Greetings to our readers throughout the beautiful Pacific Northwest! It’s our Spring/Summer Edition, so “Happy Spring and Summer to You!”

As I took to pen in writing this note, it was a rainy Saturday…and I was feeling a bit sleepy. I realized that I needed to get out and get some fresh air to wake up my brain. There is a senior couple that walk and bike daily by our country road on the edge of town. Every day I see them getting exercise and fresh air in all manners of weather year-round – rainy, sunny, cold, hot – they never seem to miss a step or cycle no matter the weather. They inspired me to get off my duff and get moving! So, I dressed warmly, grabbed my gloves and coat, put on my helmet, and jumped on my bike. That half-hour bike ride in the rain brought me fresh air for some clear thinking. Guess I need to make it a daily routine.

I enjoy venturing out with my wife, family, and friends to new and old destinations. Getting out of the house helps us feel more alive. Not that we can’t feel alive at home, but it’s good for the brain to mix things up a bit. Here are some outings we like, pending on our stamina for the day: drive to a local park, walk or bike on a park trail, take a hiking or biking adventure, or join an exercise or walking group. Later this year we hope to book an airplane flight. “The sky is the limit!”

Other ideas to get some vibrancy and social activity into our lives may include local entertainment outings, such as live theatre, music concerts, dance clubs, etc. If you have family, grandkids, or a friend in the area, invite them to join you on a special outing.

This is why the plank is most effective at building core strength and protecting the low back and spine.

Diabetes and the Health of Your Feet

Did you know that according to the American Diabetes Association, 29.2% of Americans age 65 and over have diabetes? Additionally, up to 10% of diabetics develop foot ulcers and up to 25% of foot ulcers lead to limb amputation within 6 to 18 months of initial evaluation. These numbers only increased during the COVID-19 pandemic due to diabetics deferring their health care, with many primary care physicians and podiatrists reporting their case load of toe amputations increasing by as much as 50% in 2020 and 2021. A study conducted at the US Department of Veterans Affairs in Seattle a few years ago found that 30 percent of amputations started as improperly fitting footwear, which can lead to ulcers and amputations.

The Therapeutic Shoe Bill

We don’t have a cure for diabetes. But fortunately, we know how to properly take care of diabetic feet which are subject to complications caused by diabetes. In 1993, the US Congress passed the Therapeutic Shoe

Bill, mandating Medicare to pay for therapeutic shoes and inserts for qualifying diabetics with foot conditions such as poor circulation, neuropathy, and callousing. Most private insurers have followed Medicare’s lead and cover therapeutic shoes and inserts with little or no coinsurance.

Even with this benefit, fewer than 20 percent of eligible beneficiaries use the Medicare benefit provided in the Therapeutic Shoe Bill to receive therapeutic shoes and inserts. There are two primary reasons for this: 1) lack of patient and doctor awareness, and 2) lack of providers with the expertise to properly fit the shoes, manufacture the accommodative inserts, and carry the patient through the necessary doctor and insurance paperwork.

Saving Diabetic Toes and Feet

Raising awareness of this benefit is the purpose of articles such as this. There are a number of providers who are able to fit and provide therapeutic footwear, including most podiatrists and companies like Priority Footwear, the only company in the Pacific Northwest that is exclusively dedicated to the mission of saving diabetic toes and feet.

Eating for a Healthier Heart: Tips from a Cardiologist

Aplant-based, healthy diet can “make a huge difference, not only in preventing heart disease, but also treating it and possibly even reversing it,” according to Cardiologist Jammie Menetrey, DO, MBA, FACC, who practices at Skagit Regional Health.

Whole Food Diet

Dr. Menetrey recommends a plant-based, whole food diet that includes more vegetables, fruits, whole grains and legumes, such as peas, beans and lentils.

“Eating this way goes beyond just helping to improve our heart health, it has positive implications for so many other organs from our skin to our brain health, to our gut and our microbiome,” Dr. Menetrey said.

Foods to Avoid

She also encourages avoidance of processed foods, refined sugars and packaged goods that can be high in salt.

“We really should try to avoid foods that are overly processed and things that don’t look like it was just picked off the tree or pulled out of the ground. Food that’s highly refined, such as refined grains that have been milled to the point where we removed all the good fiber and the iron and the vitamins, those are the types of grains that we want to avoid,” Dr. Menetrey said. “[Foods] that come in cans [or] are packaged in

bags and boxes, generally, those are preserved with salt. We really want to try to minimize that. We want to eat the foods that are provided freshly from the earth and have the least amount of processing to them.”

Benefits to Whole Food Diet

Benefits to the plant-based, whole food diet include a lower caloric intake while the nutritional value is higher.

“Because the plants themselves have so much fiber in them, we fill up much more quickly when we’re eating plants, as opposed to sweets, oil, dairy and meat,” she said. “The way I like to think about it is variety. The more variety that I can see on my plate, the types of food, the more color I can see on my plate, the more nutrients I know that I’m going to be consuming.”

Shopping and Food Preparation

Dr. Menetrey offers a few tips for grocery shopping and food preparation:

• At the grocery store, focus on the fresh produce aisle for the vast majority of your purchases.

• Make a big meal or two on the weekends to have ample healthy leftovers for lunches and dinners during the week.

• Plan ahead to avoid impulse purchases of fast food or refined snacks.

Medicare: A federal program that provides health insurance coverage for seniors in the U.S. who have paid into the benefit for a certain number of periods. There are different parts of Medicare that individuals can purchase and individuals must choose which plan type they would like. Typically, individuals become eligible at the age of 65, but individuals under the age of 65 with certain disabilities may be eligible.

Medicaid/COPES: A state program that provides comprehensive care management and helps pay for personal care, home delivered meals, home health care, adult day services, respite, transportation and other services for older adults in their homes. COPES may also help pay for care in senior housing and care options. The care homes that do accept Medicaid/COPES require two years of private pay before allowing Medicaid/COPES funding.

Selling Your Home? Minimize Your Tax!

Most clients we meet with have a loose idea of what Capital Gains taxes are and how it might affect them. You likely know that to AVOID capital gains tax, you need to have lived in the home you are selling a minimum of two out of the past five years. You might also know that if you are single, you get up to $250,000 free capital gains or if married, $500,000 free capital gains on your primary residence which means you don’t pay capital gain tax up to that amount.

But do you know how your capital gains are calculated? What if your gain is more than the free amounts and what rate would you pay? What happens when a spouse passes away, what can you do to minimize the capital gains? So many questions and we have some great answers!

Please note: we always recommend that you speak to your accountant/tax professional for specific advice for your situation. Here are some basic rules to follow though that will help you when you go to sell your home.

Keep Receipts

First, Keep ALL your receipts from any improvements you have put into your home over the years as you can subtract them from

your gain. Your gain is calculated the following way: NET SALE PRICE –BASIS = GAIN

NET SALE PRICE

= Final sales price – all fees to sell (commissions, excise tax, title & escrow fees, etc.). Note: if you have a current mortgage on the property, the remaining balance is NOT considered a fee and is not to be calculated anywhere in regard to your gain.

BASIS = Initial purchase price of the home + all home improvements

For example: Let’s say you are now single and you purchased your home for $200,000 and over the years you put another $100,000 into it with new roof, deck, siding, kitchen, etc. Your BASIS would be $300,000. You sell the home for $600,000 with $50,000 in fees to sell. Your Net Sales Price would be $550,000 in which your gain would be $250,000 and you are in the clear – no capital gains tax.

Set Some Money Aside for Taxes

What if you sell that home for $900,000 instead? Your GAIN would be $550,000. You would get $250,000 of that gain FREE and would owe capital gains tax on the remaining $300,000. You tax rate on that gain would be approximately 15-20% depending on your tax bracket and income (talk to your accountant).

Those taxes would be due the year AFTER you sell, when you file for your taxes, so make sure to set it aside!

If a Spouse Passes Away, Hire an Appraiser

Lastly, it is extremely important that if a spouse passes away, you should hire an appraiser to do a private appraisal as soon as possible. Why? As the surviving spouse, you will “inherit” the property at the value it was worth on the day your spouse passed away. Which means that your NEW BASIS is at today’s market value and not when you purchased it and almost guaranteeing that you won’t pay any capital gains tax if you sell in the somewhat near future.

An Important Note for Your Heirs

One more thing…if you hold onto your home and your heirs inherit when you pass, they will

also inherit at the current market value, also eliminating the capital gains tax if they sell right away.

What about non-primary homes you ask?

Capital Gains taxes are always paid on 2nd/ vacation homes and investment properties. You can defer them on an investment property by taking all your proceeds and purchasing a new investment property. That’s a different topic for a different day with a set of it’s own rules.

Home Care: Supportive care provided in the home by a professional agency. Certified Caregivers provide assistance with daily care needs and companionship. Is Your Deck Safe?

According to the Consumer Product Safety Commission, an average of 33,000 people are injured per year because of structural failure of a deck, porch, railing, or staircase. Now is the time to ensure your deck is safe to enjoy the sun. A little maintenance will go a long way toward your summer enjoyment and safety. Following are some steps towards outdoor safety.

Clutter Is the Enemy of Safety

Most decks and porches don’t have built-in storage. Chairs, cushions, barbeque tools, planters and hoses are some of the items that may end up on your deck, causing tripping hazards. Attractive weather-resistant storage boxes provide a way to stow the stuff and provide additional seating.

Clean and Check the Deck

A winter’s worth of weather will add debris and dirt to the deck. In some cases, mold and mildew can develop, making the deck slippery. Sweep the deck first, getting rid of leaves and dirt.

Once the surface is clear, it’s time to use a deck cleaner for the wash cycle. The cleaning agents TSP and bleach have been used for years, but they are toxic to the user and the environment. Consider using a homemade mixture of vinegar and water or baking soda and water, or an environmentally approved cleaner like Simple Green. Look for the Green Seal to know a product has been certified for environmental and health. Some people like to pressure wash at this point. Before starting, read the warning labels about the recommended pressure to use on wood or composite deck.

Winter weather can damage decks with snow and rain, causing cracks and loosening connections. Look around your deck, including underneath

if possible, checking for loose or rotting boards, rusty connections and screws in the structure. These things need to be fixed right away.

If you don’t have the skill or experience to do this kind of work, find a handyman. The BIAWC website and Angie’s List are great referral sites for qualified contractors.

Test the Railings and Stair Treads

Just like the deck, stairs and handrails can get cracked or loosened over the winter. Make sure that treads are sturdy, with no wiggles. Handrails should be tightly fastened. Harsh weather can make paint blister, causing cracks and splinters on the handrail. Sand these down until they’re smooth and repaint as necessary. Also, add handrails to stairs that don’t have any, even short sections of stairs. They’re good for everyone using the stairs.

Add the Finishing Touch

After cleaning and repairing, consider using a paint that has non-slip texture additive in it for increased safety. Anti-slip stair tread tape is easy to install. A thick adhesive handrail and guardrail protection product make handrails safe. Add lighting to avoid tripping accidents and a nice glow in the evening. Solar-powered sensor lights are great for this purpose.

Teepa’s Suggestions for Successful Visits

When we invite people into our lives with an in-person visit, it can be a bit overwhelming, intimidating, uncomfortable, and exciting all at the same time. Sharing space with someone that we do not usually share space with can present unique opportunities and challenges. The person being visited may believe they should plan to do something special for the visitor. It is also possible that the visitor has a similar sense of obligation or feels the need to offer something more than just their presence. Here are six engagement options to consider for more successful visits:

Offer to show the visitor around your place

This could be a small walk around a room, around the house, around the yard, around the neighborhood, or even around town. It provides both parties with things to see and talk about.

Play games together

Consider what you and your visitor might enjoy having fun doing, and pull together the necessary resources prior to meeting. It may be a tabletop game, a sports activity, or maybe a brain game. If you both agree, consider a group gathering first to take the pressure off of a more intimate one-on-one interaction. If you enjoy a one-on-one connection, then consider what you can do so that each of you is experiencing moments of pleasure and satisfaction. Keeping it friendly should feel right, not overly complicated. If there is too much effort on either person’s part, then it may be time to switch gears and transition to another option.

Provide an interactive gift

The gift can be something to look at, talk about, or do together. For instance, select a construction or craft kit that is within your scope

of comfort. Or, create a photo album that contains images of interest that offer possibilities for connections, enjoyment, shared memories, or conversation. It can be as simple as a sheet of images, a small booklet, or a tabletop book or magazine. Other ideas might include flowers in a bundle that can be cut and arranged, potting items that can be placed on a windowsill, or seeds and containers for planting. Consider the interests and abilities of all involved, so that the gift is enjoyed as much as possible by all parties.

Connect around food and drink

Gather together to share sustenance and fellowship. Whether it’s a full meal or simply a cup of tea or coffee, it allows us the comfort of doing many things with our eyes, our mouths, our senses, our interactions, and our bodies. It doesn’t have to be about consumption. It could involve the preparation of the space, the items to eat or drink, or it might simply involve the actual sharing of the meal. Maybe serving others or cleaning up provides a sense of purpose in connecting.

Look around and enjoy the scenery

Pick a spot and sit. Watch kids at play, a sporting event, dogs in a park, or flowers in bloom. Consider taking pictures or videos to review when you are in your home space – an additional opportunity for pleasure and connection.

Get out and move

Take a hike, walk, a stroll, or even a drive. Explore what you see, hear, feel, or smell. Get an energy boost by getting active, based on your (Continued on next page)

The Evolution of Phones

It all started out with poles and lines along the road and rotary phones on the kitchen counter. Yes, the Party Lines. If an older sister or a neighbor wasn’t already on the phone, I could call my friends in the next town. This was the day of one phone per household and one line per neighborhood. To dial the next county we had to call a phone operator, who was an actual human being! To dial another country the process was the same, except for a lower sound and a higher bill.

As great as it was to “Reach Out and Touch Someone,” these early phone lines had issues. My mother found it disturbing when the neighbor - we’ll call her Mrs. B. - was asking Mom about how one of her kid’s doctor appointment went that morning. Whoa! Outside the immediate household, only the doctor’s office knew about that appointment. Of course, this had been arranged over the phone. Mrs. B. mysteriously seemed to know about it.

There was another neighbor who was eavesdropping from farther down the road. This nosey and noisy kid seemed to think that no one could hear his breathing and humming. We'll call this teenager Kid M. One day Mrs. B. complained to my mother that the Kid M. “always listened in on everyone’s phone calls.” What comes around, goes around.

Now We are Private

Then the “private” lines came in, and Mother thought nothing of the cost! The long-distance fees went up. Alas, neighborhood gossip eased down. No more strange sounds from the Kid M. Finally, we could have a private conversation on the phone.

Remember the "private" Phone Booths? Just put a dime in the coin slot, to call for help anytime.

This worked when we could find a booth and a dime. Would we have had Superman without the old phone booth?

Technology Goes Remote

As noted by the Washington Post, “1973 saw the first cellular telephone call - made from a giant brick of a thing that weighed more than two pounds and provided just 30 minutes of talking time after a 10-hour charging cycle.” (www. washingtonpost.com, September 9, 2014) These were mostly used for military communications.

Sometime in the 1980’s, phones did not stay tied to a wall. Eventually, we could walk away from the wall and keep talking. No more phone cord tangles! This remote freedom was great, until you stepped outside only to lose the phone signal. Bummer.

Call Waiting

Remember the days when our phones let callers hear a busy signal when we were already busy with another call? Now we have Call Waiting, which lets incoming callers hear the “ring” signal, and you hear an incoming clicking sound while you're already talking to someone else. A great way to never miss a call again! Simply push the “hold” button to put the first caller on hold, then push the “answer” button to talk to the next, incoming caller. Is it just me or has anyone else frequently lost both callers this way? Another bummer.

Remote Reaches Beyond the Horizon

The remote phones finally traveled beyond the house, office, neighborhood, and even beyond the horizon. As noted above, first there were the large, expensive cell phones. But it wasn’t many years

before the prices and size came down. Access and usage of today’s cell phones have grown to almost one per adult with constant and instant phone capacity. With this technology comes the ability to send short “texts” to each other. We now can send and call from the middle of anywhere and nowhere. Then there is the Video Chat function…

Phone technologies have significantly expanded our electronic communication options! But what are the results of this personal and social

expansion? I say “social” tongue-in-cheek, because, as a society, our ability to really talk face to face seems to have diminished. At the same time, something out there, called “Google” or “Alexa” or “Cortana” makes me wonder who might be eavesdropping on my electronic conversation. Our modern means of constant communication is littered with advertising hum and social media distractions galore!

Maybe the old Party Lines weren’t so bad after all.
