New Scottish DriveAbility headquarters

DriveAbility has opened new Scotland headquarters in Glasgow, ensuring more Scottish people with disabilities can benefit from greater independence.

The opening recently was attended by senior representatives from Police Scotland, DVLA and Transport Scotland, as well as DriveAbility Scotland clients.

Andrew Swain, CEO of DriveAbility Scotland, welcomed professional delegates to the new home of DriveAbility Scotland. He commented: “We’ve welcomed many partners,

stakeholders and service users to come and celebrate with us. They’ve seen our facilities for the first time and have been very impressed, particularly with our new state-of-the-art vehicle simulator for specialist driving tuition.

“This new centre is vital for the people of Scotland as it provides more equal access to our assessment services.”

Founded in 2021, DriveAbility Scotland is the only Driving Mobility accredited centre in Scotland and is the only service to actively reach clients in the Highlands and Islands.

Dr TRU showcases his tardis

Accessible bathroom specialists PROCare was awarded ‘Best Stand 2025’ at the Occupational Therapy Show, which took place at Birmingham’s NEC recently. The award-winning installation featured the newly launched TRU Cubicle transformed into a science fiction style tardis, where visitors met Dr TRU and the Mobilons.

Visitors at the show, many of them occupational therapists, were invited to join the mission by exploring PROCare’s core product range and identifying the solutions needed to help residents return to safe and independent living.

Designed for temporary use in hard-to-adapt homes, TRU Cubicle includes a watertight shower and WC, half-height doors, grab rails, a raised tray lip and pumped waste system. It installs in just hours and is fully relocatable.

Feeling the heat

Adjustable beds and mattresses supplier Opera Beds has announced the launch of its first electric blanket for adjustable beds.

The "world first" blanket utilises a smart fabric called graphene, a material that offers exceptional thermal conductivity and emits far-infrared heat. The soothing heat it produces penetrates deeply to support circulation, alleviate joint pain, enhance immune function, and improve metabolism.

Lightweight, flexible, and wire-free, the blanket has been designed to deliver consistent, balanced warmth that adapts to the user’s body. Its advanced design heats up rapidly and distributes heat evenly across the surface, eliminating cold spots and reducing the risk of overheating.

The blanket will be available in a single bed size, with the double size launching soon. It features handset controls (single or dual), as well as app technology to ensure full user control at the touch of a button, or via a smartphone.

The Motability Scheme removes luxury cars

The Motability Scheme has announced that premium brand cars will no longer be available to its customers, with more British cars available to lease.

The Scheme is a lifeline for disabled people in the UK, leasing cars, wheelchair accessible vehicles, scooters and powered wheelchairs in exchange for their enhanced mobility allowance.

It has stated that half of the vehicles leased through its scheme every year will be British built from 2035. It is also refocusing vehicles available to its customers, with luxury cars, such as Mercedes Benz and BMW, removed immediately.

By increasing demand for vehicles built in Britain, the Scheme aims to strengthen domestic production and encourage new investment in the UK’s growing electric and accessible vehicle sectors. This commitment is

SOFTWARE BOOSTS GROWTH

Adapted bathrooms and wetroom solutions provider PROCare has stated that it has significantly expanded its operational capacity and warehouse efficiency after implementing Forterro’s ERP and WMS solution Orderwise.

The business claims to have increased the number of daily orders it can process from 50 to 200+, boosted its picking accuracy to 99 per cent, and scaled its warehouse operations to 75,000 square foot as demand continues to grow nationwide.

RGK WHEELCHAIRS STEPS UP

Bespoke sports wheelchair manufacturer RGK Wheelchairs has announced its new partnership with England Athletics, becoming the Official Wheelchair Racing Partner of the national governing body for grassroots athletics and running in England. Its RGK Sprint wheelchair, built from lightweight aluminium, has already become a favourite among racers for its strength, speed, and precision. With the upcoming launch of the Sprint X, it is hoping to take that legacy further, with its carbon fibre construction and refined aerodynamic design.

PUTTING ON A SPREAD

Scottish mobility retailer Thistle Help raised £1,925 for Ayrshire Hospice recently by holding a coffee morning at its Irvine showroom. The event was held to support the annual Go Purple day, where Ayrshire schools, businesses and community groups come together to raise money for Ayrshire Hospice, a local charity which looks after terminally ill patients. Customers, friends and family attended Thistle Help’s showroom, as well as staff from Ayrshire Hospice team. Laura Hays, Director at Thistle Help, said that despite the weather being miserable, it didn’t stop people from coming along!

HEADWAY COURT UNVEILING

Habinteg Housing Association’s first-ever residential scheme designed for head injury survivors in Kent to live independently has celebrated its 30th anniversary with the unveiling of a commemorative plaque for its founders.

Headway Court, near Tunbridge Wells, was opened in 1995 and it still remains the only residential scheme for head injury survivors in Kent.

The scheme was founded by the late Pam Munro MBE, and her husband Michael for their son, John, who survived a head injury. The Munros helped raise funds for the construction of the block, which features eight one-bedroom accessible flats. The block has been owned by Habinteg since 2011.

New Group CEO at Tunstall Healthcare

Technology provider Tunstall Healthcare has appointed Paul Adams as Chief Executive Officer.

Paul will lead Tunstall Healthcare globally, supporting local leadership teams across all markets to navigate complex challenges and opportunities, while continuing to deliver on Tunstall’s mission to empower people to live more independently through connected care and health solutions.

With over two decades of leadership experience across healthcare, technology and manufacturing, most recently as CEO at Lloyds Clinical.

Mobility lift solutions specialist Terry Lifts has announced the appointment of Steven Hill as the newest member of its board, taking the position of Sales Director.

With a proven record in building strong relationships and delivering customer-focused solutions, Steve’s approach reflects the values that define Terry Lifts, the firm stated, as he “understands that every decision we make ultimately impacts the people who rely on our lifts every day.”

Unique Mobility has welcomed Exeter Disability Centre into the family with a new showroom and workshop...

South West retailer Unique Mobility has announced the acquisition of Exeter Disability Centre in a move which will bring two long-standing mobility businesses together under one roof in Exeter.

The move was prompted by the retirement of David and Pat Denning, the former owners at Exeter Disability Centre (EDC). Dave, who has spent over four decades in the mobility industry, and his wife Pat have owned and run Exeter Disability Centre for over 25 years.

On his decision to retire and sell the business, Dave shared with THIIS: “After more than 40 years of helping many clients with their mobility challenges, I have decided it is now time to sell Exeter Disability Centre and retire.

“I’m incredibly grateful to all our customers for their trust and support over the years – it has been a privilege to serve them. I also thank Pat for all her work behind the scenes and my wonderful staff for their hard work, loyalty and humour”

Dave’s decision to sell the business as he moves into retirement ensures

continuity for EDC’s staff and customers. Ian and the Unique Mobility team share many of the same values as Exeter Disability Centre. The joining of these two businesses brings more services under one roof for the local community.

All existing EDC staff have joined the Unique Mobility team, ensuring customers continue to see the same familiar faces and receive the same trusted support.

The two businesses have now come together creating a new shared home, which opened on 24th November, at Unit 5 Merriott House, Hennock Road, Marsh Barton, Exeter EX2 8NP.

The brand-new, dual-branded showroom brings together the strengths of both organisations. Customers can now explore a wider range of mobility scooters, powerchairs, wheelchairs, daily living aids, paediatric equipment, plus Unique Mobility’s full vehicle adaptations service and an expanding range of wheelchair accessible vehicles – all in one place.

The upgraded site which is very close to both businesses’ previous sites on the Marsh Barton trading estate, offers a bright, modern showroom, improved accessibility, more parking and expanded workshop facilities for both EDC’s mobility engineers and Unique

Embracing a digital age

Able-Aid recently celebrated 30 years in business with the launch of a new website and re-brand…

Since 1994, Able-Aid has assisted thousands of residents throughout Essex with their disability and specialist mobility needs. The business was founded by Peter Webberley, who originally started out in the construction trade but wanted to move into an industry where he could directly help people. It helped too that he had contacts and friends in the care industry.

A family business; each one of Peter’s children has worked at AbleAid at some stage or another, explains Phil Cattley, Head of Digital Sales at Able-Aid. Unfortunately, due to health concerns, Peter retired last year, but his children, once teenage part-time employees, are now key to the running

of the business and its development.

While the core has always remained the same – it is proud to be a mobility retailer that puts genuine care at the forefront of what it does – the company has evolved over the years.

It has always operated from its Leigh-on-Sea showroom; however, over the years half of the showroom has been rebuilt and it has expanded. For a time, the business became a specialist in paediatric and bariatric

products, Phil explains, but these sections of the business were sold off so Able-Aid could focus on general care.

As the head of digital sales, Phil says that his role is as much about marketing and website management as it is about sales. Phil says: “I maintain the company’s entire digital presence including email marketing, social media marketing, graphic design and more.”

include Clive Webberley, who is the day-to-day manager of the showroom. Described by Phil as “the lifeblood” of the business, Clive does everything from manning the showroom to performing installations and deliveries. Phil himself also helps to man the showroom on a part-time basis, along with Dawn Webberley, who also manages the firm’s accounts and finances.

Bruce Webberley and Roscoe Price, two of Able-Aid’s primary partners, also work behind the scenes, contributing to the long-term strategy of the business.

DIGITAL FOCUS

In fitting with its 30-year anniversary, Able-Aid’s new website and rebrand were launched during the summer. Phil explains: “With Peter’s children

at the helm, we are now focusing on digitalisation, with new marketing channels and a web store – bringing a successful physical retailer into the online space.”

Phil, who helped to oversee the website’s build and launch, explains: “The new branding is supposed to be fresh but familiar. Keeping the essence of our old logo while freshening up the style and colour palette for our new direction.”

The website has become increasingly important over the past few months since its launch. “It has been an important online catalogue for buyers,” says Phil. “Even those customers who do not purchase online, many choose to browse our site before they come into the store and confirm the purchase.”

Able-Aid’s hiring services are now also all handled online, providing both an easy means for clients to book and an online portal for the business to organise bookings.

As with all industries, everything is flowing towards e-commerce as a key part of the industry’s future, Phil says. “Mobility and care are no different; people increasingly want quick and easy access to the products they need.

“While our customers tend to skew older, the older demographic is more digitally fluent than ever before. This trend will only continue too. As today’s workforce grows older and begins to retire, the stereotype of the digitally illiterate pensioner will be a thing of the past.”

That is not to say that there is no place for physical stores. Phil

continues: “Living aids, mobility equipment and care furniture are purchases people need to be sure are right for them.

“Even after securing a sale on the phone, the final step is often for the customer to see and try the product for themselves. Comfort and pain relief is something that needs to be felt before.”

COMMUNITY HERO

Based in the heart of Leigh-on-Sea, Able-Aid’s showroom is located on the London Road, which runs parallel to the town’s high street and stretches all the way to Southend-on-Sea, through Hadleigh, South Benfleet and beyond.

include those looking to make a purchase on behalf of a family member, as well as anyone else who finds themself in sudden need of mobility aids.

Able-Aid has been a proud and consistent presence for the locals of Leigh-on-Sea for the last 30 years.

“Peter deserves a lot of credit for fostering long-term relationships with customers by providing quality care products and support to both the client and family,” explains Phil. “This way, when the friends or family of a client require care in the years to come, they know they can trust us. We hope to carry this ethos forward.

“We pride ourselves on offering genuine care in every service we provide, trying to find the right product for them rather than always seeking the largest return.”

Giving back to the local community has been a key area for Able-Aid throughout 2025, Phil says. “Not only have we broadened our selection of charitable pursuits, supporting Dementia UK, Dignity and Sue Ryder, we have been trying new ways of reaching out to the local community. For example, we have run several walkin coffee mornings from our showroom.

“We have also been in direct support of STORM Southend, a team within Southend Hospital dedicated to the rehabilitation of patients leaving the hospital. We have both donated products to help their efforts and also hosted one of their monthly team gettogethers.”

SERVICE FIRST

Able-Aid provides repairs and servicing in-store, where it has an in-house workshop, or for smaller repairs it will offer home or workplace visits to make sure people are up and running.

Marlin and the Scooterpac Savvy 8 proving to be particularly popular.

Care beds and riser recliners are its main focus at the moment, with warmth and comfort currently a priority for its customers, explains Phil. He expects the Alerta Carebed range to continue to do well in this category, as well as products such as One Rehab’s Admiral riser recliner.

Among the newer products to be stocked lately Phil says that the Sherborne Recliners have been a hit due to the vast array of fabrics.

Able-Aid has been working very closely with local care facilities to provide both care beds and seating options. Phil says that it has been offering discounts for local facilities on its entire range of care beds, including installation and set-up.

Mobility scooters tend to be AbleAids’s main focus during the summer months. Bestselling mobility scooters for the business include the Scooterpac Savvy 8, the One Rehab Marlin and the Monarch Mobility Genie Autofold.

Phil adds: “All-in-one fold-up

scooters are often requested by our customers who don’t want the hassle of taking apart a boot scooter.

“Until now, we have been hesitant to request folding scooters, especially those that mechanically fold, as they tend to be far too heavy to lift into a boot or store. We’re hoping the Genie is the answer to a lot of people’s requests.”

Able-Aid is careful about the suppliers it uses, and Phil says consistency in delivery times for them is just as important as speed so that its staff can reliably tell its customers where and when they can expect their care or mobility aid to arrive.

“Quality of products almost goes without saying,” he says. “We don’t want to supply products that may be faulty or not provide a great quality of care for the client.”

LOOKING TO THE FUTURE

Looking ahead to 2026, Phil says that Able-Aid is looking to widen the kind of content its puts out across its online channels. For example, it is hoping to expand its advice hub on its new website so that it can grow into a reliable digital space for quality care advice and info.

its storefront to reflect its refreshed modern outlook on the market.

Phil has observed that issues like the cost-of-living crisis and the winter fuel allowance changes have meant that some customers have been more conservative with their purchasing choices. “Even when they really could benefit from a certain product, economic conditions have led them to play it safe with their finances rather than their health,” he says.

Taking a broader view, the retail market has shifted quite a lot over the last 30 years and Able-Aid has adjusted accordingly.

Phil comments: “When Peter opened the business, the high street and physical retail were still king. Now, a lot of trade is done online.

“Fortunately, many customers do their product research online, but still choose to come into the store to talk and try a product before buying. Our webstore provides a good option for those who want to purchase immediately.”

Suffice to say that Able-Aid plans to continue to digitalise its services, offering this as an option, while remaining accessible to its traditional customer base.

We were fortunate enough to discover that our local Nottingham Trent University had a wealth of talented individuals who were excited to work with us on new projects through the governmentfunded KTP (Knowledge Transfer Partnership) initiative.

“The first project was to replace our paper-based production planning system with an automated system that has PC monitors at each stage of production, this process shows the production team exactly where every chair and bed is in the production process.

“Each individual enters the work they have done on each product which means we can trace every action back to a member of the team which gives us total quality control. The dashboard then shows when a product will be available for dispatch and sends a confirmation to the customer. “Naidex last year proved to be an enormous success as it gave us the opportunity to meet the majority of our customers in one place and showcase our new products.

“Not only did we have the chance to show our previous year’s new range of the riser high back chairs and our triple motor Infinity chair, but we have added the new triple motor riser swivel chair alongside the new quad motor Horizon chair, plus our new range of ultra-low bed lifters which has created a profit

opportunity for our retailers.

“We offer full training to store staff so they can understand how to sell these products in a way that their customer will see the benefits to them.”

HAVE YOU NOTICED ANY TRENDS WITH REGARDS WHAT UK CUSTOMERS ARE LOOKING FOR IN A SEATING PRODUCT?

“We have noticed an upsurge in requests for higher quality fully bespoke chairs and beds driven by a growing requirement for higher levels of comfort. Mobility customers come in all shapes and sizes so we feel it’s better to make a chair to fit the customer than fit the customer to a stock chair that maybe over time they will find does not deliver on comfort. If your customer is happy, and you are happy, that makes us happy!”

WHAT WOULD YOU SAY IS THE BEST-SELLING PRODUCT IN THE RISE & RECLINE RANGE CURRENTLY?

“This should be an easy question to answer, however, as we manufacture so many bespoke products for individual customers there doesn’t seem to be a trend. By far the most common requirements are our ultimate comfort seating system in the chairs and our latex comfort mattresses. Our customers are always putting quality, longevity, comfort and service first.”

HAVE ANY NEW MEMBERS OF STAFF JOINED THE FIRM?

“Our main objective this year has been to upgrade our staff in key areas, and we have Neha, who is a computer systems expert, who has joined our administration and project management team.

“We have a new general manager, Alan, who has previously worked for a number of large furniture companies such as DFS and brings a wealth of management experience to R&R. He has been used to the responsibility of managing large numbers of staff producing over 1,000 chairs per week.

“Alan finds that working with a smaller, focused team, is very rewarding as it allows him to get much more involved at every level to ensure that we continue to make the highest quality products that the original Rise & Recline are renowned for.

“We have also employed a specialist fabric cutter, Adrian, who brings over 25 years of furniture experience and product design. Together with our experienced team leaders, Steve, Luke and Sue, we have a strong product development capability within the business.

"Early next year we will be taking

delivery of our new fabric CNC which features the latest technology from Kimla and uses the same software as our existing wood cutting CNC which has given us a much larger capacity to increase production and help develop a much larger range of frames.”

More time to care

How one business is redefining pressure relief...

After more than three decades in pressure care, Ray Booth could have easily rested on a legacy of innovation, following the successful sale of Select Medical, in 2021. Instead, he’s returned with an identical purpose — manufacturing surfaces that deliver comfort and safety with the feeling of being cared for… a holistic approach to specialising in pressure care – adding more time to care. His new venture, OptiMat by Vaditeq, is reimagining the humble mattress and seating surface as a powerful ally for healthcare operators, care professionals and those people under care.

We sat down with Ray to talk about what brought him back, and how his immix and immix-air brands are helping caregivers create what they value most: time to care.

You’ve had a long and successful career in healthcare. What brought you back to start OptiMat?

After stepping away for a few years, one of our earliest manufacturing partners was set to retire and thinking about closing his small business, and with it the potential loss of jobs. I realised how much I missed the energy of our earlier days growing and establishing Select Medical — the people, the purpose, and the real impact innovation can make; we struck a deal, and I bought the business. After a year stabilising finances, I looked to diversify, reconnecting with people across the care industry and what I kept hearing from clinicians and care providers was that time was their most precious resource. With so much to consider, they need products that don’t just perform short term, but devices that last longer, providing more time to focus on patient care. The purpose —more time to care — has become the foundation of our OptiMat division.

How does your new immix brand deliver on that promise?

Pressure injuries are a huge issue, not just for patient comfort but also for

staff workload. Our aims with immix and immix-air are to reduce repositioning interventions and limit interruptions caused by malfunctioning components, so the surfaces work to optimal levels for longer. All our active and reactive mattresses are unique to us, arriving to market after multiple attempts to strike a balance between effective support and pressure redistribution. Many iterations during the development process allows us to refine and improve, continuously. Our surfaces are designed and tested to adapt to body movement, but crucially, our control testing ensures we’re investing in the right materials to redistribute pressure for longer — freeing buyers, caregivers and maintenance staff to focus on other critical aspects of delivering a high-quality patient care service,

while ensuring a low total cost of ownership. Our new immix hc-200, for example, combines three grades of foams that complement each other… eight combinations of foam grades creating several devices under test were utilised during development, before deciding the best solution that blended performance with longevity. At the budget end of high-specification foam grades, our nh-200 is a one-piece foam surface with its own unique CNC cut profile, designed to work effectively for longer, supporting judicious care operators.

What’s different about your approach compared to traditional solutions?

Innovators in pressure care largely focus on talking about their technology — sensors and automation. All that is naturally important, but we believe technology should serve care, not the other way around. Every product and feature we design is tested to ensure it saves time, reduces strain, minimises risk and integrates seamlessly into existing workflows. Our approach is always person-first, and one way doesn’t suit all. It’s innovation that respects realities of multiple healthcare environments, tailoring to individual needs. An example in our category is the shift towards hybrid surfaces, across different areas of care. Hybrid is at its core an optimisation model for pressure care delivery – stepup, step-down faster with a simple time and space saving solution. While it’s generally accepted that stepping-up patient care earlier to an appropriate surface for improved care, can be of benefit, it isn’t always valid. In longterm care, real value in enhanced health outcomes is witnessed over a longer period than much shorter hospital stays, and often the benefit is precisely achieved in the inefficiency of transferring the person from reactive foam to an appropriate true active air surface.

competition at the of the market, too much emphasis is given to imitating what’s out there already, growing a huge and often complicated set of choices. The principal players are competing mainly on price, to grow their market share, with many brands sharing the same appearance, copying the look, but not the science. I want us to focus on moments that matter most. That’s what we want OptiMat to stand for: not just price, but improved outcomes and better experiences for everyone – buyers and finance people inside care operator services; caregivers and maintenance staff on the frontline of care delivery, but most of all the very people living in care. It’s the whole meaning of creating more time to care that we’re concentrated on.

What’s next for OptiMat?

“More time to care” is a powerful message. How do you see that influencing the industry? I’ve always maintained that surfaces are tools to aid good nursing care.

I’m an idealist at heart with tireless self-belief and a big believer in building partnerships for mutual benefit. I have spent the past year working closely with good people that can help us build a sustainable future – some partnerships are old, and some are new. Together with one of our key collaborators, we have a couple of new immix-air active surfaces under development, targeted to launch around April, next year. Finding partners and dealers that share the

same values isn’t straightforward. It’s important we grow a dedicated dealer network, but we want our dealers to be able to reshape how support surfaces contribute to patient well-being and care service efficiency. Do you think diversification is essential for mobility retailers given the current economic climate? THIIS asked mobility retailers for their thoughts…

“As an industry, most people can relate to the pitfalls of the 2008 global financial crisis. I will admit that was probably the worst timing to start a business. I received a comment from a very respected senior director in this industry: “Well, if anybody is going to make a go of it, I put my money on you!” It was a memorable moment that represented support for my success. Often the lines brought in are everyday lines which need to be at a low gross profit percentage to ensure sales. Often customers will then wonder if they are coming into a department store or a mobility and stairlift retailer.

“So as always, we are looking for new promotional lines within our image of a leading mobility and stairlift retailer with a wide range of mobility products –certainly for our upcoming sale.

“We, like everyone else, had to think about what damage the Budget would do again to businesses and our customer base so that we could react accordingly, Last year’s Budget increased our overhead costs by around £600k.

“The end result is that the customers need a wide range of quality mobility products for their independence, along with the correct service, quality and price. Remembering the range in the main is a need and not a desire.”

Arm positioning

An arm support on a wheelchair is usually little more than an elbow rest. What do we really need for arm positioning?

Most standard wheelchairs come with what are frequently described as ‘arm rests’. These tend to be part of the wheelchair frame, and their function can often be wide-ranging, but of these functions, there is usually little to offer towards arm positioning.

What are these useful functions that the arm rests are offering? They can be providing ‘skirt guards’ to stop clothing getting caught up in or soiled by the wheels of a manual chair. They may be offering lateral thigh support to stop the legs from abducting too far from the midline. They may be the means for mounting a joystick and other controls on a powered wheelchair. They may be elbow supports. But what they are not, in most cases, are arm supports.

A true arm positioning support needs to be height adjustable to match the distance for the upper arm from the shoulder joint to the elbow, while for the lower arm they should support the elbow, forearm, wrist. and hand.

ARM POSITIONING SUPPORT

What are the elements we need to offer arm support, and even more importantly, arm positioning? First, for support, we need something to encompass the elbow and the wrist, to hold them in place. Next, we need to consider what are the anatomicallypreferable resting positions of the different components of the arm.

For the upper arm, it is critical that there are no forces on the glenohumeral joint at the shoulder, and that the humerus is not pushed upwards by too high an arm support, nor (worse) that gravity is pulling the humerus out of the joint. This damage can lead to severe chronic pain. Attempts to accommodate this position are often far from ideal (Figure 1).

This is a common presentation post-stroke where early intervention to

prevent the subluxation of the gleno-humeral joint is critical. The population of people who have had a stroke is important: the World Stroke Association reports that there are over 12 million new stroke cases each year. Globally 25 per cent of people over the age of 25 will have a stroke in their lifetime. Just over half of the strokes each year are in people under the age of 70. Approximately 15 to 30 per cent of strokes lead to the individual being permanently disabled. This means that there’s a very large number of people at risk of not receiving the early interventions that could lead to recovery of function and prevention of long-term harm and pain.

After resolving the need to get the elbow at the correct height – which needs the chair arm hardware to be height adjustable (ideally continuously rather than in stepped increments), we should address the support of the forearm and wrist. If you lift you forearm up from your side, it will naturally bring your hands towards your midline, and with the palms facing inwards.

What this tells us is that providing troughs that are parallel with the side of the chair, and that place the hand in a pronated position, are not encouraging a natural position (see Figure 2). This position parallel with the chair arm support is where the joystick of a powerchair is usually placed: for the occupant this is not ideal for fine motor control (would you place your keypad over to your side?), and can lead to the occupant developing a scoliosis from leaning over continuously to reach the joystick.

A trough restricts movement of the forearm, whereas one thing that we should be looking for post-stroke is early resumption of movement in the affected parts of the body, to reopen neural pathways affected by the stroke. We also need to facilitate hand movement to avoid the development of contractures in the wrist and hand.

DYNAMIC ARM SUPPORT

To achieve the anatomically correct position of the arm, therefore, we should have an arm positioning support which can be positioned at the correct height, and that supports the elbow, forearm, and hand in a position where they can be moved in front of the trunk. Such a support can be seen in the Bodypoint Dynamic Arm Support (Figure 3).

This support can be moved by the occupant in a 60 degree arc from outside the width of the wheelchair to across the body, or be locked into a fixed position as and when required. Alternatively, an elasticated strap can be tied between the support and the opposite side of the seat, whereby the occupant can exercise lateral arm movements against for the forces in the strap.

There is an adjustable support along the forearm to help keep the forearm in position. A critical element is the design of the hand support (Figure 4): the thumb is positioned away from the fingers to help resist tendencies towards contractures in the wrist and

hand, and the fingers wrap around a flexible sprung support which allows the fingers to flex and extend. There is a strap to help to hold the hand in place.

STROKE WARD APPLICATIONS

In the UK, on hospital stroke wards, the bedside seating provision on the whole is not best suited to help protect the individual from developing a subluxed shoulder joint, nor provide the immediate interventions required for optimal early recovery. What is on offer tends to range from transfer-type wheelchairs to upholstered static chairs – neither of which have the facility to get the elbow at an optimal height.

The Dynamic Arm Support is available with different attachment hardwares to fit the arm rests on most wheelchairs, which means that the support can be attached to existing wheelchairs, but the correct height adjustment needs to be incorporated into the design of the chair arm rests.

For a number of upholstered static chairs, BES Healthcare has been working with a couple of the manufacturers of these seating systems. Cura Seating has designed an adjustable height arm support that they can fit onto their chairs, to which the Dynamic Arm Support can be attached.

For the Careflex range, there’s now the byBES interface (Figure 5) that fits into the arm mounting systems which provides for continuous (rather than stepped) height adjustment, which also can be set up to accommodate increased or decreased flexion at the elbow.

Standing for at least an hour a day is recommended for all mobility impaired individuals. A number of therapists have added Dynamic Arm Supports to their patients' standers to provide additional upper extremity exercise.

ISO STANDARDISED SAFETY TESTING

To date the ISO wheelchair seating standard ISO 16840-31 has not provided for static or repetitive load testing for arm positioning supports. This is currently being addressed: at the recent meeting of the ISO Wheelchair Seating Working Group, tests were proposed that address static downward (1), upward (2), lateral (3), and angular (4) loads (Figures 6 to 8).

A failure mode arises if the maximum allowable deflection distance or angle is exceeded, or permanent deformation or breakage occurs.

Lateral forces arise from the seated user or the caregiver pushing against the arm support or pivoting it through a range of motion (Figure7). For ‘dynamic’ devices (3a), repetitive forces can be especially useful tests to identify failures by shifts in position or loosening of fasteners. For fixedposition devices (3b), lateral forces are equally relevant, even though no movement is expected.

For occupants exhibiting lateral leaning or users with strong tone, the angular forces can be quite high (Figure 8), and therefore this has been addressed in the new tests.

A rig has been designed to address repetitive loads which might be experienced in dynamic supports (Figure 9).

IN CONCLUSION

The arm support supplied as part of the framework of a wheelchair is normally little more than a poorly positioned painful elbow rest.

Functionally, an arm support should

be a positioning device which can be placed to optimise the individual’s function and well-being, and without risk of inducing long term harm. Appropriate solutions and their safety testing are becoming available.

Static Foam Mattress from Drive DeVilbiss Healthcare

This mattress is a high specification foam mattress with a multi-stretch vapour permeable PU coated fabric cover, which includes a 270-degree zip, and protective flap.

It provides pressure redistribution for individuals up to 187kg in weight. The top cover has a white inner substrate to assist in identifying signs of cover damage and strike through.
