We all agree skin health is difficult to assess in persons with medium and dark skin tones and there are challenges in this area. What else can we do to decrease the disparities in skin health related to detection of damage among persons of color? Of course, the answer is use of technology such as the SEM Scanner or thermography, but not all health care institutions will be able to access technology. So, while we wait for technology we can also conduct better visual skin assessments by learning what skin discoloration and skin damage looks like across a spectrum of skin tones. We can better educate providers using data from diverse skin tones and importantly, we can ask for more information from those who live in diverse skin tones. This panel will present basic skin assessment parameters and then focus on best practices for skin assessment in medium and dark skin tones. A panel of WOC Nurses will discuss how skin damage appears in medium and dark skin tones with audience participation.

Nursing sensitive quality measures are important for wound, ostomy, and continence (WOC) nurses to use as they demonstrate the value of the care WOC nurses provide. Nursing has a responsibility to measure, evaluate and improve practice.
The Wound, Ostomy, and Continence Nurses Society (WOCN) has identified a set of measures for WOC nurses to use in practice to demonstrate the value of the care, both quality and costs, provided by WOC nurses that improve patient care and outcomes: The WOC Quality Measure Set. The WOCN WOC Quality Measure set includes measures focused on pressure injury and continence.

Clinical lab values provide objective assessment of a patient’s status and have become an essential component of care in modern medicine. In the context of wound care, lab values provide important insight into the nutritional status of the patient. In addition to traditional “point in time” measurement of clinical status, the use of the electronic medical record to rapidly search lab values and demonstrate trends in labs over time has improved our ability to prognosticate a patient’s clinical status. Lastly, clinical labs, including nutritional labs, may be affected by inflammation and the acute phase response, causing false normalization. Optimal interpretation requires lab interpretation in the context of ongoing medical issues and past surgical history. Optimal wound care can benefit from proactive assessment of nutritional labs with nuanced interpretation.

UPMC Presbyterian Shadyside provides care to hundreds of patients each year who undergo a radical cystectomy as part of urologic cancer treatment. The Wound Ostomy Continence Nursing Department identified an opportunity for improvement for patients
with urinary diversions. An observation was made that many patients were experiencing leakage in the postoperative period, resulting in peristomal skin complications and increased clinic visits related to difficulty managing urinary diversions. This resulted in negative impacts to our ostomates quality of life. In order to improve outcomes for our ostomates and provide high-quality care, a test of change was implemented. The certified ostomy nurses worked collaboratively to embrace a practice change to utilize convex barriers proactively in the immediate postoperative period to help prevent leakage, increase wear time, and maintain peristomal skin health. Selecting the right type of convexity from the start is essential to ensure optimal pouching results for patients. By utilizing this strategy, we were able to reduce postoperative leakage, decrease peristomal skin complications, and reduce clinic visits related to complications with managing urinary diversions. Thus, the proactive use and proper selection of convex barriers in the postoperative period for patients with urinary diversions has shown to improve outcomes for ostomates within our facility.

Though urinary incontinence has become a mainstream topic and easily discussed with providers and promoted treatments flood the television market, fecal incontinence is the silent counterpart: rarely discussed, infrequently treated and suffered by many. In the community setting, 7-15% of the population experiences fecal incontinence, but few seek treatment due to social stigma. Many become isolated. This presentation will highlight causes and treatments of FI as well as the social and emotional impact on the patient.

Remember when using convexity in the post-op period was taboo? For decades WOC nurses have had to rely on their own clinical knowledge and expertise when it came to using convexity in the immediate post-operative period because there was little evidence to support this practice. TODAY there is evidence that allows WOC nurses to be PROACTIVE in their practice and use convexity safely to support healthy peristomal skin regardless of when the stoma was created.
During this talk show style symposium, an acute care and home care WOC nurse, will look at the recent evidence around the use of convexity and discuss how they use soft convexity earlier in their patient’s journey to achieve positive outcomes and set their patients up for success from the start.

The diagnosis of wound infection is made clinically, based on signs and symptoms that a patient has, or non-healing despite the best care. Although empiric antibiotics are started on a patient with a wound infection, cultures are also obtained to guide antibiotic therapy moving forward. What are covert and overt signs and symptoms of infection? How should a culture be taken to optimally identify infecting bacteria? What does the clinician do with the information from the culture report? This session will address these questions and provide guidance regarding best practice.

Although urinary symptoms in adults are considered to be urological concerns, Nocturia and Nocturnal Enuresis are commonly associated with much broader medical and psychosocial diagnoses. One must create a comfortable environment in which the patient can be interviewed in a manner which assuages embarrassment and permits thorough review of other medical history, lifestyle habits or changes and psychological concerns.
While bladder dysfunction manifests with both daytime and nighttime frequency- the latter termed nocturia, most cases of nocturia are due to nocturnal polyuria. In order to make this distinction, use of a voiding diary is paramount. This will be demonstrated.
Treatment of underlying causes of nocturia and enuresis will be reviewed. Most cases, however, require a multi-pronged approach, which will be discussed.

When done poorly, it can make a bad situation worse. In this session, nursing, physician and risk management leaders will guide learners through the process of error disclosure, step-by-step. Learners will be provided a framework on how to conduct these critical conversations with patients and families that includes a focus on the initial communication and the aspects of necessary on-going follow up. As a result, learners will feel empowered to conduct robust disclosure conversations.

Evidence concerning the epidemiology of ostomy surgery performed in the United States is lacking, leading to speculations about current trends in ostomy surgery without foundational evidence needed to support ostomy care during the initial hospital course and postoperative care during the first 30 days following stoma surgery. Recently, an analysis of data from the PINC AI Healthcare Database was published in the Journal of Wound, Ostomy & Continence Nursing. This large database stores de-identified data from 25% of all hospital admissions in the US and this article analyzed data from 27,658 adult patients cared for in 658 acute care facilities across the US. This presentation will briefly summarize major findings from this landmark article, followed by an in-depth and lively discussion and analysis of implications for ostomy nursing practice, research and public policies.

Pressure injuries are a serious health care problem and threaten the health and well-being of millions of people. Most pressure injuries are avoidable with the application of standard practices and proper utilization of existing technology. Support surface technology has been shown to prevent pressure injuries, but the effectiveness of specific features designed for management of tissue loads, microclimate (i.e., heat and moisture), and/or other therapeutic functions are unknown.
Preclinical research has demonstrated that heat and moisture are important pressure injury risk factors. Some support surfaces include a feature designed to manage the heat and moisture at the interface between the person and the surface using active airflow (sometimes referred to as a low air loss feature).
Clinical research has produced evidence to support the use of reactive support surfaces with good immersion and envelopment characteristics, but there is a gap in evidence regarding the effectiveness of microclimate management features.
This randomized clinical trial aimed to first determine if low air loss is effective for preventing pressure injuries. Second, to facilitate translation of the results to clinical practice, the study is designed to identify which low air loss performance characteristics and what levels of those performance characteristics are necessary for the technology to be effective.

Collaboration between WOC Nurses and Senior Director Outpatient Clinical Operations and Data Analytics led to the development of a new infrastructure and methodology for Hospital Acquired Pressure Injury (HAPI) data collection, reporting, and summarizing trends. This strategy continues to guide and set accountability measures and action planning to reduce HAPI trends throughout the 2 campuses of an Academic Medical Center. HAPIs, especially Stage 3 and above, can be considered Never Events and have detrimental consequences on hospitals by increasing the cost of non-reimbursable care (i.e., more labor, more tests, higher length of stay, more complications, more surgical time), as well as ensue pay-for-performance penalties and negatively impact public report for quality outcomes. It is important to understand baseline HAPI volumes, with specificity to where they originate, how they progress during a patient’s hospital stay, and patient demographics. WOC Nurses can more easily coordinate HAPI prevention tactics with inpatient nursing and providers to reduce HAPIs if they create actionable goals around trends observed in HAPI analytics. Quality improvement is the foundation to identify gaps in practice and implementing small change using the PDSA cycle to close those gaps or pivot to another methodology. Quality improvement project effectiveness is measured with data to identify gaps in practice and effectiveness of intervention. Integrity of data is critical for accurate measurement of outcomes.

A burn is defined as damage to the skin and underlying tissue caused by heat, chemical or electricity. Each year in the United States more than 480,000 people receive medical attention for burn injuries and worldwide nearly 11 million receive care. Effective wound healing treatments are vital components of early burn care that aim to alleviate the physical and psychosocial challenges patients experience following a burn injury. Goals of modern burn care now focus on efficient wound healing whilst optimizing functional and aesthetic outcomes. Being able to appropriately assess and put into place an appropriate treatment plan is critical as burn assessment is slightly different than a wound assessment.

Recent literature and research findings indicate that people with darker skin tones are more likely to develop higher stage and more severe pressure injuries. One thought is current skin assessment protocols are less effective for people who have darker skin tones resulting in early damage arising from pressure not being visualized as easily as it can be in lighter skin toned Caucasian patients. We have identified within our own medical center the need for our clinicians to consider skin tone variations (specifically darker toned skin) when providing initial and subsequent skin assessments. More research and descriptive assessment protocols are needed.

The WOCN® Society recently announced the start of a major initiative that aims to support and PROVE the value of wound, ostomy, and continence nurses in healthcare.
This collaborative initiative between the WOCN® Society and its members aims to amass a database of quality measures across WOCN® member facilities to benchmark our rates against national averages to:
Demonstrate the clinical value of WOC nurses within our community
Provide greater access to quality improvement resources and tools
Find linkages between compensation, satisfaction, and quality measures
Effect policy changes that will improve WOC care and patient outcomes
Data to draw these important comparisons and conclusions hasn’t existed in our field—and we’re ready to change that! But we need your help…
We’re asking ALL members to help us gather this information. Visit wocn.org/the-value-of-a-woc-nurse for more details, instructions, and resources to help us, help you!
