Phone: 510-679-3469 Email: chagan@tenderrose.com Colleen manages and supports a team of care coordinators while also monitoring client shift reports and care-related updates, and provides ongoing training and general coaching of MCPs. She helps start new client cases, creates individualized care plans, completes assessments of clients and their homes, and participates in team conference calls to make adjustments to care plans. Colleen has a rich and diverse professional background in science and healthcare. She originally studied biochemistry with her sights set on becoming a marine biologist. She later applied the skills she acquired in college to the pathology department at Massachusetts General Hospital where she was instrumental in determining diagnoses for cancer and kidney disease patients. After the birth of her three children, Colleen relocated to the SF Bay Area and several years later found her way to caregiving, appreciating the fexibility the schedule offered her as a mom. Colleen was promoted several times at the traditional home care agency where she worked, spanning roles in scheduling, marketing, and operations. She joined Tender Rose in 2016 as a Memory Care Professional (MCP), was promoted to the role of Care Coordinator in 2019, and again promoted to Director of Care Coordination in 2021.When she’s not working with clients, Colleen enjoys embroidery, singing in her choir, learning to play the fiddle, and spending time with her children.
