SUCCESSSTORY

AseniorcouplereachedouttoCity Netwhentheywereunabletopayfor theirmotelstayafterlosingtheir apartmentinFebruary2025.Case managersimmediatelywenttowork enrollingtheclientsintotheprogram tobeabletobridgethematthemotel inwhichtheywerecurrentlystaying. Aftersecuringtheirimmediate shelter,thecouplebeganapplyingfor seniorhousingopportunitiesbutwere unabletomeet(continuedonpage3).

TheteamreceivedvariouscallsfromthecityforserviceupanddownLakewoodBlvd.Anotherstreetthatreceived multiplecallsforservicefromthecitywasFlorenceAve.Theteamengagedthefolksonthesetwoboulevards,but manyindividualsrefusedservices.CityNetpartneredwiththecityofDowney’sHomelessSolutionsManagertofind andengageindividualsonthelistedstreets.

•(Continuedfrompage1)theincomerequirementsoraffordapplicationfeesinseveralcases.So,thecasemanagers researchedotheraffordableseniorhousingunitsthattheywouldqualifyforandarrangedapplicationappointmentsforthe couples.CityNetprovideddirecttransportationfortheappointmentsandpaidtheapplicationfeesforthecouple,togive themtheirbestchanceforsuccess.Totheirsurpriseanddelight,theywereapprovedsoonafterandreceivedtheirmove-in dateforearlyJune!

•Casemanagerscollaboratedtoassistaclientwhowasworkingonobtaininghisessentialdocumentstoapplyforgovernment benefits,achallengewithoutareliablewaytoconnecttotheinternet.Whiletheteamwasmakingprogress,theclient unfortunatelydisappearedforamonthwhileenrolled.Whiletheteamlookedfortheclientduringoutreach,theyonly receivedwordabouttheclient'swhereaboutsinlateMay,fromthePacificClinicsTeam.Theclienthadmettheircaseworkers inLaMirada,havingnorecollectionofthepreviousmonthsduetosufferingaseizure.Aftertheclientrecoveredinahospital, PacificClinicstransportedthemtoDowneyCityLibraryandmetwithCityNetcasemanagersagain.Thecasemanagers offeredashelterplacementtohelptheclientachieveasenseofstability,whichhereadilyacceptedandcompletedtheintake appointmentonthesameday.Thankfultohaveashelterwherepeoplewerelookingoutforthem,theclientisnowreassured thattheywillmakeprogressinachievingtheirgoalsandobtainingtheirbenefits.
