A peek into Chai Care’s exam room

A BITE MORE INFORMATION Hot summer days are a fun reprieve from the brutal winter. Unfortunately, more time outdoors often results in more visits to our Urgent Care.

Over the years I’ve gotten the same questions and concerns regarding the most prevalent summer-related medical problems.

This week I’ll be focusing on bug bites.

Being out is fun till you’re batting your hand to shoo away those pesky bugs. Mostly they’re harmless and simply annoying. Occasionally, maybe frequently something will bite or sting you and at times require medical care.

Mosquitos: Bet you didn’t know this, mosquito bites are caused by female mosquitoes only. Females, unlike their male counterparts, have a mouthpiece that enables them to suck blood.

They choose their target by evaluating the scent and chemicals in your blood.

When the female mosquito bites and feeds off your blood it injects saliva in its place. Your immune system reaction is what causes that itching bump that generally clears after a few days.

Interesting fact: Children react more severely than adults. As you grow your body builds some immunity to the saliva resulting in a

Got a bite, now what?

You can relieve the itching by using Caladryl. It’s an anesthetic that blocks the itching pain signals from the nerves to the brain.

Another option is Benadryl or Afterbite.

Even if you have nothing handy, you can always use a cool compress to relieve the pain caused by a mosquito bite.

Whatever you do, don’t scratch yourself. Doing so may cause a break in the skin. An open wound is an invitation for bacteria to grow and cause infection.

Come see us if the bite is accompanied by fever, headache, body aches, and signs of infection.

Bee Sting: When you’re stung by a bee, it's not quite as sweet as the honey it produces.

The fear of it makes you run for the hills. I get it, it’s scary.

However, it’s not that harmful.

Bee stings are painful and mostly harmless. The most common reaction is swelling with some redness.

Usually, the stinger will remain in your skin. Remove it using tweezers, you can use an ice pack to cool the area and then apply some Caladryl. If needed Benadryl can help with additional swelling. It can take five to ten days to be completely healed.

Medical care is a must if there’s swelling in your face, especially if stung elsewhere.

You should also seek medical care if there is swelling in the lips, tongue, throat, or eyes.

This is an indication of a severe allergic reaction that can lead to anaphylaxis.

Ticks: “You must check for ticks!”

The most common advice for country dwellers in the Summer months.

Is it so important?

Absolutely!

Ticks transmit Lyme disease which is an illness caused by bacteria in deer ticks. They transmit this to humans and it takes anywhere from fortysix to seventy-two hours for transmission to occur if bitten by a Nymph tick* and ninetysix hours for an adult tick.

It’s important to recognize the initial symptoms of Lyme disease since it won’t show up in a blood test.

Common symptoms include fever, headache, fatigue, and a distinct rash. The rash resembles a bullseye pattern which starts with a red patch that expands over time with some lighter skin tone around it and a dark rim.

It’s important to note that you won’t always develop a rash.

When Lyme disease is detected early and treated properly you avoid late Lyme disease which can cause damage to the heart nerves and joints.

Tick season is a looonnngg season, it starts in April and stretches through October.

They are most commonly found in wooded areas and areas with high grass.

How to remove a tick?

Tweezers. Do not use baby oil, vaseline, nail polish remover, or matches. These items will kill the tick but it will also cause the tick to deposit additional

bacteria into your wound.

If you don’t feel comfortable removing the tick on your own, come into our office and we will properly remove the whole tick.

If you do remove it on your own, use tweezers and pinch it as close as you can to the bottom of the tick where it’s embedded in your skin. Don’t squeeze the tick. Slowly pull it out in a smooth continuous manner.

It’s always a good idea to have the tick brought to your doctor for analysis.

Tick Free Zone: • Keep your lawn manicured and short.

• Avoid areas with tall grass when hiking or at the park.

• Wear protective clothing.

• Apply insect repellent such as Deet.

• Shower immediately after hiking.

• Remove ticks using a tweezer within the first twenty-four hours.

• Check your body for ticks.

The most common areas to check are behind the ears and knees, under your arms, in your scalp, along your waistline, and back areas.

For most places in the country, summer is probably the best time to get some good hiking done. Clear, sunny skies, vacation time, and long daylight hours can all mean a great, full day spent on the trail.

Those same clear, sunny skies and long days can bake the ground you’re walking on when temperatures hover near (or into) the triple digits.

However, it is possible to still enjoy the outdoors in our brutal late summer weather. You just have to adjust your thinking a bit and take a few extra precautions beyond the basics (like making sure to tell someone where you’re hiking). You don’t have to let high temperatures keep you off the trails!

1. Start Early It's important to get an early start. 11AM to 2PM is usually when the thermometer peaks, so if you can get most of your elevation gain over with before then you’re going to have a much nicer time on the trail. If starting early isn't an option, leave later, when the heat has dissipated a bit, usually after 3 pm.

2. Cover Up You may have heard about the 3 L’s of summertime clothing: lightweight, loose fitting, and light-colored. This may seem counterintuitive, but long sleeves are actually your friend here. The more of your body you can shield from the sun, the happier you’ll be. Loose-fitting long sleeves and pants, skirts or tights paired with a wide-brimmed hat will do wonders on a summer hike. Remember to shield your eyes with some UVblocking sunglasses and slather that sunscreen on every exposed part of your body – especially if you’re hiking at altitude. The sun is stronger up there and you’ll get burned faster – even more so if you’re constantly sweating it all off.

Also, don’t wear cotton. Go with wool or wicking fabrics.

3. Hydrate During most hiking, your body will lose 159

about a liter of water every hour, and strenuous hiking in hot weather can more than double that amount. Be sure to bring more water than you think you’d need – and remember to sip often.

Under most conditions, your body can only efficiently absorb about a half-liter of water every hour and in many cases chugging a bunch of water at once can actually do more harm than good.

Remember – if you’re thirsty, you’re already dehydrated.

Trail mixes are great for this – and even better when paired with a starchy fruit like an apple.

Or you can pack some electrolyte drink mixes or tabs along with your regular drinking water, too.

Although skies may be clear in the city, the mountains can make their own weather. Oftentimes when it’s very hot – and especially when it’s more humid than usual – the mountains can trigger surprise monsoon downpours that can do some serious damage whether you’re on the peak or in a nearby canyon. Check the weather before you go and check with a ranger, too. You don’t want to get caught in one of those storms.

While you’re eating those snacks and sipping that water, why not also find some time to sit down in the shade?

Now is not the time to be a hero. Pick a trail that realistically matches your fitness level. If your trail is too easy, you can always take the harder one next time.

Resting for a bit will give your muscles a chance to recover and also give your sweat some time to evaporate and cool down your body temperature.

6. Don’t Forget the Extras When hiking in hot weather, bring two pairs of socks. Even if you don't usually have a problem with blisters, your feet sweat, too and if it’s hot out, they’re going to sweat more than they usually do. Also, don't forget the bug spray.

The easiest way to benefit from this advice is to simply dip a bandana or cloth in a cool stream and wear it around your neck (the site of some major blood flow between your heart and brain. Your cool, wet clothes will dissipate the heat you’re building up while hiking and the sun working with your own body heat will have you nice and dry by the time you reach home.
