WVMA Student Members Attend AVMA Fly-In In April, the Wisconsin Veterinary Medical Association (WVMA) helped sponsor two student members from the University of Wisconsin School of Veterinary Medicine to attend the AVMA Fly-In. Here are their reports on what they learned and experienced while in Washington D.C. Rachel O’Leary, Class of 2019 What was your biggest take-away? My biggest take-away from this was how large the veterinarian’s role can be in the legislative process. A veterinarian can become part of the legislative process in multiple ways. It doesn’t require them to be in D.C. all the time, or even ever for some roles. Whether it’s calling your local representative to further push the importance of a bill or it’s going to D.C. to lobby for important acts Rachel like we did, it’s all critical and something that needs to be done. O’Leary The AVMA has helped make this more of a reality by providing information regarding important acts in the veterinary profession and contacts relevant to these acts much more accessible for those interested in getting in touch with their legislators. How did this trip impact your view on the veterinarian’s role in the legislative process? This trip made me realize how much we can do as veterinarians in the legislative process. We’re a profession that is looked up to by many, but sometimes forgotten in the lawmaking process. Many laws are made for us without actually including us on the discussion. This trip showed me just how we can step up to become a part of the legislative process and just how important it is for us to do so. One of the ways we can help to get legislators on the same page as veterinarians, and the issues in our profession, is to urge them to join the Veterinary Caucus. The Veterinary Caucus is co-chaired by two veterinarians, Dr. Ted Yoho of Florida and Dr. Kurt Schrader of Oregon. The goal of the Caucus is to inform Congress of the challenges facing the veterinary medical profession and ensure the support needed for public and animal health and welfare, education, and research is given. Legislators and their staff are ready to listen, we just have to be ready to talk. After attending, what is your advice to students and veterinarians questioning the impact an individual can make? My advice to those questioning the impact they can make is that the only thing not being heard is the thing you’re not saying. We can’t make an impact on the legislative process if we don’t stand up and make our stories heard. This point was made very apparent by going to D.C. and meeting with legislative staff. Being able to tell a story that is important to you and your profession that supports your cause is the greatest help in these situations. We all CONTINUED ON PAGE 9

Animal Welfare; What’s Your Role?

Over the past few weeks the veterinary role in animal welfare has been front and center for me. I recently presided over the day long WVMA sponsored, Animal Welfare Seminar, attended by nearly 125 veterinarians, law enforcement and district attorneys. What’s your role in Animal Welfare? My role in animal welfare was framed in large part as a farm kid growing up in west-central Wisconsin. My view of animal welfare was based in animal husbandry from the agrarian point of reference. Early in my career, as a recent veterinary school graduate practicing in Rock County, I was exposed to numerous instances of animal neglect or abuse. For reasons I don’t fully understand, I tended to look the other way; maybe even finding excuses for caretakers responsible for neglected animals. My animal welfare epiphany occurred in 1995; the Rock County Sheriff’s Department requested my assistance at an Avon township farmstead. I arrived on scene late afternoon on one of those clear, windy, absolutely frigid winter days. There were approximately 60 head of cattle; mostly beef breeds of varying ages. Nearly half of the cattle were dead; carcasses frozen solid. The other half were emaciated, nary a stem of hay to be found and a half mile trek across an open field through deep snow to the only source of water available - the nearly completely frozen Sugar River. We were fortunate to have Rock Humane Society volunteer’s assistance and within a few hours, the surviving animals were watered and had access to plentiful forage. The surviving animals were confiscated and sold over the following few weeks; the caretaker was found to be suffering from mental disease. As I write these words, the horrific images vividly race through my mind, the empathy overcoming me, just as it did that cold winter afternoon so long ago. This was my wake-up call. No longer would I look the other way, no longer would I make excuses for the caretakers. I became an advocate for neglected and abused animals. How can we as profession not assume this noble role? I owed this to so many of my clients, tirelessly providing the very best of care to animals in the most extreme of winter weather, they didn’t make excuses, they just did it. They didn’t eat, they didn’t drink, they didn’t warm themselves, until the animals under their care were watered, were fed, were sheltered. I was recently recognized by the Rock County Sheriff’s Department as a Citizen of the Year for my work with the Rock County Sherriff’s Department humane officer, Deputy Bambi Stoikes. I was thrilled Deputy Stoikes attended the recent Animal Welfare Seminar with me. She is the one who should receive the recognition; she’s on the animal neglect and cruelty front lines every day. Thank you Deputy Stoikes and all those in law enforcement serving these roles, your work is so important. What’s your role? Your role is being informed and educated regarding the process to successfully assist law enforcement and humane organizations in relieving animal abuse and neglect. Your role is to be vigilant, your role is to not make excuses, your role is to not look the other way. Err on the side of the animal. Most importantly, have the courage take action on behalf of the helpless. If not us, who? What’s your role? l

My oldest child is graduating from high school this month. I am filled with mixed emotions. I am proud of the young man he has become, but am sad I just attended his last track meet, his last FFA banquet and bought from his last dreaded school fundraiser. As I worked on pulling pictures together for his graduation party, it forced me to pause and think about the last 18 years. The dreams, the hopes, the challenges and most of all, the opportunities for him. It has allowed me to sit back, watch and listen to his excitement on entering the next chapter of his life – college. Do you remember the excitement, dreams and goals you had when graduating from high school? College? Veterinary medical school? Does that excitement still exist? Have you accomplished your goals? Have you taken the time to pause and reflect on where you have been and where you are now? If not, take ten minutes to go for a quiet walk, sit by a lake or river or at a park and embrace thinking about how far you have come, what drives your passion and where you are headed – professionally and personally. I have not done enough of that through the years. I get caught up in the hustle and bustle of everyday life and forget to “ground” myself in what matters. My family, my health and making a difference in the lives of others. I found this quote from a college commencement speech that Maria Shriver gave a few years ago. I think it’s impactful and good advice for all of us. “Pausing today, and throughout your entire life, allows you to take a breath, it allows you to take a beat. It allows you to be in the moment. As everybody else is rushing around like lunatics out there, I dare you to do the opposite. Dare to trust in the Power of the Pause.” - Maria Shriver l

Changes Announced for 2017 WVMA Convention Attendees at the 2017 WVMA Convention will notice several changes during the three day event. “We decided after the 2016 Convention that it was time to reevaluate certain aspects and adjust them to best accommodate attendees,” says Kim Brown Pokorny, WVMA executive director. The changes include: • The traditional four day convention has been shortened to three days with the rescheduling of the small animal wetlab. This year, the wetlab will held on the morning of Saturday, October 13. Attendees will bussed from the Alliant Energy Center to the University of Wisconsin School of Veterinary Medicine. • A new exhibit hall layout has been created to allow a better flow down the aisle. This layout will give exhibitors more booth exposure and better interaction with attendees. • Along with the new exhibit hall layout, a Learning Lounge is being introduced. This space will allow exhibitors to display new products to attendees. A Learning Lounge schedule will be available at convention. • The Large Animal Lunch Bunch will be held on Friday instead of Thursday. The Small Animal Lunch Bunch will remain on Thursday. • The Friday lunch Annual Meeting has been moved to Friday morning but will not include the awards presentation. All voting will take place via mail. • All three organizations, WVMA, Wisconsin Veterinary Technicians Association, and Wisconsin Veterinary Practice Managers Association, will host their awards ceremonies during an exhibit hall reception on Thursday, October 12, allowing all of those in veterinary medicine to come together to celebrate each other’s accomplishments. This same reception will also take the place of the WVMA President’s Reception that has traditionally been held on Wednesday night of convention. • Attendees who register for convention online will receive a $10 discount. “The WVMA is always growing and looking for its next step,” says Brown Pokorny. “We are excited to see how these new changes will help make our Annual Convention more enjoyable for all our attendees.” Look for updates on the 2017 WVMA Convention at: http://wvma.org/2017convention. l

Governor Walker Proposes Professional Assistance Program for Veterinarians and Veterinary Technicians By Jordan Lamb, DeWitt Ross & Stevens, s.c. The debate and approval of the 2017-19 state biennial budget is taking center stage in the Wisconsin Legislature this spring. Every two years, the State of Wisconsin must adopt a two-year budget. Governor Walker delivered his proposed biennial budget to both houses of the legislature on February 8th. The legislature will review and amend the budget throughout the spring before sending it back for final gubernatorial review near the end of June. For Wisconsin veterinarians and veterinary technicians, one budget provision is of particular importance. In his budget bill, Governor Walker has proposed to authorize the Veterinary Examining Board (VEB) to establish a procedure to permit the Department of Agriculture, Trade and Consumer Protection (DATCP) to identify professional assistance programs that would provide chemical dependency treatment services or assistance in obtaining mental health services to individuals licensed by the board who are in need of these services. The language in the bill directs the VEB to facilitate early identification

of chemically dependent veterinarians or veterinary technicians and to encourage their rehabilitation. Importantly, existing veterinary and veterinary technician licensure fees would be used to pay for the program. (There is no change to veterinary or veterinary technician license fees needed to fund this program.) The creation of such a chemical dependency and mental health professional assistance program for licensed Wisconsin veterinarians and veterinary technicians is much needed by our community. In recent years, the stress of this profession has produced a rise in self-reported struggles with substance abuse and workplace stress. The creation of an accessible professional assistance program for Wisconsin veterinarians and veterinary technicians is needed and supported by our community. The WVMA sincerely appreciates the Governor’s leadership on this critical issue and we urge the Wisconsin legislature to support and maintain this provision in the final version of the budget that is passed this summer. l

Chemicals in the Veterinary Clinic By Leslie Ptak, Department of Labor/OSHA compliance assistance specialist A vast array of chemicals is routinely used in veterinary clinics. Cleaning compounds, pharmaceuticals, pesticides, compressed gases, sterilizing agents and anesthetic agents—are used or handled by clinic workers or contractors, such as janitorial crews. While these chemicals have utility and benefits, exposure to them has the potential to cause adverse effects. These adverse effects include both health hazards (such as carcinogenicity and sensitization), and physical hazards (for example, flammability and reactivity properties). In veterinary clinics, some significant chemical exposures include: • Anesthetic agents, such as isoflurane • Chemotherapeutic agents, such as doxorubicin and cyclophosphamide • Prostaglandins, such as Prostaglandin F2α • Sterilants (ethylene oxide) • Disinfectants, such as iodine, bleach and phenolics like Lysol™ • Compressed gases, such as oxygen • Cleaning compounds for janitorial purposes, such as bathroom cleaner, dish soap, and floor cleaner • Pesticides used for insect and vermin control Sources of exposure. There are four ways for employees to be exposed to chemicals: inhalation, ingestion, skin and mucous membrane contact and absorption. Chemicals can become airborne when they take the form of dusts, fumes, mists, vapors, gases and aerosols. Liquids, lotions, gels, and even dusts, can get on skin and clothing and result in unintended ingestion, skin contact and skin absorption. Enlist the employees. Because veterinary clinics are almost universally small entities, employees need to take on collateral duties. One of those duties is helping to develop and implement the safety and health measures for employee exposures to hazardous chemicals. At first glance, this may seem problematic because it is yet another task that has to be completed by staff. However, employee involvement in developing and implementing safety and health measures is highly recommended. Employees are usually the persons with the most exposure, and they are most likely to understand the solutions. They are also likely to have opinions as to whether a suggested control will work or not for that particular clinic. Identify and evaluate the exposures. From the time the container of chemicals is opened until it is used up or disposed of, employees can potentially inhale, swallow or contaminate

their skin and clothes with dangerous products. If the veterinary clinic does not have the professional staff necessary to evaluate workplace risks, other options are available. An industrial hygiene consulting firm, an occupational health clinic, a consulting occupational physician or the Wisconsin Onsite Consultation Program can help. The individuals should visit the clinic, talk to the employees to determine what the exposures are, and consider conducting an industrial hygiene survey. This may involve direct reading instruments, such as TIF refrigerant leak detectors for halogenated anesthetic gases, or passive sampling badges for ethylene oxide. All aspects of chemical exposure should be evaluated, from the time the chemical is brought onsite, to how it is stored, prepared and used, and finally disposed of. Comply with established regulatory standards. The safety professional you enlist to conduct your clinic hazard evaluation should be able to direct you to the regulatory standards that exist for the chemicals. Some of them, such as 1910.1047 for ethylene oxide, are comprehensive, and will be addressed in future WVMAOSHA articles. Follow established guidelines for hazardous chemicals. The National Institute for Occupational Safety and Health (NIOSH) has established guidance for veterinary clinics and hazardous drugs in one document, Safe Handling of Hazardous Drugs for Veterinary Healthcare Workers. Because it is comprehensive and readily available at no cost to clinics, the recommendations will not be repeated in this article. Please download a PDF of the document from their web site and make use of it when evaluating your workplace and establishing the controls needed to protect employees. NIOSH also regularly updates a list of drugs considered to be highly toxic. Called the NIOSH List of Antineoplastic and Other Hazardous Drugs in Healthcare Settings, 2014, it should be reviewed as part of the clinic evaluation. Follow the hierarchy of controls. If there are potential exposures to chemicals, they should be addressed using the hierarchy of controls, which is a ranking of control methods from most to least effective. OSHA’s longstanding policy is that engineering and work practice controls must be the primary means used to reduce employee exposure to toxic agents, as long as feasible, and that respiratory protection and other personal protective equipment is required when engineering or work practice controls are infeasible or while they are being implemented. Elimination or substitution of the agent is the most desirable option. Relying on personal protective equipment is the least. CONTINUED ON PAGE 12

saw first-hand the impact it made on those we met when we made the issues personable and shared with them a piece of our livelihoods. It was then no longer just an act printed on a piece of paper that we were handing them. It was someone’s life and future career that will in turn affect the whole population. How important is the power of organized veterinary medicine, on the state and national level, when dealing with policy and legislation impacting veterinarians and small business? Organized veterinary medicine at the state and national level is of the utmost importance when dealing with policy and legislation. A group of individuals all going towards a common goal can speak much louder compared to just one. While in D.C., one of the things we were trying to push was the Veterinary Caucus. Currently, only one Wisconsin representative, Mark Pocan, is a part of the Caucus. The more representatives we can get involved in this, the more we can share our ideas and express our thoughts about the legislation impacting veterinarians and small business. I feel with the Caucus growing and having representatives from Oregon, Louisiana, and Florida, as well as the new Secretary of Agriculture, all of whom are veterinarians, we are taking a step in the right direction. With saying this, I want to urge that we can still do more. The more we get involved and the more our voices are heard, the faster we start going in the right direction for our patients, our clients, our families, and our futures. Lucas Budden, Class of 2018 What was your biggest take-away? My biggest take-away was that veterinarians have an important role to play in policy as individuals and also as part of their larger community. As small as many people feel they are as individuals, making issues personal and having direct communication with representatives makes us all larger than we would think. How did this trip impact your view on the veterinarian’s role in the legislative process? Lucas Budden

This definitely changed my view on my role as a future veterinarian in the legislative process. I never saw myself getting involved or being effective, but this experience showed me how important and influential I can be on an individual basis. It is the role of the veterinarian to educate elected officials on how laws are affecting veterinarians personally. We are the only ones who can do this because we are the ones that experience that life every day.

After attending, what is your advice to students and veterinarians questioning the impact an individual can make? I would say that individuals are the ones that make policy personal. Rachel and I were able to bring our personal stories into the conversations we had with our representatives and show how these policies would affect us directly. Hearing individual stories will open anyone’s mind to a wider perspective. This may not always change someone’s mind on an issue, but may force that individual to reexamine their previous views. If you are questioning how effective you can be as an individual, just realize how unique your own story is and use that perspective to make your voice heard. How important is the power of organized veterinary medicine, on the state and national level, when dealing with policy and legislation impacting veterinarians and small business? Organized veterinary medicine can take an individual’s voice and amplify it. It can take the voices of many communities and combine them to show how many people certain issues may be affecting. It allows ideas to be spread faster and among a much wider variety of individuals. This is important for veterinarians because the profession is still small and there is such a wide range of careers that veterinarians pursue. Organized veterinary medicine allows these different individuals to come together and get behind one solid front. On both a state and national level this can be incredibly effective compared to many smaller communities attacking issues from different directions. l
