WVMA Student Day a Success On Saturday, February 4, the Wisconsin Veterinary Medical Association (WVMA) hosted a Student Day in Madison for student members. The event consisted of speakers sharing their knowledge on a range of topics selected by the students. The schedule included the following: • WVMA Member Panel: What I Wish I Knew Then Drs. Professional Wellness: Break the Dam!

Why wellness? Nearly 10 percent of veterinarians characterize themselves as experiencing severe psychological distress. Why wellness? Nearly 17 percent of veterinarians have contemplated suicide since graduation. Why wellness? How do you respond when confronted with a phone call from a veterinarian’s spouse informing you the veterinarian is threatening suicide? Why wellness? Does your practice have a drug testing policy? Why wellness? Over 72 percent of survey respondents say they’ve worked with someone abusing substances. Hypotheses abound regarding our profession experiencing far higher than average levels of psychological distress, depression, suicidal ideation and substance abuse. I challenge you: we need more than hypotheses, we need more than task forces; we need more than summits. Personally, I’m sick and tired of hearing of these woes affecting the profession. We have the knowledge; we have the resources. We must take action to assist those members of the veterinary profession suffering from mental illness and substance abuse. What can be done? Educate yourself! Educate your colleagues! Can you recognize symptoms of psychological distress? How do you respond? What resources are at your disposal? Are your practice’s controlled substances secure and records kept current? Does your practice have a substance testing policy in place? What resources can you offer the spouse of a veterinarian contemplating suicide? Have you looked at the Colorado State University College of Veterinary Medicine’s “Veterinary Wellness” website – http://veterinarywellness.colostate.edu? Did you know you can access informative webinars sponsored by Pet Poison Helpline and AVMA Life – www.petpoisonhelpline.com/webinars/? Recognize the profession’s paradox of extraordinary psychological distress and easy access to powerful substances. When will we do something about it? What’s holding us back? Let’s break the dam and implement resources already available! Break the dam and implement assistance programs! Break the dam and implement stringent drug control procedures! Break the dam and knock down stigmas associated with psychological distress! It’s time to end the suffering. It’s time to end the stigmas. It’s time to provide support to those afflicted with psychological distress and substance abuse, especially if related to profession stressors. Join me to Break the Dam! l

As I monitor the bills being introduced, discussions at our state agencies and the agenda of the Governor and newly installed legislature, there is one common theme – curbing the abuse of prescription drugs and opioids. Veterinary medicine in Wisconsin has a good track record of proactively addressing this issue with their clients and patients. We need to aggressively continue to stay engaged in these efforts. With headlines from other states hitting the main stream press, veterinary medicine in Wisconsin is under the microscope again. I need each and every veterinarian and the staff team you employ to make this issue a priority. If you suspect client abuse of a patient prescription, the following are proactive options: 1. Consider changing the medication the animal is on – more and more clinics won’t prescribe controlled substances, unless it is an end of life situation 2. Limit the number treatments/days you prescribe the medication 3. Call local law enforcement and share your concerns/suspicions 4. If a controlled substance is needed for the patient, write the prescription to a “human” pharmacy. By doing this, the client will be entered into the state’s Prescription Drug Monitoring Program (PDMP) by the “human” pharmacy and the client will be monitored by state officials. Yes, it is sad to think that animal owners will use their animals to access these types of medications – but anything is possible when an individual is desperate. We don’t want to compromise animal welfare. The ultimate treatment decision is yours. However, if something seems “off”, it probably is. Take the signals seriously and address it proactively. Together, we will continue to demonstrate our commitment to addressing these disturbing addiction trends to validate our commitment to public health while proving we don’t need to be regulated. This is an issue where together we are stronger than any one of us! l

Dr. Jonathan Garber Elected to AVMA Council on Biologic and Therapeutic Agents WVMA Member, Dr. Jonathan Garber, was elected to the American Veterinary Medical Association (AVMA) Council on Biologic and Therapeutic Agents. This Council serves as an informational and advisory resource to government agencies and to other veterinary medical groups on issues pertaining to veterinary biologics, drugs, and other therapeutic agents. The Council on Biologic and Therapeutic Agents consists of 12 voting members of the Association and are elected by the House of Delegates for two 3 year terms. Members represent the following areas of veterinary medical activity: microbiology; immunology; pharmacology; clinical pharmacology; private clinical practice, predominantly small animal; private clinical practice, predominantly food animal; private clinical practice, predominantly equine; epidemiology; industry, exclusive; and at-large. l

Legal Considerations of Using Social Media to Advertise Your Practice By Bryan C. Esch, DeWitt Ross & Stevens, s.c. Social networking and media sites like Facebook, LinkedIn, and Twitter and websites and blogs can be used by veterinarians to cost effectively advertise and market their practices to clients and potential clients. By using social media to market your clinic, veterinarians can enlist their client base in their promotional efforts by leveraging the viral nature of the internet. Through client surveys, requests for recommendations and endorsements, email marketing and other internet advertising strategies, veterinarians can have their clients “spread the word” about their services and help to grow their practices. It is reported that more than 66 percent of Facebook users login to Facebook every day to interact with friends and almost more than 20 percent of adults regularly use Twitter to obtain news and other information. It is expected that all businesses maintain a client-friendly and informative website. Many consumers, including veterinary clients, investigate professional services firms by reviewing the firm’s websites before deciding to obtain services. However, even though social media and the web are powerful marketing tools, veterinarians need to be aware of relevant legal issues to protect their practices from liability. Social media advertising is a visually-based method of communicating and it works best by pairing written material with photos and videos. However, veterinarians need to take care when using client and patient photos on their social media and websites, without client consent. Individuals have certain legal rights if their photo was taken and posted to the internet on social media or a website without permission. An individual may potentially stop the unauthorized use of his or her image in a commercial way if there has been an invasion of privacy, defamation or the violation of the individual’s right of publicity. An individual’s right of publicity has been violated if an image of the individual is used to sell products or services or to imply an endorsement of a product or a service, without authorization. Likewise, a client’s general right of privacy and

the statutory obligation to keep veterinary records confidential under Wis. Stat., §453.075, may be breached if the veterinarian inadvertently discloses private information about a patient’s health status by posting a photo or other information. In order to avoid these concerns, it is strongly recommended that veterinarians obtain written authorization from their clients, in their client/patient agreement or in a stand-alone authorization, before posting a photo of a client or patient to their website or social media. Soliciting user generated content by requesting that clients post content, including reviews and endorsements of veterinary services, is an effective way to utilize social media in advertising. However, doing so exposes the veterinarian to potential legal liability for copyright and trademark infringement (when user-generated content contains another person’s copyrighted material or uses a trademark without authorization), privacy and publicity violations, deceptive advertising and libel. Therefore, veterinarians should monitor and screen client and other third party material posted to their social media and websites. Inappropriate posts that pose a legal risk should be removed. Social media and other internet advertising is subject to the same general consumer protection provisions as traditional advertising. The Federal Trade Commission Act and state consumer protection laws, like Wis. Stat., §100.18, prohibit engaging in deceptive or unfair trade practices or advertising. Veterinarians must ensure that their advertisements are accurate and truthful, and not misleading, before posting them on social media or otherwise on the internet. The requirement to confirm the truthfulness or accuracy of an advertising claim extends to claims made by a veterinarian’s advertising agency and to posts from clients or employees. Social media and website marketing is an effective way to stretch your clinic’s marketing dollars and to spread the reach of your veterinary practice. However, veterinarians must pay attention to important legal considerations when marketing with social media and otherwise on the web. l

Ionizing Radiation Protection for Veterinary Medical Clinics – Part I By Leslie Ptak, Department of Labor/OSHA compliance assistance specialist Radiographic imaging is an important tool in veterinary medical practice. Radiation exposure is also universally considered a potential health hazard to employees. Because of the nature of veterinary x-ray exposures – an employee typically holds the animal to keep them still, preventing unnecessary exposures to ionizing radiation requires a program of control that involves every member of the care team. This article discusses clinics’ regulatory responsibilities, as well as the policies and processes that veterinary medical clinics can incorporate into their operations to minimize employee exposures to ionizing radiation. This article is presented in two parts. Part II will be published in the next newsletter.

Health effects – The health effects of ionizing radiation are well recognized. Latent effects increase with the amount of cumulative lifetime exposure (i.e., all exposures added up over the life of the individual). The most significant risks are an increase in the possibility that a person exposed to ionizing radiation will develop cancer, bone marrow suppression, as well as other health outcomes,

Regulation of workplace radiation exposures – Two agencies have jurisdiction over radiation safety in Wisconsin veterinary medical clinics: the Wisconsin Department of Health Services Radiation Protection Section (WDHS RPS) and the Occupational Safety and Health Administration (OSHA). Another organization, the National Council on Radiation Protection and Measurements provides leading scientific thinking on information, guidance and

recommendations on radiation protection and measurements. The information contained in NCRP Report No. 148, Radiation Protection in Veterinary Medicine is considered the standard in keeping radiation exposures as low as achievable. Ionizing Radiation Protection for Veterinary Medical Clinics – Part I

The WDHS RPS has authority over the registration and inspection of all machines that produce ionizing radiation (x-rays, computerized tomography, fluoroscopy, etc.), whether they are used in human medical or veterinary medical applications, industrial settings or research. They also have regulatory authority over the radiation exposures of patients undergoing X-ray procedures, the general public and the employees who take the X-rays. The regulations that apply to veterinary clinics are concentrated in DHS 157.74 through 157.81. Parts DHS 157.22, .23, and .25 address dose limits and also apply.

programs. The first is a template “GUIDE FOR REGISTRANTS,” which is currently being revised and will be available on the WDHS RSPS web site by spring 2017. The template is intended to be the foundation on which a clinic’s site-specific radiation safety program is developed. The benefit to an employer of a written program is that it (1) communicates your concerns for employees (2) standardizes your policies and procedures (3) addresses each of the regulations that all staff must comply with, and (4) is available for new staff to review when there is turnover. The Radiation Safety Policy must be available to employees at all times.

WDHS RPS updates its rules every three or four years and aligns its requirements with the NCRPM, US Food Drug Administration and Nuclear Regulatory Commission regulations. The agency is currently working on a revision to its Radiation Protection Chapter (DHS 157) and expects to have a final version in the spring of 2017. The revision is not expected to be substantially more protective of employees than what is now required.

Provide appropriate training – Well trained employees are those best able to follow the principles of ALARA. This is essential because ALARA requires that a combination of engineering controls and work practices be followed. All employees are required to have access to and understand the Radiation Safety Policy. The WDHS RPS sets some training requirements and reviews curriculum and training when they evaluate sites. The agency requires training in the operations and safety measures of clinics, as well as in the specific devices used. The training should be conducted on initial hire and should be reviewed annually. The WDHS RPS also outlines training guidelines for the technicians who take CT scans.

Every x-ray machine has to be registered before it can be used, and the registration is renewed annually in December. The WDHS RPS sends registration renewal letters out each November and the registration fee is due by the end of the year. Any changes to the registration (i.e., ownership, new address, new device installed, new contact person, etc.) are to be submitted to WDHS RPS within 30 days of the change made. The allowable exposure dose limits in OSHA’s Ionizing Radiation standard are outdated (29 CFR 1910.1096); they are based on scientific knowledge from the 1950s. However, the rest of the standard is still relevant. When OSHA evaluates the hazards of x-rays in workplaces, it uses parts of 1910.1096, the agency’s personal protective equipment standards (Subpart I of 1910), and its general duty clause to address them. Have a written program – A written Radiation Safety Policy is a requirement of the WDHS RPS. It also helps a clinic comply with OSHA’s requirements. Fortunately, the WDHS RPS has created a series of tools to help employers develop their radiation safety

Signs and Notices – DHS 157.88 requires that the Notice to Employees (Form No. P-45027, dated 12/10) be posted where employees can see it. OSHA requires that each radiation area be conspicuously posted with a sign or signs bearing the conventional radiation caution symbol. This is the three-bladed design (called a trefoil) that is magenta (purple) or black on a yellow background. Get employees involved – The ALARA concepts are effective, and they rely on individuals adhering to certain behaviors and working as a team. For some staff, that may mean changing attitudes as well as behaviors. Getting employees involved in the development of policies and their implementation will get more results than giving them the message to “just do it.” Encourage employees to help with all aspects of ALARA, such as tracking dosimetry usage, inspecting personal protective equipment, and communicating policies and procedures, etc.

New Ways to Manage Ear Diseases in Rabbits By Christoph Mans, Dr. med. vet. DACZM and Robert Hardie, DVM, DACVS, DECVS UW Veterinary Care, School of Veterinary Medicine, University of Wisconsin-Madison Lop-eared rabbits are particularly prone to ear diseases due to the particular ear anatomy. In lop-eared rabbits, the external ear canal is effectively closed over by the pinna, which makes it hard for cerumen to leave the ear canal. Therefore, in lop-eared rabbits the build-up of cerumen predisposes them to secondary bacterial or fungal infections, which can progress into the middle ear. Frequently in lop-eared rabbits, swellings at the ear bases can be palpated. Infection of the middle ear can lead to deafness, changes in behavior and appetite, and if the infection spreads to the inner ear, can cause neurological signs, such as head tilt and vestibular signs.

Diagnostics The external ear canal can be examined by using an otoscope, however if a lot of inflammation or discharge is present the tympanic membrane can often not be visualized. In most cases, rabbits do not tolerate a thorough examination of the ear canal due to discomfort from the inflammation present in the ear canal. In these cases, rabbits should be sedated or anesthetized for a thorough examination of the ear canal and tympanic membrane. Flushing the ear canal helps remove material from the ear canal to make sure no other abnormalities, such as foreign bodies or masses are present. Flushing should be performed under deep sedation or general anesthesia. The examination of the ear using video otoscopy and specialized equipment allows for removal of discharge and debris from the ear canal using small instruments, irrigation and suction. Also, samples for bacterial cultures can be collected and images taken to document findings. Video otoscopy is also very helpful Medical treatment Medical treatment of bacterial infections of the ear canal may include repeated flushing of the ear canal, and placing antibiotic soaked ear wicks in the ear canal, and/or oral or injectable antibiotics. In rabbits with upright ears, this approach can often eliminate the infection. However, in lop-eared rabbits, because of the particular anatomy of their ear canal, medical treatment will usually only improve the infection for a certain period of time before reoccurrence is noted. Ear surgery in rabbits has historically been perceived as being too invasive or risky and therefore has frequently not been recommended by veterinarians or not considered by rabbit owners. to visualize and incise the tympanic membrane (myringotomy), to collect purulent material for cytology and bacterial cultures, in cases of middle ear infections. However, the middle ear cannot be completely examined by otoscopy alone. Skull radiographs can help determine if fluid has accumulated in the tympanic bulla, but they can be challenging to interpret due to superimposition of the adjacent areas of skull. Computed tomography (CT scan) is necessary for complete evaluation of the middle ear and can be performed in sedated rabbits. Therefore, a CT scan of the head is the recommended imaging modality because it allows much better visualization of the middle ears and other important structures of the skull (nasal cavity, sinuses, teeth), which are commonly diseased in rabbits. The cost of a sedated head CT in rabbits at UW Veterinary Care is comparable to skull radiographs.

Rabbits tolerate ear surgery very well, recover quickly and have a much-improved quality of life following ear surgery.


Different ear surgeries can be performed in rabbits. For treatment of severe otitis externa, usually a total ear canal ablation (TECA) with a lateral bulla osteotomy (LBO) is performed. The TECA involves removal of the entire ear canal. In some cases, part of the external ear canal can be preserved if the infection is located only in the deeper parts of the ear canal. In such cases, a partial ear canal ablation (PECA) can be performed instead of a TECA, which reduces anesthesia time and reduces the risk for complications such as wound dehiscence (break down) after surgery. In any case, a LBO should be performed and the tympanic membrane and epithelial lining of the middle ear removed, even if no otitis media is present. During TECA-LBO or PECA-LBO procedures it is critical to collect samples for bacterial cultures, in order to know exactly what kind of bacteria are causing the infection in the middle ear and to select the most appropriate antibiotic therapy. In some cases, it might be beneficial to place antibiotic containing beads in the middle ear. In rabbits, the surgical site is closed completely after surgery and drains are usually not recommended, making wound care very simple. The

surgical site heals within 7-10 days and sutured can be removed. Most rabbits do very well after surgery and often start eating on their own the day of surgery. Pain medications and supportive care is provided and most rabbits can be discharged from the hospital within 1-2 days after ear surgery. The recommended surgical option for otitis media without concurrent otitis externa is PECA-LBO, although ventral bulla osteotomy has also been reported. Prognosis for rabbits following surgical treatment of otitis externa is excellent, and good for rabbits with otitis media. However, while infection can be resolved in most cases of otitis media and interna, neurological deficits, if present prior to surgery, often will remain. In summary, chronic ear infections in rabbits should no longer be considered a difficult and frustrating disease to treat by rabbit owners and veterinarians. Improved diagnostic procedures and treatment options make ear diseases treatable and rabbits can be cured from ear diseases and associated pain and discomfort.
