The Park District Board meets the second and fourth Tuesdays of each month at 7 p.m. All meetings are held at Hoosier Grove Barn, 700 W. Irving Park Road, and all meetings are open to the public. Twilight Egg Hunt Bring your flashlights for an “egg-citing” evening of egg and candy hunting in the dark. The plastic eggs will be filled with points and candy. After all the eggs have been collected, participants will be able to take their total points and trade them in for prizes. The person who finds the golden egg will get a special prize. Pizza and juice will be served after the hunt. Please remember to bring your bag to collect your goodies.

The Easter Bunny is busy preparing for our annual Easter Egg Hunt. Children are invited to search for candy and have a visit with the Easter Bunny until 12:30 p.m. Be sure to bring a basket or bag to collect goodies. In case of rain, children can still visit the bunny indoors.

Passport to Play Your family summer adventures begin here. Tour the sights and sounds of Streamwood this summer. Visit a combination of 40+ parks while snapping pictures along the way. Passport to Play will conclude with prizes and a chance to win $100 towards your Fall 2015 program registration. Build muscle and strength through toning all major muscle groups in 45 minutes. TNT - Teens N’ Training Core fitness is established and then changes are made in individual’s programs as they progress. This class runs continuously throughout the year. Open Gym Basketball & Volleyball A current/valid photo ID is required for all Open Gym programs. Adult Open Gym requires a valid state ID or driver’s license to verify age. All Ages/Youth Open Gyms require either a state ID, driver’s license, or school ID. Expired IDs will not be accepted. Spectators will only be allowed into age-appropriate open gyms and are required to present an ID and pay the appropriate fee. Route will be held on wide, local neighborhood streets. Water stops are on the course, plus splits at all mile markers. NEW THIS YEAR: BIB TIMING

Kiddie Races (3-7 years old)

A high energy, flat run with lots of motivation from the crowd.

Pancake Breakfast

Pancakes, sausage, juice, milk, coffee, and fruit await race participants. Family members are welcome to take part in the breakfast for a charge of $3 for children 3-10 years old, and $5 for those 11 years and older. Breakfast will be served at Park Place Family Recreation Center in the gym immediately following the race.

Awards & Premiums

Children under the age of 5 must have an adult age 18 or older within arms reach at all times. • The Illinois Department of Public Health requires that all swimmers wear proper swimming attire. Swimsuits are required at all times. Cut-offs, gym shorts, sportswear, and undergarments are not allowed. Those not wearing the proper swimming attire will not be allowed in the pool. • For your protection, all flotation devices must be U.S. Coast Guard approved and labeled as such. All children wearing flotation devices must remain within arms reach of a swimming adult. • Members or any users shall abide by all State, County, Village, and Park District rules and regulations, and shall conduct themselves in a safe, considerate, and reasonable manner so as to not cause a disturbance or to damage equipment and property. Members or any users will display respect, consideration, and courtesy to all members, users, staff, and Park Place property. The Park District reserves the right to discontinue an individual’s use or membership due to disruptive or unsafe behavior. • Parents/Guardians are responsible for the supervision of their children at all times. Lifeguards are on duty to enforce rules and respond in case of emergency. • Parents with children 5 years old and younger are asked to use the Family Locker Room. Lifeguarding Certification Acquire skills and knowledge to prevent and respond to aquatic emergencies in this SAI StarGuard lifeguard certification course. Certification is for first aid, lifeguarding, AED and CPR. Prerequisites consist of the following: (1) be a minimum of 16 years of age; (2) swim 50 yards, head up without stopping; (3) swim 50 yards with a rescue tube; (4) tread water for one (1) minute without use of hands; (5) retrieve a 10 lb. weighted object off the bottom.

Egg Dive Friday, April 3

Come to the Park Place Pool for this unusual egg hunt. Eggs will be scattered around the activity pool for the kids to find. Make sure to arrive at the appropriate time, age groups will start promptly on time stated and the hunt only lasts a few minutes. The activity pool is closed until approximately 2 pm for the hunt. Open swim will last until 4:30 pm. Bring plastic bag or basket to collect your goodies; straw or wood baskets cannot be used in the pool. Baskets will not be available at the pool. Everyone must pay admission to enter the pool area, regardless of whether they swim or not. Children under the age of 10 years must be accompanied by a paying adult/guardian age 18 or older at all times. PASSES WILL NOT BE ACCEPTED FOR THIS EVENT.

General Notes: Swim Lessons • Open registration deadline is the Thursday before the class begins. After the deadline ends, a late fee of $5 will be added to the regular fee for any registration thereafter. • If you are unable to get into a class, please put yourself/your child on a wait list as classes may be added due to changes in enrollment. • In order for your child’s swim lessons to be most effective, we ask that parents watch all swim lessons from a distance that permits clear observation without distraction. Seating locations are on the bleachers or the chairs located by the concessions area. In addition, we ask that parents monitor all siblings or other children not in swim lessons during the time of your child’s lesson so that we may maximize learning for all lesson participants. • On the first day of lessons, please arrive approximately (10) ten minutes early for class. Minnows Swim Lessons This is a water orientation program emphasizing water adjustment, preparatory activities for swimming, fun, and enjoyment for the student. Students will not be learning stroke work as Minnows. They will learn the basic concepts of swimming, which will prepare them for the Fishbowl. Flying Fish Swim Lessons (Levels 4 - 6) Children must have completed Fish Lessons or its equivalent before moving into Flying Fish. There are three levels in Flying Fish. Provides a solid reinforcement of the foundation strokes. Breaststroke is introduced and sidestroke is reinforced. Intermediate survival skills and water safety are a focus for this level.

Level 5 - Stroke Refinement

Includes review of backstroke, breaststroke, and sidestroke. The Butterfly stroke will be introduced. More advanced survival skills and water safety are also included in this level.

Level 6 - Stroke Readiness

Flip turns and endurance training are the main objectives in this level. This class is a good preparation for children interested in joining a swim team.

Early Childhood

Building Block Preschool 2015-2016 September 14, 2015- May 14, 2016

Mark your calendars. Registration for the 2015-2016 school year begins March 21, 2015 from 8:30 a.m. until Noon at Park Place Family Recreation Center. Youngsters in the Building Block Preschool Program will learn through play as they are taught skills to help develop their creativity, as well as their educational and emotional well-being by qualified and caring instructors in a friendly atmosphere. The program gears its curriculum to make sure that students are taught the skills they will need to enter Kindergarten. There is a maximum student/instructor ratio of 10:1.

Enrollment Restrictions/Requirements

• Child must be appropriate age (3 or 4) before September 1, 2015 • Child must be toilet trained • Completed Registration Form • Copy of Birth Certificate Required • Proof of Residency Required • Program Information Form • Appropriate Immunization Records • Current photo ID • Parent Handbook Confirmation letter Program Information Forms and the Parent Handbook will be available online March 21, 2015.

2.5 Year Old Preschool

Parents will stay in the classroom with the children until after the Winter Holiday Break. Come January, one or two parents will be asked to volunteer in each class.

Fees

A $100 deposit is required at the time of registration. If your fee is not paid in full, your $100 deposit will be applied to your balance. $25 of all deposits is a service fee that is non-refundable. No refunds will be granted after January 30, 2016.

Payment Options

If payment is made in full, a discount of $30 will be applied to the fee. If using our payment plan, $100 must be paid at the time of registration. A second payment of 50% of your balance is due by September 4, 2015. The third payment of 50% of your balance will be due by February 2, 2016.

Group Care

Group Care Registration for the 2015-2016 school year begins on Saturday, June 20, 2015.

Participants are registered on a first-come, first-served basis by transportation routes. A minimum of two (2) days is required. All registration must occur at least two (2) full days prior to the child’s first day to allow time to notify the school and change the bus route. All Saturday registrations will be able to start the Wednesday of the following week.

Summer Day Camps CAMP REGISTRATION BEGINS MONDAY, MARCH 16 AT 8:30 A.M.

Our Parent Handbook and Program Information Form are now on-line. Please complete the Program Information Form and the Confirmation Letter that you read the Parent Handbook and return both before/on the first day of camp. Experienced Leadership

Youth Athletics

Positive Coaching Alliance The Streamwood Park District offers a wide range of youth sports and sports leagues throughout the year. Our youth sports leagues highly depend on volunteer coaches and team coordinators. We provide all the necessary equipment and training resources to ensure you are prepared to coach and have a fun and rewarding experience. All Head and Assistant Coaches must attend a Positive Coaching Alliance seminar, partnered with the Bartlett Park District. This 2-hour seminar is free for all volunteer Head and Assistant Coaches who attend. The dates for these seminars will be posted in advance to ensure that everyone involved has adequate time to properly plan. Upon completion of the seminar, all attending coaches will receive a certificate, which is valid for 2 years. Various classes will be scheduled throughout the year.

Optional Fundraiser When a parent signs up for a Youth Athletic League, they have the option to participate in a candy fundraiser. 4 years old - High School Girls

The Streamwood Park District soccer program teaches youth soccer skills, while promoting good sportsmanship and having fun. The Fall 2015 season begins in late August and runs through early November. We are always looking for volunteer coaches. Once volunteer coaches are chosen they will contact team parents in mid-August. Soccer players are expected to purchase their own shin guards and soccer shoes before the season starts.

U6 & U8 Divisions

The U6 & U8 divisions are strictly instructional. Games are held at a Streamwood Park District soccer field on Saturdays. U8 practices one (1) night during the week, and U6 practices before games on Saturdays.

U10, U12, U14, & U18 Divisions

Both the Girls and Co-ed leagues are part of the Inter-village Soccer Program, which will require some traveling to neighboring towns. Teams will practice one (1) or two (2) nights a week depending on availability.

In-House Girls All-Star Tournament Team This competitive team is for girls already enrolled in the 2014 Girls In-House Softball program. All Star teams could play up to 3 tournaments, which translates to a minimum of 12 games, maximum 30 games. Practices will be held once a week during the regular season, and up to 3 times a week after the regular softball season has ended. A minimum of 10 girls must make the team at each level in order for this program to run successfully.

The Chicago Fire Ambassador Park District Program aims to support current soccer programs at all local Park District locations. This program can help kick off a great season for Park District leagues through our player and coach education clinics. These free clinics are available to players and coaches who have registered for the Youth Soccer Program. Coaching Clinics will be available at the start of each soccer season. A free players clinic will be scheduled for the beginning of summer.

Chicago Storm Soccer Camp Age: 4 - 13 years old

Chicago Storm Soccer Camps offer the unique opportunity for children to work with professional and collegiate soccer players and coaches. Bowling (CP) Did you know that bowling is offered to girls and boys as a Varsity sport in High School? Here is your chance to begin sharpening your skills. The Streamwood Park District, in conjunction with Streamwood Bowl / Bradford Bowling, has teamed up to offer participants a chance to learn bowling fundamentals from certified coaches. Fee includes instruction, shoe rental, and bowling. Golden Lion Academy Martial Arts Golden Lion Academy Martial Arts is operated through various Park Districts by the Golden Lion Academy. The focus of the Golden Lion Academy combines the disciplines of Tae-kwon-do, Hapkido, Judo, and Kung-fu. Students six years and up learn the rudiments of self-defense from day one. They build upon these features by adding more advanced techniques from the various styles, while also learning the Korean language and the history of martial arts. As students progress, they earn opportunities for advancement through the various ranks in a safe, friendly, and enjoyable environment. Illinois Shotokan Karate Children and adults universally enjoy learning the art of karate. Classes are taught by Illinois Shotokan Karate Club’s expert instructors under the direction of John DiPasquale, four-time National Champion, U.S. Karate National Coach, and President of the National Karate Federation of the U.S. Beginner students will not be permitted to register into the current session after the second class has met. Teaching Assistant Program Intermediate and advanced dancers have the opportunity to assist Dance Academy instructors with Pre-K and Kinder level classes. Responsibilities may include taking attendance, learning all student’s names, remembering recital dances, leading dance activities and exercise, assisting the lead teacher, and attending all scheduled classes.

Summer Dance Camp 6 - 14 years old

Summer dance camp is a fun alternative to the traditional day camp. Kids get the camp experience with games, field trips, crafts, and swim days while also learning new dance skills and fun choreography. Each camp will put on a showcase on the last Friday of each session for parents, family, and friends. Please remember to send your camper with a water bottle, lunch, and gym shoes each day. Campers must also have appropriate dance shoes for their camp (ballet camp: ballet shoes, Jazz camp: jazz shoes, etc), as well as comfortable dance clothing. Golden Agers Social Club is the place to see old friends and make new ones. Attending meetings and events are great ways to get out and stay active, so adventure out and join us for games, activities, and seminars. There is always someone to welcome you at the Parkside Room at Park Place Family Recreation Center, hosted by our Senior Coordinator Tracey Stanley. Play your favorite card games, and enjoy our outdoor patio. Active members receive a free Fitness Center Track use.

Parks Park Pride Day 2015 Saturday, May 16, 2015 9 a.m. - 12 p.m.

This year we celebrate our 5th Annual Park Pride Day. The Streamwood Park District Parks Department will provide the tools, flowers, and lunch. Just join us with some hands ready to work!

What is Park Pride?

During Park Pride Day, volunteers from all over the community are invited to join us in one of our 47 parks. Volunteers can plant colorful flowers in our sign beds, pick up trash, and pull pesky weeds from the park to prepare for a busy spring and summer. Directly following your hard work, join us at Hoosier Grove Barn for a complimentary lunch. We can accommodate any size group. Hurry, parks are going fast as we continue to make Park Pride Day better each year.

Memorial Program Become a part of the Streamwood Park District. Memorial Tree

The fifteen trees offered within the Memorial Tree Program have been selected to provide a variety of aesthetic and environmental benefits for Streamwood’s parks. Trees will be planted in either spring or fall during optimal planting conditions to ensure health and longevity. All trees will be between 2.5”-3.5” in diameter to give the tree the best chance of survival after transplanting. Please visit our website to see all of the tree options available. Please note: the Streamwood Park District reserves the right to edit text and determine the location of bricks in the pathway pattern. Bricks are $75 each. Price includes the brick, engraving, installation, and maintenance. Memorial Bench

A 6 foot recycled material bench that can be placed at the park of your choice is available through the Memorial Program. All benches include an engraved plaque that can include up to four lines of text. Plaques will be embedded into the back of the bench for comfort and durability. Benches will be installed by the Parks Department within 4-6 weeks of the request, as weather permits. Memorial benches cost $1,400. Price includes the bench, engraved plaque, installation, and maintenance.

Sunny Hill Splash Pad

Emerald Ash Borer Pardon Our Dust.

You may have started to see some big changes to the Streamwood Park District’s landscape over the past year. The Park District has about 325 Ash trees throughout our facilities and parks. Due to the invasion of the Emerald Ash Borer (EAB), over half of these trees have died with more to come. EAB is responsible for the death of over 20 million trees in the United States. In the upcoming months, the Parks Department will be removing these trees from our facilities leaving a noticeable change in our landscape. We appreciate your patience as we work to remove the infected Ash trees. In order to reestablish the Park District’s tree canopy, Parks Staff will be replacing a good percentage of these trees over the next several years.

Disc Golf Our 9-hole Disc Golf Course is located at Sunny Hill Park on Irving Park Road, west of Route 59. Hole One is located near the top of the hill in the southeast corner of the park. Our 18-hole Disc Golf Course is located in a beautiful setting of Shady Oaks Park behind the Administration Building at 777 S. Irving Park Road. Parks Vandalism Hotline Public playgrounds have become the targets of arsonists and when a loss occurs, an entire playground worth tens of thousands of dollars can be lost to the children of the community within a matter of minutes. Other park amenities such as shelters have also been damaged or destroyed. Park staff also spends hours removing graffiti on park structures and playgrounds. Registration information

Registration for all of our programs and classes is on a first-come, first-served basis. Proof of residency is required (state IDs or driver’s license) every time you register for a program. Participants will be charged nonresident fees without appropriate proof of residency. Mastercard, VISA, and Discover Card, as well as cash, personal checks, and money orders are acceptable forms of payment. Post-dated checks will not be accepted. For your convenience, we have five easy ways for you to register: Walk-in, drop-off, fax, mail-in, and phone.

Walk-in:

Walk-in registration will be taken on a first-come, first-served basis. Simply fill out the registration form completely, and bring in proof of residency, along with payment. The Clerk will confirm the status of your registration and it will be processed that same business day.

Mail-in:

Complete your program registration form, along with a copy of your proof of residency and payment, and mail to the Streamwood Park District Registration Office, 550 S. Park Blvd., Streamwood, IL 60107-2000. This method of registration is best with check or money order payment methods; if there is a waiver on file, the Clerk will contact you to complete payment information if by credit card. Do not send credit card information through the mail. Once the application has been processed, the registration clerk will contact you by phone to confirm your registration status.

Drop-off:

Complete your program registration form, along with a copy of your proof of residency and payment. If paying by credit card, you will be contacted for your credit card information the next business day. The drop box is located inside Park Place Family Recreation Center at the front counter for your convenience. You will be notified of class status by phone.

Fax:

Complete your program registration form and fax with proof of residency to 630.483.0562. You will be contacted on the next business day for your credit card information. Do not record any credit card numbers on the faxed form.

Phone:

A “Waiver on File” form, along with proof of residency, must be on file at Registration prior to using this registration option. Once on file, you can simply call the Registration office and process registration using a credit card. If a Clerk is not available when you call, please leave only your phone number, name, and class number desired; a Clerk will return the call to retrieve credit card information; do not leave credit card information in your message. (Note: due to additional forms that are needed for certain classes/programs, phone registration has some exclusions. Please call the office for more information.)

Summer is a great time to visit museums and nature centers in the Chicagoland area. The Streamwood Park District Passport Program will have those located in your region. Many sites have activities for your child to complete while visiting.

Passport Program Guidelines

• Each child participating must have their own passport; which can be picked up at the Hoosier Grove Museum beginning Labor Day Weekend. • Suggested for children 4 years and older. • The passport should be stamped before leaving the museum or nature center. • Participants visiting seven (7) or more sites can receive the general KDRMA prize from any museum. However, site-specific prizes are only given to the participants returning the book to the same location it was picked up from.

Streamwood Historical Society Are you interested in local history? If so, then the Streamwood Historical Society is for you. The Society assists with museum-related programs and projects throughout the year. New members are always welcome. Meetings are held the first Monday of each month at 7 p.m. in the Farm House, unless otherwise noted. If you are not able to attend a meeting, but are still interested in volunteering, there are always activities or projects needed to be completed. We have opportunities for home-school groups, as well as Boy Scout/Girl Scout cultural and history-related badge programs.

Splash & Pool Parties Looking for a fun and innovative place to hold your child’s next birthday party? Park Place Family Recreation Center has many amenities to suit you and your child’s ideal birthday parties wishes.

A Park Place Splash and Semi-Private Pool Party includes:

• Access to the 25 yard lap pool and the Activity Pool. The Activity Pool is a zero-depth entry pool featuring a Kiddy Slide, Horseshoe Channel, two-story high water slide, and water play structures. • Three (3) hours in the Party Room. • One and one half (1.5) hours in the pool area (part of your 3-hour room rental). • Plastic table covers in the color of your choice. • A cart to bring in and take out your party supplies. • Thirty (30) minutes before and after your party to set-up, decorate, and clean up. Trash bags and trash removal are included. •Your Party Host/Hostess will be dedicated to help you in the beginning of the party, will periodically check on you and your guests during your time in the party room, and will also be available after your party to help clean up. • Ice, ice bucket with scoop. General Information

NWSRA exists to provide outstanding opportunities through recreation for people with disabilities. Celebrate ability throughout the Northwest suburbs. Join the celebration! The team of full-time Therapeutic Recreation professionals and trained part-time staff at the Northwest Special Recreation Association (NWSRA) offer safe, fun, and quality year-round opportunities to individuals with disabilities of all ages and ability levels. Inclusion assistance provides individuals with disabilities the opportunity to participate in their home park district program. When registering, the parent of the individual should inform the park district (and NWSRA, if desired) of any accommodation needed in order for the individual to successfully enjoy the program. The park district staff will then contact NWSRA for assistance with the accommodation. For more information on NWSRA, check out nwsra.org. Your Cooperation Is Needed Parks are for everyone to enjoy and should not be used as a dumping ground for litter, garbage, or waste. Keep your parks clean and free from debris and waste. If you walk your pet in or around a park, please be sure to pick up after it. Keeping your parks clean will enhance the beauty of the park. We appreciate your cooperation. Control of Pets Pets, including dogs, cats, and other domesticated animals kept as pets, may be brought into any park, but only when leashed in accordance with Village of Streamwood ordinance(s), licensed as necessary, and controlled by the person bringing such pet into the park. It is expressly prohibited for any pet, even though on a leash, to enter or be on a park playground. Defecation by any pet while on Park District property shall be immediately and completely removed by the person in control of the pet. No pets shall be allowed in any Park District facility or building unless brought in for a Park District program or event involving such pet. In such case, the pet must be leashed, licensed and controlled as aforesaid. The provisions of this section shall not apply to service animals in the performance of their duties. All Streamwood Park District ordinances are enforced by Streamwood Police Department. Smoking Policy In compliance with state and federal laws, the Streamwood Park District has set the following policy: No cigarette, pipe, or cigar smoking will be allowed in any of the Park District facilities. The policy includes Park District activities as well as private rentals. Your cooperation is appreciated.

Child Car Seat Requirement All parents and guardians of children participating in a Park District trip or program where they are being transported by a Park District vehicle, must provide a child restraint system for any child 7 years old or younger or sign a waiver giving permission for the child to be transported without such system. All participants must wear seat belts. For trips using a school bus, there are no restraints for adult passengers or children.

Photo Disclaimer Registrants and participants permit the taking of photos and videos of themselves and their children during Park District activities for publication and use as the Park District deems necessary.

Emergency Shelter In case of a disaster situation as identified by the Emergency Services and Disaster agency, Park Place will be used as an emergency shelter. If this occurs, it may be necessary for some programs to be postponed or canceled. Lightning Prediction System The Streamwood Park District has installed three lightning prediction systems. One located on the roof of Park Place Family Recreation Center, the second at Hoosier Grove Park, and the third at Rahlfs Woods Park. These systems gather atmospheric information to accurately predict when conditions are conducive for a lightning strike. When conditions are “right,” the warning will sound (a single fifteen (15) second horn blast) followed by an orange strobe light located outside on the roof at all locations and inside of the building at Park Place. At this point, participants in outdoor activities need to cease all activity and seek shelter immediately. When the hazardous weather has passed, the “all clear” will sound (a series of three-5 second horn blasts), and the orange strobe will turn off. Only at this point should outdoor activities resume. It is important to remember that even though the system may not sound when weather conditions are threatening, you should exercise good judgment. Proceed to stop all outdoor activities and move to a safe location until the dangerous conditions have passed. Horns from the Park Place location can be heard at Dolphin Park baseball fields. These warning systems will be active during the months of April through October. Further, the District will be checking the systems manually, on the first and third Tuesdays of every month (April-October) at 10:30 a.m. Please contact Superintendent of Parks and Planning, Keith Gorczyca, at (630) 483-3161 for additional information. Camera Restrictions The use of any visual recording device in any protected areas is prohibited. Visual recording devices include, but are not limited to, any camera, camcorder, or videotaping device, and cellular phone cameras. Protected areas are defined as any locker room, dressing room, bathroom/rest room, lavatory, shower, or bathing facility located in any District facility. Any person caught violating this section of Park rules may be banned from Park District facilities and subject to a fine. Brochure Changes/Errors Disclaimer Due to the large amount of information available in the Streamwood Park District brochure, errors and changes before or after publication may occur. We apologize for any errors in this brochure and will advise you of any changes as quickly as possible. We will also attempt to keep the information on our website (spdcares.com) as current as possible. Thank you for your patience and understanding when these situations arise. Privacy Policy The personal and private information collected by the Streamwood Park District from any individual(s) is intended for the sole use of the District, and such information will not be sold or distributed to other persons or agencies without the written permission of the individual(s) providing it, except as required by law. If information is provided to any other person or agency pursuant to any legal requirement, the District will advise the individual(s) of the disclosure. Resident and Business Rates A Streamwood Park District resident, via property taxes, makes a significant financial contribution to the operation of the District, whether or not he or she takes advantage of the services offered. The nonresident fees are required of nonresidents to apportion an equalizing fee to contribute to the overall financing of the park system on an equitable basis with the resident. Resident Fees: A “resident” is defined as anyone living with in the geographic boundaries of the Streamwood Park District. Residents of the Streamwood Park District have priority when registering for most programs. Business Rate: All people that work at a business within the Streamwood Park District boundaries may receive resident rates on memberships (proper identification required along with current pay stub, program participants will be randomly sorted in the nonresident lottery).

Service Charge Policy All refunds issued which are not a result of a Park District cancellation will be assessed a service charge. The service charge for programs with fees of $10 or more will be $5; programs with fees less than $10 will be charged 50% of the program fee. Class Cancellations Every effort is made to avoid canceling classes. However, a minimum number of participants must be registered. Notification of class cancellation may be made with as little as 24-hour notice. In cases where a program is canceled, you will be notified prior to the start of the program and a full refund will be issued. Transfer Policy All transfer requests will be honored if there are openings in the class requesting to be transferred to and the participant is the appropriate age for the program. If additional payment is due, this must be made at the time of the transfer request. For refund see #7 under Refund Policy. Program Scholarships The Park District recognizes that there are people in the community who are often unable to participate in recreation programs due to economic hardships. If any resident individual or family would like to apply for a scholarship, pick up an application packet at Park Place Registration Office or call the Registration Counter, (630) 483-3025, or Recreation Manager, Cindy Babicz at (630) 483-3730 or cbabicz@spdcares.com to have one mailed or emailed. All requests will remain confidential.

Important Information regarding Waiver and Release The Streamwood Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Streamwood Park District continually strives to reduce such risks and insists that all participants and parents/guardians of minors registering for programs/activities must recognize that there is an inherent risk of injury when choosing to participate. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury, or impairment, to consult a physician before undertaking any physical activity. Warning of Risk Recreational activities/programs are intended to challenge and engage the physical, mental, and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers, and injuries due to inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities/programs exist. In this regard, it must be recognized that it is impossible for the Streamwood Park District to guarantee absolute safety. Behavior Policy Appropriate behavior is expected from all participants and observers in programs and in facilities to protect the enjoyment and safety of all patrons. Behaviors that will not be accepted include, but are not limited to, the following: • Disrupting a program or creating a disturbance at a facility. • Continuous refusal to follow program and/or facility rules and guidelines. •Use of verbal harassment, profanity, vulgarity, obscenity, or racial slurs. •Blatant disrespect of staff and program or facility rules and guidelines. •Damage, vandalism, or theft of private or Park District facilities, equipment or supplies. • Possession or use of illegal substances or medication, including the use of alcohol or smoking where prohibited. • Inappropriate or offensive dress, including, but not limited to dress which is too revealing and clothing which displays obscene or offensive words or pictures. • Possession or threat of a weapon. •Gang activity including display or possession of symbols, hand signals, soliciting membership, intimidating or threatening any individual, wearing or displaying colors or items of dress, etc. • Inappropriate display of affection, including but not limited to prolonged kissing and touching of any private area of the body. The following outlines steps that may be used with patrons who are not behaving appropriately. Membership or program fees will not be refunded for any person removed from a program or facility due to violation of this behavior policy. of a minor, a copy of the notice will be provided to such minor’s parent/guardian. If a participant receives three notice of inappropriate behavior, or if the participant is involved in one severe incident, as determined by District staff, the participant may no longer participate in the program until a meeting is scheduled and held. The participant, a parent/guardian if the participant is a minor, the program instructor, the program supervisor and any other persons whom the District considers necessary for a resolution of the inappropriate behavior, shall be present at such meeting. At the meeting, the question of continued participation in the program will be considered and, primarily for minor participants, a Behavior Contract may be prepared as a condition of continued participation. Such a Behavior Contract will be personalized to the circumstances and the participant, and will include specific expectations and consequences for failure to meet such expectations, including possible suspension and/or termination from program participation. Inappropriate behavior by adults may result in more rapid progress toward suspension or termination from program participation. Facilities All facility participants are expected to follow the rules of the facilities. Specialty areas of the facilities (example: fitness center and pools) have posted signs stating special rules specific to that area. Disregard or abuse of facility rules may result in removal by staff or police from the facility for a designated length of time or permanently. Observers Observers of programs are also expected to behave appropriately. Parents, friends, and any observers are held to the same standards of behavior as participants. If inappropriate behavior of a program observer is reported to or observed by a staff person, the observer will be asked to leave by the staff or police, if necessary. Appeals Any participant who is suspended or terminated from program participation shall have the right to appeal to the Executive Director within fourteen days of any notice of suspension or termination. The Executive Director shall thereupon schedule a meeting concerning such appeal, and the appellant shall be entitled to present his or her version of the facts at such meeting. Within five days after such meetings, the Executive Director shall notify the appellant in writing that the suspension or termination has been affirmed, modified, or reversed. The Executive Director shall notify the Board of Park Commissioners of the meeting and the decision. It is a vision of the Streamwood Park District to provide safe and wholesome programs in well-maintained facilities for our residents and guests. Rules must be followed and policies such as this must be established to protect this vision for all involved. Our goal is not to remove patrons from programs or facilities, but to work together with participants and/or parents/guardians to the best of our ability to correct or deal with a problem. Program Participants All program participants are expected to behave appropriately and follow the rules of the program and facility. A participant will receive written notice for inappropriate behavior, and in the case

Refund Policy To request a refund on a program, a Refund Request Form must be completed and submitted at the Registration Counter at either center. Administrative approval must be received before a refund will be issued. 1. A full refund will be given when the Park District cancels a trip/program or a refund is requested as a result of being placed on a waiting list. 2. A refund minus a service charge will be given when a refund request is received a minimum of four days before the start of the registered program. 3. A prorated refund with a service charge may be granted when a refund request is received with less than four days before the start or after the start of the registered program and the reason is approved. Documentation may be requested with your refund request in order to be approved. The prorated refund will be based on the date the request was received, not the last attended class. A refund will not be approved if the session is more than half over. No refunds will be issued for individually missed or forgotten classes. 4. No refund requests will be granted that are made more than two (2) business days after a one day program or special event. 5. Refunds for trips will not be given unless one of the following occurs: A. Park District cancels the trip. B. A replacement is found to take the trip. 6. Deposits for certain programs and trips are identified as nonrefundable and therefore the refund policy does not apply. 7. The cost of purchased individual supplies or equipment such as t-shirts, uniforms, trophies, costumes, etc. will be deducted from your refund. No reimbursements will be given for required equipment or supply purchases made by the participant or parent. 8. Please allow 3-4 weeks for your refund to be processed. If originally paid by check or cash, a check will be mailed to you. If paid by VISA, Master Card, or Discover Card, we must credit the card with which payment was made. 9. Any programs with special policies void the preceding. See page 9 for membership and punchcard refund policies
