SUCCESS STORY

A family of five had been working with City Net for three months to get into a family shelter after being referred by Downey Police after they had been engaged while sleeping in their vehicle. The family was eager to enroll with City Net and accept formal services and resources. Case managers immediately referred the family to the Whole Child while helping the parents set up and utilize their Cal Works benefits (continued on page 3)

Teams focused their outreach along Firestone Boulevard as City Net received many dispatch calls pertaining to individuals who were staying along the boulevard. In an effort to connect more new clients with services, case managers focused on engaging new individuals who were experiencing homelessness and staying in front of buildings and at transportation centers downtown.

• (Continued from page 1) While waiting to hear back on their shelter options, the family received motel vouchers through Cal Works and found a place to stay while waiting for their shelter placement. City Net also facilitated having the family's vital documents renewed and collected together before entering the shelter. At the end of April, the family connected with The Whole Child, and, thanks to having all of their essential documentation ready, they entered a family shelter aimed to connect them with permanent housing.

• City Net had been working with a woman for 6 months who has worked tirelessly to meet her housing goals. In the past, the client had been enrolled in another program aimed at connecting her to a temporary shelter, but it had not been a good fit for her. But in her time enrolled with City Net, she became document-ready and received assistance in setting up her public benefits After several months of support from City Net through services, she reached out to say that she wanted to get off the streets and into a shelter. City Net referred her to Little House in Bellflower, which was a relief to the client as it was the city where she had grown up, and the familiarity would be a source of stability for her Case managers were able to complete her screening and provide transportation to the residential program all on the same day.

Positive exits indicate clients moved off the streets or out of shelter into a more stable living situation.

Temporary housing includes rehabilitation programs, institutional care facilities, and all transitional housing placements.

Permanently housed clients rent a private residence where they may receive ongoing subsidies or have moved into a residence with family or friends permanently.
