As the first UK nation to embrace clinical services in optometry primary and community care, Wales is once again leading the charge to widen optometry-based clinical services.
Having received the go-ahead for its long-awaited eyecare reform plans (see news page 9), the priority now is to get on with implementing – and increasing –clinical services delivered by optometrists to reduce referral numbers and improve access to hospital eye departments.
With GP and hospital services now in crisis, there has never been a greater need to ensure timely – and potentially sightsaving – access to eye health care. Inextricably linked to this is the need for practices to have the latest eye health technology and training – not just in clinical imaging and diagnostics – but across the entire practice e-health IT system. Telemedicine is ever-expanding and, as was raised at the recent inaugural meeting of the Optical Suppliers Association’s E-Health Focus Group (see news age 14), instrumentation and software providers must work together to ensure optimum patient care.
At the same time, the profession and industry must look to how they can be more sustainable – whether it be within the product and supply chain or in simple day-to-day operations. Taking their eco responsibilities ever-more seriously are a number of NEG lens suppliers: turn to page 25 to read about the ‘labs protecting the planet’.

A national digital media campaign for Essilor’s Eyezen lens portfolio is running this month in parallel with a social media drive to reach patients living near participating practices. Alan Pitcher, Essilor commercial director, said: “Our autumn marketing drive hopes to create awareness of the single vision lenses and generate footfall for our Essilor Experts. Consumers will either be sent to the optician finder on our consumer website or receive an Eyezen ad in their Facebook social media feed where they can make an appointment directly with their local optician.”

At the recent International Myopia Conference in the Netherlands, experts shared their latest findings of the clinical application of Hoya’s Miyosmart lens and its role in myopia management. Dr Andrea Lembo of Milan shared the results of a clinical application of the lens combined with 0.01 per cent atropine drops in European myopic children and adolescents. The results demonstrated that over one year, “axial elongation and myopia progression were reduced with maximum effect using the combination treatment”.
Professor Hakan Kaymak shared data of German children wearing the lenses, which reconfirmed previous clinical study results. Further findings showed that “children adapted very well to the lenses with no safety concerns even in combination with low-dose atropine”. Professor Carly Lam of the Hong Kong Polytechnic University shared data showing that the myopia control effect was stronger in children with baseline hyperopic relative peripheral refraction (RPR) than in children with baseline myopic RPR.

The British Contract Lens Association (BCLA) is inviting nominations for two of its top honours – the Irving Fatt Memorial Lecture and the BCLA Medal – both to be presented at its 2023 clinical conference. The BCLA Medal recognises outstanding achievement in the field of contact lenses and/or the anterior eye. An ambitious programme for eyecare reform is underway in Wales after receiving ministerial sign off last month. It will mean a substantial change within primary care optometry in the nation over the next 18 months, announced health and social care minister Elenud Morgan. The reform process will see the profession delivering more clinical work in the primary care setting and greater involvement in local decision making at primary care cluster level.
Sarah Schumm, chair of Optometry Wales, commented: “This is a fantastic opportunity to truly be part of the primary care community, to fully utilise the skills we have to be able to truly be part of the diagnosis, management and treatment of citizens in Wales. The World Council of Optometry (WCO) and CooperVision shared their vision for myopia management at the International Myopia Conference in Rotterdam last month. A poster presentation reflected on changing approaches to the disease – co-authored by Dr Sandra Block, WCO president elect, Elizabeth Lumb, CooperVision director of global professional affairs, myopia management, Professor Peter Hendicott, WCO president, and Dr Susan Cooper, managing director of the World Council of Optometry.
Full training and support will be arranged over the coming months as more details are shared about what will be required, and what the financial package will look like. “This is a new era of change for the profession and we are incredibly excited to help set the pace for change,” added Sarah. “We will be continuing to offer individual sessions to help all practices across Wales understand how the landscape will be changing and how to manage that change effectively.”

Third-year clinical trial results of Essilor Stellest lenses have shown they continue to exhibit strong efficacy in slowing myopia progression and axial elongation. The clinical trial was conducted in China, where 151 children from the first two years of the study were switched to spectacle lenses with highly aspherical lenslets while a new set of 56 children were recruited and wore single vision lenses.
Key highlights from the clinical trial include: Essilor Stellest lenses saved more than one dioptre of myopia on average over three years; children who wore Essilor Stellest lenses full-time saw a marked increase in myopia control efficacy; the lenses are effective in slowing myopia progression and axial elongation in older children.

Installing large solar panels on the roofs of its eyewear production facilities, along with further carbon reduction projects and off-setting, has resulted in a 50 per cent reduction in Silhouette’s carbon footprint over the last seven years. The company can now claim carbon neutral status for eyewear production with off-setting, having cut emissions to approximately 2,000 tonnes last year alone.
By 2023, Silhouette plans to install solar panels on all suitable roofs at its headquarters in Linz, Austria. Annually, this will prevent the release of 450 tonnes of CO2 emissions, the equivalent of what would require 13,361 trees to absorb, said the company. Part of its long-term plan to cut out remaining emissions focuses on solar energy production in the Global South, a Gold Standard carbon off-setting project

Silhouette has purchased Gold Standard certificates for a 6MWh solar energy farm in Kolanupaka, India, and is championing a solar farm supplying 9,899MWh of clean energy each year to the local power grid, producing a cut of 9,535 tons of CO2 equivalent each year. The solar farm also improves air quality and living conditions locally, with a health camp in the works to provide medical treatment in co-operation with Kamineni Hospital for 400 people.
NEG managing director, Phil Mullins, took part in the inaugural E-Health Focus Group of the Optical Suppliers Association (OSA) last month – urging independent practitioners to embrace new technology. Underpinning the meeting was the enthusiasm of industry colleagues to work together, to ensure practices had access to the most advanced technology.
Group chairman, Karl Jeebaun, told attendees that a set of standards for e-health was required, a process that could be optimised by the industry working together. “Building a bridge between instruments and software will ensure that new technology can be used for optimum patient care,” he said. “There are many challenges, particularly issues of liability and who might end up before the General Optical Council [GOC]. We have seen this being used as a block to taking digital images already. Conservatism is currently delaying the adoption of digital technologies in many practices, and we are eager to understand everyone’s perspective.”
Philip Mullins told the group: “Independents need to embrace new technology, but the practice management software is the central core of everything. We also need to engage with the NHS and to establish the most effective pathway for care.” The group discussed many of the issues of integrating e-health devices, telemedicine and apps into traditional eyecare, which needed standardisation and verification.
With patient care being the primary concern, the group agreed to: identify all stakeholders and their sub-specialties within e-health and the ophthalmic sectors; work with all stakeholders to develop guidance and standards for the safe use of technology; meet requirements of regulators including the Medicines and Healthcare products Regulatory Agency and GOC; define the distinction between devices suitable for patient use or use by a qualified clinician; and address concerns the profession may have.
Vice chairman of the group, Jason Higginbotham, urged everyone with an interest to become involved: “There is an opportunity for everyone to have a voice and to add their expertise as we create new pathways. By setting specific definitions and minimum standards, we have the potential to be a global leader. Our businesses want to function on an international platform and beacon status would be very useful. We have a real opportunity – no-one wants to stifle innovation but at the moment there is no clear path.”
The AIO is occasionally approached by optometrists and dispensing opticians who are interested in setting up their own practice, or purchasing an existing independent practice.
We suspect that those lifting their head above the parapet in this way, particularly employees of the multiples, are the tip of the iceberg. In other words, there are many others who have that desire, or perhaps are just interested, that do nothing about it. This is a crying shame.
There is always a steady flow of independent practice owners who are nearing retirement, who would love to sell their practice to young aspiring independents. The likes of Myers La Roche do a good job in trying to match up those wishing to sell and those interested in buying, but the AIO believes that there is an untapped cohort of interested practitioners who never progress their dreams.
The Association is determined to find ways to connect with this cohort and help them to make an informed decision about whether practice ownership is for them. Setting up a new practice, whilst very challenging, is also an option, providing always that the appropriate research and planning has been undertaken before taking the plunge. Either way, the AIO Practice Support Network (PSN) can be a catalyst to help make the dream a reality.
The PSN was created before the pandemic to help aspiring independents to own their own practice, and support those who have already taken the plunge. The pandemic was an obvious inhibitor to the PSN progressing its objectives, but as we move on, the partners (AIO, NEG, Eyeplan, Myers La Roche and Seiko Optical), have committed to relaunching the network and making a serious impact in supporting the longterm health of the independent sector.
The challenge is getting to those people who aspire to independent practice ownership, but never do anything about it. The PSN is actively looking at how to address this challenge and enable the dreams of young practitioners to become a reality. Several approaches have been used to reduce the impact of soft contact lenses on the ocular surface, for example, increasing wettability and oxygen transmissibility, changing lens replacement frequency, and material enhancements to reduce protein and lipid deposition.4,16 Building on these, an additional, innovative approach involves incorporating comfort and eye health ingredients into the contact lens itself.

Highly accurate dry cutting technology, facilitated by the MEI 641 TBA, which negates the need for disposable blocking pads, has made a profound difference to several UK labs. Shaun Bace, commercial manager at Jai Kudo lenses, part of the Millmead Optical Group, said: “Processing a lens without water is much faster and there are less variables. Making sustainable decisions often makes commercial sense too.”
Millmead is manufacturing frames and cases at its north west facility in Maghull, but conscious of the tremendous waste from dummy lenses and lab swarf is shining a light on the issue with a life size sculpture to be unveiled later this year.
“We are all conscious that dummy lenses go straight into the bin, but we are aiming to collect these when deliveries are dropped off at the practice,” said James Conway, Millmead CEO. “Some dummy lenses are biodegradable but we need them all to be biodegradable. Our goal at Millmead is to recycle everything and to close the loop on waste.”
Essilor’s global Eyes on the Planet initiative is backed up by the management undertaking sessions on carbon neutrality. Thornbury lab manager, Dan Peacey, explained: “We have fitted meters to all devices using water so that we can monitor consumption at any one time. This computerised system enables us to really interrogate what our processes are doing.
Cuttingwater usage for a surfaced lens from 12 litres to four, and reducing the estimated 3,000 tonnes of waste that goes to landfill each year, are driving labs – NEG preferred suppliers among them – to review their business plans. Implementing strategies such as harvesting rain water, recycling lens production water, installing solar panels on lab roofs, double glazing and LED lights are all contributing to a more sustainable industry.
A move away from heavy metal alloys to ART (alloy-free technology) from Satisloh – estimated to be 15-20 per cent cheaper for each lens produced – is a key element to reducing waste, says the OSA. Replacing print catalogues with online ones is becoming the norm.

'Harmful blue light' is calculated between 380nm and 460nm. Based on tests on polycarbonate grey lenses at 23˚C.

2. Plastic neutrality is established by purchasing credits from Plastic Bank. A credit represents the collection and conversion of one kilogram of plastic that may reach or be destined for waterways. CooperVision purchases credits equal to the weight of plastic in our one-day contact lens orders in a specified time period. One-day contact lens plastic is determined by the weight of plastic in the blister, the lens and the secondary package (outer carton), including laminates, adhesives, and auxiliary inputs (e.g. ink).
