WVMA Member Spotlight – Dr. Scot Hodkiewicz

After graduating from UW Madison SVM in ‘93 I went into mixed animal practice in Wisconsin Dells. That same year I married Mona Anderson, another veterinarian from the class of ‘94. After five years of mixed animal practice, we had the opportunity to buy our own practice in Lake Geneva, Wis. We took the chance and bought it! The next five years were spent building our practice, along with having three kids and developing a relationship in the community. The clinic started to thrive, and we thought we had a great plan for how things were going to run. Everything was falling into place. On October 22, 2006 the plan changed. Our family of five (plus two dogs) were hit head-on by an eight-time drunk driver at 70 mph on the interstate. Dr. Scot Hodkiewicz Myself, Mona, and our oldest daughter were severely injured and spent the next year in and out of hospitals. I was in the hospital for three months, out of work for six months and limited for a year or two more. Down two vets in a 3 ½ vet practice and desperate for some stability at our business, Mona was back at work in four weeks in a full body splint with three vertebral fractures. I can honestly say that during this period, we found out what it truly means to be a veterinarian. People came from everywhere to help (I call them our “angels”). Daily food deliveries were coordinated by our church, cooked by volunteers, and dropped it off at our doorstep. One client who owned a dry cleaning business would stop by weekly to pick up our laundry and have it washed, pressed and folded and returned to our house that night. Another client who owned a gas station offered free gas to anyone that needed to drive to the hospital. The client who does our clinic snowplowing would clear our house driveway and send his son to sweep and salt our steps. The local barber even showed up at the hospital to cut my hair. All these people came to help because of our close relationship as their veterinarian. The veterinary community also stepped up. My roommate visited me in the hospital and gave me a blanket hand-made by his wife. Veterinary colleagues Mike Aushwitz, Jeff Korosek, Tracy Kusik and Chris Holder all came and worked at our office for free while we were out. Professor Chuck Hendricksen from the SVM kept everyone informed with email blasts to both Mona and my classmates. We, as veterinarians, are a tight family and this was evident when we were down. >>> Continued on Page 6

Isn’t it Great Being a Veterinarian?... Dr. John T. Been

It’s hard to believe that my year in this position is already coming to a close and that it is time to compose this last president’s message for the WVMA Voice. Time sure does go by quickly and it seems to be picking up speed as I grow older! What an honor it has been to help guide the WVMA through its 101st year – a brand new century of opportunity for our Wisconsin veterinary profession. In my first president’s message last November, I mentioned that I was very proud of how vitally the WVMA helped to shape the course of veterinary medicine in both Wisconsin and nationally. This past year has continued that tradition. We are completing year two of our five year strategic plan and have launched several new initiatives in line with that plan. A quick summary of some of those efforts include: n WVMA Foundation – this philanthropic effort is now moving

forward with the establishment and populating of our foundation board who will guide it’s actions. n Foor Armor® – Our unique and successful WVMA HACCP program for Proper Drug Use has established Phase II training so that accredited veterinarians can qualify producers for full certification under the standards of the program. Third party auditing is now in place to assure accountability. Furthermore we are in the process of separating Foor Armor® into a more independent non-profit foundation to insure it’s scope, visibility, and sustainability. n Business Management Task Force. - Addressing the business/ practice management concerns for our membership. n OSHA Alliance Task Force - Addressing safety and hazard

analysis in our unique veterinary workplace environment. We will work collaboratively with OSHA to provide this member service to both increase safety and to better prepare our practices for an OSHA inspection. This would be the first such veterinary OSHA alliance in the nation. Online CE is now available for the membership on our WVMA website. Mind, Body, & Soul - Weekend for Team Wellness focusing on compassion fatigue was held in Wisconsin Dells in April, 2016. Successful legislative advocacy regarding the move of VEB to DATCP, UW School of Veterinary Medicine tenure for clinical staff, and maintaining WVDL within the UW system, among other efforts. Another great annual convention planned for this October with exciting activities and valuable CE scheduled.

It has been a great year for me personally and I can’t help but reflect a bit on some of the personalities who have helped shape me along the way in my career. I have such good memories of those days of my training in the brand new UW School of Veterinary Medicine and of my superb instruction from Drs. Easterday, Wilsman, Oakes, McGuirk, Manley, Czuprynski, Bristol, Dahl, Young, Backstrom, Thomas, Bjorling, Duncan, Dueland, Schultz, Rosin, Allenstein, and MacWilliams. Those are some rather legendary educators that I had the good fortune to have rubbed shoulders with. I also had great business partners in my practice life for some 24 years at the River Valley Veterinary Clinic in Plain. A couple of years ago, the WVMA worked to exempt veterinary medicine from the Prescription Drug Monitoring Program (PDMP) in Wisconsin. Not all states have been this fortunate and it is creating a lot of extra work and stress for veterinary clinics in other states across the country. I feel it is a good time to remind everyone we were able to obtain the exemption for several reasons: 1) Pet owners weren’t using veterinary clinics to fulfill their addictions; 2) Veterinarians and veterinary clinics were proactive in reporting suspicious activity to local law enforcement; and 3) If veterinarians were able, they would also change the animal’s medication; to name a few. If we are going to remain exempt from PDMP, we need to remain committed and diligent to these commitments. If you are suspicious of clients shopping for these substances, I would like to add another solution. Human pharmacies must track all controlled substance prescriptions they fill and upload that information into the PDMP database. If you write a prescription to a “human” pharmacy, the animal owner will be entered into the PDMP database, even though the prescription is for their animal(s). Regulators will be able to track animal owners looking to divert their animal’s medications through this process. We are able to remain exempt due to the profession’s proactive measures and the responsibility and accountability to the issue. Thank you for continuing to be a part of the solution! n

DATCP Action on Informed Consent and Scope of Practice By Jordan Lamb, DeWitt Ross & Stevens, s.c.

The Wisconsin Veterinary Examining Board (VEB) moved to the Department of Agriculture, Trade and Consumer Protection (DATCP) just over a year ago. As a result of this move, DATCP has taken over the investigation and enforcement of scope of practice issues. At the July 2016 VEB meeting, DATCP reported to the Board about the following recent cases that they have reviewed and addressed. Spay/Neuter Video Does Not Meet Informed Consent

A Wisconsin veterinary clinic that is primarily engaged in spaying and neutering animals recently inquired whether it could meet Wisconsin’s informed consent requirements by using an online video and quiz, rather than an in-person meeting between the veterinarian and the pet owner. The clinic proposed to show an online video of their veterinarians explaining the spay/neuter procedures and risks to pet owners and then require the pet owners to complete an online “quiz” that would prove that they watched the video. The online quiz would also provide an opportunity to request a personal call from the veterinarian. Under Wis. Admin. Code s. VE 7.06 (23), it is considered unprofessional conduct to fail to “inform a client prior to treatment of the diagnostic and treatment options consistent with the veterinary profession’s standard of care and the associated benefits and risks of those options.” Accordingly, DATCP determined that the proposed video would not meet Wisconsin’s informed consent requirement. Unlicensed Practice Cases

DATCP recently closed two unlicensed practice cases. The first involved a dog groomer who was providing an over-the-counter product to a customer in order to calm a dog for grooming. Wisconsin law defines

the practice of veterinary medicine as, “to examine into the fact or cause of animal health, disease or physical condition, or to treat, operate, prescribe or advise for the same, or to undertake, offer, advertise, announce, or hold out in any manner to do any of said acts, for compensation, direct or indirect, or in the expectation thereof.” Wis. Stat. s. 89.02(6). DATCP determined that although the product provided to the dog was an over-the-courter product, it was offered for the purpose of treating the dog’s anxiety and the treatment was offered for compensation. Even if the compensation was for the grooming service, the Department concluded that this was the practice of veterinary medicine. The Department exercised its discretion to treat this unlicensed practice of veterinary medicine as a minor violation of the public interest and simply issued a warning notice to the groomer. However, the warning did include notice that any future violations could be met with more strict enforcement measures. The second unlicensed practice case involved a nurse who was administering rabies vaccines to dogs. According to the stipulated special order issued in this case, the Department found that the nurse was selling and administering rabies vaccines to dogs and also issuing certificates of rabies vaccinations. Wisconsin law prohibits a person from engaging in the practice of veterinary medicine without a license. See Wis. Stat. s. 89.078(2). The Department determined that this action constituted the unlicensed practice of veterinary medicine and the order enjoins the nurse from engaging further in the practice. If the nurse fails to comply with the order, then a legal action could be brought to recover the statutory forfeiture of not more than $10,000 for each offense. n

Surviving this extreme experience opened our eyes to how important it is to become part of the community. We work hard to help people with their animals during the toughest of times. These animals are an important part of our client’s lives and they trust us to keep them well. We, as veterinarians, have been trained with incredible skills to accomplish this. For many of us, we go from treating a dog’s allergies one minute to removing an eight pound mass the next. We see them as brand new puppies and help them have a good life until the very end where we also are there to help them. There are not many jobs that allow this much variety and do this much good. I realized, after being out of practice for those six months, that we as veterinarians are so very blessed.

The WVMA and the AVMA also stepped up. Our insurance was through these organizations and the offices helped get our medical bills paid and our disability started. They are powerful groups to have on your side when looking at over a million dollars in medical expenses. After I recovered, I served on the legislative committee helping draft laws to help veterinarians and animals. I now serve on a committee helping to better coordinate veterinarians and OSHA. The WVMA does so much behind the scenes to help veterinarians that I am happy to give something back. I would encourage new graduates to work hard at perfecting your craft. I read once that it takes 10,000 hours to master anything from a musical instrument to carpentry to a skill such as ours. This would be about five years at 40 hours a week. Those first 10,000 hours can be stressful. Things do not always go well and mistakes are prone to happen. After 10,000 hours, it gets easier. Like they say in football, after a while the game slows down. Mistakes are fewer, you have seen the strange cases before, the spay now takes 20 minutes instead of 45, even on the 9-year-old, fat Rottweiler, and clients stop mistaking you for a technician or asking that you “check with the experienced vet”. New veterinarians have to keep reading, studying, and asking questions to get to that point. Us “old” veterinarians do this also; it is part of the job. I would also encourage new veterinarians to develop some area of expertise. At our practice, all doctors specialize in something they enjoy. I do surgery, scoping and ultrasound, one doctor does oncology, two do acupuncture, two do ophthalmology, one does dermatology, and one specializes in feline medicine. This is best for the animals, good for the veterinarians, and grows the business. I recently published a book about the crash and our recovery called “Getting to Heaven By Going Through Hell” (available on Createspace, Amazon and Kindle). It recounts the crash and recovery highlighting the many “angels” that came and helped us. It is about traveling through the “Hell” that many of us face at some point in our lives and finding the “Heaven” that is all around us on the other side. My “Heaven” includes the job I get to do and the clients we get to bring into our lives. The crash made me a better veterinarian (I now am all about pain control) and a better person. Oddly, the crash gave me great peace knowing that I don’t have to try to control everything and that my life’s direction may not go the way I expect. As I say in the book, “If you want to hear God laugh, just tell Him your plan”. Again, the local community as well as my veterinary family have been wonderful at helping promote the book and it has helped many dealing with their own “Hell”. As part of giving back, a portion of all proceeds go to our church that was so vital in our recovery, the local animal shelter and Fellow Mortals Wildlife Hospital where Mona and I volunteer our veterinary skills. n

and humor and was an excellent veterinary role model for me to emulate. My involvement with organized veterinary medicine allowed me to engage with other industry leaders at the local (SW) and then the state (WVMA) level and I very much appreciate all the collaboration and contribution made by our excellent committees, staff, and allied individuals. That work, entirely volunteer, is something I truly consider to be as cherished and rewarding as anything I’ve ever done in veterinary medicine. I recommend it to you all. This year I was blessed with a really dedicated and engaged executive board who helped us, as Larry the Cable Guy would say, “get ‘er done”. I especially admire the dedication of such individuals as Drs. Doug Kratt and Steve Erickson (AVMA delegates), Chet Rawson (AVMA Executive Board) and, of course, Rene Carlson who is now the president of the World Veterinary Organization after serving as president of the AVMA and WVMA in her past. I have no doubt that she will be willing and able to serve as the

president of the Intergalactic Veterinary Organization if it ever is formed – and I’d be the first one to nominate her! My last note of appreciation, and the most important one, goes to my family who are the real inspiration for most everything I do. As all of you know, a veterinary career is often not an easy one and the family has to absorb lots of the unique stresses that might come with the job. My wife and three boys shared in all the fun and stresses alike with a great attitude. Probably nothing measures time as starkly as watching the kids grow up and start their own careers and families. However, I must say, ‘Grandpa’ has a great ring to it. It’s really true – time flies. In any case, best wishes to you all and to Dr. Scott Spaulding as he takes over as your new president this fall. ...Yes, it is! n

Acupuncture: Its History, Effects, and Use in Veterinary Medicine

By Lindsey Snyder, DVM, MS, DACVAA, CVA Clinical Instructor, Department of Surgical Sciences, University of Wisconsin School of Veterinary Medicine and UW Veterinary Care Acupuncture has been described in writing as a part of Traditional Chinese Medicine (TCM) for more than 3,000 years, and some artifacts depicting this practice are more than 8,000 years old. Written references to veterinary medical acupuncture in particular go back 1,400 years. Acupuncture is one of the five components of TCM, the others being herbal medicine, massage (Tui na), gentle exercise (qigong), and food therapy. Recent decades have seen the introduction of a neurophysiologic approach to acupuncture called medical acupuncture that aligns with Western medical philosophy; it utilizes nerve pathways and neurochemistry to teach acupuncture’s effects. Both the traditional and Western approach to acupuncture produce similar analgesic and therapeutic effects. Defined as the stimulation of specific anatomic points in the body to produce therapeutic or analgesic effects, acupuncture involves the use of fine gauge needles to puncture the skin in specific areas called acupoints. The effect can also be accomplished by heating, applying pressure to, or injecting liquid (e.g., saline, vitamin B, or even the patient’s own blood) into the point. To enhance the therapeutic effects, the needle can be stimulated through the addition of manual rotation, heat, or a mild electrical current (electroacupuncture). Traditional acupuncture is based on patterns of energy flow, or Qi, throughout the body. Disruptions in the flow of energy along hypothesized lines or channels are thought to be the basis of disease in the body. These channels, or meridians, have never actually been proven to exist, but evidence shows that they follow nerve pathways or fascial planes. Acupoints are thought to be locations where energy is distributed or gathered. Western science has confirmed that TCM acupoints are areas of decreased electrical resistance that can increase electrical conduction. They also have a high concentration of free nerve endings that specialize in pain perception, arterioles, lymphatic vessels, and mast cells. Acupuncture has many indications, from pain to neurologic deficits to urinary disease. Because it is devoid of virtually all side effects, acupuncture can be thought of as a low-risk treatment option. However, it may not be universally effective for patients. Two patients with the same symptoms might respond very differently to acupuncture therapy; therefore, similar to other therapies, its degree of success varies for every patient. That being said, for some patients it can be used as the sole approach to some conditions. More commonly, acupuncture is used as an adjunct to Western therapies. Acupuncture research has primarily focused on the mechanism of analgesia secondary to treatment. Muscle spasms have been proven to be decreased with the use of acupuncture therapy. Additionally, acupuncture therapy induces the release of neurotransmitters. The effect is an alteration in the processing of pain impulses, thereby reducing

the transmission and perception of pain. Endorphins have been found to be released secondary to needle placement, with electroacupuncture having a greater increase in endorphin release. In simple terms, placing acupuncture needles will change the chemistry of the nervous system so that pain is perceived as less severe. Because this process utilizes the nervous system, acupuncture can help “wake up” damaged or traumatized nerve pathways. Typically, in chronic conditions, more therapy is needed to break the cycle of pain. Therefore, clients should commit to several therapy sessions before determining the effect acupuncture has on their pet. That is not to say that improvements can’t be seen in one session. Therapy is not limited to dogs and cats. In fact, acupuncture patients at UW Veterinary Care (UWVC) include everything from ferrets to cows to birds. The UWVC Pain Management Service includes two veterinarians who are certified in acupuncture and one who is finishing medical certification. There is no referral necessary for making an appointment, only a desire to improve quality of life for an animal or pet. n

In an effort to get more clients in the door, some veterinary hospitals are starting to second guess their shoppable fees. This cost / benefit analysis is the true story of a veterinary hospital that recently lowered their vaccine fees to gain a competitive advantage over the competition. Spoiler alert: there is no benefit.

for vaccines. More phone shoppers were coming through the door, but regular clients were now paying a lot less for their vaccines. There was much analysis done leading up to the fee change, yet no one looked into the implications on total revenue or more importantly, net income. Here is that analysis.

It all started during a discussion on how to get more clients. The receptionist offered that as soon as she tells a phone shopper the price of their vaccine, they either say that the price is too high or they politely say their goodbyes. “We are not getting any shoppers because our fees are too high,” she concluded. After some fee analysis, the staff decided that they could get more price shoppers if they lowered their vaccine fees to compete with area practices. They went from Fee Guide (or very close to Fee Guide) to around $100. Dog vaccines (with exam) went from $139.50 to $107.50 and cat vaccines (with exam) went from $132.00 to $94.50.

The hospital in question will remain nameless, and the numbers have been rounded to keep the math simple. However, the spirit of the argument holds true, and any smoothing of the data was done conservatively, so the real life impact would be worse.

Before the new vaccine fees were introduced, the veterinary hospital was down roughly 10 percent compared to the last year, with revenue slipping to $750,000. They had 1,250 active clients with revenue per client of $600. They were suffering the same fate as every other hospital that was down in revenue – clients were down. They did a great job of lowering their vaccine fees. They dropped a $213,806, and with the new fees, they were on target to hit $159,075 whopping 23 percent for dog vaccines and 29 percent for cat vaccines. with the same number of clients. Don’t forget, the reason they lowered the fees was to get more new clients. With the lower vaccine fees, they now need to get 182 more new clients just to get back to the revenue they were originally predicted to earn. That translates into a 47 percent increase in new clients each year to equal previous revenue. They would be no further ahead financially, they would simply be busier. So far we have only been discussing the impact on revenues. Net income suffers a lot more than revenues when we lower fees. Because each client is spending 5 percent less, the hospital’s margin, or net income to gross revenue ratio goes from a respectable 30 to 25 percent. This means that before the fee change, 30 cents of every dollar went to veterinarians; after the change, this drops to 25 cents from every dollar. When you lower (or raise) fees, hospital expenses don’t change, so a lower fee for each client causes net income to decline. To simply get

back to the old net income with the newly lowered net income to gross revenue ratio, this hospital needs to see 362 more clients every year. With the newer, lower fees, everyone has to work harder to get the same results. The shortfall in revenues from the change in fees brings revenues down to $695,269 and if the number of clients does not change, the net income would fall to $170,269. To get back to $225,000 in net income, target revenue is now $900,000 ($204,731 higher). Because fees were lowered, the only way to get to this newer, higher gross revenue is to see 362 more clients.

The true cost of lowering vaccine fees more than thirty dollars is more than just a $54,731 shortfall in revenue. After factoring in earning less from every client moving forward, you now need to attract 362 more new clients and increase gross revenue 32 percent just to get back to the net income you started with. The benefit to lowering exam fees is more clients. Unfortunately, the number of new clients required to cover the revenue loss from all clients is likely out of reach.

A better solution to too few clients would be further training of staff members. Provide staff with opportunities to hone their communication and customer service skills, and allow them the time to connect with potential clients. This will yield better results over the long term, but you must be willing to put off the instant gratification afforded by dropping your fees. In the end, your bank account will thank you. n
