CONTINUUM-OF-CARE
Wrapping services around people at every point One of the things that separates us from many other organizations is our Continuum-of-Care model. This practice provides us with the tools, timing, and the flexibility to ensure clients get the support they need, when they need it, and in the environment that works for them. Because every client is different, and they enter our programs and services at different times and stages, it is important that we meet them where they’re at; that might be a hot meal, a place to sleep for the night, somewhere to detox or someone to talk to. Our Continuum-of-Care model removes barriers and allows clients to enter our services and programs–in most cases–with a seamless transition to the next phase. Our programs complement one another and are supported by several other wrap-around services, all designed to take our clients from one level of support to the next until eventually they have completed their transformation. No matter where in the program or which level a client begins their journey, we are here to help. And because of your ongoing support, we can do this 365 days a year.
