Reliability Great Catch of the Month Joseph Szgalsky, M.D.

Inspira Medical Group Primary Care Glassboro This Great Catch highlights the significance of Applying a Questioning Attitude using the High Reliability Tool STAR (Stop, Think, Act, Review). In a recent incident, Inspira Medical Group (IMG) discovered that several Diabetes Eye Testing results did not meet the Quality Metric due to a reporting error regarding test dates (performed versus resulted). In response, the IMG Clinical Training team distributed additional educational resources via the IMG e-bulletin, reaching both providers and staff. Dr. Joseph Szgalsky received this bulletin, reviewed its contents and Applied a Questioning Attitude. He identified an issue with the wording on the ad hoc form, which inaccurately classified the new Retina-Vue testing as a dilated exam. Using STAR (Stop, Think, Act, Review), Dr. Szgalsky took the following actions: • Stop: Stopped and reviewed the bulletin and tip sheet. • Think: Thoughtfully identified the wording discrepancy on the ad hoc form.

• Act: Acted by emailing the IMG Clinical Training team and notifying them of the error in Cerner. • Review: Acknowledged the potential issues the wording in the form could lead to with CMS for falsely reporting a dilated exam when Retina-Vue is not a dilated eye exam. The IMG Clinical Training team used Dr. Szgalsky's insights to address the issue. They communicated with Ambulatory Cerner analysts and Value-Based Care team members to change the wording on the ad hoc form from "Dilated Retinal Exam Date" to "Diabetic Retinal Exam Date," aligning with the actual RetinaVue exam and ensuring compliance with the measure. Dr. Szgalsky's use of High Reliability Tools prevented IMG staff from continuing to use an inaccurate form, ultimately leading to error correction. His Questioning Attitude and STAR Moment exemplify the importance of Teamwork in ensuring accuracy and patient safety.

Great Catch of the Month Shannon Cronk Scrub Tech, Labor and Delivery, Inspira Medical Center Elmer This Great Catch highlights the importance of consistently Paying Attention to Detail using our High Reliability Tool STAR (Stop, Think, Act, Review) to ensure fall prevention measures for newborns. Shannon Cronk, a scrub tech at Inspira Medical Center Elmer Labor and Delivery, was Paying Attention to Detail when she noticed a support person walking in the hallway while carrying a newborn. Shannon immediately took a STAR Moment to Speak Up for Safety, recognizing the potential fall risk for the newborn. • Stop: Shannon paused upon witnessing the support person walking in the hallway with the newborn. • Think: Shannon thought about the risks of walking in the hallway with a newborn and the potential harm that could arise from a trip or accidental dropping of the baby. • Act: Shannon Spoke Up for Safety and provided education to the support person.

• Review: The unit's care team diligently worked to mitigate risks to the baby, delivering education on fall prevention to both the support person and the mother. Additionally, the care team shared the incident with their manager and the entire unit for heightened awareness and collective learning. Had Shannon not taken a STAR Moment to ensure all fall precautions were in place for the baby, the baby could have potentially been harmed. This is why we must be Paying Attention to Detail using our High Reliability Tool STAR (Stop, Think, Act, Review) to reinforce our fall prevention measures for newborns. Thank you Shannon for Paying Attention to Detail using our High Reliability Tool STAR (Stop, Think, Act, Review) and living our Inspira Value of Reliability.

DAISY Award Winners The DAISY Award® recognizes outstanding licensed nurses for their skillful and compassionate care.

Mary MacNeil, R.N. Pediatrics, Inspira Medical Center Vineland “Nurse Mary went above and beyond to make sure we were comfortable during my son’s stay. She even brought coloring books and crayons for him and his little brother.” –Patient’s parent

Dolores Ziegler, M.S.N., R.N. Adult Mental Health Unit, Inspira Health Center Bridgeton “Ms. Dee advocated (along with the other nurses) on my behalf when I wanted to be released a day early so I could see my younger brother before he moved to Illinois. She helped me with coping skills I could use in my everyday life and she was there for me whenever I needed to talk or had a question. She has helped me over the last three years be a better person and inspires me to want to become a better advocate for myself and others in the community. I would not be where I am if it wasn’t for Ms. Dee and her words of encouragement.” –Patient

PETAL Award Winner

Every DAISY has its petals. The PETAL Award is a recognition program that honors the performance, dedication and diligence of Inspira’s nursing support partners.

Kevin Hills Security, Inspira Medical Center Mullica Hill “Kevin reinforces our mission and exemplifies our I.C.R.E.A.T.E. values by treating both patients and staff with compassion. He was instrumental in assisting with a long-term patient on our unit and visited every day he worked. He brought the patient coffee, clothes and decorations and was concerned with the patient’s well-being during the months he was here. Not only did Kevin show compassion and empathy toward the patient, but during each visit, he asked the staff how they were doing, offered his contact number and assistance and was always present during our most difficult situations.” –Colleague
