I

f you’re currently on Medicare this applies to you if you have a Medicare drug plan or if you are enrolled in a Medicare Advantage plan (HMO or PPO). This does NOT apply to your Medicare Supplement plans; such as Plan F, G, N etc.

The Annual Notice of Change is a document you receive showing if your Medicare plan made changes for the upcoming year.

Some changes that you would see on your ANOC are increases or decreases in your monthly premium, changes to your co-pays, deductibles and the plan’s drug formulary.

An ANOC will list 2 columns, one is what your benefit is for the current year, the other column will show what it will change to in the new year.

The biggest offender of changes prompting people to look elsewhere is the “Stand Alone Medicare RX plans” that people that use Medicare as their primary insurance coupled with a Medicare supplement plan have. These plans seem to always be giving an increase to their deductible, premium and co-pay amounts.

Unfortunately knowing how to go about finding the right Medicare drug plan on their own isn’t something most Medicare beneficiaries know how to do.

Home Safety Advisors: Helping Seniors Live Safely in Their Own Home Longer!

y the time my friend’s grandmother passed away at 92, she had fallen six times. Two of those falls resulted in broken bones - once, her hip. Horrifying, to find oneself in that position. Tripped up by your walker or the corner of a rug. Often, a wet bathroom or kitchen floor. She was in a home for five out of the six falls, and still, it took some time to respond, get her stabilized, diagnosed, and fully recovered. Falls, my friend and her family quickly learned, are the leading cause of injury related deaths in Americans aged 65 and older. She didn’t end up passing because of a slip up directly, but her bones certainly didn’t give her a pass either, in those last days.

Home Safety Advisor’s President Dave Riggle has a similar story, as do so many around the country. The Beauty in Ritual

As we are looking for ways to reduce the environmental anxiety that today’s world causes so many of us, how can we bring more intentional ritual into our lives?

I recommend a great book, The Power of Ritual: Turning Everyday Activities into Soulful Practices by Casper ter Kuile. A Fellow at Harvard Divinity School and cohost of the awardwinning podcast Harry Potter and the Sacred Text, ter Kuile examines how our everyday rituals can help us restore our sense of purpose.

I think it’s my Midwest upbringing when September was a natural transition time signifying summer was coming to an end and the cool days of Autumn were fast approaching.

M

erriam-Webster defines the word “ritual” as the established form for a ceremony; a ceremonial act or action, an act or series of acts regularly repeated in a set precise manner.

For centuries we have used ritual to help us come to terms with the reality of death. It connects us to our cultural heritage and provides significant symbols to help us honor our loved ones and mark their deaths. So often in our grief, we feel a profound sense of “lostness” as we try to navigate a new world without our person physically with us. We rely on ritual to comfort us and to give us a sense of direction at a time when our world seems to be spinning out of control. Ritual speaks when words fail us.

Our rituals tell a story; they tell our story. It’s how we communicate and pass down from generation to generation our cultural and religious traditions, our values, and our beliefs.

Ritual creates connection, something so many of us are desperate to find right now.

Much like when we experience grief, our world feels very fragmented right now. Many of our rituals, our cues, that help us navigate our world have been taken away from us. Some of these include the start of the school year in which kids are actually in the classroom, back-to-school shopping, the first football games of the season, family gatherings celebrating milestones like weddings, anniversaries and birthdays.

Think of all the rituals you have in your daily life; your morning ritual, your bedtime ritual, what you eat, where you shop, how you pray or meditate, how you prepare for a big meeting or a speaking engagement. Even some of our most beloved sports stars have rituals; Michael Jordan always wore his North Carolina college shorts under his Chicago Bulls uniform. NHL players grow “playoff beards” until they hoist the coveted Stanley Cup or get eliminated. Tiger Woods always wore a red shirt while competing on Sundays. Rituals provide us with a routine which makes us feel comfortable and reduces our anxiety.

Now, more than ever, we yearn for community. We want to deepen our connection both with ourselves and with others in a way that is personally meaningful. Many of our daily habits can help us connect; ter Kuile states that reading, walking, eating, resting, and reflecting can become our foundation of a life of deep connection by practicing these in a way that helps us connect to ourselves, to others, to nature, and to our Higher Power. It’s about shifting our mindset to experience these everyday rituals as spiritual practices.

Take a moment to pause this week.

Get away from technology, turn off the news, don’t check that text.

Breathe in deeply, take a walk in nature, truly savor each sip of your morning cup of coffee, connect creatively with one or two people in your life. Find your ritual and embrace it, allow it to help you find connection, a sense of calm and a feeling of control in our chaotic world.

The Hidden Risks and Costs of Selling a Home in Uncertain Times As the residential real estate industry is forced to re-think how it operates during the global pandemic, many homeowners are wondering how to “safely” approach the home selling process in this uncertain time.

S

enior Home Purchase Program (SHPP) offers a unique service which addresses the hidden risks and costs which homeowners now face with the traditional home selling process.

“Selling a home, even in “normal times” can be overwhelming, especially for seniors who have been in their homes for years and feel emotional about moving; and now there is the added concern of health risks during the sales process,” says Rob Perkins, SHPP co-founder.

Senior Home Purchase Program, based in San Diego and now serving Orange County, offers a process with built-in health and safety measures, as well as no seller costs, which is in stark contrast to the traditional home selling model.

“Many homeowners don’t realize the true selling costs of a home sale,” says Perkins. “The SOLD price does not reflect the hidden expenses charged to a seller, significantly affecting their net proceeds,” he adds. Additionally, the many unknowns inherent in the process- showings, cancelled escrows, time on market, logistics of next purchase- all contribute to high anxiety and stress levels for home sellers, especially seniors who often need to move quickly.

Rob and his sister Corinne Ross had this experience with grandparents who had lived in their homes for decades and needed to move. “It took a small village of relatives to manage their home sales,” remembers Perkins.

After a decade in residential real estate investing, Perkins and Ross created the Senior Home Purchase Program (SHPP) in 2015, a niche service offering seniors a transparent and simple solution to home selling.

SHPP purchases homes directly in as-is condition, alleviating the stress, risk, and costs associated with a traditional home sale.

With SHPP, there are no agents, no home preparation, no showings, and no costs to the seller. Homes are sold “as-is” with clutter, unwanted fur-

niture, and all. There are no failed escrows, commissions, closing costs or repairs. The SHPP process is a staggeringly simple one compared to selling on the open market. “The first step is a home visit to gather details about the home, the client’s needs, and their timelines,” Perkins said. Next, SHPP copiously reviews the local market in comparison to the home. “We then meet and present our absolute best offer based on our careful evaluation.

We are transparent in illustrating our analysis and how it differs from the traditional offer. With no costs or fees to the seller, our SHPP offer is very compelling,” he explains.

Transparency, integrity, and client comfort level are top priorities for SHPP. “Most of our customers are referrals, so the trust factor is big for us,” Ross explains. Stella, a former client, shared, “Their honesty, transparency, and diligence made me feel completely comfortable with them.

It is safe to say that I know nothing about pipes. I know a little about drywall, and some about framing; my husband has his general contractor’s license so I’ve picked up a few nuggets of knowledge over the years - albeit in conversation, not in practice.

I have family who live on the East Coast, where pipes seem to burst with some degree of frequency in the winter, but that is the extent of my experience via six degrees of separation. Since our family moved into our “new” home in San Juan Capistrano a few years ago, we have since become something of an expert on experts: those who do and don’t know anything about pipes.

There are a bunch of things I can now catch you up on: leaks, sprinkler lines, lead testing. But truly, all you need to know is this: In many cases, ePIPE can have your pipes restored the same day without displacing you from your home.

McDonnell’s “masterpiece fighting jet,” the F-4 Phantom, rolled off the production line in December 1960, as a fleet defender built to intercept nuclear foes. Delivered to Miramar Naval Air Station, the jet was eventually bound for nearly four decades of service in the U.S. military.

or a rookie pilot, the F-4 could be a little hard to handle, its two-seater capacity requiring a little more of a team effort than fighter pilots were used to; and a nose that rose notoriously slowly off the deck of an aircraft carrier. For those of us who know nothing about planes, let alone what Mach 1 or 2 mean, a slow-going-nose doesn’t sound like much. But for Colonel Gregory G. Raths, USMC (RET) it was as much of a trial as any: with a bit of speed, that slow nose could over rotate to a stall. A rather rude initiation to the Marine Corps, if you ask me.

Luckily, meeting Col. Raths is nothing like stalling out over the deck of a 45,000 ton aircraft carrier, even though his sights are currently set on becoming a U.S. Congressman this year perhaps a task equal to lifting off the deck in a multi-million dollar war machine.

budgets. My first assignment was in Orange County, CA - at El Toro Marine Corps Air Station.”

and three sisters he grew up with in Phoenix, Arizona. The fifth of eight children, Col. Raths had an early view into the life of a serviceman. His father served in the Army Air Corps from 1941-1945, as a bomber pilot during WWII, and flew combat in the European theatre in the A-20, a night fighter and adaptable light bomber mostly used by the Allied forces in World War II. His mother stayed home full time with the family, keeping the household running.

Closed now, El Toro was long part of the vision of a developing community - a vision that Col. Raths would like to see brought back to American towns. Dwight Whiting, Boston-born founder of the town of El Toro, helped establish the first church, negotiated the arrival of the Santa Fe railroad through the valley, and saw the future of the community in its gentleman farmers: working hard and living off the wealth and proceeds of their walnut and apricot groves. Replaced now by shopping malls and a new town moniker, the vision Whiting and Col. Raths share for the country is fading, and Col. Raths believes it is time for a change.

“The American people have had enough,” he says. “We have been deceived, betrayed by our elected officials.

“As a kid, I mowed lawns, delivered handbills, went door to door selling anything from light bulbs to Christmas lights,” he says. Each step around the neighborhood was a step towards a life of service: to his community first as an Eagle Scout, class president, and multi-sport athlete, and eventually to his country - joining the Marine Corps Officer Candidate Program in 1972 after his freshman year at Arizona State University (ASU).

Enduring summer training at Quantico during the summers of 1973 and 1974, Col. Raths also received his Bachelor’s Degree in Business in 1975. By the time he graduated flight school in Pensacola, FL, “Vietnam was over, and the military began drawing down its forces, and reducing its

same politicians who got us into this mess to get us out.

There is only one special interest group I am interested in serving: the people of the 45th Congressional District.”

Col. Raths brings a unique experience and perspective to negotiating Washington’s political scene. Previously having served as Chief of Staff of the White House Military Office taught Raths much about how career politicians function, and he’d like to be a force that changes those functions.

We need new leaders with honesty and integrity” - two traits Col. Raths learned en force through his tours aboard the USS Midway, and later as squadron commander off the carrier USS Abraham Lincoln. “Washington D.C. has proven its ineptitude, and we must not allow this dreadful sequence to continue.”

His new assignment is a personal one: One he believes must be based on solutions, not politics as usual. Courage, rather than the opinion of a special interest group, or the hope of reelection.

Col. Raths has no aspirations to become a career politician - he’s already completed a 30 year military career and spent the past fifteen years in the private sector as a commercial airline pilot, president of an automobile parts distribution firm, and is currently an author and publisher. He is the current Mayor of Mission Viejo and volunteers within his community as a member of the Elks Lodge, president of the Mission Viejo Rotary Club, American Legion, VFW, St. Kilian Knights of Columbus and Vice-Chairman of the OC Board of Supervisors Veterans Advisory Council.

He is running for office because he simply believes that “a drastically new course for this nation must be drafted” and is committed to leading that charge on behalf of his friends, neighbors and fellow citizens in California’s 45th Congressional District.

At Mach speeds, most modern aircraft must compromise in order to maintain handling ability. Supersonic speeds create shock waves, and the stronger the shock wave is, the greater the pressure difference, which causes a sonic boom. While those outside the cockpit are witness to the boom, those inside the aircraft cannot hear it. America is at the edge of Mach speed right now, cruising into the path of governmental dependence and the “damaging impact of complacency and apathy.” Col. Raths would like to take back the controls.

Overseeing military support from the White House is no small task, especially after serving in the military at such a volatile and unpopular time, and Raths is all too familiar with the stresses and victories surrounding military legislation coming from Congress. He would like to make it clear that his next battle is back in Washington, fighting for the people of the 45th Congressional District where he’d like to “fix the mess that has been created by politicians invested in maintaining their power, rather than doing what they know is right based on the principles that made this nation great to begin with.”

For our future and that of our children and grandchildren: the time to survey the scene has passed. Start those engines, ladies and gentlemen.

A SENIOR FOR SENIORS When asked what types of current policy he sees as going in the wrong direction that he would correct if elected to office, Col. Raths points to the “fiscal mismanagement of programs such as Medicare and Social Security.”

“Why are members of Congress receiving pay raises and huge benefits when Social Security recipients are only receiving a 1.6 percent cost-of-living adjustment in 2020?” he asks. “Medicare co-payments, prescriptions and hospital bills are skyrocketing. I will focus on our seniors and see that they are taken care of.

How Can I Tell If I Have Skin Cancer?

There are three main types of skin cancer: basal cell carcinoma, squamous cell carcinoma, and melanoma. Because each has many different appearances, it is important to know the early warning signs.

ook especially for change of any kind. Do not ignore a suspicious spot simply because it does not hurt. Skin cancers may be painless, but dangerous all the same. If you notice one or more of the warning signs, see a board-certified dermatologist right away, preferably one who specializes in diseases of the skin.

WHAT PATIENTS NEED TO KNOW 1. Everyone needs to perform a self-skin exam once a month.

ACTINIC KERATOSIS

An actinic keratosis (AK) is a crusty, scaly growth caused by damage from exposure to ultraviolet (UV) radiation. An AK is considered a precancer because if left alone, it could develop into a skin cancer. AKs are the product of a lifetime accumulation of UV damage and typically appear on areas most commonly exposed sun, such as the backs of the hands, forearms, face, neck, and scalp.

BASAL CELL CARCINOMA

Basal cell carcinoma (BCC) is the most frequently occurring form of all cancers, with more than 4 million cases of diagnosed in the U.S. each year. BCCs may look like open sores, red patches, pink growths, shiny bumps, or scars. Any skin lesion that doesn’t heal, or bleeds with even gentle trauma, such as drying with a towel, is concerning for a BCC.

2. Melanoma can appear anywhere on the skin, including the palms/soles, under your nails, and places...for most, have never seen the light of day (genitalia). For this reason, it is important to look everywhere. Lock the bathroom door, get out a hand mirror, and check, it could save your life.

3. What to look for: ABCDEs, and an ugly duckling A - Asymmetry: If I draw a line down the middle, do the sides match?

B - Border: Is there a nice crisp border, or does the mole melt into the surrounding skin?

C - Color: Moles should be one color, one shade of brown. Is there a variety of shades or additional colors, such as black, red, white, or blue?

D - Diameter: Any mole larger than 6mm, ie the head of a pencil eraser, has a higher risk of becoming melanoma.

SQUAMOUS CELL CARCINOMA

Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the epidermis, the skin’s outermost layer. SCCs often look like scaly red patches, crusty bumps, warts, or elevated growths with a central depression. SCCs may appear suddenly and some forms are tender to touch. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of SCC are diagnosed each year in the U.S., which translates to about 115 cases diagnosed every hour.

Incidence has increased by 200 percent in the past three decades in the U.S., and more than 15,000 Americans die each year from this type of skin cancer.

MELANOMA

Melanoma is a very dangerous form of skin cancer. Melanoma is caused mainly by intense, occasional UV exposure (frequently leading to sunburn), especially in those who are genetically predisposed to the disease. An estimated 178,560 cases of melanoma are diagnosed in the U.S. each year and is fatal for an estimated 9,320 of these patients. Fortunately, if melanoma is recognized and treated early, it is almost always curable.

E - Evolution: Has it changed?

F - The Ugly Duckling Sign: Most people have a “signature mole,” their own personal mole appearance and architecture. Is there a mole that seems different from all the others?

4. Everyone needs a full body skin exam by a board-certified dermatologist once a year, more frequently if you have a family history of skin cancer, a personal history of skin cancer, or live in an area with substantial sunshine.

In the Medicare insurance world we refer to it as an “ANOC” (we love using acronyms unfortunately we’re the only ones that know what they mean).

I

f you’re currently on Medicare this applies to you if you have a Medicare drug plan or if you are enrolled in a Medicare Advantage plan (HMO or PPO). This does NOT apply to your Medicare Supplement plans; such as Plan F, G, N etcetera.

The Annual Notice of Change is a document you receive in the mail (or online if you signed up for that) that explains the changes that your Medicare plan may have made for the upcoming year. Some changes that you would see on your ANOC are increases or decreases in your monthly premium, changes to your co-pays, deductibles, the annual out of pocket maximum and the plan’s drug formulary. (the prescription drugs that the plan gives you access to at a set copay or co-insurance amount) You will receive your Annual Notice of Change every year at the end of September. It’s not a fancy envelope with bright red letters reading, “Open Me, I’m Important to Read” rather it’s generally printed on an inexpensive paper resembling a newspaper, and it usually reads and looks 46 September 2020

like a pamphlet. All plans have some type of change for the upcoming year. An ANOC will list 2 columns, one is what your benefit is for the current year, the other column will show what it will change to in the new year.

Sometimes the change is good; a lower premium, deductible, co-pay or maximum out of pocket known as a MOOP (see, there’s another acronym for you!)

What you want to make sure of is that you are aware and informed when it’s not good news; a higher deductible, higher co-pays or premium or MOOP. At that point, you can decide if it’s going to affect you enough to want to look at making a change to your plan.

In the Medicare Advantage plan world, the HMO plans seem to have positive changes for the most part,

but the few PPO’s out there in Orange County not so much.

The biggest offender of changes prompting people to look elsewhere is the “Stand Alone Medicare RX plans” that people that use Medicare as their primary insurance coupled with a Medicare supplement plan have. These plans seem to always be giving an increase to their deductible, premium and co-pay amounts. Unfortunately knowing how to go about finding the right Medicare drug plan on their own isn’t something most Medicare beneficiaries know how to do.

With state and county guidance continuing to evolve, the district developed flexible plans that can adjust as the pandemic situation changes, allowing families to choose the plan that works best for them. In addition, the district has shifted to a new K-12 digital learning platform.

CUSD hosted 30-minute webinars in August for families to learn the basics of the new Canvas distance-learning platform. Canvas, a pre-K to grade 12 learning management system, will replace the School Loop system.

“Although this school year opened very differently than what we are all used to, we are committed to supporting families and students as we navigate this adventure together,” said Superintendent Kirsten Vital. “We welcomed back over 47,000 young people for the new school year.”

SEPTEMBER IS PROSTATE CANCER AWARENESS MONTH FOUR FACTS TO PREVENT ADVANCED PROSTATE CANCER Robert Pugach, MD

This month is Prostate Cancer Awareness Month. As always, I remind men that it is time to schedule their annual prostate cancer screening exam.

P

lease keep reading to learn why it is important to detect prostate cancer early and why it is a myth that prostate cancer is a slow growing tumor that never causes serious health problems. It’s more than a myth – it’s downright wrong! As we have already passed the midpoint of the year, we have seen more advanced, incurable prostate cancers at Pacific Coast Urology Medical Center and Western States HIFU than ever before.

FACT #1: Once prostate cancer gets out of the prostate it is not curable.

This year, up to 32,000 men will die from prostate cancer and another 50,000 will have either locally advanced or metastatic cancer. Those are frightening statistics and we want to do what we can to prevent our patients from becoming one of those statistics.

FACT #2: You can prevent advanced cancer by getting screened for it every single year. I recommend that most men start their prostate cancer screening at age 45, or at age 40 if there is a family history of prostate cancer.

FACT #3: Screening is easy – a brief prostate examination and a PSA 56 September 2020

blood test. It’s that simple, but you have to be sure you get it done!

Make sure you receive the following as part of your screening:

• For many men, your annual health examination is done by your primary care physician (family practitioner or internist). However, many primary care doctors omit the prostate examination. At Pacific Coast Urology Medical Center and Western States HIFU we see many patients each year with advanced cancers because abnormal blood tests were not acted upon. If your doctor says either PSA level is abnormal, request that he or she send you to a urologist.

• You also need to have the prostate cancer blood test every year, called a PSA. When your doctor orders your annual blood work, ask if it includes both a “total” and “free” PSA. Either one of these can make the difference between finding prostate cancer early or allowing it to spread. That’s how my prostate cancer was diagnosed – total PSA was normal but the free fraction was low. That led to a bi-

ravel for our family has always been an exciting opportunity to explore both far-flung destinations and closer-to-home hidden gems.

During these unprecedented times defined by shelter-in-place and safer-at-home, we - like many families - pressed pause on several planned summer vacations and weekend getaways to destinations farther than an easy driving distance. For Southern Californians, the beaches are open, and the weather is bright and warm well into the fall and few places are as idyllic as Coronado Island to while away a couple of days in the surf and sand.

crowded white sand beaches, a rich, romantic history and laid-back, small town atmosphere. Although locals refer to it as “the island,” Coronado is really a peninsula connected to the mainland by a finger of land called the Silver Strand. With wide leafy streets lined with Victorian homes and Southern California beach bungalows, the island is small enough to walk almost anywhere (it’s only a mile from the San Diego Bay side of the peninsula to the Pacific Ocean). Once we parked upon arrival for a recent stay at the lovely Glorietta Bay Inn, we discovered that Coronado truly is a “car-optional” environment and were able to easily navigate the relatively flat paths on foot and by bicycle.

We chose the Glorietta Bay Inn (www. gloriettabayinn.com), a beautiful

boutique hotel and historic landmark famous for its grandeur architecture, as “home base.” The former private residence of sugar baron John D. Spreckels, the hotel (which features 11 historic mansion rooms in the main original building circa 1908 and 89 contemporary rooms and suites in structures added to the property in later years) is situated across the street from Coronado Beach and a short walking distance from an array of retail shops and restaurants.

Defined by an artful blend of Old World charm and contemporary coastal comfort, the Inn features a scenic outdoor verandah, a storied music room complete with a baby grand piano and a sun warmed lobby that once served as the grand foyer to Spreckels’ home.

Each of the historic mansion rooms are uniquely styled: the Sugar Baron Room, originally the private bedroom of Mrs. Spreckels, features king-bedded room with a large balcony and a spectacular view of Glorietta Bay; the Spreckels Suite the private bedroom of John D. Spreckels, this beautiful room has a living room area with a convenient kitchenette, and a separate bedroom with a king size bed. As our family discovered on similar sojourns to “the island,” it doesn’t take much planning to have a perfect day in Coronado. We had brought our bicycles and started off along Coronado Beach - a picturesque stretch of sand and surf that stretches for over a mile along Ocean Boulevard in front of historic

and glamorous houses. Voted in the top 10 beaches in the USA by the Travel Channel, this long and wide beach is great for walks, runs, beach volleyball or sandcastle building. If you bring along a furry friend, there is a leash-free dog zone at the far end of the beach where pups chase each other in the waves.

Fans of author L. Frank Baum’s iconic classic “The Wizard of Oz” will enjoy pedaling past the quaint yellow Cape Cod-style home (located right across from Star Park) that he rented during his stays and the place where he penned three of the books in the Oz series.

We spent the better part of an afternoon tooling around town, along the Bayshore Bikeway, past Glorietta Bay and the Coronado Golf Course and under the Coronado Bridge (if you continue following the well-marked bike path, you’ll end up at the Coronado Ferry Landing, where a ten-minute ride would bring you to downtown San Diego.)

After our bike ride, we strolled Orange Avenue, a beautiful commercial boulevard that curves back to the Glorietta

Bay Inn, and stopped in Bay Books where staff members leave heartfelt handwritten recommendations under their favorite reads and stumbled upon MooTime Creamery - where locals and tourists alike were lined up for what they all (accurately) claimed was “the best handcrafted ice cream and hand-rolled cones” in town.

We kept the “enjoy the outdoors” theme going through the end of the day, spending the afternoon and early evening playing in the waves and soaking up the sun on Coronado Beach. The kids spent the entire trip outside and active - and much to their parent’s pleasure, off their respective electronic devices.

My husband and I thoroughly enjoyed kicking back on the balcony with a glass of wine each evening and even managed to finish a couple of books we’d been wanting to get to for weeks (you’d think we would have found the time at home, but it seems as though there’s been no shortage of home improvement projects to focus on).

Our weekend away in Coronado was just what we all needed - a change of scenery and experience - at a destination fast becoming one of our favorites.
