CFT Quality Leads

We are an NHS community and mental health provider Trust. Around 4,000 people make up the Trust and we support over 500,000 people who call Cornwall and the Isles of Scilly home. During the summer months the demand for our services increases significantly to support people who come here on holiday.

Our quality leads are key members of the operational teams within each service line. Building rapport and relationships in their specific areas, the quality leads are responsible for working alongside senior managers to provide leadership in embedding the quality strategy and patient safety agenda with a culture of continuous improvement. We have 9 Quality Leads employed across the Trust in different areas as detailed below:

The quality leads act as mentors and role models in supporting staff to adopt quality improvement approaches and methodologies. They also support the Trusts larger transformational projects. Recent examples of some of the work they have been involved in includes:

• Self-harm and suicide prevention

• Least restrictive practice

• Gold standard framework for End-of-Life Care

IMPROVEMENT AND INNOVATION TEAM

Torbay and South Devon NHS Foundation Trust serves a resident population of approximately 286,000 people, plus an additional 100,000 visitors at any one time during the summer holiday season. An Integrated Care Organisation since 2015, we provide health and social care services to our people in their own homes or in their local communities. We run Torbay Hospital (providing acute hospital services) as well as five community hospitals stretching from Dawlish to Brixham.

A key function of our team is to provide the infrastructure to grow organisational capability to improve and innovate. We achieve this by providing expert QI and project management methodologies to our six commissioned portfolios of work and through delivery of our Building Capability programme, coaching offer and curriculum ladder. We are an agile team of individuals skilled in improvement science, committed to creating a culture of continuous improvement where everyone is enthused, inspired and confident to implement changes that make a positive difference to safety, quality of care, patient experience and outcomes, sustainability, and staff wellbeing.

We offer a range of support to all staff and volunteers, from an introduction to Quality Improvement methodologies and project management principles through to our bespoke team days and 4-day expert training courses for practitioners and coaches.

Royal Cornwall Hospital Trust: Quality Improvement Hub

Royal Cornwall Hospital Trust provides Acute care to a population of over 500,000 people within Cornwall and the Isles of Scilly with one main Acute hospital and two peripheral sites.

The idea of a Quality Improvement (QI) Hub initially came through staff feedback in 2018. Our QI team members offer advice, support and inspiration on QI initiatives as well as support the major Transformational Programmes within our Trust and systemwide. All staff are welcomed & supported to learn QI tools and techniques.

What is the Integrated Care Board?

Our job is to plan and buy health and care services for the people registered with GP practices in Cornwall and Isles of Scilly. These services include planned hospital care, mental health and learning disability services, urgent and emergency care, children’s health services, community services, NHS 111, a GP out of hours service and some primary care services.

We are part of the wider Cornwall and Isles of Scilly Integrated Care System. This is a partnership of health and care organisations who will work together to plan and deliver services and improve the health of people who live in Cornwall and the Isles of Scilly.

What do the Integrated Care System (ICS) want for Cornwall & Isle of Scilly?

People living in Cornwall and the Isles of Scilly should be able to live happy and healthy lives, to be in control of their future, free from harm and have the same opportunities as everyone else, regardless of where they live. Cornwall & Isles of Scilly integrated care system (ICS) provides the foundation to achieve this by bringing together the NHS, local authorities, the police, the voluntary sector and other independent providers of care and services we all rely upon.

Aims of the integrated care system:

1. Improve outcomes in people’s health, and healthcare.

2. Tackle inequalities in outcomes, experience and access to care.

3. Enhance productivity and value for money.

4. Help the NHS support broader social and economic development.

We want to ensure that providing place-based care is at the heart of everything we do. This means delivering services closest to where people live and involving communities in making the decisions which will affect them.

What we want to achieve?

We want Cornwall and the Isles of Scilly to be a great place to be born, live and grow old. We want it to be a place where people thrive, and live happy, healthy and independent lives, where they are in control of their future, but know we are there to support them when they need us.

Focusing on the priorities set out in our health and wellbeing strategy and the NHS long term plan, we want to support people to stay as healthy as possible for as long as possible and help people to help themselves and each other so they stay independent and well in their community. This means providing joined-up, high quality services that we can all be proud of, with more services delivered in people’s homes or within their community. This will allow us to make the very best use of the funds which are available to us.

One Devon is a collaboration of the NHS and local councils, as well as a wide range of other organisations like the voluntary sector, who are working together to improve the lives of people in Devon. We want to make our system as strong and effective as possible, through partnership working and with the ambition to tackle health inequalities, help communities thrive and achieve the very best for everyone.

One Devon Partnership

The One Devon Partnership is a new committee that includes a range of organisations and groups in the NHS, health, care and voluntary sector who can influence people’s health, wellbeing and care. Its primary aim is to drive integration by producing a strategy to join-up services, reduce inequalities, and improve people’s wellbeing, outcomes and experiences. All partners will be jointly accountable for delivering this strategy by:

• Facilitating joint action to improve people’s health and care, and reduce inequalities

• Influencing wider factors that affect health (like housing) to create healthier environments

• Building a culture of collaboration to promote and support wellbeing, and involve people Provider collaboratives

Provider collaboratives are partnerships that bring together two or more NHS trusts and/or providers of NHS services to work together to benefit their populations.

While providers have worked together for many years, the move to formalise this way of working is part of a fundamental shift in the way the health and care system is organised, moving from an emphasis on organisational autonomy and competition to collaboration and partnership working.

Devon has three provider collaboratives:

1. Peninsula Acute Provider Collaborative (joint with Cornwall and Isles of Scilly)

2. Acute Mental Health, Learning Disability and Neurodiversity Provider Collaborative

3. Primary Care Provider Collaborative (representing all GP practices in Devon)

Local care partnerships (LCPs)

LCPs are collaborations between a wide range of organisations and groups who help people live happier and healthier lives. Many factors affect people’s health and wellbeing from their homes and personal finances to their education and employment. LCPs bring together the organisations and groups that provide these services (and many others) to better support people and communities. Devon has five LCPs across the county.

Primary care networks (PCNs)

All general practices across Devon are working together formally within local primary care networks (PCNs). There are 31 PCNs in Devon, typically serving patient populations of 30,000-50,000. PCNs support each other and offer more specialist services and roles to patients such as pharmacists, physiotherapists, paramedics, physician associates and social prescribers.

NHS Somerset is responsible for commissioning health and care services for our 580,000 residents. We play a pivotal role in bringing organisations together to work as one integrated care system (ICS), for Somerset.

An ICS is a partnership between the NHS, local council, voluntary services and other organisations working together to plan and deliver joined up services to improve the lives of people in our area.

This is our mission:

Driving collaboration, promoting innovation, supporting good health. We will drive collaboration between the NHS, Somerset Council and our voluntary sector partners to improve the health and wellbeing of people in Somerset. We will listen to our residents and work with communities to develop innovative ways to prevent people from becoming unwell, whilst also delivering high quality services when people need them.

We want all people in Somerset to live in thriving communities and have healthy and fulfilling lives from birth, through to adulthood and into old age. We want people to live well for longer than they do now.

NHS Dorset is the public name of NHS Dorset Integrated Care Board (ICB). NHS Dorset will undertake the statutory responsibilities of the clinical commissioning group (CCG) and will also be responsible for planning to meet the health care needs of people and communities in Dorset.

We will work more closely together across the NHS, local authorities, and many others in what is called the Dorset integrated care system.

NHS Dorset will work with others to:

• Improve outcomes in population health and healthcare

• Tackle inequalities in outcome and access

• Enhance productivity and value for money

• Help the NHS deliver broader social and economic development, particularly post the COVID-19 pandemic
