Student Cardholder Agreement

Please carefully read the Student Cardholder Agreement. It constitutes a legal obligation between you and Concordia University.

Overview

As you read this Student Cardholder Agreement (the “Agreement”), remember that the words “you” and “your” refer to the person named on the Concordia ID card. All cardholders are bound by the terms of this Agreement. For greater clarity, by requesting a Concordia ID card at Concordia University (“Concordia” “we”, “our”), you agree to and accept all of the Agreement, as applicable.The Agreement is subject to change by Concordia from time to time and without notice. Future changes will become effective and will apply to all Concordia ID cards in circulation at the time the amended Agreement is posted on the Card Services website and will supersede the previous Agreement in effect at the time you acquired your Concordia ID card. Be sure to review the Agreement regularly 1. About the Concordia ID Card The Concordia ID card is your official identification piece at Concordia and is valid for 4 years. It is managed by Card Services and allows students to access facilities and services on Concordia premises, as well as certain off-campus student pricing. The use of the Concordia ID card is governed by the present Agreement and Concordia’s policies and procedures, as they may exist from time to time. 2. Use and Ownership of the Concordia ID Card Your Concordia ID card remains the property of Concordia and must be presented to campus authorities upon request.

Your Concordia ID card is not transferable, and you are responsible for any/all use of the card, including but not limited to library items borrowed and charges applied.

Using another person’s Concordia ID card, or altering, falsifying, selling or tampering with a Concordia ID card, is prohibited and subject to sanctions, pursuant to Concordia’s policies and procedures, and may also be subject to civil or criminal proceedings.

You may not use your Concordia ID card to commit or facilitate any illegal, improper, or unlawful purposes or acts or otherwise in a manner that contravenes this Agreement.

3. Obtaining a Concordia ID Card There are two ways to process a request for a Concordia ID card. Depending on your choice, one of the two subsections below shall apply. i) Image Upload Terms and Conditions (if applicable) General Provisions The Concordia ID card photo upload tool (the “Portal”) allows you to upload a chosen photo of yourself to be used for your Concordia ID card. For further information on what constitutes an acceptable photo, please refer to Concordia.ca/card and the admissible photo guidelines. In addition to your chosen photo, you will need to upload a digital copy of your government-issued photo ID, which will be used by Card Services to verify your identity and to validate the chosen photo submitted to produce your Concordia ID card.

By uploading your photo ID through the Portal, you are agreeing to the present section 3 i) (Image Upload Terms and Conditions).

Cannot be used to renew or replace a Concordia ID card Temporary Storage on the Cloud Once uploaded through the Portal, your chosen photo and government issued photo ID are securely stored in an encrypted format on a third-party cloud-based storage until the verification step for your Concordia ID card is completed by Card Services. The duration of the verification process may vary, however, once approved, your chosen photo and government-issued photo ID are automatically destroyed and no copy is retained in the third-party cloud that was used to produce your Concordia ID card.

Temporary Storage on the Cloud Once uploaded through the Portal, your chosen photo and government issued photo ID are securely stored in an encrypted format on a third-party cloud-based storage until the verification step for your Concordia ID card is completed by Card Services. The duration of the verification process may vary, however, once approved, your chosen photo and government-issued photo ID are automatically destroyed and no copy is retained in the third-party cloud that was used to produce your Concordia ID card.

Accepted Government Issued Photo ID Accepted government issued photo ID include a valid passport, driver’s license, provincial/state ID card with photo, provincial health card with photo, Canadian asylum seeker document, or permanent resident card. Note identification cards issued by high schools, post-secondary institutions, or hospitals are not considered government issued.

ii) Visit the Card Services Office (if applicable) You may visit the Card Services office to request your Concordia ID card and have your picture taken in person.

4. Protection of Personal Information Concordia is committed to protecting the personal information in its custody and control. We only collect, use and store personal information that is directly related to, and needed for, operating our programs and activities. Employees, service providers, and volunteers must comply with applicable legislation and Concordia’s policies, including the Policy Concerning the Protection of Personal Information, whenever third party personal information is entrusted to Concordia’s care.

The digital photo used to produce your Concordia ID card, whether your chosen photo uploaded through the Portal or the photo taken at the Card Services office, will remain in Concordia’s CFI Awards Management System (CAMS). It will be used for the renewal/replacement of your Concordia ID card and used by the Société de transport de Montréal (STM) to produce the student reduced-fare transit pass, Opus card.

Individuals requesting a Concordia ID card acknowledge the above and consent to the collection, use and storage of their digital image used to produce the Concordia ID card and Opus card.

5. Lost or stolen Concordia ID cards You are responsible for safeguarding your Concordia ID card against loss or theft. If your Concordia ID card is lost or stolen, you must immediately deactivate it.You can do so by contacting Card Services or presenting yourself in person at its office, as well as contacting any other unit as may be required pursuant to other terms and conditions (e.g. for meal plans with Food Services). Proper notification of a lost or stolen card requires your receipt of an acknowledgement response from Card Services. A replacement fee is charged for lost or stolen Concordia ID cards.

If you find another person’s Concordia ID card, you are strongly encouraged to drop it off at the nearest Protection and Prevention desk or at the Card Services office.

6. Confiscation of Concordia ID cards and Termination Concordia may terminate this Agreement with you, confiscate your Concordia ID card and/or prohibit your access to services and to Concordia premises, without notice to you. Concordia and any service providers are not liable for any losses, damages, expenses or inconvenience that results from the termination of this Agreement, from confiscating your Concordia ID card, from the withdrawal of your access to services and/or to Concordia premises.

In addition to the above, Concordia may decide to impose additional consequences or sanctions, if appropriate. Decisions on sanctions, are made on a case-by-case basis in accordance with Concordia’s policies, procedures, guidelines, and practices. If your Concordia ID card is confiscated, you may be denied services and you may be denied access to Concordia premises (including access to student residence buildings/rooms and to the shuttle bus) until your ID card is returned to you, which shall occur at Concordia’s sole discretion.

7. Limitations on Concordia’s Liability and Indemnity Concordia is not liable to you for any loss, inconvenience to you or to others, or any damages or expenses of any kind that may result from the use, misuse, loss or destruction of the Concordia ID card, or if, for whatever reason, your Concordia ID card is not accepted for its intended use.You agree to indemnify, hold harmless and release the Concordia from any and all claims for losses, damages, injury, fees, expenses, charges or debts made by any party against Concordia arising out of the use, misuse, loss or destruction of the Concordia ID card.

8. Disclaimer Concordia does not guarantee uninterrupted or secure access to the Portal. We cannot guarantee that the Portal will be compatible with any specific hardware or software. We reserve the right to suspend use or access to the Portal at any time, for any reason.
