What is Quality Improvement?

Quality Improvement is simply about doing things better, safer, more effectively and efficiently.

By placing QI at the heart of everything we do we want to instil a culture where everyone feels empowered to influence change and deliver Brilliant care for our patients.

What does QI mean for you?

The idea of a QI hub initially came through staff feedback. We want everyone to have QI skills relevant to their role; to know how to access support and to ensure we place quality at the centre of everything we do.

QI at RCHT is embedded and supports our whole portfolio of the work we do.

QI Cafes

We are in the progress of re-launching our QI Cafes at Royal Cornwall Hospital Trust. The QI Cafes act like drop in sessions, (at physical locations and held virtually) where you can access advice, signposting or just talk an improvement idea through with us.

Coin Spinning

(physical or virtual activity)

Learning Objectives: At the end of this activity, you will be able to:

• Explain how to conduct small, rapid PDSA cycles.

• Discuss why theory and prediction are critical to learning when conducing a PDSA cycle.

• Collect real-time data for measurement.

Task Explanation:

• Each have one coin

• Each spin the coin at the same time and record your time

• Feedback to the group and see who has the longest time

• Longest time discuss what they did (surface, coin used etc)

• Agree as group 1 change to make to improve the length of time that you can spin the coin from (taking from learning of longest spinner)

• Repeat process etc.

IHI Quality Improvement Games: Learn How to Use PDSA Cycles by Spinning Coins

You may have thought your coin spinning days ended in school, but it turns out the activity can help you learn about theories, predictions, and PDSA cycles. In the first video in this activity, Niñon Lewis explains what participants will do in the game and what they’ll need to play. In the second part, she talks with Rebecca Steinfield, IHI Improvement Advisor, about some of the key learnings from the game. You can show these videos in a group setting to help you facilitate this activity or watch them by yourself ahead of time.

Build A House

(physical or virtual activity)

Task Explanation:

You can see the house you are building, the architect was very specific. It’s a beautiful home, with sleek black lines, grey sash windows, a blue chimney to blend in with a summers day sky. The architect designed the path with a calming curvature using the finest orange coloured brick. To ensure curb appeal she used her favourite flowers –purple pansies. No house would be complete without a yellow welcome sign. This is the house we want you to replicate.

Organisations are often run according to "the super chicken model," where the value is placed on star employees who outperform others. And yet, this isn't what drives the most highachieving teams. Business leader Margaret Heffernan observes that it is social cohesion — built every coffee break, every time one team member asks another for help that leads over time to great results. Systems Thinking: A Cautionary Tale

This whiteboard animation video about systems thinking tells a story of cats in Borneo (a.k.a. Operation Cat Drop parachuting cats) that occurred in the 1950's. It is a reminder that when solutions are implemented without a systems perspective they often create new problems.

We live in complex systems. Systems thinking is important to take into account in our sustainable development to make sure that we take all systems into account before acting and trying to solve our sustainability problems. As a result of not using systems thinking in this story, the World Health Organization decided to parachute live cats into Borneo. “Operation Cat Drop” occurred courtesy of the Royal Air Force and eventually stabilized the situation.
