The National Benefit Fund believes it is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted in 2010. Being a grandfathered health plan means that this plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for an external review process for claims appeals. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on benefits. Questions regarding which protections apply and which protections do not apply to a grandfathered health plan and what might cause a plan to change from grandfathered health plan status can be directed to the plan administrator at (646) 473-9200. Make Sure Your Tax Refund Is All It Can Be with the Earned Income Tax Credit Take Advantage of Our Free Tax Preparation Assistance Program For years, the 1199SEIU Benefit Fund’s Tax Preparation Assistance Program has been available to help you claim the Earned Income Tax Credit (EITC) and get the most out of your tax return. The program has served thousands of members, and in 2016, the Fund is again partnering with the New York City EITC Coalition to help you get your taxes prepared accurately and make sure your refund includes everything you’re entitled to. The EITC Coalition is led by the New York City Department of Consumer Affairs, and includes the city’s Office of Financial Empowerment and the Food Bank For New York City.

Visit the Food Bank For New York City and Ariva Locations for Free Tax Preparation As long as your 2015 household income was below $54,000 (or $30,000 if you are single and have no dependents), our partners at the Food Bank For New York City are offering free tax preparation events at locations throughout the five boroughs of New York City.

The nonprofit financial education program Ariva is also offering free tax assistance at sites throughout New York City. In order to receive assistance at Ariva’s sites, your 2015 household income must be below $53,000 if you have dependent children or below $30,000 if you are single and have no dependents. If you qualify for the EITC, the tax experts onsite will help you file for the credit. Even if you don’t live near one of these sites, be sure to check the “Claim Your EITC” guide on this page to find out whether you qualify for the credit before filing your return. It’s Not Too Late to Get Your Flu Shot!

We’re right in the middle of peak flu season, but it’s still not too late to protect yourself, your family and your patients or residents from the potentially serious virus. If you still haven’t gotten your shot, here are a few good reasons why you should:

Getting vaccinated is the best way to protect yourself against the virus, say health experts.

As a healthcare worker, you have a greater chance of getting the flu. You’re also at a higher risk if you’re pregnant, age 50 or older, or have a chronic medical condition such as asthma, diabetes or heart disease.

A flu shot cannot make you sick. This is one of the biggest myths surrounding the flu vaccination. The flu viruses in the vaccine are inactivated (killed), so they can’t cause infection, and the vaccine is tested for safety.

Getting your flu shot is convenient—and covered! Flu vaccinations are available through most healthcare employers and worksites, and at local Rite Aid pharmacies. And remember that your Benefit Fund covers the vaccination, so you can also make an appointment to see your healthcare provider.

The Benefit Fund Launches a Coordinated Care Program with Northwell Health In July 2014, we entered into our first Accountable Care Organization (ACO) agreement with Montefiore Medical Center. Now, your Benefit Fund is partnering with the Union and the North Shore-LIJ Health System—now known as Northwell Health—to launch our second ACO, the 1199SEIU Northwell Health Coordinated Care Program, which means our two ACOs combined will cover more than 25,000 members.

Beginning January 1, 2016, all 1199SEIU members and their eligible family members who already receive most of their healthcare services from providers affiliated with Northwell Health, will be covered by the program.

Through this coordinated care program, your providers will work together closely to help ensure you receive the best possible care.

The program connects you with a care management team of nurses, social workers and health educators who can create

a customized health plan to help you manage a chronic condition, recover from a serious illness, or transition back home or to another setting after a hospital stay. Members of your Coordinated Care team will have access to specific information—such as the dates you visited a doctor or hospital, the treatment you received, any tests or screenings you underwent and prescriptions you received—which eliminates duplicate medical tests and referrals, and helps ensure you get the care you need when you need it. This information is protected under regulations of the Health Insurance Portability and Accountability Act (HIPAA), and the only people who see it are your healthcare providers and Coordinated Care team.

Gwendolyn Williams Gwendolyn Williams credits her Greater New York Benefit Fund health coverage with helping her manage two chronic conditions while maintaining a full-time schedule.

As a Certified Nursing Assistant (CNA) at the Pavilion at Queens Rehabilitation & Nursing, Gwendolyn Williams has administered daily medications to her residents with chronic conditions for the past 18 years. About 10 years ago, Gwendolyn began taking her own daily medications when she was diagnosed with a thyroid condition and high blood pressure.“I went to my doctor for an annual checkup, which I never miss because of my benefits with the Greater New York Benefit Fund, and he gave me the news soon after,” she said. “To be honest, I was a little bit afraid and didn’t know what it all meant at first.”

Fortunately, Gwendolyn said, her doctor eased her fears, telling her that with medication and some changes in diet and exercise, she could continue to lead a full life.

“I wasn’t totally surprised when I got the diagnosis,” she said. “I had been feeling tired and just not myself for a few months.”

To help keep her two conditions under control, Gwendolyn has made an effort to adopt healthier habits. She joined a health club near her home in Brooklyn and gets there about three times a week, she said. She also eats a healthier diet, with more lean meats, less fried food and lots of water. And while she still needs to take two pills a day to manage her conditions, she is no longer afraid. “I know that I have the best care, between my primary doctor and my thyroid specialist, who I see twice a year,” she said.“It’s incredibly reassuring.”

As a Greater New York member, Gwendolyn has a modest co-payment when she sees her thyroid specialist and no longer has a co-payment for prescription drugs—which, she said, gives her great peace of mind.“To me, my benefits mean I never have to skip an appointment, I never skip my medications and I always get the care I need,” Gwendolyn said.“It doesn’t get better than that.”

Members Helping Members Inspired by the success of her Mount Sinai Beth Israel co-worker Merle Warren (featured in our Fall 2014 issue of For Your Benefit), fellow 1199SEIU member Patricia Blount made a few diet and exercise changes to avoid the health problems for which her family history of diabetes and high blood pressure put her at risk. In the past year, Patricia has lost nearly 30 pounds and feels better than ever. Watch for her full story in an upcoming issue of For Your Benefit.

New Blood Pressure Guidelines:

What You Need to Know You know that high blood pressure can increase your risk of stroke and heart attack, but do you know how high is too high? And how often should you have it checked? The National Heart, Lung, and Blood Institute and the U.S. Preventive Services Task Force have issued new blood pressure screening recommendations, which we’ve outlined below.

Of course, you should always talk with your own healthcare provider about what’s best for you.

•

If you’re being treated for high blood pressure, current guidelines say your target should be under 140/90—but lower may be better. A recent study from the National Heart, Lung, and Blood Institute suggests that 120 may be the new target for systolic blood pressure (the top number that measures the amount of pressure in your arteries as your heart muscle contracts). Researchers found that people who had high blood pressure and used medicines to lower their systolic blood pressure to below 120 lowered their risk of heart attacks and strokes by 30 percent and their risk of death by nearly 25 percent.

If you’re healthy and under age 80, you should aim for a blood pressure at or below 120/80. According to the American Heart Association, if you’re over age 80, your target should be below 150/90.

Healthy News You Can Use One more reason why it’s not too late to quit smoking: It can lower your risk for diabetes. Researchers looked at 88 studies for a recent review in The Lancet Diabetes & Endocrinology and found that the more cigarettes people smoked, the higher their diabetes risk; but those who quit smoking gradually lowered their odds of getting the disease.

Know your numbers. The latest guidelines from the U.S Preventive Services Task Force recommend regular blood pressure screenings for all adults ages 18 and older. If you’re between 18 and 39 years old and your reading is 130/85 or lower, you should get screened every three to five years. If you’re age 40 or older or are at high risk for high blood pressure, you should be screened every year. Risk factors include: • Being overweight or obese • Smoking • Getting little or no exercise • Excess salt in the diet • Drinking too much alcohol • Stress • Ethnic background • Family history of high blood pressure

Get screened outside the doctor’s office before starting treatment. If your healthcare provider thinks you have high blood pressure, the U.S. Preventive Services Task Force recommends having your blood pressure taken outside your doctor’s office to make sure.Your blood pressure varies throughout the day, and studies have shown that some people’s readings are higher at the doctor’s office due to what’s known as “white coat syndrome”—stress experienced when visiting a doctor. Look for opportunities for free screenings at health fairs.

You can also find free blood pressure stations at many pharmacies, although the accuracy of these machines varies widely. Talk to your doctor about what would work best for you.

Start 2016 the Healthy Way at Our January 30 Wellness Workshop We all know how tough it can be to stick to a New Year’s resolution, and your Benefit Fund is here to help. Join your fellow members on Saturday, January 30, at our Wellness/ Member Assistance Program (MAP) Department’s “Keep Your New Year’s Resolution” workshop,* where you’ll get tips on healthy eating, fitness and stress management.

Guide to a Healthy Blood Pressure Eat a heart-healthy diet. Be sure to eat plenty of fruits, veggies, whole grains, lean meats and low-fat dairy.

You should also limit added sugars from sweets and drinks like soda and juice.

Exercise regularly. Being active helps you maintain a healthy weight and keep your blood pressure under control.

The American Heart Association recommends at least 150 minutes a week of moderate exercise, such as brisk walking.

You can meet that goal in just 20 to 25 minutes a day.

Quit smoking. Each cigarette you smoke raises your blood pressure for long after you’ve put it out—and increases your risk for heart disease—so set a date to quit today.

Take medications “as directed.” If you take medication(s) to keep your blood pressure at a healthy level, make a plan that will help ensure you never miss a dose, such as taking your medication(s) at mealtimes.

At our nutrition seminar, a registered dietitian will discuss how to create a healthy eating plan for the whole family, including tips on getting the right nutrients and making well-balanced meals that are quick to prepare. You’ll also learn strategies to help reduce stress at home and at work, and will have the chance to participate in an all-ages aerobics class. So remember to wear comfortable clothing—and bring the whole family!

Healthy Eating

Decoding Food Labels Shopping for healthy food can be a challenge. More and more foods whose packaging reads “light” or “all natural” can be anything but. The good news is that nutrition labels can give you all the information you need to eat healthfully and steer clear of marketing gimmicks. Here’s a quick guide to some of the trickier labels at the supermarket:

Fat Free

All Natural

To be labeled “fat free,” foods must have less than half a gram of fat per serving, but they’re often high in calories, since food manufacturers often replace the fat with sugar and flour.You might also be tempted to overeat these foods since they seem healthier. So limit your portions and, when you can, choose veggies, fruits, beans and whole grains more often. They are lower in calories, can be more filling and are chockfull of fiber and other nutrients. It’s important to note that some fats are necessary for a healthy diet—such as olive oil, safflower oil, avocados and nuts. But these foods can be high in calories, so you should eat them in moderate amounts—and avoid foods high in saturated and trans fats altogether.

Although “all natural” sounds healthy, it can be misleading. In reality, there is no recognized, standard meaning of “natural” for most foods. That means that food companies can claim their products are “natural” even if they’re loaded with fat, sugar and carbohydrates. When you’re grocery shopping, be sure to read the ingredients on nutrition labels before you buy. Better yet, stick with truly natural foods: fresh fruits and vegetables.

Healthy News You Can Use Eating a healthy diet and exercising may be more effective than medicine for reducing your diabetes risk, according to a recent study in The Lancet Diabetes & Endocrinology. Participants who ate a low-fat, low-calorie diet and exercised for 15 minutes a day lowered their diabetes risk by nearly 10 percent more than those who took diabetes medication.

Low Sugar and Lightly Sweetened A “low sugar” or “lightly sweetened” label doesn’t mean a food is actually low in sugar; like “all natural,” these terms don’t have standard meanings. Check the nutrition label to see how much sugar you’re really getting. The American Heart Association recommends limiting the amount of added sugars in your food or drinks to no more than six teaspoons a day for women and nine teaspoons a day for men (one teaspoon is equal to 4 grams). If you like, go for the brands that use sugar substitutes, or better yet, satisfy your sweet tooth the healthy way, with in-season fruits like pears and apples.

Healthy News You Can Use Trading one hour of sitting each day for walking could add years to your life, according to a recent study in the International Journal of Behavioral Nutrition and Physical Activity. Researchers found that people who swapped one hour of sitting each day for walking lowered their risk of early death by nearly 15 percent.

If you have a hard time fitting regular exercise into your busy routine, you are not alone. But that doesn’t mean it can’t be done. In fact, many of your fellow members have discovered strategies to overcome barriers that get in the way of good health and an active lifestyle. Here are some tips:

Squeeze in short walks throughout the day. Every little bit counts. Try 10 minutes in the morning, 10 minutes at lunch and 10 minutes in the evening—and you’re done!

Exercise is boring.

I’m self-conscious about my weight.

Want to know a secret? Most everyone else is too, so no

one is looking at you! In the meantime, use a video at

home like our 1199SEIU Resistance Band Total-Body Workout until you feel ready to join a group. And focus

on how much better you feel after you exercise.

I’m too tired to exercise.

Keep a pair of walking shoes with you and take a brisk walk during your lunch break. On your way home, get off your bus or subway a stop or two early and walk. Before long, you’ll see that exercise actually boosts your energy.

I’m not motivated to exercise.

To get started, set realistic goals, like a walk around the block—and reward yourself when you meet them. That first walk may earn you a 30-minute nap. Make it three trips around the block and treat yourself to a movie.

I don’t like exercising alone.

Encourage your family, friends or co-workers to join you in a weekly walking group or other fun activity. Bonus: You’re more likely to stick with an exercise if you know someone is counting on you.

Take advantage of one of your Fund’s discounted memberships to a gym, where you can try a class one day, work out with weights the next day and, if the gym has a pool, go swimming the following day. Mix it up!

The 1199SEIU Greater New York (GNY) Education Fund is creating a new online training program, the “1199SEIU Resident Decline Detection Program,” which will be available in 2016 to all facilities and homes that participate in the GNY Education Fund.

The program satisfies New York State’s Advanced Training Initiative (ATI) requirements and will be offered during working hours. It will be offered online in a train-the-trainer format and will include topics such as creating a welcoming environment for new residents, reducing unnecessary hospitalization, assessing residents at risk, making pain assessments, and facilitating communication and care planning. Attending College in Academic Year 2016–2017?

Request the Joseph Tauber Scholarship Application The Joseph Tauber Scholarship Program offers scholarships to children of eligible 1199SEIU members who are attending accredited undergraduate programs. Eligible members are those whose employers contribute to the National Benefit Fund or the Greater New York Child Care Fund. Members must have been receiving Wage Class I or Eligibility Class I benefits for at least one year. Candidates must be high school graduates, post-secondary school students or previous awardees enrolled full time for a minimum of 12 credits each semester. All candidates must be younger than 23 years of age by September 1, 2016. Previous awardees seeking scholarship support should note that automatic award renewals are not granted; you must reapply each year for consideration.

To request an application for the Joseph Tauber Scholarship Program, provide your information below and submit it before January 31, 2016.

Scholarship award amounts vary according to need, with a yearly minimum of $750. Asegúrese de Obtener la Máxima Devolución Posible de Impuestos con el Crédito Tributario por Ingresos del Trabajo Aproveche el Programa de Asistencia para la Preparación de la Declaración de Impuestos Gratuito Durante años, el Programa de Asistencia para la Preparación de la Declaración de Impuestos del Fondo de Beneficios de 1199SEIU ha estado disponible para ayudarle a solicitar el Crédito Tributario por Ingresos del Trabajo (Earned Income Tax Credit, EITC) y a obtener la máxima devolución posible de impuestos. El programa ha brindado servicios a cientos de miembros y, en 2016, el Fondo se asocia nuevamente con la Coalición EITC de la Ciudad de Nueva York para ayudarlo a preparar su declaración de impuestos con precisión y garantizar que el reembolso que reciba incluya todos los montos a los que tiene derecho. El Fondo de Beneficios Lanza un Programa de Atención Coordinada con Northwell Health En julio de 2014, celebramos nuestro primer acuerdo de Organización Responsable de la Atención (Accountable Care Organization, ACO) con Montefiore Medical Center. Ahora, el Fondo de Beneficios se asocia con el sindicato y North Shore-LIJ Health System (conocido ahora como Northwell Health) para lanzar nuestro segundo acuerdo ACO, el Programa de Atención Coordinada de 1199SEIU conocido ahora como Northwell Health, lo que significa que nuestros dos acuerdos ACO combinados cubrirán a más de 25,000 miembros. Gwendolyn Williams Gwendolyn Williams afirma que su cobertura de salud del Fondo de Beneficios del Gran Nueva York la ha ayudado a controlar dos enfermedades crónicas y, al mismo tiempo, a mantener un programa de tiempo completo.

Como Asistente de Enfermería Certificada (Certified Nursing Assistant, CNA) en Pavilion at Queens Rehabilitation & Nursing, Gwendolyn Williams ha administrado medicamentos diariamente a los residentes con afecciones crónicas durante los últimos 18 años. Hace aproximadamente 10 años, Gwendolyn comenzó a tomar sus propios medicamentos a diario cuando le diagnosticaron una afección de la glándula tiroides y presión arterial alta. “Fui al médico para que me hiciera el control anual, que nunca paso por alto ya que cuento con los beneficios del Fondo de Beneficios del Gran Nueva York, y me dio la noticia poco después”, cuenta. “Para ser honesta, tenía un poco de miedo y, al principio, no comprendía”.

Se inscribió en un gimnasio cerca de su casa en Brooklyn y asiste tres veces por semana, según dice. Tome los medicamentos “según las indicaciones”. Si toma medicamentos para mantener

la presión arterial en un nivel saludable, elabore un plan para asegurarse de que nunca omita una dosis, como tomar los medicamentos a la hora de la comida.

Cómo Decodificar las Etiquetas de Alimentos Comprar alimentos saludables puede resultar un desafío. Hay cada vez más alimentos con etiquetas que rezan “dietético” o “ingredientes completamente naturales”, pero puede que no sea así. La buena noticia es que las etiquetas de información nutricional pueden brindarle toda la información que necesita para alimentarse de manera saludable y alejarse de los engaños de marketing. A continuación, incluimos una guía rápida de algunas de las etiquetas más engañosas que se encuentran en un supermercado:
