Patient Care Coordinator
Start your career in healthcare and become an important member of the healthcare team. As a patient care coordinator, you will have an active role working one-on-one with patients to help them meet their goals. This position can be found in clinics, nursing homes, community and non-profit organizations, home health agencies and other healthcare providers. In this program you will learn the essential roles and responsibilities of a care coordinator and how to best communicate, engage and respond to patients’ diverse needs. Pay average is $26 per hour ($54,000 per year).

Intro to Care Management
This first course will cover the essential roles and responsibilities of care coordinators, including documentation, ethics, and effective communication. The focus will be on the perspective of the patient with an examination of the barriers to patient care and how best you can help.

Motivational Interviewing (MI)
During this course, you will learn to apply motivational interviewing techniques when communicating with your patients to help them identify and change behaviors that place them at risk for further health issues. Prerequisite: PCO 10.
PCO 20 / 5 sessions / 15 hours / Zoom / $245 Mon/Wed 6:30-9:30 pm Jul 6-20
We will identify what constitutes a chronic condition and how to work with clients with chronic conditions. The focus will also be on the complexities of addiction counseling and on the key facts behind alcohol and substance abuse. Prerequisite: PCO 20.

Mental Health and Emotional Support
In this course, you will gain a better understanding of the levels of engagement needed when working with patients with mental health conditions. Included are identifying unique barriers to successful outcomes; managing a patient in crisis; and recognizing schizophrenia, bipolar disorder, and major depression. Prerequisite: PCO 30.
