Benefits of pregnancy

We know you have lots on your mind, but don’t forget your pregnancy rights and entitlements. As an expectant mother, you are entitled to paid time off for antenatal appointments, including midwife and medical appointments and doctorrecommended appointments. Your baby’s father or partner can take unpaid time off to accompany you for two appointments, each capped at six hours and 30 minutes. You may also qualify for a one-time payment of £500 as part of the Sure Start maternity grant (or Best Start grant if you are in Scotland). If you are an employed pregnant woman with average earnings of at least £123 per week and have been working with your employer for a minimum of 26 weeks, you are entitled to a year’s maternity leave, along with 39 weeks of statutory maternity pay. If you cannot claim this, you may be eligible for maternity allowance.

Pregnancy can be truly beautiful. It is a time that is often celebrated by society. However, alongside the physical changes, the journey to motherhood can also change your mental health. While every woman’s experience is different, there are some common mental health challenges that many expecting mums may encounter.

MOOD SWINGS

Hormonal fluctuations during pregnancy can lead to mood swings, ranging from elation to tearfulness. These mood swings are entirely normal and are often a result of the body adjusting to surges in hormones. However, as a pregnant mum going through this, you may often feel ‘out of control’ and find it hard to keep up with this emotional rollercoaster.

LOW MOOD OR DEPRESSION

For some mums, you may have periods of consistent low mood and lack of motivation. Many pregnant mums find it difficult to adjust to being pregnant. It can feel really difficult when many of the usual coping strategies that you would use to maintain your mental health, such as exercise or socialising, can be limited. Although this can be true for

anyone during pregnancy, this is made worse if you are experiencing health conditions such as pregnancy sickness.

STRUGGLING TO SLOW DOWN

Pregnancy is a time that most of us would benefit from slowing down from our usual fast-paced lives. However, as a society, we are not taught to listen to our needs and to take things

slow. Often, it may feel like your self-worth is equated to how productive you are. This can make it feel difficult to comfortably take the time that you need during pregnancy. It can feel even harder to slow down if you are working in a fast-paced job, or if you have other children or responsibilities.

Growing another human being comes with a lot of pressure. It can feel overwhelming to navigate medical appointments, remain safety conscious for your baby, keep up with your own physical body changes and learn what foods to eat and avoid. For some, this can lead to anxiety about your health and the health of your baby. It is also common for mums to experience excessive guilt when they perceive that you may have got something wrong.

One of the first things I did when I was pregnant was to seek resources to stay active. I was an athlete, and fitness was a massive part of my lifestyle and my identity. However, I quickly realised that there was so much conflicting information around the subject, and the general advice was to take it easy and leave my active days behind. That answer didn’t sit well with me, and I knew in my gut that it wasn’t the healthy route.

As a pre and postnatal fitness trainer, the concept of safe and effective exercise is at the core of what I do. But fitness during pregnancy isn’t just about staying active; it’s about feeling confident and preparing for motherhood’s challenges, it’s about reducing pregnancy complications and giving your baby a head start in their future health. MAKE A REALISTIC SCHEDULE

Workout motivation often gets derailed when we set unrealistic expectations. You’ll say to yourself, “I’ll work out five times a week”, then when life gets busy, it feels like you’ve missed the mark. Achieving goals motivates us, so make a plan and schedule workouts like unmissable appointments!

The food we eat is directly linked to how we feel, so choose wisely, avoid processed foods, and eat fresh where possible. It’s also important to listen to your body. Pay attention to how it reacts to certain foods and how you feel after eating. It will tell you much about what is good for you and what’s not.

LISTEN TO YOUR BODY

The #1 rule is exercise should make you feel good, and it can be demotivating when it doesn’t. If you’re trying to finish a HIIT class and feel terrible, Maybe you need to slow things down with a restorative yoga practice instead! If your workouts feel forced, let your body (instead of your mind) lead.

START TODAY AND JUST DO ONE WORKOUT

That’s it. Just get out there today for one workout. It can be challenging to remember that feel-good feeling when you haven’t exercised in a while, so do one workout and write down that feeling on a piece of paper so you can go back to it and remind yourself of what exercise gives you.

Knowledge is power, especially during pregnancy, but my final tip is to be cautious when searching the web for guidance. A lot of the information can be misleading and will only add to your worries. Buying for the baby

What you really need

If you're gearing up for your baby's big arrival, here's the ultimate checklist of must-have's.

As if becoming a parent isn’t bewildering enough, you’re suddenly expected to be familiar with lots of babyspecific jargon you’ve most likely never heard before and parenting items you never knew existed. It can be overwhelming working out what you need, and we know that parents-to-be often end up spending a lot of money on baby products that just get left in the cupboard gathering dust.

We regularly ask thousands of parent’s which products they owned – either given or bought – during pregnancy and the first two years of having a baby. We also rigorously lab test a massive range of baby equipment, including bedside cribs, cot mattresses, stair gates, slings and carriers. Find the musthave products you should invest in and what you need to know about them.

Car seat

If travelling by car, you’ll need an infant car seat to bring your baby home from the hospital. Some baby products are okay to buy second-hand, but a car seat is not one of them.

While a pre-owned car seat may look in good condition, it could have internal damage that you can’t see, for example, a

hairline crack caused by dropping it. This could affect the structural integrity, preventing it from adequately protecting your little one in a collision. We crash-test each car seat we review, and we’ve found several Best Buys for around £100, so you don’t need to spend lots to get a safe car seat.

Pushchair

Not all buggies are suitable from birth. You either need a pushchair with a lie-flat seat or, ideally, a travel system-compatible buggy that can take a carrycot or infant car seat. Pushchairs can be expensive and it’s no surprise many parents look to save by purchasing pre-owned ones.

If you’re buying a pushchair secondhand, make sure to look for a safety label confirming compliance with BS EN 1888-2:2018+A1:2022 (the British Standard for safety testing), and if you can’t see one, be wary. Also, check that it hasn’t been involved in a product recall by searching for the product name online along with the word ‘recall’ or by contacting the manufacturer. Although our most expensive Best Buy costs more than £1,200, our testing has uncovered excellent Best Buy pushchairs costing £100-£200.

Baby Monitor

There are three options - basic audio, video and smart. Audio monitors can cost as little as £25 and will help you to hear when your baby needs you. If you want to be able to look at your baby too, go for a video or smart monitor which will cost between £50-£200.

Smart monitors are typically the priciest, but they are more versatile because unlike audio or video monitors, you can check in on your baby via your smartphone when away from home. But if you go for a smart wi-fi monitor it’s essential you set a strong password and activate two-factor authentication to protect yourself from hackers. Also make sure to regularly check for any security software updates too.

HOW TO PREPARE YOUR BODY & PELVIC FLOOR FOR BIRTH

A pelvic health physiotherapist’s top tips

Our focus can be on prepping for how we want birth to look, alongside thinking about what our baby needs; however, we are not often given much information on how to prepare our body for birth. I always say that birth is like a marathon, and pregnancy is the training season before the race. However, in reality, none of us have much time, so what should we focus our energy on?

1WORK ON PELVIC FLOOR EXERCISES THROUGHOUT PREGNANCY

Pelvic floor exercises are important during pregnancy due to the added weight on the muscles as your baby grows. Research has shown that working on pelvic floor exercises throughout pregnancy reduces the risk of incontinence in the 3rd trimester and postpartum.

Ideally, you should be working on pelvic floor exercises daily, remembering, like all muscles, that the pelvic floor needs to relax fully as well as contract.

The best way to do pelvic floor exercises is to follow the steps below:

• Sit or lie in a comfortable and relaxed position.

• Think about holding in wind and squeeze around the back passage.

• Then, think about bringing this contraction forward towards the pubic bone at the front of your pelvis.

• Then, allow the muscle to relax fully.

• Repeat this ten times.

• You should not feel your legs or bottom muscles contracting. Instead, you should feel this movement internally.

PRACTICE PERINEAL MASSAGE FROM 34 WEEKS

Perineal massage is a technique that can be used to gradually stretch the perineal tissues in the weeks running up to birth. The perineum is the area between the vaginal opening and the anus, which is most stretched during vaginal birth and is where most tears occur. Perineal massage can be used to help reduce the severity or likelihood of tearing during vaginal birth. Therefore, it is recommended that all women are informed on how to do it. This is supported by extensive research, so it is worth trying if you haven’t already.

Here is how to do it:

• Sit in a supported and relaxed position.

• Apply a natural oil to the vulva and your thumb.

• Insert your thumb just inside the vagina and gently sweep side to side, applying gentle pressure onto the perineum.

• Then, think of the area like a clock. 12 o’clock at the pubic bone and 6 o’clock towards the anus.

• Hold and stretch for 30 seconds in each direction from 3-9 o’clock. This should feel like an intense stretch but not painful.

• You can repeat this a few times a week from 34 weeks.

Please check with your medical professional if you have any concerns about your cervix or placenta or have a high-risk pregnancy about whether it is appropriate for you to do.

WORK ON DIAPHRAGMATIC BREATHING

The diaphragm is closely related to the pelvic floor, and they naturally move together. Therefore, working on diaphragmatic breathing is not only calming and suitable for managing stress but also has many physical benefits.

Try it using the tips below:

• When sitting, place one hand on your lower ribs at the side and one on your tummy.

Pregnancy is a beautiful and exciting time, but it can also come with its unique set of considerations, especially when it comes to travelling. You will most likely start to question things that you have always taken for granted. As a midwife and travel agent (a funny combination, I know!) I understand the importance of ensuring both the health and comfort of expecting mothers while still encouraging them to enjoy travel.

policies vary between each airline but generally most commercial airlines accept pregnant travellers up to 36 weeks if you are having one baby or up to 32 weeks if you are having twins or more. This is because labour is more likely after 37 weeks, or around 32 weeks if carrying an uncomplicated twin pregnancy.

Please always check with your travel agent or the airline for individual policies. Some airlines may require a letter from your Midwife or Doctor. It is important to

know that if you do get a letter, it is only stating that you are fit on the day you have been seen, although most airlines seem to be happy with this.

My number one top tip for travelling in pregnancy is that you have a great insurance policy and that you take your maternity notes with you. Most destinations will have English speaking Doctors across Europe, Asia and The Americas. In fact, even if you go away overnight in the UK, it is recommended that you have your notes with you.

Eating & drinking on holiday

For many years there has been conflicting research about alcohol in pregnancy but it is now officially advised that it is not safe to drink alcohol at all in pregnancy. This is mainly because your baby’s liver cannot process alcohol in any form and we now know that it can have a detrimental effect on the placenta – your baby’s lifeline!

I recommend drinking bottled water while away from the UK.

As for food, it’s best to follow the same precautions you do at home. I recommend you avoid the following...

WEAR COMPRESSION SOCKS AND KEEP HYDRATED

Although compression socks are not sexy, they are very Important. Long flights, especially during pregnancy, can increase the risk of developing deep vein thrombosis (DVT), a condition where blood clots form in the legs. Compression socks are an excellent way to reduce this risk by promoting healthy blood flow. They increase your circulation, reduce swelling and can make you

These can carry harmful bacteria such as listeria, which can be dangerous during pregnancy. When in doubt, choose pasteurised options.

UNDERCOOKED MEATS AND SEAFOOD

To avoid foodborne illnesses, ensure that all meats, poultry, and seafood are thoroughly cooked. Sorry, but that means some sushi too!

RAW OR UNDERCOOKED EGGS

Whether it's in sauces, dressings, or desserts, it's best to avoid dishes that contain raw or undercooked eggs due to the risk of salmonella.

When travelling to destinations where water quality is questionable, opt for fruits and vegetables that can be peeled or washed thoroughly with bottled water.

STREET FOOD

While street food can offer delightful local flavours, it's essential to be cautious. Choose vendors with high turnover rates, where food is cooked fresh and served hot.

Despite taking all of the recommended precautions, you may still become unwell during your pregnancy. Whether you are at home or away, always seek advice if you have persistent vomiting and diarrhoea.

Experiment with different positions until you find what works best for you and your baby. Whilst you may experience some tenderness, breastfeeding should not be excruciatingly painful. If it is, seek support sooner rather than later. A painful latch could be a symptom of oral ties, which require professional assessment and intervention.

STAY HYDRATED AND NOURISHED

Maintain a healthy diet rich in fresh, nutrient-dense foods and stay hydrated by drinking plenty of water. Breastfeeding requires extra calories and fluids to support milk production, so prioritise self-care and nourishment for optimal lactation. Your milk will remain nutritious even if your diet is far from perfect, but your health will suffer if vital nutrients are not being replaced by the foods that you eat. Taking a prenatal or breastfeeding vitamin or supplement can help fill in gaps in your diet.

PRACTICE SKIN-TO-SKIN CONTACT

Skin-to-skin contact promotes bonding and stimulates milk production. Spend time cuddling with your baby, especially immediately after giving birth and in the early days postpartum, to establish a strong breastfeeding relationship and boost your milk supply.

LISTEN TO YOUR BODY

Pay attention to your body’s cues and trust your instincts as a mother. Rest when you’re tired, prioritise self-care, and don’t hesitate to ask for help when needed. Taking care of yourself is essential for your overall well-being and breastfeeding success.

MANAGE ENGORGEMENT

Engorgement is common in the early days of

breastfeeding as your milk supply adjusts to your baby’s needs. Use warm compresses, gentle massage, and express milk to relieve discomfort and prevent blocked ducts.

STAY PATIENT AND PERSISTENT

Breastfeeding is a learning process for both you and your baby. Be patient with yourself and your little one as you navigate this journey together. Remember that every breastfeeding experience is unique, and it’s okay to seek guidance and support along the way.

Plan for the most important day of your life

If you are reading this then it is very likely you are expecting a baby – wahoo! Huge congratulations. Somewhere during your pregnancy, you will likely be thinking about a birth plan. As a midwife, I have supported with and read hundreds, likely even thousands, of birth plans – so let’s dive in to everything you need to know.

WHAT IS A BIRTH PLAN?

A birth plan is a document you can give to the people looking after you during birth. It contains your preferences for birth and the kind of birth you’d like to have. You should cover everything that is important to you during your labour and birth.

WHY IS A BIRTH PLAN ESSENTIAL?

A birth plan is essential not because you are considering planning your whole labour to a T – as I am sure you know, labour and birth can sometimes be unpredictable. Having a birth plan means that during pregnancy, you’ve thought about your options and the ones you’d like to choose. It means you are going into birth with knowledge. It’s an excellent tool to communicate with your midwife or consultant and will allow you not to have to repeat things as your key information and preferences will be there for anyone caring for you to look at.

WHAT KEY INFORMATION SHOULD I INCLUDE?

This is going to be completely individual to you. What key information do you want your midwife and/or consultant to know? Is it your first baby? What were your previous births like? Do you have any medical conditions the team must be aware of? Is there anything you are nervous about, e.g. a phobia of needles – we can then do things like avoid you seeing them if that would be helpful.

Questions like....

Would you like to be offered pain relief, or would you like to ask for it? Are there certain pain relief types you want/ don’t want? Would you like a waterbirth? If you needed to go to theatre, who would you want to go with you? What positions do you want to try for birth? How do you want to deliver your placenta? Do you want your baby to have the Vitamin K injection? Do you know the gender of your baby? Do you want your birth partner to announce this to you? Do you have any special requirements like needing an interpreter or any religious customs that need to be followed?

HOW SHOULD I WRITE MY BIRTH PLAN?

You can write your birth plan on a piece of paper or use a template. Keeping the points short and sweet can be helpful to ensure it can be read quickly but still thoroughly. One thing I will add is that because birth can deviate from the “plan”, I like to call a Birth Plan your Birth Preferences instead. This is because it’s really important to be flexible in birth and know that sometimes birth can end up being completely different to the original plan. This is when knowledge of antenatal and hypnobirthing can be extremely helpful as they teach you the tools and techniques to feel calm, confident and empowered even if your birth goes from Plan A to Plan B (or even C!). It can also be helpful to ask your care team to follow EBRAN should things not be going to Plan A.

What is EBRAN?

EBRAN is a tool to use during pregnancy and birth to help with your decision making. Maybe you are offered an induction after a growth scan, or during labour, you are recommended a caesarean section or episiotomy. The list is endless. EBRAN is a thought-process tool for you to work through when making any decision to help you remain informed and in control.

There are lots of templates for birth plans available, the NHS has one and The Bump to Baby Chapter has one within their course. Your community midwife will also be happy to help you write yours if you need some guidance, so just ask.

Having this on your birth plan can be helpful to ensure whoever is caring for you knows the process you want to go through should you need to make any decisions during labour.

Supporting your labour & birth

Pip Davies is an experienced midwife, hypnobirthing and antenatal educator, creator of the Midwife Pip Podcast, and most importantly, a mum. She has been a practising NHS midwife for more than a decade and is a pelvic health specialist, Mummy MOT practitioner and pre and postnatal exercise qualified. I firmly believe ALL women can have a positive and empowered birth experience with the correct education, preparation, and support. This does not mean all women will have a nonmedicated vaginal birth. Still, it means that planned or unplanned caesarean births, hypnobirthing, waterbirths, epidural births, assisted births… every single type of birth… and crucially, YOUR birth can be positive.

Your mind and body are so connected, which is critical to understanding when preparing for labour and birth. This means you must prepare and condition your mind and body. If you believe birth will be painful and fearful, that likely is what your body will experience. But if you challenge this,

use tools and techniques to prepare and support your mind and body. We can change this, and birth can be calm and empowering. It is the most incredible time in your life as you embark on the precious journey to motherhood.

It is not about teaching your body how to give birth; it knows how to do that, and women have been doing it for many years and generations. Convincing your mind is the hardest part. The more that you can understand precisely what happens during labour, the more you learn to realise just how incredible your female body is.

It is helpful to understand that there are four stages of labour - the latent, first, second, and third stages. In

the first stage of labour, the main change happening to your body is that your cervix is pulled upwards into the body of the uterus. So, to support this physical change, you can use breathwork, an essential tool in every woman’s labour toolbox. Think about taking long, deep breaths inwards and upwards. Long, slow breaths help suppress adrenaline and tell your body that you are safe and calm- exactly how you want to feel during labour. Then, in the second stage, the main action is your baby moving down the birth canal, so you want to switch the breath to be a more purposeful, active breath in a downwards motion, taking a quick inbreathe followed by a sharp downward focused out-breath. Practising these breathwork and visualisation techniques daily will help you slot into them like second

nature when your labour begins.

I think we must also address the word pain in relation to labour. I like to think we have two different types of pain- pathological pain and physiological pain. Pathological pain occurs when we stub our toe or break a limb. In other words, something goes wrong in your body. We are naturally fearful of this, which makes us tense, causes adrenaline levels to spike, and the pain becomes worse as you continue to cycle through feelings of pain, fear, and tension. Spikes in adrenaline are essential for survival; imagine being in a jungle chased by a lion- it is super helpful in such times that our blood is directed to our arms and legs so we can run away and make an escape. This response, however, is very unhelpful when it comes to labour because it means

blood is directed away from your uterus, suppressing any contractions. So, instead, we need to recognise labour as physiological pain; it isn’t because of illness or injury. It is a normal process that you do not need to fear. The more you can embrace a cycle whereby you feel relaxed and even begin to enjoy the process, the lower your adrenaline levels, the higher your oxytocin levels, and the more you produce a set of hormones called endorphins. Oxytocin is the hormone responsible for generating contractions, and endorphins are your body’s natural painkillers, so they are one for your labour toolbox, and the more relaxed you are, the more you produce.

Remember- proper preparation can transform your birth experience, and you deserve a positive and empowering birth.

Breastfeeding is the most natural way to feed your baby and a great opportunity for you both to bond. Many Mums find that using a breast pump to express their milk can be a great help, as it can allow them to continue offering their baby breastmilk, for longer than they might be able to if putting baby exclusively to the breast. CLARE, WHAT ARE THE BENEFITS OF A SOOTHER?

Well, there are lots of benefits. Soothers can be used for many reasons, but their main aim is to soothe a baby to sleep, that’s been fully fed and winded, but is still finding it hard to settle down to sleep. Yes, I think it’s fine to introduce a soother as soon as it becomes apparent that the baby needs that extra comfort and is unable to settle without one. But it is really important only to use a soother when the baby has had all their needs met. You shouldn’t use it just to

stop your baby crying when they’re crying to tell you that they’re still hungry.

I breastfed my first two daughters beyond two years and currently breastfeeding my third daughter. To me soothers were a great help, but I never use them to replace breastfeeding. Soothers help me settle them down so I can have a break or for practical reasons, like driving the car.

Combination Feeding

BREAST IS BEST? OR FULL THROTTLE ON THE BOTTLE?

In reality, many Mums find a combination of the two offers the most practical way to feed their baby. We asked a trained nurse and midwife for her advice on how to blend breastfeeding and bottle-feeding with confidence.

COMBINATION AND MIXED FEEDING

Combination feeding is all about flexibility and this might be a factor for Mums choosing to opt into mixed feeding once their breastfeeding is established. Whether the Mum might be away from her baby or needs more flexibility with feeding; mixed feeding can be an effective way to combine bottle and breast. For example, if going back to work, some mothers opt to breastfeed morning and

evening and then express their milk at work, so that their baby can be bottle-fed breast milk during the day. If a Mother does not want to express at all during work hours, she should spend a week or so before returning to work gradually reducing feed times during the day to help her breasts and baby adjust to the new routine.

Mothers shouldn’t be discouraged and feel like they need to drop breastfeeding when their lifestyle changes, instead it is far better, in my opinion, for these Mothers to give themselves more options when feeding their baby, rather than to give up breastfeeding altogether. And not only do I think it is fine to give the occasional bottle of expressed milk, but I also think it can enhance the whole breastfeeding experience and offer flexibility.
