What a gift it is to be a member of Wisconsin’s veterinary community! I have been blessed and honored to serve as a Student Representative on the WVMA’s Executive Board for two years. As a result, I possess a broader and richer perspective of the veterinary profession. This position enhanced my educational experience, helping me to grow, mature and further deepen my commitment to a veterinary career. My experiences as representative fostered a deep appreciation for our state’s professional association, and it is with great pride that I reflect on my good fortune to have had a small role within the WVMA.

The camaraderie between the WVMA and the University of Wiscosnin School of Veterinary Medicine is special, and students are fortunate to benefit from this partnership. Student WVMA membership includes countless benefits - from a plethora of learning opportunities to involvement on subcommittees within the organization. The spectrum of experiences planned and sponsored by the WVMA spans from gaining hands-on clinical skills to preparation for the mental and emotional challenges of modern medicine. The Annual Convention offers a full day of student wetlabs and lectures - for free! (a vet student’s favorite four letter word). Membership also provides numerous opportunities for student career-building. I found that attending Networking Dinners hosted by local clinics provided much more than a free meal - these were important opportunities to leave campus, visit the real world and enjoy the company of practitioners who warmly shared advice, tips for success and encouragement. These and other networking events serve as an invigorating reminder of what lies beyond the SVM doors. You, the DVM members taking the time to mentor and teach are vital elements of the WVMA’s spirit of student support. Thank you. I have come to also value the immense energy the office staff invests in making student events possible. Bailey Quam works tirelessly as our fabulous student liaison (and receives way too many emails from me), making events engaging, professional and highly relevant to our careers. These activities certainly boost our knowledge base, but more importantly, shape us into diverse and dynamic leaders. There are so many chances to gain a perspective many students would otherwise never obtain without the WVMA. Student members not only have an excellent (and fun) route to enriching their educations, but also gain critical insight into life after veterinary school. Perhaps the most impactful quality of the WVMA is the association’s drive to promote and sustain the linkages between veterinary, industrial, and agricultural communities present in Wisconsin. Hoarding Discussed at Welfare Seminar

We have all heard about or witnessed animal hoarding. It is also fair to say it often isn’t addressed as seriously as other types of animal neglect or abuse. During the Animal Welfare Seminar held on March 30, Dr. Randall Lockwood discussed animal hoarding and the disturbing trends of repeat violators. The definition Dr. Lockwood used for an animal hoarder is as follows: “Someone who accumulates a large number of animals; fails to provide minimal standards of nutrition, sanitation and veterinary care; and fails to act on the deteriorating condition of the animals (including disease, starvation and even death) or the environment (severe overcrowding, extremely unsanitary conditions) or the negative effect of the collection on their own health and well-being and on that of other household members.” – Dr. Gary Patronek. He went on to state that of the documented hoarding cases that involved criminal charges, 46 percent were dogs, 34 percent were cats, 5 percent birds, 5 percent farm animals, 4 percent rabbits, 4 percent horses and 2 percent exotic animals. Source: Berry, Patronek and Lockwood. While dogs and cats are the most common cases seen, no animal is immune from animal hoarding. The take home message from Dr. Lockwood is “since animal hoarding is associated with a high likelihood of recidivism, there is likely to be greater risk of future instances than in other neglect cases. Although most of the efforts of forensic veterinarians will focus on documenting the injuries and illnesses seen in animals removed from a hoarding situation, it is also important to document the presence of health and well-groomed animals at the scene. Such evidence indicates that the suspect was knowledgeable of what actions are needed to keep animals healthy and still made the decision to deny that treatment to the animals that suffered. Similar evidence may be provided by the presence of quality food or appropriate medications that may have been found on the scene but not made available to some or all the animals.” Based on the above information, it is important for everyone involved in animal hoarding cases to take them serious. I often hear statements from the public, media and decision makers that it is just a case of the individual “loving animals” too much. Based on what we learned in March, that is not a valid excuse and these situations should not be swept under the rug. The likelihood of the reoccurring again, with the same individual is high. l

102nd Annual Wisconsin Veterinary Medication Association Convention to be Held October 12-14 Mark your calendar for the 2017 WVMA Annual Convention – Licensed to Practice – being held October 12-14 at the Alliant Energy Center in Madison, Wis.

The convention will be held in conjunction with the Wisconsin Veterinary Technician Association and Wisconsin Veterinary Practice Managers Association conventions, and features an immense lineup of nationally acclaimed speakers and trade show vendors. Beyond the state lines of Wisconsin, the scope of connectivity between the WVMA and national groups including the AVMA keep us well-represented throughout the country.

Moving On Up or Moving On Out: Whatever the Reason for Ownership Changes, Planning is Critical By Gregory E. Scallon, DeWitt Ross & Stevens, s.c. It is inevitable in the life of a veterinary medicine practice that there will be ownership changes. During 2016, we saw a great deal of activity involving ownership transitions in very diverse ways. The most common, by far, involved the purchase of an ownership interest by a current employee of the practice. Frequently, this buy-in was coupled with the retirement of a current owner, or was part of a plan for a future retirement, and in other instances simply reflective of the growth of the practice and the desire of both the employee and the practice to take this next step. We also saw increased activity on the part of large national firms buying-up local practices. We assume this may have been the result, in part, of a general sense that improvements in the economy and in consumer confidence levels feed into a growing market for veterinary medicine services. Valuations for practices have been solid and growing. If we look at valuation models used by appraisers and buyers of veterinary practices, they support the higher valuations. This avenue can be a good exit strategy for a practice that does not have any candidates within its employee base to take on ownership of the practice. Less common, in my experience, but certainly viable options, are a sale, or a merger, with another existing practice, perhaps a friendly competitor, and the sale to a person with no prior relationship to your practice, whether a veterinarian or not, who prefers to purchase an existing practice rather than start a new practice. As with most aspects of owning and operating a veterinary medicine practice, it is critically important that you begin planning for any such ownership transition years in advance. Having the tools in place in advance, will allow you to move quickly and confidently when the time is right. This article, and others to follow as we discuss various aspects of this topic in greater depth, is aimed at the practice that does not already have an established ownership transition process in place. If you are part of a multi-owner practice with established procedures and agreements in place that are working well for bringing in new owners and transitioning out retiring owners, you can skip this article; although, I would appreciate any insights you may be willing to share about any unique features of your system. For the practice without a “tried and true” system in place, I suggest the first consideration is to look at your current ownership structure (i.e., “look in the mirror”) and to decide what is your preferred course to prepare for a transition. If you are a sole owner, have always enjoyed being the sole owner, and don’t relish the concept of having other owners with whom you share management decisions, it doesn’t seem advisable that you go down the path of bringing current associates, or an outsider, into ownership. Rather, it seems

that you should anticipate a plan to sell your practice to a third party (perhaps a neighboring practice that would like to expand, one of the large national firms, etc.) that affords you the opportunity to sell and retire from the practice with a short, if any, transition period. If you are the sole owner of a practice with one or more employed veterinarians, or a co-owner with others of a practice, without an established track-record of ownership buy-ins and buy-outs, the initial analysis is the same as discussed in the previous paragraph, which is to decide what exit strategy seems likely to work best for you, your co-owners, if any, and your practice. As noted above, a process that is commonly followed, and, in my experience, with frequent success is to grow the future owner(s) of the practice within the practice. That is, as the growth of the practice allows, hire additional veterinarians with the intent that some or all of the new hires will grow into management and eventual ownership of the practice. The process of growing the practice through hiring associates who are elevated to ownership requires, first and foremost, that you, as the owner, are interested and willing to share ownership, and, secondly, that you hire one or more associates who have the desire and are good candidates to become owners. As noted above, not every owner wants to have co-owners, and, similarly, not every employed veterinarian seeks, or is a good candidate for ownership status. This process requires a thorough self-evaluation of whether you, as owner, are a good candidate for co-owner status, and, similarly, a rigorous evaluation of the employee, both a selfevaluation by the employee and an evaluation by you, to be sure that elevating the employee to ownership is a good match from both owner and employee perspectives. While we do not recommend any contractual commitments regarding future ownership opportunities at the time an offer of employment is made to an associate veterinarian, we do suggest that it is good to have a very preliminary discussion during the interview stage to explore the potential employee’s interest in acquiring an ownership interest. The ownership transition process is multi-faceted, and is extremely important to provide for the continued success of your practice, for an opportunity for an employed veterinarian to advance to ownership and for the older veterinarian to realize the value you have built-up in the practice and go forward comfortably into retirement. In future Legal Brief articles, I intend to expand on this discussion, and to delve more deeply into the considerations with respect to the various types of buy-ins and buy-outs. Question: Does OSHA cover any laser use issues in the workplace, for instance surgery lasers and therapy lasers? Or is that a State issue? OSHA does cover laser issues. Class of 2018 Blue Coat Ceremony Held On April 2, students of the University of Wisconsin School of Veterinary Medicine class of 2018 participated in the Blue Coat Ceremony hosted by the Wisconsin Veterinary Medical Association (WVMA) at the Union South Varsity Hall. During the ceremony, third-year students were presented with their “blue coats” signifying the end of classes and beginning of clinics. Family and friends of the students attended the event which included special presentations from Dr. Scott Spaulding, WVMA president; Dr. Mark Markel, dean of the School of Veterinary Medicine; Dr. McLean Gunderson, Kristen Cooley and Dr. Calico Schmidt, School of Veterinary Medicine. USDA Announces $4.2 Million to Repay Educational Loans for Veterinarians The U.S. Department of Agriculture’s (USDA) National Institute of Food and Agriculture (NIFA) today announced $4.2 million in available funding to help eligible veterinarians repay a portion of their veterinary school loans in return for serving in areas of the United States lacking sufficient veterinary resources. Funding is made through NIFA’s Veterinary Medicine Loan Repayment Program (VMLRP), authorized by the 2014 Farm Bill. “Many veterinarians are facing insurmountable student loan debt, and they choose to work in urban locations that offer higher pay. This is causing a critical shortage of veterinarians in rural America,” said NIFA Director Sonny Ramaswamy. “This assistance will help veterinarians return to rural America where they can provide needed services to our farmers and ranchers, and continue to keep our food supply secure from diseases of farm animals.” Studies indicate significant and growing shortages of food supply veterinarians. A leading cause of the shortage is the heavy cost of four years of professional veterinary medical training, which can average more than $150,000. Without the Veterinary Medicine Loan Repayment Program to help relieve that financial burden, many veterinarians might otherwise select more lucrative career options in the urban areas, such as companion animal care. Recipients are required to commit to three years of veterinary service in a designated veterinary shortage area. A map of veterinary service shortage areas by state is available online. Loan repayment benefits are limited to payments of the principal and interest on government and commercial loans received for attendance at an American Veterinary Medical Association (AVMA)-accredited college of veterinary medicine resulting in a degree of Doctor of Veterinary Medicine or the equivalent. Eligible applicants: • Have a degree of Doctor of Veterinary Medicine by July 1, 2017, or the equivalent, from a college of veterinary medicine accredited by the AVMA Council on Education; • Have a minimum qualifying educational loan debt of $15,000; • Secure an offer of employment or establish and/or maintain a practice in a NIFA-designated veterinary shortage situation within the time period specified in the VMLRP service agreement offer; • Provide certifications and verifications as defined in 7 CFR 3431, Veterinary Medicine Loan Repayment Program; • Not owe an obligation for veterinary service to the federal government, state, or other entity under an agreement with such federal, state, or other entity unless such obligation will be completely satisfied prior to beginning service under the VMLRP; and • Not have a federal judgement lien against his/her property arising from federal debt. Advancing Patient Care Through Team Commitment to Forward Booking The Roaring Fork Valley provides some of the best fly fishing in Colorado, and the town of Carbondale – with its 6,500 residents located in the mountains 170 miles west of Denver – lies at its center. Resting in the magnificent shadow of 12,953-foot Mt. Sopris, Carbondale offers plenty of activity and enjoyment for tourists and residents alike.

What has also been important is for our staff to be unafraid of saying to the client what the patient needs. The best care for the pet should not be posed as a question, rather it’s “here’s what’s best for your pet.” You have to believe in providing the best care and then it’s natural to say, “let’s schedule that exam” instead of asking for permission.

Carbondale also provides the home to Alpine Animal Hospital, a thriving, AAHA-accredited mixed animal practice with five veterinarians and 12 staff members. Mary Fox, MBA, who characterizes Carbondale as “a fabulous town” and serves as Alpine’s hospital administrator, recently shared her observations about successfully implementing forward booking into the protocols and culture of the hospital.

A: 90% there! It took two years because our goal is to help our staff understand that we’re all in this together. It’s really a switch in practice culture, which will have a longer lasting effect than just dictating protocols to the staff.

Q: How did your practice implement forward booking? A: It was a process, and it took some time because part of it was spending time with the staff to figure out practice goals and values, and how we express them in a positive way. We quickly learned that if we talked about forward booking in isolation and didn’t attach it to preventive healthcare, the staff thought it seemed pushy. Once we got staff buy-in that they were acting in the best interest of the patients, they started to feel comfortable and confident talking about forward booking.

Q: How good is the practice at forward booking?

Q: Any other advice for practices? A: Have definite, purposeful goals that will bring your team together – Why are we here? What do we want our culture to be? Then, build it out and ask, “How do we accomplish this?” Sure enough, you circle back to where you started and the initial things that were difficult to implement, you either find they fit in or they don’t and you reject them. We are always trying to do things better. l

Q: What was the biggest barrier to implementation? A: First, the staff did not want to be “pushy salespeople.” Second, communication in the practice needed improvement. The front desk often did not know what the next step was in the preventive healthcare process so we improved the communication between our receptionists and our exam room team. We now use the code “RTG” in our software system, which stands for “Ready To Go.” No patient can leave without that code, and next to the code the doctor indicates the next steps in the patient’s healthcare. Q: How does forward booking tie into your patient reminder system? A: We’re right in the middle of evaluating the process of how we want to remind our clients. We have a new communication platform, which starts with an email reminder, then goes to a text, then a call. Once the appointment is confirmed, the client doesn’t receive any other messages. Because we use all mediums (email, text, phone), a lot fewer cards go out, which saves us time and money. In the beginning, the staff was very tentative about asking for client email addresses. It’s taken about a year, but now they say to clients, “and your email address is…” without becoming apologetic.
