Cabometyx on ensimmäinen ja ainoa TKI lääke, jolla on käyttöaihe levinneen munuaissyövän hoitoon monoterapiana ja yhdessä IO lääkkeen kanssa.

HILA korvattavuuspäätös 2022 Edenneen munuaissyövän hoito – monoterapiana tai yhdistelmähoitona nivolumabin kanssa aiemmin hoitamattomille aikuispotilaille, joiden ennuste on kohtalainen tai huono, tai – monoterapiana aikuispotilaille aiemman endoteelikasvutekijään (VEGF) kohdistetun hoidon jälkeen.

CABOMETYX 20 mg, 40 mg ja 60 mg kalvopäällysteiset tabletit. Vaikuttava aine: Kabotsantinibi. Käyttöaiheet: Munuaissyöpä (RCC): monoterapiana edenneen munuaissyövän ensilinjan hoitoon aikuispotilaille, joiden ennuste on kohtalainen tai huono, sekä aikuispotilaille aiemman endoteelikasvutekijään (VEGF) kohdistetun hoidon jälkeen. Yhdistelmänä nivolumabin kanssa edenneen munuaissyövän ensilinjan hoitoon aikuispotilaille. Maksasyöpä (HCC): monoterapiana maksasyövän hoitoon aikuisille, jotka ovat aiemmin saaneet sorafenibihoitoa. Erilaistunut kilpirauhassyöpä (DTC): monoterapiana aikuispotilaille, joilla on paikallisesti edennyt tai metastasoitunut, erilaistunut kilpirauhassyöpä ja joiden sairaus on radiojodihoitoon vastaamaton tai joita ei voida hoitaa radiojodilla, kun sairaus on edennyt aiemman systeemisen hoidon aikana tai sen jälkeen. Annostus ja antotapa: Hoito on aloitettava syövän hoitoon tarkoitettujen lääkevalmisteiden antamiseen perehtyneen lääkärin valvonnassa. Monoterapiana suositeltu annos on 60 mg kerran vuorokaudessa. Hoitoa on jatkettava, kunnes potilas ei enää kliinisesti hyödy hoidosta tai kunnes potilaalla ilmenee liiallista toksisuutta. Edenneen munuaissyövän ensilinjan hoidossa yhdistelmänä laskimoon annettavan nivolumabin kanssa kabotsantinibin suositeltu annos on 40 mg kerran vuorokaudessa. Lääkkeen epäiltyjen haittavaikutusten hoito saattaa edellyttää hoidon tilapäistä keskeyttämistä ja/tai annoksen pienentämistä, ks. valmisteyhteenveto. Iäkkäät: Erityistä annoksen muuttamista ei suositella ≥ 65-vuotiaille. Munuaisten vajaatoiminta: Käytettävä varoen potilaille, joilla on lievä tai kohtalainen munuaisten vajaatoiminta.

Kabotsantinibin käyttö ei ole suositeltavaa, jos potilaalla on vaikea munuaisten vajaatoiminta. Maksan vajaatoiminta: Annosta ei tarvitse muuttaa potilaille, joilla on lievä maksan vajaatoiminta.

Kohtalaista maksan vajaatoimintaa koskevia annostussuosituksia ei voida antaa, kokonaisturvallisuuden tarkkaa seurantaa suositellaan. Kabotsantinibin käyttö ei ole suositeltavaa, jos potilaalla on vaikea maksan vajaatoiminta. Sydämen vajaatoiminta: Erityisiä annostussuosituksia ei voida antaa. Pediatriset potilaat: Turvallisuutta ja tehoa alle 18 vuoden ikäisten lasten ja nuorten hoidossa ei ole vielä varmistettu. Lisätiedot, ks. valmisteyhteenveto. Antotapa: Tabletit niellään kokonaisina eikä niitä saa murskata. Potilaita on neuvottava olemaan syömättä vähintään 2 tuntia ennen CABOMETYX-valmisteen ottamista ja 1 tunti sen jälkeen. Vasta-aiheet: Yliherkkyys vaikuttavalle aineelle tai valmisteen apuaineille. Varoitukset ja käyttöön liittyvät varotoimet: Koska useimmat haittavaikutukset ilmenevät hoidon alkuvaiheessa, lääkärin on seurattava potilaan tilaa huolellisesti kahdeksan ensimmäisen hoitoviikon aikana mahdollisten annosmuutosten tarpeen arvioimiseksi. Seuraavia haittavaikutuksia esiintyy yleensä hoidon alkuvaiheessa: hypokalsemia, hypokalemia, trombosytopenia, hypertensio, palmoplantaarinen erytrodysestesia (PPES) oireyhtymä, proteinuria ja ruoansulatuselimistön haittavaikutukset (vatsakipu, limakalvojen tulehdus, ummetus, ripuli, oksentelu). Ruoansulatuskanavan häiriöiden aiheuttaman kuivumisen, elektrolyyttihäiriöiden ja painon laskun välttämiseksi on välittömästi aloitettava lääkinnällinen hoito. Maksan ja kilpirauhasen toiminta on suositeltavaa tutkia ennen kabotsantinibihoidon aloittamista. Maksan toimintaa, verihiutaleiden määrää, elektrolyyttiarvoja ja kilpirauhasen toimintaa sekä verenpainetta on seurattava hoidon aikana. Maksasyöpää sairastavia potilaita on seurattava maksaenkefalopatian oireiden ja löydösten varalta. Kabotsantinibia käytettäessä on havaittu vakavia ruoansulatuskanavan perforaatioita ja fisteleitä, laskimoiden ja valtimoiden tromboembolisia tapahtumia, vaikeaa verenvuotoa, trombosytopeniaa ja verihiutaleiden määrän pienenemistä, haavakomplikaatioita, verenpaineen kohoamista, hypertensiivisiä kriisejä, PRES-oireyhtymää, proteinuriaa, PPES-oireyhtymää, ja leuan osteonekroositapahtumia. Kabotsantinibin käyttö saattaa edistää aneurysmien ja/tai valtimon dissekaatioiden muodostumista. Potilaat, joilla on ollut tällaisia tapahtumia aiemmin tai joilla on niiden riski, on arvioitava huolellisesti ennen hoidon aloitusta ja heitä on seurattava hoidon aikana huolellisesti mahdollisten oireiden varalta. Kabotsantinibia on käytettävä varoen potilaille, joilla on aiemmin esiintynyt QT-ajan pidentymistä, jotka käyttävät rytmihäiriölääkkeitä tai joilla on merkityksellinen sydänsairaus, bradykardia tai elektrolyyttihäiriö. Suu pitää tarkastaa ennen kabotsantinibin aloittamista ja säännöllisesti kabotsantinibihoidon aikana. Yhteisvaikutukset: Varovaisuutta on noudatettava, jos potilas saa samanaikaisesti voimakkaita CYP3A4:n estäjiä. Lisäksi on vältettävä voimakkaiden CYP3A4:n induktorien pitkään jatkuvaa samanaikaista käyttöä. Samanaikaisessa käytössä varfariinin kanssa on seurattava INR-arvoja. Kabotsantinibi saattaa suurentaa samanaikaisesti annettujen P-gp:n substraattien pitoisuuksia plasmassa.

Potilaita on varoitettava P-gp:n substraattien käytöstä kabotsantinibihoidon aikana. MRP2-estäjien (esim. siklosporiinin, efavirentsin, emtrisitabiinin) käyttö saattaa suurentaa kabotsantinibin pitoisuuksia plasmassa. Hedelmällisyys, raskaus ja imetys: Kabotsantinibia ei pidä käyttää raskauden aikana, ellei raskaana olevan potilaan kliininen tilanne edellytä kabotsantinibihoitoa.

Mies- ja naispotilaiden sekä heidän kumppaneidensa on käytettävä tehokasta ehkäisyä hoidon aikana ja vähintään neljä kuukautta hoidon päättymisen jälkeen. Lisäehkäisyn, kuten estemenetelmän, käyttöä suositellaan steroidiryhmän ehkäisyvalmisteita käytettäessä. Äidin on lopetettava imetys kabotsantinibihoidon ajaksi ja vähintään neljän kuukauden ajaksi hoidon päättymisen jälkeen. Kabotsantinibihoito saattaa vaarantaa miesten ja naisten hedelmällisyyden. Sekä miehiä että naisia on kehotettava hakeutumaan hedelmällisyysneuvontaan ja harkitsemaan mahdollisuuksia hedelmällisyyden säilyttämiseen ennen hoidon aloittamista. Vaikutus ajokykyyn ja koneidenkäyttökykyyn: Väsymystä ja heikkoutta saattaa esiintyä ja siksi suositellaan varovaisuutta ajettaessa tai koneita käytettäessä. Haittavaikutukset: Monoterapiana: Hyvin yleiset: Anemia, trombosytopenia, hypotyreoosi, ruokahalun heikkeneminen, hypomagnesemia, hypokalemia, hypoalbuminemia, makuhäiriö, päänsärky, huimaus, hypertensio, verenvuoto (ml. nenäverenvuoto), äänen käheys, hengenahdistus, yskä, ripuli, pahoinvointi, oksentelu, suutulehdus, ummetus, vatsakipu, dyspepsia, PPES-oireyhtymä, ihottuma, raajakipu, uupumus, limakalvojen tulehdus, voimattomuus, perifeerinen edeema, painon lasku, seerumin ALAT- ja ASAT-arvojen nousu. Yhdistelmänä nivolumabin kanssa: Hyvin yleiset: ylähengitysteiden infektio, hypotyreoosi, hypertyreoosi, heikentynyt ruokahalu, makuhäiriö, huimaus, päänsärky, hypertensio, äänen käheys, hengenahdistus, yskä, ripuli, oksentelu, pahoinvointi, ummetus, suutulehdus, vatsakipu, dyspepsia, PPES-oireyhtymä, ihottuma, kutina, luuston ja lihasten kipu, nivelkipu, lihasspasmi, valkuaisvirtsaisuus, uupumus, kuume, turvotus, ALAT- ja ASAT-arvon nousu, hypofosfatemia, hypokalsemia, hypomagnesemia, hyponatremia, hyperglykemia, lymfopenia, veren AFOS-ja kreatiiniarvojen nousu, lipaasiarvojen nousu, amylaasiarvojen nousu, trombosytopenia, anemia, leukopenia, hyperkalemia, neutropenia, hyperkalsemia, hypoglykemia, hypokalemia, kokonaisbilirubiiniarvon nousu, hypermagnesemia, hypernatremia, painon lasku. the breast cancer research being done today.

Over the past 25 years, there have been remarkable strides in breast cancer treatment. Extensive research and the development of innovative therapies have significantly increased survival rates. Compared to earlier, twice as many patients are triumphing over the disease today, and even those who experi-

ence relapses are living longer. Notably, this year commemorates the 25th anniversary of the FDA’s approval of trastuzumab (Herceptin), a monoclonal antibody, for the treatment of HER2-positive metastatic breast cancer, alongside the immunohistochemical assay known as HercepTest.

Trastuzumab has revolutionized the landscape of HER2-positive breast cancer

treatment. The development of a diagnostic test to pinpoint the patients who

stand to benefit the most from this therapy marked a pivotal moment in the evolution of cancer treatment strategies. Since the approval of trastuzumab, a

multitude of new drugs with diverse modes of action have been developed, and the future holds even more promising treatments for the well-being of cancer patients.

Vyxeos liposomal 44mg/100mg (daunorubisiini ja sytarabiini) Käyttöaihe: Aikuispotilaille, joilla on äskettäin diagnosoitu aiempaan hoitoon liittyvä akuutti myelooinen leukemia (t-AML) tai joilla on AML yhdessä myelodysplasiaan liittyvien muutosten kanssa (AML-MRC). Annostus ja antotapa: Laskimoon. Hoidon saa aloittaa ja sen kulkua pitää seurata kemoterapeuttisten lääkevalmisteiden käytöstä kokemusta saaneen lääkärin valvonnan alaisena. Annostus perustuu potilaan kehon pinta-alaan. Ks. annostusohjelma valmisteyhteenvedosta. Potilaita on seurattava hematologisen vaikutuksen ja toksisuuksien varalta. Yliherkkyysoireiden ilmaantuessa hoito on lopetettava. Antihyperurikemiahoitoa on harkittava ennen hoidon aloittamista. Erityisryhmät: hoitoa saa antaa vaikeaa maksan vajaatoimintaa sairastaville ainoastaan jos siitä saatavat hyödyt ovat riskejä suuremmat. Muut erityisryhmät, ks. valmisteyhteenveto. Vasta-aiheet: Aikaisempi vakava yliherkkyys vaikuttaville aineille tai apuaineille. Varoitukset ja käyttöön liittyvät varotoimet: Valmistetta ei tule korvata muilla daunorubisiini- ja/tai sytarabiinivalmisteilla.

Myelosuppressio ja verenvuodot: jopa kuolemaan johtaneita infektioita ja verenvuototapahtumia on ilmoitettu tapahtuneen valmisteen terapeuttisen annoksen annon jälkeen. Vakavia tai kuolemaan johtaneita trombosytopeniaan liittyneitä verenvuototapauksia, ml. kuolemaan johtaneita keskushermostoverenvuotoja on esiintynyt hoitoa saaneilla potilailla. Verenkuvan analyysi lähtötilanteessa on tehtävä ja potilaita on seurattava huolellisesti hoidon aikana myelosuppressiosta johtuvien mahdollisten komplikaatioiden varalta.

Pitkästä puoliintumisajasta johtuen absoluuttisen neutrofiilimäärän (ANC) ja verihiutaleiden palautumisaika saattaa pitkittyä ja vaatia lisäseurantaa. Infektiolääkkeitä voidaan käyttää ennaltaehkäisevästi vakavan neutropenian aikana, kunnes ANC palaa vähintään arvoon >500/µl. Kardiotoksisuus on tunnettu antrasykliinihoitoon liittyvä riski. Aikaisempi antrasykliinihoito, taustalla oleva sydänsairaus, aikaisemmin saatu välikarsinan sädehoito tai kardiotoksisten valmisteiden samanaikainen käyttö saattaa lisätä daunorubisiinin aiheuttamaa kardiotoksisuutta. Ks. tiedot kumulatiivisesta altistumisesta daunorubisiinille valmisteyhteenvedosta. Sydämen toimintaa on seurattava tiukasti. Hoito on lopetettava potilailla, joilla sydämen toimintakyky on heikentynyt, ellei hoidon jatkamisesta saatava hyöty ole suurempi kuin siihen liittyvät riskit. Yliherkkyysreaktiot: Vakavia yliherkkyysreaktioita, ml. anafylaktisia reaktioita, on raportoitu daunorubisiinin ja sytarabiinin käytössä. Ks. ohjeet infuusion lopettamisesta ja esilääkityksestä valmisteyhteenvedosta. Daunorubisiinin käyttöön on liittynyt paikallista kudosnekroosia ekstravasaation yhteydessä. On huolehdittava siitä, ettei ekstravasaatiota pääse tapahtumaan. Maksan vajaatoiminta saattaa lisätä daunorubisiiniin ja sytarabiiniin liittyvän toksisuuden riskiä. Saattaa aiheuttaa hyperurikemiaa leukemiasolujen nopean lyysin jälkeen. Wilsonin tautia tai muita kupariin liittyviä sairauksia poteneille potilaille käytettävä ainoastaan siinä tapauksessa, että hoidosta saatavat hyödyt ovat suuremmat kuin hoitoon liittyvät riskit. Hoito on lopetettava potilaille, joille ilmaantuu akuutin kuparimyrkytyksen merkkejä tai oireita. Elävien rokotteiden käyttöä hoitoa saaville potilaille on vältettävä.

Maha-suolikanavan limakalvojen tulehdus ja/tai ripuli, joita usein esiintyy intensiiviseen solunsalpaajahoitoon liittyen, voivat vaikuttaa merkittävästi suun kautta otettavien lääkevalmisteiden imeytymiseen. Yhteisvaikutukset: Kardiotoksisuus: Ei saa antaa yhdessä kardiotoksisten aineiden kanssa, ellei sydämen toimintaa seurata tiukasti mahdollisesti lisääntyneen kardiotoksisuusriskin vuoksi.

Valmisteen käyttö potilaille, jotka ovat aikaisemmin saaneet doksorubiinia, lisää kardiotoksisuuden riskiä. Hepatotoksisuus: Maksan toimintaa on seurattava tihennetysti, jos valmistetta annetaan yhdessä hepatotoksisten aineiden kanssa mahdollisesti lisääntyneen toksisuusriskin vuoksi. Hedelmällisyys, raskaus ja imetys: Raskauden poissulkemiseksi naisille, jotka voivat tulla raskaaksi, on tehtävä raskaustesti ennen hoidon aloittamista. Sekä miespotilaiden, joiden kumppani voi tulla raskaaksi, että naispotilaiden on käytettävä tehokasta ehkäisyä hoidon aikana ja 6 kuukautta viimeisen annoksen jälkeen. Valmistetta ei saa käyttää raskauden aikana, ellei naisen kliininen tila edellytä ja oikeuta hoitoa sikiölle koituvasta mahdollisesta riskistä huolimatta. Jos valmistetta käytetään raskauden aikana tai potilas tulee raskaaksi hoidon aikana, naista on informoitava sikiöön mahdollisesti kohdistuvasta vaarasta. Kardiologista tutkimusta ja verenkuvaa suositetaan joka tapauksessa hoitoa raskauden aikana saaneiden äitien sikiöille ja vastasyntyneille. ENHERTU® 100 mg kuiva-aine välikonsentraatiksi infuusionestettä varten, liuos. Vaikuttava aine: trastutsumabi-derukstekaani Käyttöaiheet: HER2-positiivinen rintasyöpä: Enhertu on tarkoitettu monoterapiana sellaisten aikuispotilaiden hoitoon, joilla on leikkaushoitoon soveltumaton tai metastasoitunut HER2-positiivinen rintasyöpä ja jotka ovat aiemmin saaneet vähintään yhtä anti-HER2-pohjaista hoitoa. Annostus ja antotapa: Enhertu-valmisteen määrää lääkäri ja se on annettava sellaisten terveydenhuollon ammattilaisten valvonnassa, joilla on kokemusta syöpälääkkeiden antamisesta. Suositeltu Enhertu-annos rintasyövässä on 5,4 mg/kg ja mahasyövässä 6,4 mg/kg molemmissa annettuna laskimoinfuusiona kolmen viikon (21 vrk) välein kunnes sairaus etenee tai esiintyy ei-hyväksyttävissä olevaa toksisuutta. Aloitusannos on annettava 90 minuuttia kestävänä infuusiona laskimoon. Enhertu on emeettinen. Ennen jokaista Enhertu-annosta potilasta on esilääkittävä kahden tai kolmen lääkevalmisteen yhdistelmällä solunsalpaajahoidon aiheuttaman pahoinvoinnin ja oksentelun ehkäisemiseksi. Lääkitysvirheiden välttämiseksi on tärkeää varmistaa injektiopullon etiketistä, että valmisteltava ja annettava lääkevalmiste on Enhertu (trastutsumabi-derukstekaani) eikä trastutsumabi tai trastutsumabiemtansiini. Enhertu-valmistetta ei saa korvata trastutsumabilla tai trastutsumabiemtansiinilla. Biologisten lääkevalmisteiden jäljitettävyyden parantamiseksi on annetun valmisteen nimi ja eränumero dokumentoitava selkeästi. Interstitiaalisen keuhkosairauden (ILD) ja/tai pneumoniitin tapauksia on raportoitu Enhertu-hoidon yhteydessä ja kuolemaan johtaneita tapauksia on havaittu. Potilaita on neuvottava ilmoittamaan välittömästi yskästä, hengenahdistuksesta, kuumeesta ja/tai kaikista uusista tai pahenevista hengitystieoireista. Potilaita on seurattava ILD:n/pneumoniitin merkkien ja oireiden varalta ja ne on tutkittava viipymättä. Potilaat, joilla epäillään ILD:tä/pneumoniittia, on arvioitava kuvantamistutkimuksella.

Keuhkolääkärin konsultaatiota on harkittava. Oireettomassa (asteen 1) ILD:ssä/pneumoniitissa on harkittava kortikosteroidihoitoa ja Enhertu-hoito on keskeytettävä, kunnes haittavaikutus paranee. Oireelliseen ILD:hen/pneumoniittiin (aste 2 tai korkeampi) on viipymättä aloitettava kortikosteroidihoito (esim. ≥1 mg/kg/vrk prednisolonia tai vastaavaa) ja Enhertu-hoito on lopetettava pysyvästi. Neutropeniatapauksia, myös kuolemaan johtanutta kuumeista neutropeniaa, raportoitiin kliinisissä Enhertu-tutkimuksissa. Täydellinen verenkuva on tarkistettava ennen Enhertu-hoidon aloittamista, ennen jokaista annosta ja kliinisen tarpeen mukaan. Vasemman kammion ejektiofraktio (LVEF) tulee arvioida tavanomaisella sydämen toiminnan tutkimuksella (sydämen kaikukuvaus tai MUGA) ennen Enhertu-hoidon aloittamista ja säännöllisin väliajoin hoidon aikana kliinisen tarpeen mukaan. Enhertu-hoito on lopetettava pysyvästi, jos LVEF on alle 40 % tai absoluuttinen lasku lähtötilanteesta on vahvistetusti yli 20 %. Enhertu-hoito on lopetettava pysyvästi, jos potilaalla on oireellinen kongestiivinen sydämen vajaatoiminta (CHF). Maksan vajaatoiminta: Enhertu-valmistetta on annettava varoen potilaille, joilla on keskivaikea tai vaikea maksan vajaatoiminta. Ks. tarkemmin valmisteyhteenveto. Hedelmällisyys, raskaus ja imetys: Enhertu-valmisteen antamista raskaana oleville naisille ei suositella. Highlights from ASCO 2023 – New Insights into Luminal Breast Cancer Traditionally, the ASCO conference has mainly focused on other subtypes of breast cancer, but this year’s conference broke with that norm and brought forth a range of interesting presentations within the field of luminal breast cancer.

(ASCO) presents new and groundbreaking research findings in cancer treatment, and in 2023, it was no different. Among the many presentations at this year’s

conference, there was a significant focus on luminal breast cancer, i.e., hormone-sensitive breast cancer that is HER2-negative.

Menopause and Chemo Impact on Ovarian Suppression in Breast Cancer One of the remarkable presentations at ASCO23 came from Richard Gray, MA, MSc, a professor from Oxford, who presented a meta-analysis on ovarian suppression or

castration for women with hormone-sensitive breast cancer.1 The results showed an 11.9% reduction in the risk

of recurrence with ovarian suppression. It should be not-

ed that the study included data from studies conducted from 1948 to 2009, which posed challenges due to

differences in study design over time. An important observation from this meta-analysis was that the effect of ovarian suppression was primarily limited to women who were definitely still premenopausal after or had

not received chemotherapy. This highlights the importance of menopausal status and treatment history.

CONCLUSION: ASCO 2023 has undoubtedly shed new light on luminal breast cancer and introduced exciting perspectives for future treatment. It is important to remember that while these results are promising, further research and follow-up are required before they can be implemented as standard treatment.

Useampaan kuin yhteen elinjärjestelmään vaikuttavia immuunijärjestelmään liittyviä haittavaikutuksia voi ilmetä samanaikaisesti. Yhdistelmähoidossa on potilaita seurattava sydämeen ja keuhkoihin liittyvien haittavaikutusten varalta jatkuvasti sekä elektrolyyttihäiriön ja kuivumisen kliinisten merkkien ja oireiden sekä laboratorioarvojen huononemisen varalta ennen hoidon aloittamista ja säännöllisesti sen aikana. Nivolumabi–ipilimumabi-yhdistelmähoito tulee keskeyttää, jos hengenvaarallisia tai uusiutuvia sydämeen ja keuhkoihin liittyviä haittavaikutuksia ilmenee. Potilaita on seurattava jatkuvasti (vähintään 5 kuukautta viimeisen annoksen jälkeen), sillä haittavaikutus voi tulla missä vaiheessa yhdistelmähoitoa hyvänsä tai vasta sen päätyttyä. Haittavaikutuksen vakavuuden perusteella nivolumabihoito, nivolumabi-kabotsantinibi-yhdistelmähoito tai nivolumabi–ipilimumabiyhdistelmähoito on keskeytettävä ja annettava kortikosteroideja. Nivolumabihoito tai yhdistelmähoito on lopetettava pysyvästi, jos yksikään vakava immuunivälitteinen haittavaikutus uusiutuu tai on hengenvaarallinen. Kliinisissä tutkimuksissa on raportoitu vaikeita infuusioreaktioita nivolumabi-monoterapian tai nivolumabi–ipilimumabiyhdistelmähoidon yhteydessä. Vaikeissa tai henkeäuhkaavissa infuusioreaktiotapauksissa nivolumabi-infuusio tai nivolumabi–ipilimumabi-infuusio täytyy keskeyttää ja antaa asianmukaista lääketieteellistä hoitoa. Klassinen Hodgkinin lymfooma: Nivolumabihoito saattaa lisätä vaikea-asteisen ja akuutin käänteishyljinnän ja kuoleman riskiä, jos sitä annetaan ennen allogeenista hematopoieettisten kantasolujen siirtoa tai sen jälkeen. Lääkärien tulee huomioida nivolumabin ja nivolumabi–ipilimumabi-yhdistelmähoidon viivästynyt vaikutuksen alkaminen ennen hoidon aloittamista potilaille, joilla on nopeasti etenevä sairaus sekä potilaille, joilla on huonompi ennuste ja/tai aggressiivinen sairaus tai ruokatorven levyepiteelikarsinooma. Annoksen muuttaminen ei ole tarpeen hoidettaessa iäkkäitä potilaita (≥ 65 vuotta). Valmisteen turvallisuutta ja tehoa alle 18 vuoden ikäisten lasten hoidossa ei ole varmistettu. Tutkimustiedon puuttuessa nivolumabia ja nivolumabi-yhdistelmähoitoa tulee käyttää varoen erityisryhmille (ks. valmisteyhteenveto). Ks. valmisteyhteenvedosta indikaatiokohtaiset lisätiedot erityisryhmistä, joille hoitoa on käytettävä varoen vähäisen tutkimustiedon takia. Yhteisvaikutukset: Systeemisten kortikosteroidien ja muiden immunosuppressiivisten lääkkeiden käyttöä ennen nivolumabihoidon aloitusta on vältettävä, koska ne saattaisivat vaikuttaa nivolumabin farmakodynamiikkaan; niitä voi kuitenkin käyttää hoidon alettua immuunivälitteisten haittavaikutusten hoitoon. Hedelmällisyys, raskaus ja imetys: Nivolumabia ei suositella käytettäväksi raskauden aikana eikä sellaisten naisten hoitoon, jotka saattaisivat tulla raskaaksi mutta eivät käytä tehokasta ehkäisyä, paitsi siinä tapauksessa, että kliininen hyöty on merkittävämpi kuin mahdollinen riski. Tehokasta ehkäisymenetelmää on käytettävä vähintään 5 kuukauden ajan viimeisen nivolumabiannoksen jälkeen.

Ei tiedetä, erittyykö nivolumabi ihmisen rintamaitoon. Nivolumabin vaikutusta miehen ja naisen hedelmällisyyteen ei tiedetä. Haittavaikutukset: Nivolumabihoitoon ja nivolumabi–ipilimumabi-yhdistelmähoitoon liittyy immuunivälitteisiä haittavaikutuksia. Immuunivälitteiset haittavaikutukset hävisivät useimmissa tapauksissa oikeanlaisella lääkehoidolla. Nivolumabia monoterapiana, nivolumabi–ipilimumabi-yhdistelmähoitoa, nivolumabi–kabotsantinibi-yhdistelmähoitoa tai nivolumabi–kemoterapiayhdistelmähoitoa saaneiden potilaiden hyvin yleisiä haittavaikutuksia ovat: ylähengitystieinfektio, lymfosytopenia, anemia, leukopenia, neutropenia, trombosytopenia, leukopenia, lymfopenia, neutropenia, kilpirauhasen vajaatoiminta, kilpirauhasen liikatoiminta, ruokahalun väheneminen, hyperglykemia, hypoglykemia, painon lasku, perifeerinen neuropatia, dysgeusia, päänsärky, heitehuimaus, kohonnut verenpaine, dysfonia, yskä, hengenahdistus, ripuli, suutulehdus, oksentelu, pahoinvointi, vatsakipu, ummetus, dyspepsia, käsi-jalkaoireyhtymä, ihottuma (toisinaan rakkulainen), kutina, muskuloskeletaalinen kipu, artralgia, lihaskouristukset, proteiinivirtsaisuus, uupumus, kuume, turvotus (mukaan lukien perifeerinen edeema), lisääntynyt alkalinen fosfataasi, lisääntynyt ASAT, lisääntynyt ALAT, hypernatremia, hyponatremia, hypoalbuminemia, lisääntynyt alkalinen fosfataasi, lisääntynyt kokonaisbilirubiini, lisääntynyt kreatiniini, lisääntynyt lipaasi, lisääntynyt amylaasi, hypofosfatemia, hyperkalemia, hypokalemia, hyperkalsemia, hypokalsemia, transaminaasiarvojen suureneminen, hypomagnesemia, hypermagnesemia.
