Breast Cancer Awareness Every year, October marks Breast Cancer Awareness Month, which is a dedicated campaign aimed at promoting early detection and the prevention of

breast cancer, a disease that profoundly impacts millions of women worldwide

each year. In this issue of the BestPractice Nordic oncology-hematology magazine, we’ve adorned select pages in pink to show our solidarity and to highlight

the breast cancer research being done today.

Over the past 25 years, there have been remarkable strides in breast cancer treatment. Extensive research and the development of innovative therapies have significantly increased survival rates. Compared to earlier, twice as many patients are triumphing over the disease today, and even those who experi-

ence relapses are living longer. Notably, this year commemorates the 25th anni-

versary of the FDA’s approval of trastuzumab (Herceptin), a monoclonal antibody, for the treatment of HER2-positive metastatic breast cancer, alongside the immunohistochemical assay known as HercepTest.

Trastuzumab has revolutionized the landscape of HER2-positive breast cancer

treatment. The development of a diagnostic test to pinpoint the patients who

stand to benefit the most from this therapy marked a pivotal moment in the evolution of cancer treatment strategies. Since the approval of trastuzumab, a

multitude of new drugs with diverse modes of action have been developed,

and the future holds even more promising treatments for the well-being of cancer patients.

As research continues, we anticipate further advancements in targeted treatments, transforming the landscape of oncology and improving patient outcomes.

(ASCO) presents new and groundbreaking research findings in cancer treatment, and in 2023, it was no different. Among the many presentations at this year’s

conference, there was a significant focus on luminal breast cancer, i.e., hormone-sensitive breast cancer that is HER2-negative.

Menopause and Chemo Impact on Ovarian Suppression in Breast Cancer One of the remarkable presentations at ASCO23 came from Richard Gray, MA, MSc, a professor from Oxford, who presented a meta-analysis on ovarian suppression or

CONCLUSION: ASCO 2023 has undoubtedly shed new light on luminal breast cancer and introduced exciting perspectives for future treatment. It is important to remember that while these results are promising, further research and follow-up are required before they can be implemented as standard treatment.

CONCLUSION: Given the increasing prevalence of breast cancer survivors, oncologists and healthcare personnel caring for breast cancer survivors ought to be aware of this excess risk, particularly the increased risk of AML in women treated with chemotherapy. Still, our study highlights that the absolute risk of subsequent diagnoses of hematological cancers remains low.

survivors had increased risk of myeloid leukemia (both AML and CML) 1–<5 years (SIR 1.92, 95% CI: 1.47–2.47) and

Cabometyx «kabozantinib» filmdrasjerte tabletter. Reseptgruppe C. Indikasjoner: Voksne: Som monoterapi ved behandling av avansert nyrecellekarsinom (RCC) som førstelinjebehandling ved intermediær eller dårlig prognose, og/eller etter tidligere vaskulær endotelvekstfaktor (VEGF)-rettet behandling. I kombinasjon med nivolumab til førstelinjebehandling av avansert RCC. Som monoterapi ved behandling av hepatocellulært karsinom (HCC) etter tidligere sorafenibbehandling. Som monoterapi ved behandling av lokalt avansert eller metastatisk differensiert tyreoideakarsinom (DTC) hos voksne, som er refraktære eller ikke kvalifisert for behandling med radioaktivt jod og har progrediert under eller etter tidligere systemisk behandling. Dosering: Monoterapi: 60 mg 1 gang daglig. Bør fortsettes så lenge klinisk nytte sees eller til uakseptabel toksisitet. Kombinasjon med nivolumab: 40 mg 1 gang daglig i kombinasjon med nivolumab i.v. enten 240 mg hver 2. uke eller 480 mg hver 4. uke. Behandling fortsettes inntil sykdomsprogresjon eller uakseptabel toksisitet. Se SPC for dosering av nivolumab. Dosejustering: Midlertidig avbrudd og/eller dosereduksjon kan være påkrevd ved mistenkte bivirkninger. Ved monoterapi anbefales først reduksjon til 40 mg/dag, og deretter til 20 mg/dag. I kombinasjon med nivolumab anbefales først reduksjon til 20 mg/dag og deretter til 20 mg annenhver dag. Behandlingsavbrudd anbefales ved CTCAE grad ≥3 eller uakseptabel grad 2-toksisitet og ved ALAT/ASAT >3 x ULN, men ≤10 x ULN, uten samtidig total bilirubin ≥2 x ULN. Ved nedgang til grad ≤1 kan behandling gjenopptas med redusert dose.

Spesielle pasientgrupper: Ikke anbefalt ved alvorlig nedsatt leverfunksjon (Child-Pugh C). Ikke anbefalt ved alvorlig nedsatt nyrefunksjon. Kontraindikasjoner: Overfølsomhet for innholdsstoffene. Forsiktighetsregler: Hepatotoksisitet: Unormale leverfunksjonstester er hyppig sett. Leverfunksjonstester anbefales før behandlingsstart, samt nøye regelmessig overvåkning under behandling. Ved forverrede leverfunksjonstester bør anbefalte doseendringer følges. Sjeldne tilfeller av vanishing bile ductsyndrom er sett. Immunologiske sjekkpunkthemmere ble gitt, enten før eller samtidig med kabozantinib i alle tilfeller.

Leverencefalopati: Overvåkning anbefales for tegn/symptomer på leverencefalopati. Perforeringer og fistler: Alvorlige GIperforeringer og fistler, av og til fatale, er sett. Bør seponeres ved gastrointestinal perforasjon eller fistler som ikke lar seg behandle adekvat. Gastrointestinale sykdommer: Diaré, kvalme/oppkast, nedsatt appetitt og stomatitt/smerter i munnen er vanlige GIhendelser. Rask behandling, inkl. støttebehandling bør innføres. Behandlingsavbrudd, dosereduksjon eller permanent seponering bør vurderes ved vedvarende eller gjentatte signifikante GI-bivirkninger. Tromboemboliske hendelser: Venøs og arteriell tromboembolisme (inkl. fatal) er sett. Brukes med forsiktighet ved risiko for, eller tidligere tromboembolisme. Pasienter med tidligere infiltrasjon til portvene har høyere risiko for portvenetrombose. Bør seponeres ved utvikling av akutt myokardinfarkt eller klinisk signifikante tromboemboliske komplikasjoner. Blødning: Alvorlige blødninger (inkl. fatale) er sett. Ved tidligere alvorlige blødninger bør pasienten vurderes nøye før behandlingsstart. Bør ikke gis ved alvorlig blødning eller risiko for dette. Aneurismer og arteriedisseksjoner: Kan fremme dannelsen av aneurismer og/eller arteriedisseksjoner. Før oppstart bør risikoen vurderes nøye ved risikofaktorer som hypertensjon eller aneurisme i anamnesen. Trombocytopeni: Er sett. Blodplatenivå bør overvåkes under behandling, og dose endres iht. alvorlighetsgrad av trombocytopeni. Komplikasjoner med sårtilheling: Er sett. Behandling bør stoppes minst 28 dager før planlagt kirurgi hvis mulig. Beslutning om å gjenoppta behandlingen bør baseres på vurdering av sårtilheling. Bør seponeres ved sårtilhelingskomplikasjoner som krever medisinsk intervensjon. Hypertensjon (inkl. hypertensiv krise): Er sett. Blodtrykk bør være velkontrollert før oppstart. Blodtrykket skal overvåkes tidlig og regelmessig, og egnet antihypertensiv behandling gis etter behov. Ved vedvarende hypertensjon til tross for antihypertensiv behandling, bør kabozantinibdosen midlertidig seponeres til blodtrykket er under kontroll. Deretter kan kabozantinib gjenopptas med redusert dose. Bør seponeres ved alvorlig og vedvarende hypertensjon, til tross for antihypertensiv behandling og reduksjon i kabozantinibdosen, samt ved hypertensiv krise. Kjeveosteonekrose (ONJ): Er sett. Munnhuleundersøkelse bør utføres før og med jevne mellomrom under behandling. Behandling bør om mulig stoppes minst 28 dager før planlagt dentalkirurgi/invasiv tannbehandling. Forsiktighet utvises ved bruk av legemidler forbundet med ONJ, f.eks. bisfosfonater. Seponeres ved ONJ. Palmar plantar erytrodysestisyndrom (PPES): Er sett. Ved alvorlig PPES bør det vurderes å avbryte behandlingen. Kan gjenopptas med lavere dose når PPES er gått tilbake til grad 1. Proteinuri: Er sett. Urinprotein bør måles regelmessig under behandling. Bør seponeres ved utvikling av nefrotisk syndrom. Posterior reversibelt encefalopati-syndrom (PRES): Er sett. Behandlingen bør seponeres ved PRES. Forlenget QT-intervall: Brukes med forsiktighet ved tidligere QT-forlengelse, ved bruk av antiarytmika eller ved relevant eksisterende hjertesykdom, bradykardi eller elektrolyttforstyrrelser. Regelmessig EKG-overvåkning og elektrolyttmåling bør vurderes. Tyreoideadysfunksjon: Måling av tyreoideafunksjonen anbefales ved baseline, samt nøye overvåkning for tegn/symptomer under hele behanlingen. Behandling iht. vanlig medisinsk praksis. Unormale biokjemiske laboratorietester: Økt forekomst av elektrolyttforstyrrelser er sett og tilfeller av leverencefalopati ved HCC kan tilskrives dette. Hyppigere frekvens og/eller økt alvorlighetsgrad av hypokalsemi, inkl. grad 3 og 4, er sett hos pasienter med tyreoideakarsinom sammenlignet med pasienter med andre krefttyper. Overvåkning av biokjemiske parametre anbefales. Behandlingsavbrudd, dosereduksjon eller permanent seponering bør vurderes ved vedvarende eller gjentatte signifikante avvik. Graviditet, amming og fertilitet: Graviditet: Skal ikke brukes under graviditet, hvis ikke klinisk tilstand nødvendiggjør behandling. Amming: Skal opphøre under og i minst 4 måneder etter endt behandling. Fertilitet: Kan reduseres.
