Regulatory bodies have not yet set parameters for measuring convex ostomy product characteristics and as a result, manufacturers have developed different means for measuring and describing them. Clinicians consequently select convex skin barriers based on subjective assessments rather than objective criteria. An international group of ostomy care nurses met to discuss and put forward five objective and easily identifiable characteristics that can be used when considering a convex skin barrier for a person with an ostomy’s individual need. Attend this session to learn more about the proposed standardized nomenclature and enhance your selection criteria of convex ostomy skin barrier products.

Machines are evaluated based on the output of the entire unit, not the impressiveness of their individual parts. Extraordinary teams, like great machines, require certain components that hold their pieces together. While most organizations focus exclusively on building the skills of their individual members, elite organizations invest in the “stuff” that transforms a collection of talented individuals into extraordinary, machinelike teams.

Peristomal skin health is a priority for promoting quality of life in persons with an ostomy. However, patients, caregivers, and staff can have challenges in sizing stoma apertures to fit oval, oblique, and irregularly shaped stomas with the risk of peristomal skin breakdown. This session will address how scissorless (Shapable) ostomy barrier can expedite care in selected settings (e.g., operating room, long-term care) while also discussing cost savings and improved patient outcomes in real-world utilization.

Speakers: Donna Bliss, PhD, RN, FGSA, FAAN Mikel Gray, PhD, FNP, PNP, CUNP, CCCN, FAANP, FAAN Laurie McNichol, MSN, RN, CNS, GNP, CWOCN, CWON-AP, FAAN MASD is a complex condition that, when coupled with a shift in demographics towards an aging population and unspecified diagnosis codes, has placed a significant burden on patients and the health system. The WOCN Society responded to this need by applying for ICD-10-CM codes specific to MASD conditions related to irritant contact dermatitis. On October 1, 2021 those new codes for MASD conditions went into effect, having been approved by the ICD-10 Coordination and Maintenance Committee. Attend this session to learn more about the new codes and what actions YOU need to take to incorporate them into your practice.

Globally there are around 1.8 million new cases of colorectal cancer per year with rectal cancer accounting for one third of all colorectal cancers. Sphincter sparing surgery with low anterior resection of the rectum is commonly the surgery of choice. Low anterior resection syndrome (LARS), faecal urgency, incontinence and difficult evacuation, often combined with urinary and sexual dysfunction, may be a consequence. Advances in the treatment of rectal cancer have increased cancer survivorship leading to an increase in individuals with LARS. Enterostomal Therapists, WOC Nurses and Colorectal Nurse Specialist, are well positioned to assess and manage many of the LARS symptoms.

This session will provide a holistic, global approach comprehending skin and sepsis, nutrition, patient anatomy, and surgical procedures with the fistula patient population. Patient case examples will illustrate medical and surgical techniques that may be used to improve patient quality of life. Provide practical “Here’s how I do it” wound care methods and will cover products used and when they are appropriate for complex fistula challenges.

Does poor diet contribute to the burden of chronic disease and premature death? The Global Burden of Disease Study demonstrated that poor diet is an equal opportunity killer linked to 11 million deaths worldwide. Poor diets and many of the health consequences that follow are modifiable. This session will look at ostomy and skin health and through the lens of nutrition as a critical component of overall well-being, healing wounds, and sustaining skin integrity. We will examine trends in health and popular diets, role of key nutrients in the medical management of selected chronic diseases and provide actionoriented strategies to meet nutrient requirements for skin integrity, wound healing and to manage nutrition-related ostomy problems.

Contemporary WOC practice is focused on quality patient care with identifiable positive outcomes. WOC nurses may also have to provide data that demonstrate their clinical effectiveness and efficiency. This session will describe data management for WOC nurses focusing on pertinent data for collection, data entry and analysis, and crucial aspects of data interpretation. Implications for personal and professional development and success will be emphasized.

GENERAL SESSIONS

Ostomy care nurses use multiple clinical parameters and clinical judgement, based on evolving education and experience, to help each patient find the best pouching system to minimize leakage. Ideally, this process is based on clinical practice guidelines; however, when it comes to the use of convexity as a first-line choice there is a lack of guidance in the post-operative period. In this session, you will hear from a panel of experts as they discuss the use of a convex pouching system in the post-operative period – including the immediate (0-8 days), post-operative (9-30 days), and transition (31-180 days) periods - based on their involvement in a recent national consensus.

Nurses can develop creative ways to overcome symptom management in their patients especially when it involves bodily fluids such as diarrhea. We found that nurses were using Nasopharyngeal Airway devices as off label rectal trumpets to manage fecal incontinence. However, in addressing the diarrhea containment problem other unintended patient problems soon developed resulting in several adverse patient safety events including: rectal mucosal injury, bleeding, and perirectal injuries. The team revised the existing fecal containment policy which now includes a focus on patient assessment of reversible factors resulting in diarrhea, and initiation of nursing interventions to address those factors. A formal protocol was developed to address inappropriate utilization of fecal containment systems.

Speaker: Sarah Russell, MSc In this presentation, Sarah will share the evidence and research about the importance of core exercises for ostomy patients. Instead of ‘rest for 6 weeks’ or ‘avoid core exercises’ she will talk about the important role of early rehabilitation and how this could reduce the risk of parastomal hernia. In the presentation, she will also share exercises that nurses can teach their patients right away, plus highlight some exercises that patients might want to avoid. After this presentation, participants will feel more confident to talk about safe and appropriate core exercise with their ostomy patients.

The world of intestinal failure has changed markedly over the last 30 years. From TPN dependence and the complications of intestinal failure associated liver disease and central line management, intestinal transplantation came to the fore as the ultimate salvage therapy. Now some 30 years later, the pendulum has swung somewhat to fewer transplants and better long term salvage with medical and surgical therapies for intestinal failure. New pharmacological therapies may offer alternatives to transplantation. This session will review where we started, how we progressed and what is the paradigm for managing intestinal failure patients, adults and children currently.

Nurses and health care aides provide the majority of frontline continence care across settings, from hospital to long term care. Unfortunately, this care is often seen as ‘simple” or “basic”, and assigned to care aides with minimal professional involvement. Myths abound, including the myth that incontinence is an inevitable part of aging. This presentation will focus on how education for nurses and health care aides can shift to understanding continence as a complex part of care when viewed from the lens of lower urinary tract symptoms rather than a binary of people being simply continent or incontinent. The different learning needs for licensed nurses versus care a ides will be identified, and approaches to team work to improve continence promotion, assessment and management through education strategies explored.

In this session, we will explore five classes of medications that increase the risk of incontinence or increased risk of related issues including Incontinence Associated Dermatitis (IAD), adverse changes in cognition, falls and urinary tract infection. Drug classes that will be reviewed include: anticholinergics, calcium channel blockers, diuretics, psychotropic drugs, and sodium-glucose co-transporter 2 (SGLT2) inhibitors.

For millions of people around the globe, travelling is an integral part of modern life, encompassing both recreational and occupational activities, with little attention being paid to the complexities it may hold. However, since the outbreak of the novel Covid-19 strain of the coronavirus, the concept of travel has changed universally. For some people, travel has become impossible or too challenging to consider, leading to an array of physical and psychological conditions. This session aims to inform nurses how they can holistically support stoma and pouch patients who are either new to the concept of travel or those who have lost confidence to do so.

This session will provide an opportunity for you to hear the latest research findings in WOC nursing. Research-based abstracts are selected by a rigorous, two-step peer review process. Clinical investigators will present their findings in a moderated session.

This session aims to provide an overview of the roles of medical decision-making and education in pediatric ostomy surgery, with a focus on the developmental nuance this populations present. The decision-making by parents, patients, and providers are complex in navigating the child’s development, psychosocial health/considerations, and clinical equipoise of other medical options; additionally, these experiences become more nuanced in integrating the child or adolescent’s perspective in a manner that is developmentally and culturally appropriate. This session will incorporate recent research about what is known for medical decision-making and education in pediatric ostomy surgery. Implications of this research, as well as the gaps in knowledge will be discussed and will then highlight the work that needs to be done in this area. Additionally, this session will incorporate resources and clinical considerations for attendees.

The key to preventing and managing pressure injuries is reducing pressure and shear forces wherever they are – hospital, home, rehab centre, community etc. Bed rest has negative health consequences for clients, making mobilization and sitting a priority. As a result, managing pressure and shear forces in sitting is critical to the overall pressure injury prevention and management plan.

Over the last four decades, the ileoanal pouch operation has seen great improvements in surgical technique, pouch configuration, medical management and patient outcomes. However innovation in this area can be daunting for less experienced practitioners and change is not always welcome, especially if costs or risks appear to outweigh potential benefits. This session aims to inform nurses to better support pouch patients and surgical colleagues when innovation is inevitable.

This practical workshop will equip participants with the information they need to assess, trouble shoot options and decide when a referral to an interprofessional team member is warranted. Even when a referral is made, participants will develop strategies to improve pressure and shear management until the client is seen.

The first part of this session will discuss urinary incontinence, a common condition in older adults, many of whom believe that this is normal. Typically, incontinence in older adults results from a combination of factors, not only lower urinary tract and pelvic floor dysfunction. This session will provide a framework by which to consider the multi-aetiological nature of incontinence in later life as a "geriatric syndrome" and offer practical steps in management including the potential of pharmacological therapies to ameliorate incontinence in later life.

It is well established that high levels of rapidly proliferating, pathogenic bacteria (infection) can impair healing of wounds. More recently, multiple laboratory and clinical studies have documented that when bacteria attach and create a biofilm, they can exhibit very high tolerance to antibodies, antibiotics, antiseptics, and killing by neutrophils. A chronic inflammatory state also occurs that destroy proteins that are essential for healing. Biofilm-based chronic wound care emphasizes the “Step-Down Then Step-Up” concepts of initiating aggressive and frequent debridement to remove necrotic tissue and

The economic and human cost to treat wounds are high and can quickly spiral out of control. Healthcare providers are asked to do more with less.

Transportation to and from River Ranch is supported by Mölnlycke Health Care.

Did you know, 70% of dressing changes are driven by schedules? Unnecessary dressing changes consume valuable time which could be used for patients’ other clinical needs, impacting their wellbeing. 24% of patients with chronic wounds have lived with their wound for at least 6 months, with almost 16% remaining unhealed for a year or more. It’s time to take control of chronic wound care, while empowering our patients in their care.

Type 2 diabetes is a worldwide epidemic, often leading to serious complications and premature mortality. In the context of Covid-19, poorly controlled diabetes greatly increases mortality, and racial disparities in diabetes prevalence have fueled disparities in Covid-19 mortality.

This session will focus on updates in robotic surgery for cystectomy and optimization of pre and post operative cystectomy patients to improve outcomes. This will include a review of the pros and cons of a robotic approach for cystectomy. A focus on enhanced recovery after surgery coupled with a unique interactive-home monitoring program and the role of the APP and WOC nurse will be discussed. Reducing readmissions is the focus of this surgical program for optimal patient outcomes.

New nutritional interventions for diabetes have shown surprising power in both its prevention and management, and particular attention has been paid to plant-based diets. As a group, individuals following such diets have a lower diabetes prevalence, compared to others. They also tend to have lower cholesterol levels and are less likely to be overweight or hypertensive. In clinical trials, plant-based diets have led to dramatic improvements in diabetes management, typically reducing medication requirements and sometimes making the condition disappear for all intents and purposes.

(507A) Starting an Ostomy Clinic: An APRN Experience (O, PP) 9:50 AM CDT - 10:20 AM CDT 30 (Lecture and Discussion) CONTACT HOURS: 0 .5

Type 2 diabetes starts with the build-up of microscopic fat particles inside muscle and liver cells (intramyocellular and hepatocellular lipid), which causes the cells to stop responding normally to insulin, a condition called insulin resistance. A low-fat, plant-based diet—which, of course, has no animal fat and very little fat in general—directly addresses this fundamental cause of diabetes. Magnetic resonance spectroscopy shows that a low-fat plant-based diet reliably reduces intramyocellular and hepatocellular lipid, with the result that insulin resistance can be replaced by increasing insulin sensitivity.

Speaker: Jennifer Giannettino, APRN, CWOCN-AP Advance practice providers are being utilized in many different capacities to address gaps in care within our health care system. In 2012, the Wound, Ostomy and Continence Nursing Certification Board started to offer certification in wound ostomy and continence for providers at an advance practice level. This was done to recognize the benefits that additional training may offer to this patient population including ordering and interpreting diagnostic tests, medication management as well as prevention of emergency department visits/hospitalizations for skin and fluid/electrolyte balance and prescription management for ostomy supplies. This session will describe the transformation of a small nurse-run ostomy clinic into a system-wide nurse practitioner run program.

This presentation will describe the rationale for a plant-based diet and how to put it to use, including simple steps that can be used by patients in the clinical setting. Because the intervention relies on food, the result is not only improvement in the identified patient, but often also in the family.

Peri-stomal skin complication are far too prevalent and have serious quality-of-life consequences for ostomates. In this educational session, Prof. Karen Ousey will discuss the causes of peri-stomal skin damage, and the key role silicone can play to maintain and improve peri-stomal skin. The benefits of silicone in medical devices are well documented in many different product categories. However, silicone has had a minor role in ostomy management, yet may offer significant benefit for ostomates. Dr. Tom Swift a researcher who introduced Sil2, a unique, patented, silicone-based adhesive with Trans-Epidermal Water Loss (TEWL) similar to normal skin, will discuss his scientific findings when comparing the new silicone adhesive to traditional hydrocolloids. These findings include on the effects of skin stripping, breathability, pressures at the skin interface, durability, and absorbency. These scientific findings may be the answer in reducing peri-stomal skin complications impacting ostomate’s quality of life. Finally, you will hear from ostomates themselves on how the science is translating to actual ostomates improving their quality of life. From the lab to the clinician to the ostomate, you will gain understanding of the importance of this new silicone ostomy adhesive in Trio Ostomy Care’s new Genii one-piece silicone ostomy appliances with Sil2 Breathable silicone technology.
