About you… • Educated to NVQ level 4 or able to demonstrate equivalent professional experience. • Relevant experience of working within a health, social care or voluntary sector setting. • Able to build relationships with participants and partners • Good report writing skills • Experience of supporting people with health conditions and other barriers into work About the role… The Work and Health Employment Coach will undertake the following: • Manage a caseload of clients who have experienced common mental, physical and social health problems or long‐term conditions • To carry out a vocational assessment, to identify vocational needs and develop action plans in agreement with the individual. • Ensuring that a participant’s eligibility is checked as outlined in the WorkWell Grant Guidance. • To provide an evidence based, person‐centered and holistic initial assessment of barriers. • To provide a light‐touch holistic support for health‐related barriers to employment. • To provide a provision of a single joined‐up view and pathway into local services to tackle specific needs. • To ensure multiple referral pathways into and onwards from Work Well services across the range of partners to ensure maximum opportunities for people to engage in using WorkWell services. • To provide all information/documentation requested by DWP for the purposes of activities undertaken by DWP to evaluate the effectiveness and success of WorkWell.

Your main duties will include: • Undertaking statutory home visits within timescales and excellent recording and timekeeping skills. • You should have experience and be highly skilled in undertaking direct work with children. • You will be expected to have good knowledge and understanding of the Child Looked After processes using LCS. • You will be expected to prepare reports for and attend Child Looked After reviews, hold monthly Care Team meetings and record them in a timely manner. • You will also attend strategy meetings, stability meetings and be confident and competent in undertaking Child and Family Assessments, Viability Assessments, Special Guardianship Support plans and Assessments, Discharge statements for court and together or apart assessments and completing Child Permanence Reports. You will be responsible for managing your own caseload and will need excellent report writing skills. In addition, you should be able to demonstrate working in a relationship based and trauma informed way committed to supporting our children to achieve permanence in a timely way and supporting them to understand their history and journey in to care, supporting relationships with their families and re‐assessing family members to consider safe reunification planning and reducing the number of children in our care service.
