不小心就以救世主情怀或是网红猎奇的角度 去拼凑肤浅的异地印象。

然而东渡之后,这里的一切却也正如小时候常 玩的虚拟战争游戏,随着经验值提高,我进一步解 锁地图中的另一块暗影。暗影之下,是我未曾领略 的景色与人文。二十八岁的人生,第一次踏足莽莽 苍苍的婆罗洲,我难以摆脱雀跃兴奋的游客心境。

即使在山打根工作了一个月,我也只是在医院、出 租屋和几个食肆间往来,去过最远的不过是离市中 心七哩以外的地区。这块土地,毕竟给我留下太多 未知与想象。

家。很多时候,我们都在屋子前院执行任务,甚至 没有踏足屋内半步。

所以,我十分期待家访任务,借工作之名,行 发现山打根之实。家访服务,是精神科专为那些行 动不便或是没有交通工具的病人所提供的援助。它 不是常人所认知的下乡任务,也不是到老人院、孤 儿院义务性的社工服务。家访当天,八点抵达医 院,翻阅病历简单理解病情之后,一个医生随同两 个护士便会轻装上路。行程匆匆,每到一户人家, 简单关心近况,注射抗精神病剂后,就离开前往下

适于被家访的病人都经过严格筛选,毕竟资源 有限,原本一个医生搭配两个护士的组合,如果在 诊所,可以看至少三十个病人;而家访服务,我们 只能走访至多十个患者。大部分的时间,都耗在沙 巴州坑坑洼洼,满是补丁的沥青路上。有些房子位 于棕油园深处,需要通过一段黄沙漫漫的土路。

驶在其中,司机嘀咕一句“感谢上苍“,幸好昨天无 雨,否则轮胎轻易下陷,无法前行。随行的护士 语带轻蔑地问我这位西马来的“公子哥”是否已经晕 车,我当即否认,说起18岁时也曾随舅舅往胶林深 处送电脑的往事。

医务人员走进一扇又一扇门之后的暗影,抵达 家庭悲剧的现场,感受真实冲击,借以更加全面理 解有些病人发病,久久无法痊愈的根因。病人A有 一张被岁月揉皱的脸,原以为她已经够老了,未料 一个弓着背,至少八十几岁的老奶奶从厨房蹒跚走 来,热情迎迓却口齿不清。木屋和主人一样斑驳破 旧,垂垂老矣,卫生条件甚差,好似崩塌在即。探 访比较“正常”,老少齐聚的家庭时,当护士把病患 牵出,我从那些年纪尚小的孩子眼中,看见一闪而 过的恐惧,但恐惧很快转换为淡然。

心 专 栏

精神疾病是可控的,只要遵守医嘱,很多病人 都过着和常人无异的作息。只是被家访的患者,大 都比较严重,目光呆滞,行动迟缓。一个月注射一 次的抗精神病剂的确能抑制患者心中的猛兽,防止 一个家庭陷入更深的泥淖。我的前上司说过,稳健 的家访制度,可以减低一个地区精神病患的入院 率,因为家访团队可定期上门为拒绝食药的病患注 射抗精神病剂,并在疾病复发前及时发现苗头,适 时介入。

在北海医院,也曾遇见几个无家可归的流浪汉 因为短暂的思觉失调而入院。出院前,他们主动要 求院方把他们送回他们“常驻“的街头或庙口。精神 专科的出院方案往往较具弹性,和领导家访任务的 专科医生讨论之后,我们允许病患回到他所熟悉的 街头,再由家访团队每个月前去跟进病情。在我看 来,那位医生的脑袋应有自建雷达,对整个北海地 区的流浪汉的寄居位置了如指掌。

类似的情况在沙巴州同样常见。经验丰富的 司机和护士不开GPS,以“水果档旁边”、“木瓜树 下”、“一座桥之后转弯”等笼统说法,就找到病人 所在。没有固定地址,就像那些病人没有国籍,没 有身份证,没有名字的人生。于是家访任务成为探 入暗影的唯一途径。北海的家访任务关怀的是城市 夹缝中的阴影,而在山打根,零星错落不同地区的 暗块,则加剧了任务难度。家访任务并不是下乡服 务,我们关心的病者犹仍在山打根市区医院的管辖 范围,而那些被发展洪流排除在外,身处内陆地区 的病人,我对他们的命运一无所知。

一路摇摇晃晃抵达14哩的地区,我开拓了更远 的旅人视野。U转回归市中心,14哩之后,延伸到 丛林深处的故事,都只能交由想象力填充。眼不见 未必为净。一棵树在丛林倒下而无人听见,我们就 能当作它没有发出巨响?

养生先养心,医病先医心

首先,文化有三层次:“器物”、“制度”、“精神 理念”,三者形成人类文明或文化有形和无形、硬体 与软体活动的总和。有形可见的“器物”如马车,及 与法令、礼俗有关的“制度”如三年之丧,会随时空 条件不同而调整或淘汰。“精神理念”如孝顺父母, 与人心人性有关,不会随时空改变或消失。所以, 任何高明智慧,一定重视精神理念的解说与人心人 性的处理。

其次,人有自我三层次:“身”、“心”、“灵”。“ 身”是身体,与之相关的是生命、存活、发展、五官 四肢、言行与疾病等;“心”是学习与认知的器官, 与之相关的是学习、理解、判断、选择、遵礼守法 和有情有义等;“灵”是人类所独有的精神或灵魂, 与能否了解人生意义、有无尊严和悲悯之心、活得 快乐与否等有关。人若只有“身”的层次,则与其他 动物没有差别。

所以,修身先修心——修养言行,先修养自己 这颗心的起心动念;养生在养心——保养身体的 健康与生命的安全,从保养心灵开始;医病先医 心——想要预防或医好疾病,先治疗自己这颗心。 我们因而可以说:人生最重要的,就是注意自己这 颗心;因为人生最麻烦的,就是自己这颗心。

约成书于西汉的《黄帝内经》,后世尊为“医 家之宗”,早已指出“百病生于气也,怒则气上,喜 则气缓,悲则气消,恐则气下,寒则气收,炅则气 泄,惊则气乱,劳则气耗,思则气结。”(〈素问· 举痛论篇第三十九〉)又说“怒伤肝”、“喜伤心”、“ 思伤脾”、“忧伤肺”、“恐伤肾”。(〈素问·阴阳应象 大论第五〉) “心者,五脏六腑之主也。…故悲哀 愁忧则心动,心动则五脏六腑皆摇。”(〈灵枢·口 问第二十八〉)。这类说法不胜枚举。

第三,心有三种作用或功能:“知”、“情”、“意” 。“知”是透过学习以认知,进而做出选择;“情”是 情感爱恨情仇与情绪喜怒哀乐等;“意”是想望、意 图或欲念。三者中“知”是关键,会影响“情”和“意” 。即“知”一旦出问题,“情”和“意”跟着出问题,言 行也出现差错,后果不堪设想。这是“知”→“情”→“ 意”→“言行”→“后果”的因果推断。

中医的两大重点,一是未雨绸缪,预防胜 于治疗;一是身心一体,生理器官是有机体 (Organism),会相互影响。心、肝、脾、肺、 肾“五脏藏神”(〈素问·宣明五气篇第二十三〉)和 喜、怒、忧、思、悲、恐、惊“七情致病”是其理论 核心,说明情绪变化会引起生理功能紊乱而致病, 并将“心”视为人体心理和生理活动的总指挥。“故 智者之养生也,必顺四时而适寒暑,和喜怒而安居 处,节阴阳而调刚柔,如是则僻邪不至,长生久

视。” (〈灵枢·本神第八〉)因此,中医历来重视 心理因素对人体疾病的影响。

西方则认为万物包括人体是由不同复杂部 分组成,且彼此之间没有内在联系,这是机械 论(Mechanism),因此西医头痛医头,脚痛 医脚。但约近百年前,西医发展出“身心医学” (Psychosomatic medicine,也称心理生理医学) ,将身心当成一体或相关,因而有了“心理应激源” (Psychological stressor)——应激(Stress)是指 个体身心感受到威胁时的紧张状态,如压力与焦 虑;以及“认知行为疗法” (Cognitive behavioural therapy)和“理性情绪行为疗法”(Rational emotive behavioural therapy)等理论。

剖人性,直指本心。儒家重视修养,故特别强调真 诚的重要,真诚是修养的起点;道家重视智慧,故 特别强调自觉的重要,自觉是智慧的开端。

西医也发现越来越多的疾病,不能单纯从生理 学角度去研究和治疗,除非把内在的心理因素和外 在的社会因素也考虑进去。研究指出,由心理因素 导致的身体疾病,是造成现代人死亡率升高的重要 原因。而病人情绪与人格特征的不同,有明显的病 征差别,如性格冲动、急躁者易患心脏病;内向、 消极者易患哮喘;焦虑、被动者易患胃溃疡;固 执、怨天尤人者易患偏头痛;过于压抑、自我克制 者易患癌症。

中华文化的儒、道二家哲学是高明智慧,皆解

儒家谈“心”,把焦点放在心之真诚,从人际真 诚交往互动,以达至和谐关系的角度来谈。道家谈“ 心”,聚焦自觉,从察觉自己的学习与认知,以达至 自身安全与快乐的角度来谈。

因此,儒家谈“心”,关键在“情”(真诚);道 家谈“心”,关键在“知”(自觉)。儒家从真诚出 发,希望抵达人间关系的圆满;道家从自觉出发, 希望解脱人为价值观的困苦。两者的最终目标,皆 是天下太平。

身体保健从心灵保健开始,儒、道哲学提供人 格的形塑和心理的调节,在一定意义上成为防治心 理疾病的良药。在孔孟老庄思想的引导下,我们对 挫折与逆境,有着更强的承受力和化解力。因此, 本专栏旨在从儒、道二家角度,提出如何检视我们 这颗心及改善方法。

病从心起,以心治身。

极乐寺慈善医院经过两年多的发展,院内的空 间已无法满足病患的需求。因此,医院决定增设新 的空间,用于建设洗肾中心和心脏病科楼。该扩建 计划于2023年12月24日举行了动土仪式,由极乐 寺基金会主席、极乐寺慈善医院管理理事会主席释 日恒长老、槟城首席部长曹观友等人主持。这项扩 建工程预计将耗时一年完成,地点则位于现有医院 的范围内。

极乐寺慈善医院院长拿督丁福南医生表示,新 楼的扩建将用于发展洗肾中心,增设更多的洗肾设 备,以满足更多肾友的需求。此外,新楼还将设有 心脏病科的医疗设备,包括心脏导管实验室(Cath Lab),以便进行各种复杂的心脏手术。院方还计划 增设磁振造影扫描仪(MRI),以及引进双能X线吸收 测定法(DEXA)用于检测骨骼密度。所有建设和维持

设备的经费,包括扩建医院的费用,都将由极乐寺 骨灰塔的累计盈利来支持。

极乐寺慈善医院一直秉持着“慈爱、承诺与关 怀”的核心价值,并将为大众提供可负担且高素质 的专科医疗服务作为使命。极乐寺慈善医院管理理 事会主席释日恒长老表示,他们有意筹备设立另一 所更大的医院,但在此之前,院方必须先满足病人 的迫切需求。待目前的情况稳定后,才能考虑建立 更大更完善的医院,以服务更多的病人。曹观友也 表示,极乐寺慈善医院自开业以来一直赢得了良好 的口碑。许多来自威省、吉大南部和霹雳北部的病 人都选择前往槟岛的极乐寺慈善医院就医。如今慈 善医院即将迈入第三个年头,希望院方依旧不分种 族、宗教和信仰,为大众提供高素质的医疗服务, 继续维持合理的医药费用,造福更广泛的人群。

随着国内老龄人口比例的提升,迈入第三个运 营年的极乐寺慈善医院也必须应对多重疾病和治疗 的复杂性。为了更好地服务社区以及满足病人在不 同阶段的各种医疗需求,我们正在进行扩建工程, 并积极提升更全面的专科医疗服务,其中包括缓解 医疗专科服务。

重疾末期的病人提供综合性的医疗照护,减缓他们 的痛苦,同时协助病人及家属在疾病期间及临终前 身心上的调适。

《医愿 Med Talk》缘起

大众对医院普遍有着刻板印象:认为它们只是 治病平台。其实,健康教育是医院的关键环节, 它不仅与大众交流病情护理方法,还介绍医院可以 治疗的疾病及其治疗方案。目前许多团体、医生及 非政府组织(NGO)也经常举办医学讲座活动。

极乐寺慈善医院以慈善宗旨出发,推广医疗和社 会服务,将其与慈善事业结合,取名《医聊 Med Talk》,因“聊”代表聊天,旨在避免将医学讲解得

第一场座谈会于2月22日,傍晚7点半举办,主题为《关于 医疗与慈善那些事》。此次座谈会主讲人是极乐寺慈善医 院的首席执行长涂仲仪医生和菩提馨园院长陈瑞万先生。活动开场前,涂医生幽默表示今晚 绝不会介绍医药产品,更不会要求参与者签署任何配套,他旨在分享极乐寺医院的创办理念 和目标。在正式进入主题前,他分享了几个个案,指出一些病患的病情已经非常严重,但政

府医院求医者人满为患问题严峻,必须等待几个月才能进行手术。涂医生表示,政府医院并 非不愿意帮助,而是因为病患众多,导致等待时间过长,有些人根本等不起。

涂医生强调,慈善医院并不意味着提供廉价或免费医疗,而是尽量将医疗费用降到最 低,以确保来自B40和M40阶层的病患都能及时得到治疗。为了让大家更清楚理解这一概念, 涂医生举例说明了医疗费用的构成,其中包括住院空间、医疗设备、专业费用、消耗品和药 品、设备维护费用以及公用事业费用。

慈善医院该如何持续?

极乐寺慈善医院向来采取直接购买医疗设备的方式,而非分期付款,这样就可以避免每 月支付设备租赁费用的情况。在医疗用品方面,他们致力于保障品质同时寻求降低成本,以 确保有足够的经费来承担医疗设备维修费用。此外,涂医生亦透露极乐寺骨灰塔的所得盈利 会用来支援慈善医院的病患。

在提问环节中,有位听众表明自己是肾友,自己和同样患有肾病的父亲皆无法负担昂贵 的医疗费用,希望寻求涂医生的帮助。涂医生耐心听完后立即表示会考虑收留这位病患,并 邀请他在座谈会结束后留下来继续深入交谈。

座谈会: 医 院 新 事

过于生硬,巧妙将医学知识融入日常对话,使活动 更具亲和力和大众化。

《医聊》是一个围绕医疗、慈善及社会关怀的 讲座平台。它不仅传授医学知识,更注重促进社 会责任感和慈善事业,鼓励听众思考并参与社会互 动。院方希望通过这个活动开启一扇与大众共同思 考的窗口,连接社会、人际关系和慈善医疗。《医

聊》每两个月举办一次,每次主题不同,由年轻团 体组织,逐步推广,希望吸引并帮助更多人。每一 场活动都精心筹备,主办方不懈努力使活动规模扩 大,期望更多人参与。

《医聊》活动邀请的专家(医生)不仅在活动 上提供优质和专业见解,也会分享医院及社会的见 闻和感悟,关注并高龄人士问题,为听众提供温暖 和关怀。整体而言,《医聊》活动丰富多彩,涵盖 医学知识、社会问题和人文关怀,为参与者带来启 发和感动。

关于医疗与慈善

第二环节原本由陈瑞万先生分享,遗憾的是他当天临时身体不适,不得不紧急前往医 院,无法与大家见面,因此改由极乐寺慈善医院院长拿督丁福南医生上场,与大家分享医 院的创办历程和这些年经营医院的挑战。他提及,创办医院的最初想法是由极乐寺的方丈提 出,但此事最大的挑战是医疗人员的短缺。在医院经营三年后,如今医院的医疗团队已逐渐 稳定,这是令他感到最欣慰的事情之一。丁院长强调:“我们必须传承这个传统,一旦医院开 业,就必须持续运营下去。慈善医院一定能够像其他医院一样持续发展。这个过程不仅仅是 一个团体的责任,也需要大家的帮助。院方需要做的是努力付出,让大家相信我们的医院。”

这场座谈会是极乐寺慈善医院主办《医聊Med Talk》系列讲座的第一场,获得了热烈的 反响,共有45人报名参与,大家踊跃的参与也为主办方注入一支强心针。主持人即场宣布, 这场座谈会所筹得的450令吉报名费将全数捐赠给极乐寺医院,用于帮助更多有需要的人。
