Located on the Pauline Sterne Wolff Campus in Southwest Houston, Seven Acres is a not for-profit, non-sectarian, certified and licensed Medicaid, Medicare and private pay campus for the elderly. Seven Acres consists of the largest long-term care facility in Houston and together with The Medallion Assisted Living Residence creates a seamless continuum of care for those who require monitoring and support to those who need significant long-term clinical intervention.
Seven Acres provides a variety of services for all levels of need and care:
• Skilled Nursing Care
• Rehabilitative Care
• Intermediate Long-term care
• Intensive Long-term care
• Hospice Services
• Assisted Living
Seven Acres provides full-time medical services with on-site physicians and geriatric nurse practitioners five days a week and on call 24 hours/7 days a week. We also maintain on-site interdisciplinary services to enhance care.
Seven Acres’ Lou Lewis Medicare Skilled Nursing Program offers post acute services for those transitioning from hospital to home. This outcome-focused specialized care helps shorten the hospital stay and lays the ground work for a successful discharge.
Each person’s care is customized for their specific needs delivered around-the clock by trained professionals.
Because our goal at Seven Acres is for the residents to live as independently as possible, a broad complement of services is provided to residents by a highly-trained staff of licensed therapists and assistants. We offer Physical, Occupational, and Speech Therapy services.
Seven Acres offers Hospice Services designed to provide comfort and support to residents and their families when a life-limiting illness no longer responds to cure-oriented treatments. The goal of hospice care is to enhance the final stages of a resident's life by offering comfort and dignity.
The Medallion residential community, on the campus of Seven Acres, offers all the features of an elegant apartment with gourmet Kosher dining, an active schedule of social, cultural and recreational programs, a wellness center and aqua therapy pool, along with medication management and assistance with activities of daily living.
We are very sensitive to the needs of applicants requiring Seven Acres services. Our staff is available to work with those needs. Admissions packets and applications are available daily from 8 a.m. to 8 p.m.
