A round-up of the key trade stories for business leaders to digest

Thistle Help staff in Irvine held an open day to celebrate the store’s anniversary

Thistle Help celebrates 15 successful years When George Ruddick and Laura Hays opened their first Thistle Help store in Irvine, North Ayrshire, in 2007, they never imagined how successfully the business would grow. Fifteen years on, the Scottish mobility retailer boasts a team of 15, two showrooms and a workshop space. Due to the pandemic, the last two years have been the most challenging

of all, according to the co-owners, but they say that they are so proud to have come out of it stronger than ever. Although it was a difficult time for everyone, they grew their business by acquiring a second showroom in Crosshouse, Kilmarnock. The retailer recently celebrated with an open day at its Irvine showroom, which was attended by customers, suppliers and friends and

family of the business. Laura said “As I stood in the showroom getting a photo taken with some of the team, I felt proud of what we have achieved but I also felt sad that George wasn’t sat beside me. “COVID has robbed us of a lot over the past two years but of all the times, why did COVID have to catch up with him now? He isn’t made of Teflon after all!”

The SANGO Slimline powerchair

DIETZ Mobility UK partners with Motability Scheme Powered wheelchair supplier DIETZ Mobility UK has announced that its SANGO Advanced and SANGO Slimline electric wheelchairs is now available to customers on the Motability Scheme. The newly branded DIETZ Mobility UK is the exclusive UK distributor of the DIETZ Power range of SANGO powered wheelchairs. Darren Legg, Managing Director of DIETZ Mobility UK, commented: “We are very proud that we have been able to pass Motability’s stringent checks to become an accredited supplier to the scheme.” “Our SANGO electric wheelchairs have proven themselves to be very robust, durable and easily readjusted to suit changes in different needs. “This will align itself perfectly with the worry free mobility that both ourselves and Motability aim to offer.”

Mobility trade gears up for Disability Awareness Day Disability Awareness Day takes place on Sunday 17th July, making it Warrington Disability Partnership’s 31st annual disability festival. Since its humble beginnings three decades ago, DAD, as it is known, has grown immensely and has taken place in 13 countries worldwide, the latest being Cameroon. Over 20,000 visitors are expected at Walton Hall and Gardens in Warrington, with a tented village expected to house over 200 exhibitors promoting independent living, a “Can

Do” sports zone with scuba diving in the onsite pool, inclusive cricket, wheelchair American football, wheelchair basketball, tai chi, martial arts, as well as wheelchair, physical disability and learning disability rugby league. Dozens of businesses have been lined up to promote mobility and independent products, wheelchair accessible vehicles, car adaptations and Motability car dealerships, alongside a range of statutory service providers.

Brotherwood previews zeroemissions WAV Wheelchair accessible vehicle (WAV) specialist Brotherwood has teased an eco-friendly electric vehicle for wheelchair users, unveiling its ‘elektron’ conversion for the Vauxhall Vivaro e-Life. With a 50kWh battery and 100kW/136PS drivetrain, the Brotherwood elektron maximises space within the Vivaro e-Life to provide a flat, level lowered floor of 164cm in length. Much longer than previous electric WAVs, the elektron provides increased space, ride

comfort and visibility for the wheelchair user. The rear wheelchair ramp is counterbalanced, making it light enough to lift with a single finger. Only 107cm long, the wheelchair ramp sits neatly under the rear tailgate for shelter from the elements when loading. The optional VB-FullAir air suspension system replaces the traditional rear spring setup with a full air suspension system that filters out unevenness in the road surface, increasing ride comfort.

The Osprey Group raises the bar

WheelAir welcomes sales representative

Disability equipment supplier The Osprey Group was presented with the Innovative Product Award at the annual National Association of Equipment Providers (NAEP) Conference recently. Designed to assist the user to sit and stand from the toilet, the Osprey Wash/Dry Toilet Raiser impressed the judges with its multiple features. Paul King shows off the NAEP award Paul King, Director at The Osprey Group, commented to THIIS: “Our new Wash/Dry Toilet Raiser is a new addition to the range. Osprey already have a standard toilet raiser for the domestic market and it has since added a wash-dry seat option. “The new version will fit over the user’s standard home WC, with a 30-stone working load capacity.” Unlike the supplier’s standard toilet raiser, which is battery operated, the new Osprey Wash/Dry Toilet Raiser is mains-feed electric only.

Disability equipment supplier WheelAir has appointed Marloes Kijzers as Sales Representative where she will be the go-to person for dealers, rehab centres, hospitals, and endusers. Marloes has a degree in human kinetic technology and four years of sales experience with custom-built wheelchairs. Marloes commented: “I am thrilled to be joining the WheelAir team at an exciting time for the company and contributing to strengthening its presence in the BeNeLux (Belgium, Netherlands and Luxembourg) region.” She also says that she is motivated to have a positive impact on people’s lives and is aligned with WheelAir’s vision to create a world where every wheelchair user feels comfortable and in control of their temperature.

Moving and handling aids

Wondering which moving and handling aids are grabbing mobility companies’ attention? Each month, retailers in the mobility industry highlight one product that is standing out for them and why…

Retailer’s Choice: Handling Belt – Maxi Deluxe by Locomotor “Handling belts are an effective way to minimise the risk of falls, slips and injuries when carrying out assisted transfers – and there are a plethora to choose from! “The Maxi Deluxe Locomotor Handling Belt is one of the most popular transfer aids that we sell at Ability Superstore. With practicality and durability at the forefront of its design, the Maxi Deluxe boasts a range of features whilst managing to omit the hefty price tag that normally accompanies ‘deluxe’ products. “The belt is designed to aid both the user and the carer when moving a person. It is softly padded to ensure comfort for the user and is available with, or without, a non-slip inner lining which helps to prevent the belt from sliding upwards during a transfer. This is, of course, important when using for a sit to stand transfer, or with assisted walking. “These deluxe belts also benefit from a row of vertical and lateral handholds which allows a more comfortable grip, or handle, for the carer, or nurse. “Frequently used by the NHS, the Locomotor range of handling belts provides an easy to use and Julia Phipps comfortable solution for transfers.” Owner of The Ability Superstore

Retailer’s Choice: MoveAssist from Alerta Medical “If you have a moving and handling offering within your business the MoveAssist would be a great option to have within your product range. The MoveAssist fits well within the professional care environment as well as the home environment. “Having the MoveAssist at our disposal helps us to offer the customer a truly value-adding solution. The MoveAssist competes strongly against rival brands but significantly, as an Alerta dealer we benefit from the firm’s expertise, innovation, and ongoing support, which is important. “The MoveAssist is fundamentally a transfer aid but it is so well designed, user friendly, compact, and aesthetically pleasing. As well as being a functional aid it has subtle design features which can boost user interaction and promote confidence whilst promoting positive daily living. “If the user has a degree of strength in the arms and legs the MoveAssist could be a great piece of equipment as the easy grip crossbar enables the user to get to a standing position using their own strength and the flip Billy Finnie up/down padded seat ensures Operations safety, security, and movement.” Manager at Mobility Scotland

Retailer’s Choice: Birdie Evo Hoist from Invacare “When supplying to the care sector, we need to be confident that the equipment we are providing is reliable and built to last. When it comes to hoisting equipment, our go-to is the Birdie Evo range. “The new Birdie Evo family has been designed to offer a high feature-set of options that have been blended with new technologies to provide maximum comfort and security when lifting or transferring a patient to or from a bed, chair or even the floor. “Based on the renowned Birdie, the new range has been customer insight led, encompassing everything that was valued on the Birdie whilst bringing in new features to further enhance the client and carer experience. “It has an impressive lifting range up to 1660mm in height. The high boom ensures there is space for the client, even when the hoist is in its highest position, and the optimised space in front of the actuator reduces the risk of injury to the client’s knees. “Available in this range is the Birdie Evo, the Birdie Compact and the Birdie Evo XPlus which takes a user weight of 210kg. All of these hoists can be folded for storage and transportation without the use of tools. “It really is a great range of hoists to work with and we have no hesitation in recommending them to care homes.”

Retailer’s Choice: GH3 Lifting Module by Guldmann

Alastair Gibbs Managing Director of TPG DisableAids

“Without doubt the whole world of ‘moving and handling’ has evolved over recent years to ensure the products used are now far more user friendly and fit for purpose. “Our preference in many cases is the GH3 hoisting range from Guldmann. Not just for the hoist, but for the whole range of track and fixing solutions that have been designed with three customers in mind. “Customer one being the disabled client. Customer two being the care giver. Customer three being the engineer that will install and maintain the products. “All three customers have equal importance and influence over the design and capability of the range. The quality is outstanding, and the durability is second to none. “The philosophy of ‘time to care’ just sums up the superb efficiency of the range and the effective solutions provision of the range.”

Retailer’s Choice: Sara Stedy from Arjo “The Sara Stedy from Arjo is a great product to use when a client needs some assistance to transfer but does not require a hoist. “It can be used with just one caregiver and unlike a rota stand, it has pivoting seat pads and enables the user to be moved safely and securely from one place to another, such as chair to toilet, and a back strap can be used if extra support is required. “It has spreadable legs as standard, and is also available in a compact size. It is a very popular sales item and also proven to be robust and durable on Lauren our hire fleet.” Bromfield Director of Classic Mobility

Trade Talk Q&A with Thomas Bradford, Marketing Coordinator for Kymco UK WHAT HAVE BEEN THE HIGHLIGHTS FOR KYMCO OVER THE PAST 12 MONTHS? “Kymco have had a progressive 12 months, with the world starting to recover from the COVID pandemic. “We managed to maintain good levels of product stock to support our dealer network, which is something Kymco UK have always been committed to. Due to our substantial investment in stockholding, we were able to sustain supply for our dealers and maintain a strong sales performance, during difficult market conditions. “Demand for our products during this time ramped up dramatically, partly since we had good stock levels. We took extra market share from our competitors at this time as some other companies were of course less fortunate with their stock position. The unprecedented demand did create some pressure for restocking of some of our most popular models, which did go out of stock for a while due to the pandemic. “We launched the new K-lite FE and K-lite F scoooters at Naidex last year and these have proven to be a massive success, surpassing all our expectations. We have also managed to grow the team significantly, with new appointments to our sales, marketing, customer service and technical teams. “These new appointments have led Kymco on a new path which has resulted in creative brand expansion, expansions to our dealer network and more continuous improvements to customer satisfaction levels.

and durability, we responded and packed a bit more into this amazing scooter, getting as much range capability in the scooter as possible. “With Maxer ER, we’ve managed to fit 2 x 100 Ah batteries in the scooter by redevelopment of the heavy-duty chassis and an improved

motor performance. With this new beefed-up model Maxer ER can now achieve an extra 10 miles drive range, meaning it can achieve a massive 45 miles range, with one charge.” LAST YEAR, KYMCO LAUNCHED ITS FIRST EVER RANGE OF FOLDING SCOOTERS. WHAT KIND OF RESPONSE HAVE YOU HAD TO THIS RANGE SO FAR? “Our K-Lite F and FE premium folding scooters are growing at an unprecedented rate. Its popularity seems to be growing every day, with no rest. “This little scooter has been a fantastic seller for KYMCO and our dealers and is a product we are incredibly proud of. “The feedback from dealers is that it is now the leading folding scooter in the market, due to its high-quality build, exceptional performance and unique features benefiting their customers. “This exciting new model is selling so fast that it is very difficult to forecast such rapid growth trends!” HOW HAS YOUR FIRM NAVIGATED THE IMPACTS FROM EXTERNAL FACTORS SUCH AS BREXIT AND THE COVID-19 PANDEMIC OVER THE PAST COUPLE OF YEARS? “The last few years have been the hardest and most adverse conditions experienced by the industry, and we have had to deal with may difficult issues that have resulted from the impact of the COVID-19 pandemic and Brexit. “Whilst we’ve managed to maintain a good level of stock of our products, massively rising shipping costs, fuel costs, energy costs and severe delays

have been some of the main issues we have faced and are still facing, for both main product and spare parts. “Whilst these are mainly out of our control, we are continuing to make every effort to minimise the effect of these issues, so we can continue to support our dealer network and our customers.” IS KYMCO SEEKING TO EXPAND ITS DEALER NETWORK? “Kymco is dedicated to expanding its dealer network in all areas of the UK, to support its customers to the very highest level. “We are always interested in hearing from potential dealers who want to join our dealer network. The best contact to start talks about joining is with Neil Keeble who can be contacted via email at nkeeble@” WHAT’S COMING UP FOR KYMCO FOR THE REST OF THE YEAR? “Kymco still have a very busy remainder of 2022. We have Naidex coming up on the 6-7 July, of which

we are proud sponsors and will have a fantastic stand to visit at the very front of the show. “We will also be attending and exhibiting at the Motability ‘One Big Day’ events at various locations across the UK this summer. “For the first time ever we are attending The Royal Welsh Show from 18-21 July, which is an incredible opportunity for us to showcase our flagship products, as well as further expand our brand. “To wrap up on the year’s events we will be at Rehacare Dusseldorf from 14-17 September, to further expand our European and international business. “Outside of these events, we will continue our investment and dedication to other marketing activities, dealer support and expansion, and external partnerships, as well as launching new products such as Maxer ER premium mobility scooter, as well as some other surprises along the way.”

Kymco found that customers are increasingly requesting scooters with greater range

Hampshire Mobility Services: Championing inclusivity Hampshire Mobility Services in Romsey is a stairlift company providing installation and aftercare to the healthcare, local authority and private sectors markets. It is also a high street mobility retail business providing everything from walking sticks to scooters, grabrails to bathlifters, and manual wheelchairs to bespoke wheelchairs.

20 /

Founded by Kevin Fry 10 years ago this August, this privately owned family-run business started out as a small shop, gradually growing over the years into the business it is today. “We still work with the same enthusiasm to provide honest, unbiased service and products to suit the need of our client base,” says Kevin. “Through working with trusted

stairlift tracks for Wessex Medical (as it was then known) – it now called Wessex. He then moved into stairlift installation, installing the older VS and VL model vertical lifts. He continues: “Moving through the years I continued into stairlift installation via another company, working up to engineering supervisor before moving on to a role as service and install manager at a familyrun business. This was back in my

home town at a time when I needed to be close to my family and young children.” Kevin was made general manager of the larger company that brought the family-run business and merged it into a regional business. Kevin adds: “At this point I lost the reason we all work in this industry – the interaction with the end user and providing solutions to make every day as inclusive as mine/ours.” This

experience led Kevin to starting Hampshire Mobility Services. The shop, which can be found on Abbey Walk on Church Street, has always attracted plenty of interest from locals in Romsey and the surrounding area, who come for the good service and the ability to try out a wide range of products. RETAILER SPOTLIGHT and try different types of wheelchairs. For Kevin, he wants customers to feel assured that they have a product suitable for their needs, and not simply because it’s the “the only one they have in stock” in order to achieve a sale. Target customers, says Kevin, are those who want their own equipment so that they are not reliant on the state, or those who are unable to wait for assessments due to the current demands in the healthcare sector. The bestselling product, he says is the Rehasense Space LX rollator. “It’s a modern looking, lightweight Britishmade product. Since the beginning of the year we have had a constant flow of them and we will continue to support the rollator and their other products.” This year, the business has expanded into the modular ramp market. Kevin started to noticed enquiries coming through either on the back of wheelchair assessments with customers, or through onsite stairlift or hoisting assessments with OTs. The range of products offered by another British manufacturer, Enable Access, is another “go-to choice”, explains Kevin. “The quick modular products suit many situations and

they have enabled us to break into a market which complements our own range of products and services. We are already looking forward to where these products can take the company over the coming months.” COMMUNITY-MINDED BUSINESS Earlier this year, as reported in THIIS, the retailer teamed up with housing association Abri to help fund ‘Millbrook Munchkins’, a foundation football group of Millbrook Football Club. As well as helping to cover the costs of equipment, pitch hire and specialised training the investment helps to provide free football sessions to young children. The courses are allinclusive and accommodate kids with differing abilities. Kevin explains: “The Abri contract was a tender based award which we were initially awarded back in 2016 and continued until its re-tender back in May 2021. A significant part of the award was the condition to provide or fund a social value fund aimed at giving something back to the community in line with Abri’s policies. “We chose to provide Millbrook FC with regular funding to help them provide 3–7-year-olds with a pathway into football in an area

The retailer is supporting grassroots football by investing in a children’s football team

“We have established a company that can achieve what we say we can achieve with a strong team of staff” of Southampton which is socially deprived and in the past had issues with anti-social behaviour. “Bringing the children into an environment where they can be safe, welcomed and taught important skills through sport is something both Abri and I feel will benefit this local area. “I have seen myself the importance of football, sport and friendships as our own son has been with the team from a young age. He is now progressing into a football scholarship with some of his teammates and I believe some of this is due to his time with the club and volunteers and managers who have supported him over the years.” Further funding from Hampshire Mobility Services to Millbrook FC will continue with the aim to make the club even more inclusive, from investing in female football to the possibility of starting a pan-disability team in the years to come.

ADAPTING WITHIN THE SECTOR There are plenty of challenges in the industry. As Kevin acknowledges, product supply in some sectors has been difficult to maintain and he, like many, feel that there is no clear path for the short term. He does feel confident about the future however. “Yes, we have had

to adapt and look at other avenues, but we will only continue to supply products to our customers which we know we can support. “Handicare and Invacare are two regular manufacturers we have worked well with over the last 10 years. We have a solid working arrangement where we can fully support the demands of the market and the backing market leaders.” Taking a wider view, Kevin feels that the demands on the healthcare sector will be felt for a while to come, which will have repercussions on retailers who do business with local authories. “Funding caps on budgets, staff shortages and so on is going to take time to rebuild after COVID. Mixed with supply shortages this will mean that the local authority market will be turbulent in the coming months. I can already see the increase on private sales for many products.” What this means for the business? Kevin adds: “We will continue to tender for maintenance contacts and the installation of stairlifts, hoists

“Bringing the children into an environment where they can be safe, welcomed and taught important skills through sport will benefit this local area.” and ramps etc for buildings and maintenance. “These sources of regular income for the business are important for us to continue to grow and achieve our targets.” Hampshire Mobility Services remains focused on putting the needs of customers first. “We as a business will continue to support these customers’ needs with a quicker response to enable them to be as independent as possible and as long as possible.”

SUPPLIER SPOTLIGHT The curved Bruno Elite offers a high degree of adjustability

Bruno Lifts: Its smooth ride to the top 30 /

American-owned Bruno claims there is a stairlift for every budget and space in its range of products, from straight stairlifts to handcrafted curved stairlifts. With updated versions of its products being launched this year, it is clear that Bruno remains focused on continuing to innovate…

SUPPLIER SPOTLIGHT Bruno Lifts is the European sales organisation of Bruno Independent Living Aids, the largest stairlift manufacturer in the United States. Bruno manufactures a range of indoor and outdoor straight and curved stairlifts, which are sold and installed throughout the UK via its authorised trade network. Last year, the firm launched the latest version of its Elan indoor straight stairlift, with the company claiming to have produced a slim rail that installs closer to the wall than any other stairlift on the market thanks to its unique vertical rail. Bruno is also planning to introduce more products on the market over the coming months. THIIS chats to Warren Keeton, National Sales Manager (UK and Ireland) for Bruno Lifts to find out more… PLEASE CAN YOU TELL US A LITTLE BIT ABOUT YOUR STORY WITH BRUNO? “Since beginning my role as National Sales Manager at Bruno in 2021, I’ve really enjoyed getting to know our trusted dealers across the UK. Our industry is critical to enhancing the quality of life for people with mobility challenges, so it’s rewarding to work together to meet those needs. “Throughout my career, I’ve worked closely with local authority contacts and healthcare professionals, and I’m continuing to work with those groups. With the support of our dealers, I enjoy finding ways to solve some of the challenges we are faced with out in the field.”

Bruno Lifts staff (left to right): Bob Hambleton (General and Technical Manager), Warren Keeton (National Sales Manager), Mel Kawycz (Accountant), Roger Sage (Warehouse assistant) and Bill Page (Director, Product Management).

HAVE THERE BEEN ANY PARTICULAR PROJECTS YOU HAVE BEEN MOST PROUD OF SINCE WORKING AT BRUNO? “The outdoor stairlift projects are always interesting. We see dealers come across some very challenging outdoor environments, making the end-users feel isolated and trapped.

“Our Elite straight and curved outdoor lifts often fit with little or no alterations to the outdoor staircase, Bruno’s curved outdoor Elite has a hand-fabricated rail completely bespoke to the staircase. “Bruno offers a true outdoor product: marine-grade vinyl upholstery and outdoor-specific paint safeguards all surfaces.” WHAT DO YOU THINK MAKES BRUNO PRODUCTS STAND OUT IN THE MARKET? “Bruno’s 30-stone range, high performance Elite – available for straight and curved indoor or outdoor staircases – is very unique.

SUPPLIER SPOTLIGHT “The Bruno Elite is engineered and manufactured to carry a high-load capacity yet maintain a high degree of adjustability and reliability. Using the Bruno Elite with its heavy duty weight capacity can often reduce the lead times associated with more complicated options to address access issues.

Warren Keeton, National Sales Manager (UK and Ireland) for Bruno Lifts

“Bruno’s straight Elan is our go-to straight indoor stairlift for our dealers and covers loads up to 21 stone with all optional powered or standard manual features. It also has the slimmest rail-to-wall dimension on the market. At the top of the staircase the seat swivels a full 90 degrees at the top landing – with an impressive turning radius of 702mm!” CAN YOU DESCRIBE ANY NEW PRODUCTS THAT WILL HIT THE MARKET THIS YEAR? “Later in the year, we are launching our new and improved Elite seat design that will bring a fresh new look to our much-loved Elite stairlift. Whilst still keeping to its 30 stone capacity, we have developed a new design and introduced some great new functionality. “It will still be a comfy, sturdy,

“Later in the year, we are launching our new and improved Elite seat design that will bring a fresh new look to our much-loved Elite stairlift” smooth chair, but with a new modern look and feel. “We aim to be able to exhibit this at The OT Show from 23-24 November on stand D62.” WHAT PRODUCTS DO YOU REGARD AS BESTSELLERS IN THE UK MARKET? “The Bruno Elan range is our bestseller for many dealers since it lends itself to bulk purchasing. It is “non-handed” and fits on most UK staircases. Having the rail-to-wall installation so close means the chair only takes up only approximately 300mm on the staircase when not in use. “When a higher weight capacity is required, then the Elite is the solution. Bruno accommodates heavy-capacity users better than anyone. Our dealers love the 30-stone capacity because they are confident in its performance whether the person is 30 stone or much lighter. “Bruno’s generous warranty also gives dealers confidence. With Bruno having a UK office, it means parts are readily available to be collected or shipped out immediately. IN 2020 BRUNO LAUNCHED AN UPDATED VERSION OF THE ELAN STRAIGHT STAIRLIFT. WHAT RESPONSE DID YOU HAVE TO IT? “We maintained the close-to-the-wall advantages of the slim line rail and chair, leaving maximum open space on the steps. We also made changes to the styling and colour palette to give it a more modern appearance. The positive feedback has been tremendous!”

What makes a good positioning belt? 6. Safety first Positioning belts and harnesses are provided primarily, as the name suggests, for the purpose of positioning. They are not ‘safety belts’ in the way that vehicular occupant restraints are also named, in that those devices are to protect the person from being thrown forward in a car accident. However, it is important that the occupant of a seating system is safe. The safety elements include avoiding failure of the postural support device (PSD), avoiding slippage of the PSD, avoiding skin tissue damage arising from pressures from the PSD when tightened, avoiding asphyxiation, and avoiding falling out of the seat. There have been recorded injuries and deaths resulting from inappropriately placed or adjusted positioning belts and harnesses, leading to both falls and strangulation. It was for this reason that BS 8625 was published in the UK in 2019, and now adopted internationally in ISO/ TS 16840-15 (due for publication this year, 2022), to cover the ‘Selection, placement, and fixation of flexible postural support devices in seating’. In these standards, guidance is given for safe practice, and requirements that secondary supports are tested to ISO 16840-32 and ISO 16840-103. PELVIC POSITIONING Some of the deaths have occurred as a result of occupants ‘submarining’ under their pelvic positioning belts. To avoid the risk of ‘submarining’,

44 /

In previous issues of THIIS in the Let’s Get it Clear series1, we have covered various aspects of ‘What makes a good positioning belt?’. The previous articles in this series covered the anatomy of a pelvic positioning belt; choosing and adjusting a pelvic positioning belt; the features and benefits of different belt closures; means to fix the belts to the wheelchair frame in appropriate positions; and selection of anterior harnesses to optimise the position of the trunk.

able to slip under the belt (as long as it is tightened appropriately). In addition, this positioning in front of the greater trochanters means the occupant is able to reach much further forward, and there’s less pull down on the pelvis, and thus decreasing pressures on the ischial tuberosities. Interestingly, in a paper presented at the recent European Seating Symposium meeting, Van der Heyden and Siefert showed that the shear forces through the skin tissues are very high when the belt is at 45o, whereas at the preferred position they are minimal. Greater skin health all round and a much safer positioning solution!

Figure 1. Options for correct positioning of a pelvic positioning belt (ISO/TS 16840-15)

it is critical that the belts are not fitted at the junction between the back support and the seat structure – this 45o angle is appropriate for a vehicular occupant restraint belt, but not for a positioning belt. For those clients at risk of going into posterior pelvic tilt and, as a result, of sliding forward in their seat, the standards cited above require that the belt is positioned anterior to the greater trochanters (Figure 1). The shorter length of belt material involved in positioning the pelvis means that the pelvis would not be

SOURCES OF ASPHYXIATION (SEE ANNEX E OF BS 8625 AND ISO/TS 16840-15) Asphyxiation can arise from the use of postural supports for one of following reasons: 1. The support is too high and it occludes the airway (strangulation). This could be because: • the support has been fitted too high; or • the support has been adjusted since fitting to sit too high; or • the support has moved out of position since being fitted. 2. The occupant collapses down in the seat and the airway is occluded by the support. This could be because: • there is insufficient lateral trunk

support such that the occupant collapses to one side; or • there is insufficient posterior pelvic or lumbar-thoracic support to maintain an upright posture, such that the occupant collapses down into kyphosis. 3. The occupant slides down in the seat and the airway is occluded by the support. This could be because the postural support is: • fitted too loosely; • poorly positioned; or • malfunctioning. 4. Mechanical compression is otherwise applied to the neck, ribcage, and/or diaphragm, due to: • the anterior support strap/pad being fitted too tightly; • the occupant sliding underneath the postural support until stopped by their arms and ribcage, placing pressure on the ribcage; or • the occupant submarining totally underneath a postural support until they are stopped by the support being caught around their neck. PROTECTION AGAINST ASPHYXIATION Use of an anterior trunk support comes with an increased risk of injury or death due to asphyxiation/

strangulation – this is greater when a chest support is used, as compared with a shoulder harness, due to the neck shape integral to the design of the support. Any anterior trunk support should be supportive, but not restrict the physiological functions ongoing within the torso, whether this be digestive, respiratory, cardiovascular, or bladder function – and protecting all these internal functional requirements needs to be balanced with facilitating external physical functional needs. Furthermore, the risk is increased where the occupant is unable to recognise that something is incorrect about their seating, or cannot verbally express their distress. • The first aspect of importance is that an anterior trunk support should never be used without a correctly fitted and adjusted pelvic positioning belt, to control the risk of ‘submarining’ under the pelvic belt (see Part 2 in this series, and BS 8625 for further guidance). • The next aspect is that the central part of the support should be positioned no higher than the lower part of the sternum. ISO/TS 1684015 provides the graphics (see Figure 2) for a label to be placed, if possible on the PSD, as well as in the IFU: • The materials from which a chest harness is made is important. In consideration above of the dynamic elements built into a harness, the elasticity needs to be in the straps, and not in the central pad. Chest harnesses that are completely elastic, including across the central pad (sometimes referred to as

Figure 2. Suggestion for a label to indicate the correct positioning of an anterior harness

butterfly harnesses), have a tendency to ride up and there have been reports of occupants being strangulated as a result. Flexible PSDs should never be used as vehicular occupant restraints. They are by definition designed for postural positioning purposes, and their placement on a wheelchair, and their strength, for these purposes are very different from what is required from a vehicle’s safety belt. IN CONCLUSION A correctly fitted and fastened positioning belt or harness will enhance the occupant’s postural support, improving functionality and aiding good health. When done incorrectly, this can lead to harm or death. Do no harm, and follow the guidance and specifications in the standards.

Ablestand Patient Turner from Drive DeVilbiss The Ablestand Patient Turner helps a carer or attendant to transfer a user to another seat, bed, wheelchair, toilet or commode by standing and turning. Its sturdy steel frame provides stable support for user while the heightadjustable handles and padded lower leg support can be adjusted to suit user requirements. Its low profile base facilitates user feet placement and it has a substantial base area providing stability, with rear bar also allowing carer to counterbalance. The handle has multiple gripping points for varying carer/user support requirements and is angled to enable "nose-over-toe" standing movement. The opening on the central handle bar allows quick fitting of optional support belts. Two 3" castors ensure effortless maneuvering even with heavier users. The aid requires the user to pull on the handle to stand up and to be weight bearing.
