Seven Acres Jewish Senior Care Services is a not-for-profit, non-sectarian, certified and licensed Medicaid, Medicare and private pay campus for the elderly and disabled.
Seven Acres’ Lou Lewis Skilled Nursing Program offers post-acute services for those transitioning from hospital to home. This outcome-focused specialized care helps shorten the hospital stay and lays the groundwork for a successful discharge back home. Each person’s care is customized for their specific needs delivered around-the-clock by trained licensed professionals.
Skilled care begins with a comprehensive assessment by our licensed professionals and continues through discharge planning. Our licensed professionals are always available to answer questions and address concerns of prospective residents or their families.
The Lou Lewis Skilled Nursing Program includes:
• Rehabilitation care due to fractures or joint replacements
• General rehabilitation due to weakness or physical decline
• Advanced wound care management
• Post-operative care
• Chronic illness management
• Non-invasive respiratory care
• Parkinson's rehabilitation
• Stroke recovery therapy
• Post operative care
• IV Therapy
Seven Acres provides full-time medical services with on-site physicians and geriatric nurse practitioners five days a week and on call 24 hours/7days a week. We also maintain on-site services for dentistry, podiatry, and ophthalmology. These professionals and services enhance better coordinated care and allow our residents to see these specialists while residing on our campus.
Because our goal at The Lou Lewis Skilled Nursing Program is for seniors to live as independently as possible, a broad complement of services is provided by a highly-trained staff of licensed therapists and assistants. We offer Physical, Occupational, and Speech Therapy services.
Our licensed physical therapists work on improving coordination, balance, strength, endurance, and joint flexibility to achieve optimal levels of functional mobility. Our goal is to move seniors toward greater independent mobility. An integral part of the treatment is rehabilitation education for our clients and their families. Our physical therapists design individualized treatment programs for pain management, preventing disability, and restoring function.
Our licensed Occupational Therapists enable the resident to adapt their physical and cognitive capabilities to the skills needed in activities of daily living. Occupational Therapists assess the areas needed for functional independence, customizing a treatment plan with emphasis on balance, strength, perception, and cognition.
Our licensed Speech-Language Pathologists work to restore the resident's ability to communicate. They address communication problems by helping patients improve skills in speaking, listening, thinking, problem solving, as well as organizing and sequencing thoughts. Speech Language Pathologists also assess swallowing difficulties and develop treatment and managed programs.
We are very sensitive to the needs of applicants requiring Seven Acres services. Our staff is available to work with those needs. Admissions packets and applications are available daily from 8 a.m. to 8 p.m.
