A Message From Liz

Dear Team,

I’m excited to share a major milestone with all of you. After 18 months of intense work, we are nearing the completion of our new Livermore site, our 2nd Street location!! This has been an enormous project, and the transformation is just incredible.

What started as a standard two-story building (without an elevator!) has been completely overhauled to become a fully functional clinic that will meet the needs of our patients and staff. We didn’t just make cosmetic changes – the entire space has been redesigned from the ground up From installing a new elevator to ensure accessibility, to gutting and rebuilding each floor to accommodate clinical services, every detail has been carefully planned and executed.

This has been a complex process, requiring collaboration across multiple teams, and I want to acknowledge the hard work and dedication that has gone into making this happen.

Last week, we had the opportunity to provide our Board of Directors with a sneak peek of the building by holding our monthly Board meeting on-site (pictured on the front page). They were thrilled by the transformation and deeply grateful for all the work that has gone into making this space welcoming, inclusive, and accessible for the community members we serve.

We are almost at the finish line, and I couldn’t be prouder of what we ’ ve achieved together. Thank you to everyone who has played a role in this transformation, and stay tuned for more updates as we prepare for the grand opening!

Axis’s Busy Weekend

On October 19th, Axis participated in two significant community events that showcased Axis’s commitment to supporting and celebrating the diverse needs of the Tri-Valley community.

Livermore Pride: This annual event honors the LGBTQIA+ community, promoting inclusivity, acceptance, and love. Axis set up a booth to provide information about their healthcare services, including mental health support, primary care, and preventive screenings

Veterans Stand Down and Unhoused Resources Event: Hosted by Alameda County Supervisor David Haubert, this event is crucial for providing essential services to veterans and individuals experiencing homelessness, offering a range of resources. Axis’s team worked along side other local resources to ensure that attendees received the support they needed.

Congressman Eric Swalwell Visits Axis

Congressman Eric Swalwell recently visited Axis Community Health to get his flu shot, reminding us all of the importance of preventive care. By staying up to date on vaccinations, we not only protect ourselves but also safeguard our entire community especially the most vulnerable among us.

We are grateful for the Congressman’s commitment to leading by example and supporting community health efforts. Preventive care is key to staying healthy, and at Axis, we ’ re proud to offer essential services like flu shots to keep our community thriving!

Mental Health Matters Panel

Last week, Axis’s CEO, Liz Perez-Howe, took part in “Mental Health Matters,” a crucial discussion on mental health resources spanning federal, state, county, and local levels.

Axis remains steadfast in its commitment to supporting Tri-Valley residents through our Axis Bridge Mental Health Urgent Care program. We are proud to collaborate with local organizations to ensure every community member has access to the mental health resources they need.

A heartfelt thank you to the Tri-Valley Nonprofit Alliance for uniting so many influential leaders! Together, we will continue to elevate awareness about the importance of mental health, benefiting our entire community.

October is Breast Cancer Awareness Month, a time dedicated to raising awareness about the importance of early detection and treatment of breast cancer. Regular screenings, such as mammograms, play a crucial role in catching breast cancer early, when it is most treatable. This can make a significant difference in the lives of many women.

In an exciting development, Axis Community Health teamed up with Sutter Health to offer mammograms to Axis patients through a mobile unit. This initiative aimed to make these essential screenings more accessible to our community, especially those who might find it challenging to visit traditional healthcare facilities.

It is our hope that in partnering with Sutter, we can continue to increase our breast cancer screening rates. According to the 2023 Uniform Data System (UDS), Axis’s breast cancer screening rate is an impressive 59.1%. This is significantly higher than both the state average of 46.9% and the national average of 45.3%. These numbers highlight the success of Axis’s efforts and the importance of making healthcare services more accessible.

Regular mammograms are recommended for women aged 40 and above, although those at higher risk may need to start earlier or have more frequent screenings. Early detection through these screenings can lead to less aggressive treatments and better survival rates, making them a vital part of women ’ s health

The collaboration between Axis and Sutter Health is a fantastic example of how healthcare providers can work together to improve public health. By bringing mammograms directly to the community, they are breaking down barriers and ensuring that more patients have the opportunity to benefit from early detection.

In a recent feature on their news website, Vitals, Sutter Health showcased the remarkable efforts of Axis Community Health in improving access to mental health services. The article, titled “Breaking the Silence: How One Health System is Improving Access to Mental Health Services,” delves into the significant strides made by Axis in addressing mental health needs in the Tri-Valley area.

The piece highlighted our Axis Bridge Mental Health Urgent Care Program, that provides free, critical mental health treatment to adults and children aged 5 and older who reside in Dublin, Livermore, and Pleasanton. This program, supported by a substantial $300,000 investment from Sutter Health, offers crisis intervention, stabilization, care management, and counseling services, regardless of a patient’s insurance status or ability to pay.

Axis’s CEO, Liz Perez-Howe, expressed deep gratitude for Sutter Health’s partnership and support. She emphasized how the Axis Bridge program has provided urgent mental health support to over 1,000 individuals this year alone, significantly reducing hospitalization rates and offering hope and healing to those in need.

Axis is excited to announce that Gojji, our new comprehensive remote patient monitoring program designed to support individuals with conditions such as diabetes and high blood pressure, is now available to eligible patients. Earlier this month, representatives from Gojji came to orient the Axis team on how patients can benefit from their remote patient monitoring program. By providing continuous monitoring, personalized care, and easy access to essential health tools, Gojji can help our patients get healthy and stay healthy!
