LETTER FROM THE CHAIR 1 pediatrics.wisc.edu ANNUAL REPORT 2022

The University of Wisconsin School of Medicine and Public Health (SMPH) held its inaugural Investiture Celebration honoring 32 faculty members and their philanthropic partners. Three Department of Pediatrics faculty members were honored at the event. In the photo, left to right, are Ellen Wald, MD, department chair, Mary Ehlenbach, MD, O’Connor Family Professor in Pediatrics, Peter Ferrazzano, MD, Enid and Jerry Weygandt Professor in Pediatric Critical Care, Robert N. Golden, MD, SMPH dean, and Christian Capitini, MD, Jean R. Finley Professor in Pediatric Hematology and Oncology.

A photo of white matter, with its predominant orientation of fibers colored to reveal pathways traversing the regions, was a winner in the UW–Madison 2021 Cool Science Image Contest. Peter Ferrazzano, MD, professor and chief, Division of Critical Care, was one of the members of the research team that created the image using a magnetic resonance imaging (MRI) scan.

In September 2021, UW Health announced a new name for all of the pediatric services offered in Madison and beyond. UW Health Kids is care from infancy to adulthood. It is checkups, vaccinations, bike helmets, and car seats. It is specialty care for complex diseases and preventive care for wellness at home, in school, and on the playground. It is tertiary and quaternary care for acute and chronic illnesses in the clinic and the hospital.

The Reach Out and Read Podcast is a public outreach program from Reach Out and Read, a nonprofit that gives young children a foundation for success by incorporating books into pediatric care and encouraging families to read aloud together. The podcast’s host is Dipesh Navsaria, MPH, MSLIS, MD, associate professor, Division of General Pediatrics and Adolescent Medicine with a joint appointment in the UW School of Human Ecology. Find it at reachoutandread.org/podcast.

TABLE

It is with great pride that I share the University of Wisconsin Department of Pediatrics 2022 Annual Report. Inside you will find highlights of the extraordinary clinical care, research activities, and educational milestones that took place in the department between July 1, 2021, and June 30, 2022.

The 2022 academic year was one filled with great accomplishments and achievements. We saw a more than 12 percent increase in the number of patient visits over the previous year, partly a rebound from pandemic-related declines but about half from new growth. We ranked 7th in the nation among pediatric departments receiving research funding from the National Institutes of Health. And we received accreditation for a new Pediatric Hospital Medicine fellowship,

One initiative I am especially proud to share with you is the introduction of five new departmentwide directorships. When we initiated our strategic planning process in the summer of 2018, leaders determined there were some broad areas of concern relevant across all of the department’s 16 divisions. They created five directorships to develop, initiate, and oversee strategic improvements in these areas, and appointed faculty to those areas. The directorships and their inaugural directors are listed below:

• Director of Advocacy Laura Houser, MD, professor, Division of General Pediatrics and Adolescent Medicine and Division of Global Health. The department includes “vigorous advocacy” as essential to its purpose. Speaking and acting on behalf of children, who have no voice or agency in the wider world, faculty connect with schools, legislative bodies, and other communities to promote children’s health and safety. This directorship’s purpose is to spotlight existing advocacy efforts in the department and facilitate effective participation in new areas.

• Director of Diversity, Equity, and Inclusion (DEI), Mary Ehlenbach, MD, associate professor, Division of Hospital Medicine. This area focuses on the need to make the department an inclusive and equitable place to work, addressing racism, sexism, and other destructive biases both in the workplace and in society at large. This work is essential in advancing child health and the field of pediatrics.

• Director of Professional Development Mala Mathur, MD, MPH, associate professor in the Division of General Pediatrics and Adolescent Medicine. Professional development is a robust program with a long history within the department. Its many offerings include Pediatric Grand Rounds, Seminars in Pediatrics, Research Week, workshops, a professional development series, and an increasing number of lectureships.

• Director of Quality Improvement Scholarship Kristin Shadman, MD, associate professor, Division of Hospital Medicine. An overarching purpose of this directorship is to encourage and support scholarly work in QI within the department. Those who develop innovative methods to improve patient care must be able to present their work as a recognized and appreciated scholarly product.

• Director of Well-being Sarah Webber, MD, associate professor, Division of Hospital Medicine. Well-being of staff, faculty, clinicians, and trainees is a priority across our institution. This position was developed in order to encourage and facilitate a systems approach to worker well-being within the department.

Please enjoy reading about the department and learning about the work of our faculty, staff, and trainees. I take pride in their remarkable accomplishments, incredible talents, and inspiring commitments to our patients and families. I hope you do too.

A SELECTION OF NATIONAL INSTITUTES OF HEALTH (NIH) AWARDS

Studying early brain recovery and development in perinatal stroke patients

Perinatal stroke is a common cause of cerebral palsy and life-long movement impairment. Scientists predict early interventions may encourage brain recovery following perinatal stroke, but data on brain development after a stroke in the weeks around birth is lacking. Bernadette Gillick, PhD, MSPT, PT, associate professor, Division of Developmental Pediatrics and Rehabilitation Medicine, will study brain connectivity and development in 50 infants with perinatal stroke through her R01 project “Perinatal Stroke: Longitudinal Assessment of Infant Brain Organization and Recovery through Neuroexcitability, Neuroimaging and Motor Development.” This award from the Eunice Kennedy Schriver National Institute of Child Health & Human Development is expected to total $2.9 million over five years. This research could lead to the development of early interventions intended to encourage brain recovery following perinatal stroke.

Developing new cellular therapies for radiation injury

Christian Capitini, MD, associate professor, Division of Hematology, Oncology, and Bone Marrow Transplant, was awarded an R01 from the National Heart, Lung, and Blood Institute to study new therapies that could be used to treat patients experiencing radiation-induced illness. The grant for “Exosome Educated Monocytes for Acute Radiation Syndrome” is expected to total more than $2 million over four years. Capitini’s team will use exosomes from stimulated mesenchymal stromal cells to develop radioprotective monocytes as a therapy for acute radiation syndrome. This work could lead to new ways to treat patients who experience side effects from common radiotherapy treatments for cancer and other conditions, as well as nuclear accidents or terrorist attacks.

Investigating genetic variation in immune response to a toxic fungus endemic in Wisconsin

For more than 30 years, Bruce Klein, MD, professor and chief, Division of Infectious Diseases, has studied Blastomyces dermatitidis, the toxic fungus that causes the respiratory illness blastomycosis. This fungus, present in the Great Lakes states and the Mississippi and Ohio River valleys but uniquely prevalent in Wisconsin, grows in the decaying leaves and wood in acidic soils. Klein was granted his second R37 MERIT award from the National Institutes of Health for his project “Molecular Pathogenesis of Blastomycosis.” This grant is expected to provide up to $4.3 million for up to seven years to study the genetic underpinnings of susceptibility to fungal infections.

Understanding wound repair at the cellular level

Anna Huttenlocher, MD, professor, Division of Allergy, Immunology, and Rheumatology, is the recipient of an R35 Maximizing Investigator Research Award (MIRA) from the National Institute of General Medical Sciences to further research on how inflammation resolves and tissues are repaired in the context of human health and disease. The competitive renewal grant is expected to total more than $3.3 million and will fund years six through 10 of her project “Cell Migration and Wound Repair.” Huttenlocher’s research aims to uncover mechanisms of cell migration within tissues, the signaling networks that regulate these processes, and how they are integrated during wound healing. This research could lead to the design of therapeutic strategies for autoimmune disease, diabetic and other chronic wounds, cancer, and cardiovascular disease.

A SELECTION OF OTHER AWARDS

Developing a vaccine to prevent cardiovascular fungal infections Patients with a heart transplant, a heart infection, or catheters placed in their veins are at risk for Candida species infections, but there are currently no anti-fungal vaccines. The American Heart Association awarded Lucas Dos Santos Dias, PhD, postdoctoral fellow, Division of Infectious Diseases, a two-year postdoctoral fellowship in the amount of $140,752 for his proposal “Vaccine Prevention of Cardiovascular Fungal Infections.” The project is co-sponsored by mentors Marcel Wüthrich, PhD, distinguished scientist, and Bruce Klein, MD, professor and chief, Division of Infectious Diseases. The goal of the research is to investigate whether a fungal protein, DN Bl-Eng2, can be used as a vaccine for patients at risk of cardiovascular fungal infections.

Supporting health care professionals who are caring for patients with dyslexia Families often seek help from their primary care teams regarding learning and reading problems including dyslexia. Oftentimes clinicians are minimally trained in identifying and addressing reading development and learning disorders. Julie Gocey, MD, MS, associate professor, Developmental Pediatrics and Rehabilitation Medicine, and Alanna Kessler-Jones, PsyD, UW Department of Neurology, were awarded a 2022 Baldwin Wisconsin Idea Endowment Seed Grant for their project “Developing a Dyslexia Toolkit for Interprofessional Care.” This one-year grant provides $4,000 to create and present training materials to local primary care providers and staff centered on recognizing reading disorders and dyslexia in children. Gocey and her team hope to expand the availability of the training sessions and materials throughout Wisconsin and across the United States and offer continuing education credits free of charge.

Increasing medication administration accuracy for children with medical complexity Children with medical complexity may require the administration of dozens of medications per day, with a narrow window of accuracy. The Agency for Healthcare Research and Quality awarded Ryan Coller, MD, MPH, associate professor and chief, Division of Hospital Medicine, a grant for his project “Improving Medication Safety for Medically Complex Children with mHealth across Caregiving Networks.” Expected to total $1.5 million over three years, the project will create a mobile health (mHealth) platform, Medication Safety @HOME (MedS@HOME), to improve medication safety for children with medical complexity by supporting medication management across the caregiving network. The team’s goals are to increase medication administration accuracy, reduce medication-related adverse events, and ultimately improve chronic care management.

Increasing availability of doctors’ notes about patient care with parents of hospitalized children Michelle Kelly, MD, MPH, associate professor, Division of Hospital Medicine, received a grant expected to total $292,973 over two years from the Agency for Healthcare Research and Quality for the project “Getting on the Same Page: Leveraging an Inpatient Portal to Engage Families of Hospitalized Children.” The study will be the first to evaluate using a bedside computer to share in real time doctors’ notes about diagnoses, treatment, and discharge plans with parents of hospitalized children. Kelly and her research group hope the findings will advance scientific knowledge around the potential to use health IT to engage parents as partners to improve inpatient care quality and safety. 2022
