SUCCESS STORY

City Net received a call from Downey city officials regarding a veteran who was at risk of homelessness due to an unexpected notice to vacate where he had been renting. City Net met with the client right away, explaining that case managers would be able to assist immediately, but he expressed that he wasn't ready to enroll as he still had to determine a plan for his belongings The following week, the client met City Net case managers at the library, explaining that he had just lost his home that day, (continued on page 3)

Teams worked in these spots the most this month, Dippity Donuts on Firestone Boulevard and the Downey City Library on Brookshire Avenue. The team recognized that these spots have more people experiencing homelessness and have placed many referrals to the Salvation Army Bell shelter by conducting outreach in these areas.

The case managers secured a space for the client at the Salvation Army Bell Shelter where he could stay for a few days as his case management team explored other options. By the end of the month, he was transferred to a Veteran Transitional Housing Program that would better help him navigate his period of homelessness and connect him to benefits that would assist in his housing search.

• When an older gentleman reached out to the city for shelter assistance, City Net connected with him and arranged a meeting with the Homeless Solutions Manager of Downey. The client explained how he was looking to stay in the area, as he had family nearby and a part-time job in Downey While the case managers attempted to locate a shelter placement, they had no luck and worked with the client on other options. At the time, the gentleman was living out of his vehicle, and since it was still in good working condition, he agreed to try out a shelter that was further outside of Downey, but would still be an easy commute. Once he agreed, the team secured a bed reservation at the Salvation Army Bell Shelter and met him at the shelter for his intake appointment By the end of the month, the client had settled in and begun working on housing navigation with the shelter staff.

Positive exits indicate clients moved off the streets or out of shelter into a more stable living situation.

Temporary housing includes rehabilitation programs, institutional care facilities, and all transitional housing placements.

Permanently housed clients rent a private residence where they may receive ongoing subsidies or have moved into a residence with family or friends permanently.
