Vet’s Corner Stacey Chappell, DVM Dr. Stacey Chappell is a seasoned veterinarian with 20 years’ experience with small animals. He spent most of his earlier career as the owner and medical director of a veterinary emergency hospital, gaining vast experience in diagnosing, treating critical care patients and performing advanced surgical procedures. After taking over Animal Medical Centre of Greensboro, PA, in August 2014, Dr. Chappell now enjoys forming lasting relationships with his clients and patients based on lifetime preventive care.

Preventive Care: Changing Roles and Setting Goals As a society, we love our pets! There are more than 140 million pets in the U.S. with the number fairly evenly divided between dogs and cats, with about 70 million of each. For 2013 the total expenditure on these pets was a little over 55 BILLION dollars, with veterinary care contributing 15 billion to this number. “Supplies and over the counter medications” contributed a nearly equal amount at 14 billion. Those are HUGE amounts of money. Corporate America realizes it and tries very hard to capture as much of it as possible. The end result is a plethora of products, a bombardment of advertising, and a consumer who is left wondering what is best for their pet. In addressing preventive care here, I would like to offer a new perspective. I seriously doubt there is anyone left in America that does not know that their dogs and cats need “shots” and “deworming” and they sure do wish they could find a flea medicine that worked!! They have bought something different each time when they went to Costco, Walmart, Target, and the local feed store. But THEY STILL HAVE FLEAS!! Dr. Google even mentions “Brazilian oils” to control fleas. Hmmm, do we put the oils on the food, pour them on the pet, or catch the fleas and put them into the oil? This stuff is SOOO confusing! Well, settle in for a few minutes and let’s see if we can’t make this a whole lot easier and more rewarding for everyone. And no, I will not be listing diseases that should be vaccinated against and at what interval. Nor will I offer opinions on which of the previously mentioned “plethora” of products I think work best. But instead, I would like to suggest a way to better help you and your pet navigate this maze by changing your role as primary care giver for your pet, and look at setting goals for what we would all like to achieve with our furry family members. Back in the 1980’s or so we began to use the term “human-animal bond” to define the relationship we have with our pets. Overall, this concept served our pets well. By acknowledging what they mean to us emotionally, their place was elevated to that of a family member rather than just an “owned” animal with a set economic value like a farm animal. For example, if a dairy cow needs $1200

in veterinary care and she can be replaced for $1000, she likely won’t receive that care. Contrast this with the kitten your daughter got for her birthday. If that kitten breaks its leg and is given an estimate of $500-700 for repair, the responsible owner provides for that repair, even though a “new kitten” could be obtained for a seventy five dollar adoption fee….or for free in the local paper. Despite the success of elevating our animals to family member status, I would like to re-evaluate this “human-animal bond” and take it to the next level. Merely owning an animal and loving and caring for it to the best of your ability satisfies the “human-animal bond”. With animal loosely defined as any living creature and pet defined as an animal kept as a companion and cared for affectionately, the new bond we discuss should use the term pet instead. Also, we replace the broad term human with responsible pet owner, defined as someone who is committed to providing for the medical, physical, and emotional well-being of their pet for the duration of their life. To this newly defined relationship of “Responsible Owner-Pet”, we need to add the term “veterinary”. We will define veterinary as not just the veterinarian, but the entire team at a given practice, because we share a common goal. That goal is to utilize all the expertise and product development our profession provides to maximize the relationship you have with your pets. Find a veterinary team that you like and trust and you now have the newly termed “Responsible Pet Owner-Pet-Veterinary” relationship that is paramount to navigating the maze of preventive care. It is within this relationship that you are free to most enjoy your pet as a companion and family member. You no longer need to concern yourself with the specifics of vaccines and which diseases you need to protect against. You don’t worry over active ingredients of flea and tick preventives and which are best, or safest, or their duration of action. Is there an insect growth regulator present or does the product only kill adult fleas? Can I give the same product to my cat too, or is it just for dogs. Oh yeah, my neighbor’s dog had whipworms the last time they went to the vet. Let me Google “whipworms in dogs” so I can learn their life

cycle and how would my dog become infected. CAN my dog even go back to my neighbor’s house? Max has been itching a lot. The part time sales clerk at the local Big Box Pet Store said they just starting carrying a new line of dog food that touts Cassava root and a liver extract derived from the Cambodian fire eel that will stop my dog from itching. Should we buy this food? After all, have you ever seen a Cambodian fire eel itch? Makes sense, huh?...........well no, it doesn’t. You, as a responsible pet owner, should not be a consumer of veterinary products! That is what I want to illustrate. You shouldn’t come into our offices and say I want one of product X and one of product Y. Instead you and your pet become consumers of veterinary services. For example, you shouldn’t buy flea medicine from Costco and vaccines from the feed store and hope you get it right. Rather you ask your veterinarian to provide you with a protocol that meets our goals of protecting your pet from the most dangerous infectious diseases, protecting your pet from both internal and external parasites and the diseases they transmit. Your team will provide age appropriate screenings of body system functions (lab work, radiographs, advanced diagnostics), proper dental care, and sound nutrition. There are many variations to these protocols based on the lifestyle and age of your pet. But again, you don’t even need to worry yourself over that, we will. See how easy this can be? Now, before you berate me for being pompous or dismissive of your input, let me ensure you that is not the case. Our best clients are informed clients. A huge part of this relationship is ensuring that you understand why we make the decisions we make about your pet’s care. And, yes it does sound self-serving when I ask that you see us for your respective products. Again, this is not the case. Let’s not forget the age old adage, “An ounce of prevention is worth a pound of cure”. This has proven to be true SO MANY TIMES. It wasn’t my fault or his fault, just the by-product of some over-exuberant playing! There are many options when it comes to treatment, and no matter which you choose, the cost is in the thousands. Why not talk to your veterinarian about pet insurance and which brand they recommend? Luckily, since Rowan had insurance, the cost was nearly cut in half. So we’ve covered the important things to consider long term. Now let’s go back to the beginning! We push adoption so hard as the perfect choice for getting a new pet, so it’s not surprising if adopters have high expectations of what their new baby will be like. Your adopted friend can fulfill your expectations, but as with all things, not without some effort on your part. It is unrealistic to expect your newly adopted pet to transition into your family seamlessly and become the family member of your dreams overnight. Expect an adjustment period, and don’t expect it to be over quickly. Both your new pet and your family need to adjust to each other. On average, expect this adjustment period to last several weeks while you all get used to each other. The most important thing to remember is that your new friend has had a lot of upheaval in his life, no matter how long he was at the shelter or in rescue. He’s going to have to work hard to feel at home with you, and he will likely make a lot of mistakes. This includes house-training, turning shoes into

chew toys, and re-learning that the couch is not a pet’s personal “fort”. It’s always a good idea to pet-proof your home first- why not just take up cords and put shoes in the closet? Set you and your new buddy up to succeed! When you bring a new pet home- be sure to establish that any pre-existing pet(s) are STILL important. This may mean greeting and feeding your older fur babies first, so that your new pet understands pack order. Upsetting pack order is never a good idea! Rememberthis could take a few weeks. To recap, long term considerations need to be taken into account- is this going to be a forever pet? Uh oh- all of a sudden one of us has allergies- now what? It may be a good idea to allergy test each member of the family first! What if I move- can I take my pet with me? What if something happens to me- where will my pet go? There are many organizations out there with programs to take in pets after tragedy befalls a family. Make sure your pet is included in your will, as well. Another long-term consideration- do I have the time? Can I dedicate time each and EVERY day to play with and exercise my pet or pets? What if I’m going on vacation- am I prepared for the extra responsibilities of pet ownership? If you spend the necessary time with your new pet, they will be well-behaved and you will be well-trained in turn. Don’t think for a second that training doesn’t go both ways- it does! I’ve learned so much from my dogs and my cat, and I accept each new lesson with wonder and joy! Probably the most important words to remember when you adopt a new member of your family are: Patience, Love, and Learning. Your new pet will not be perfect, but with time

Laparoscopic Surgery in Veterinary Medicine

What is laparoscopic surgery? Laparoscopic, or “minimally

invasive” surgery is a specialized way of performing surgery, typically within the abdomen. This type of surgery has long been the standard of care in human medicine, and only more recently has been adopted by the veterinary field. With traditional, or “open,” surgical techniques, one large incision is made to gain access to the abdomen. With laparoscopic surgery, multiple 0.5-1.0 cm incisions are made. At each incision (which are called “ports”) an instrument called a trochar is inserted into the abdomen. A fiber-optic camera is then inserted into the patient’s abdomen, and the abdomen is inflated with carbon dioxide. This provides a working space and increased visibility for the surgeon . The camera transmits high-quality, magnified images to a video screen in the operating room, allowing for precise maneuvering. Specialized, long-handled instruments are used to cut, seal, and remove tissue from the abdomen. This technique allows surgeons to perform procedures with much smaller incisions than traditional surgery.

What are the advantages of laparoscopic surgery in veterinary medicine? Smaller incisions generally mean less pain and faster recovery for pets undergoing laparoscopic surgery instead of traditional surgery. For instance, following a traditional spay surgery (where the ovaries and uterus are removed) veterinarians recommend 10-14 days of restricted activity. Following a laparoscopic spay (where only the ovaries are removed) dogs are back to normal activity in as little as three days. Senior pets benefit greatly from the smaller incisions associated with minimally invasive surgery over traditional surgery, as they can take longer to recover from anesthetic and surgical procedures than younger dogs. Additionally, because of the excellent visualization obtained by the laparoscope during surgery, advanced procedures and diagnostics can be performed that may otherwise be challenging with traditional surgical techniques. For instance, during a laparoscopic bladder surgery (cystotomy) the camera can be used to fully examine the bladder. Small stones or masses can be magnified on the video screen and retrieved or biopsied as needed.

Laparoscopic technique can also be used to perform a procedure called a gastropexy. This is where the stomach is tacked (sutured) to the body wall. This can prevent a condition called gastric dilatation and volvulus (also known as GDV or “bloat”). This condition occurs most often in large-breed dogs like Great Danes, German Shepherds and Labradors, and it is extremely life threatening. In this condition the stomach distends with food, gas, or water and eventually twists into an abnormal position. This causes disruption of the blood supply to the stomach and eventually leads to shock, severe infection and death. Veterinarians often recommend a gastropexy at the time of spay or neuter to prevent GDV from occurring later in the life of the dog. With laparoscopic technique, this can be done in a safe and minimally invasive fashion. Laparoscopic surgery can also be used in a variety of other ways to diagnose and treat conditions that affect our pets. Liver and splenic biopsies, gall bladder removal, and bladder stone removal can all be performed using minimally invasive technique.

How safe is laparoscopic surgery? Laparoscopic surgery is as safe as traditional open surgery. At the beginning of each surgery, the laparoscope is inserted into the abdomen and the surgeon inspects the contents to ensure that the procedure can be safely performed in a minimally invasive fashion. If needed, the procedure can be converted to a traditional, open procedure. All surgeries carry risks, including anesthetic complications, infections, or bleeding. The heath status of each patient should be evaluated prior to surgery, and a surgical plan created that addresses each pet’s individual needs.

Is laparoscopic surgery always better than traditional surgery? Using minimally invasive surgical techniques, most patients experience less pain and faster recovery times. Laparoscopic surgery has become the standard of care in human medicine, and, when indicated, can offer pets significantly better outcomes over traditional surgery. However, not all surgical procedures can or should be performed laparoscopically. For certain procedures, such as those that require opening the intestines or stomach, traditional surgical methods are still required.

What kind of procedures can be performed using laparoscopic How do I know if my pet would be a good candidate surgery? He is being fostered in a home with a toddler and three dogs, so he is used to children and dogs.

was originally found as a stray. She absolutely loves other dogs, kids, cats, really everything and everyone. Kanani could stand to lose a little weight which her foster mom is working on. During her initial vet visit, we discovered that she was also heartworm positive. It appears to be a mild case of heartworms so she should make a full recovery with no foreseeable long term problems. They offer drink specials daily, and are famous for Angus beef burgers and signature sauces. Jones, along with his son Josh, opened The Corner Slice right next door at 600 North Elm Street. This location offers a wide variety of pizzas, subs and salads. …I heard coming from a back corner the frantic cries of a kitten. It looked so scared and bedraggled; it was the first time I had ever been afraid to pick up a kitten.

e have something,” were the first words I heard from my brother as I exited my car one afternoon. I always stopped by after work to feed the two cats we share at our old home place where he still lives. He led me over to an old dilapidated outbuilding, where I heard coming from a back corner the frantic cries of a kitten. It looked so scared and bedraggled; it was the first time I had ever been afraid to pick up a kitten. Fearing if I took the time to find some gloves, I’d lose it if it darted through a nearby hole; I mustered up the courage and picked it up. It was light as a feather, not much more than skin stretched over sharp, protruding bones, with pus filled eyes, and a putrid odor. It looked about a month old but its ears and back legs told me it was probably older, just stunted from malnourishment. Upon examination I could tell it was a female. Fortunately, it was still strong enough to eat some canned cat food. “Where did you come from?” I asked her. I couldn’t imagine. She was so little. I searched the other outbuildings for her mother or other siblings and found none. The only possibility was she was “dumped” as all of our other pets over the years have been. There are still people in this world

that think they are doing their pets a favor by letting them out to try and find someone to take them in. The problem is, by the time we’ve been able to adopt ours, they’ve either lost an eye, or have contracted feline leukemia or AIDS or like the last three have stunted growth with its complications. This little kitten had almost died of starvation. It was early evening, so I wrapped our new found friend in a towel and made a bed for her in the chair by where my brother sleeps not knowing if she would live through the night. My brother had just left a job he had been at for the last 15 years. His friends were there. Work was all he knew. The transition was hard for him. He is in his early 60’s, had worked constantly since he was a teenager and is one of these people who thrive on work. He was moody and heading for some serious depression. “What did you eat today?” I’d ask while keeping a watchful eye on him. “I had a few sips of Coke and a candy bar” was becoming too frequent an answer to my question. How long had it been since I had heard his usual, quick laugh? The kitten’s sudden appearance seemed to upset him, which surprised me, since he had worked with a feral colony in the past and helped tame the kittens that were

found so they could be adopted. He had often hinted about wanting to bring one home but never did. “Do what you think is best” he said not offering to adopt her himself. I knew my spitfire of a cat at my house would never tolerate her. “Weren’t our lives complicated enough?” I asked the kitten while holding her in a towel the next morning, as I tried to decide what to do with her. I had called around and been told all the rescue groups were full up on kittens and my only option may be a “kill” animal shelter. My brother came and sat beside me “What are you going to do with her?” he asked, seeming to soften. “I have to get her some medical help,” I answered. Her tail was being eaten by maggots. When I bathed her that morning with a warm soft rag it turned rusty from the blood of flea infestation. I had figured an amount of the most I could scrap out of my pitiful savings to try and save her and if that was not enough I would have to take her to the animal shelter. “Well, she can stay around here a few days. We don’t even know if she may belong to a neighbor,” he answered. “Her name is Amanda.” I told him. “So you’ve already named her,” he replied with a slight smile on his face. We both knew then we would keep her. A few days later the first time I heard

my brother laugh again was over one of Amanda’s antics. After that she became a constant joy and companion to him and a comic relief for me as I watched my brother slowly pull out of his depression. Deep down, I was relieved when I took down the signs in the neighborhood that no one responded to. I think Amanda doubled in size the first week we had her and since has doubled a couple of more times. She not only edged her way into my brother’s heart, but into his bed. Being the old confirmed bachelor he is, I’ve known the nights at times are

long for him. There is something comforting in being able to reach out into the dark and touch a warm, breathing thing, next to you, even if it is just a mite of a kitten. To hear it’s little “motor” start up in response. And I’m sure Amanda is comforted when she sprawls out on a warm breathing chest with a heart beating like the mother’s she once knew. I later looked back and saw a prayer I had written in the daily journal that I keep, “Oh Father, I release my brother to you tonight. Please send an angel to comfort him. He’s losing ground…We need a miracle.”
