Senior Management update:

CTG STUDY DAY

Dear all,

We really need your help.

As you may all know we are required to meet certain training compliance every year in order to pass CNST (MIS). If we do not pass £1.5 million is removed from our budget.

We are all trying very hard to pull us through all 10 elements. One of which is training. We didn’t achieve this last year, and lost the money : (

With the new requirements all midwives, obstetricians and ST3 and above are required to attend a full day CTG study day every year We need everyone to have attended by 30th November 2023.

Along with all other training requirements we realise this is a significant undertaking and this is why we ask for your help.

Midwives:

Some of the training days fall within the period of rotas already completed, we are therefore asking please for you to agree to attend as an extra bank shift. For rotas not already completed rota coordinators will attempt to allocate within your rota, however you may be asked if you can attend as an extra shift if this is not possible. If your rota is already finalised please book yourself a place on bank by informing Charity Davor and Amarpreet Malhotra via email as soon as possible.

Please note any bank only midwives who are not compliant with all mandatory training by mid-November will be removed from the bank.

Consultants and Doctors (excluding GP trainees)

This can be booked as study leave and should be confirmed with your line manager and rota coordinator. Please book yourself a place on bank by informing Charity Davor and Amarpreet Malhotra via email as soon as possible, once authorised.

Uniform policy reminder

The Trust has received complaints recently regarding staff wearing scrubs outside of the hospital. We would like to remind all staff of the uniform policy, which can be viewed on the intranet. If you have any questions, please speak to your manager in the first instance.

Getting to know you better………….

Hello everyone, Thank you for such a fantastic welcome.

I am covering the Deputy Head for Workforce and Education post, across site. I am lucky that I know a fair few people and have met lots more already.

For those people I've yet to meet, a little about me. I'm a midwife, a sister a mother, and (more recently) a grandmother. I am very lucky.

In my working life, I've been in more of a strategic role for the past few years as Clinical Lead Midwife for BLMK Integrated Care Board. Alongside, I also had a regional role in NHS England - East of England, as Lead for Midwifery Continuity of Carer and Personalisation. Prior to that I have been a Matron for both Community and Inpatients and have worked across all areas of maternity.

I would like to really get to know how best I can support the Maternity team and am keen to listen - both to staff and to our service users: I am a PMA, so please do stop me if you see me. There has been a lot of change all around us, some change is exciting, other change is harder for us all. If you think I can help you, please ask me. I am delighted to be working with you, striving for excellence for our staff and families to be proud of our wonderful maternity service.

Hello everyone,

My name is Lorraine Hawkins, and I am very excited to have just started in post as Consultant Midwife across both sites.

I have had the warmest welcome from all staff, and I really look forward to working with you all and learning about the great work you all do every day.

I qualified as a midwife in 2001, and have worked in different roles in maternity. My most recent roles were labour ward coordinator, ward manager and, this last year, as seconded Consultant Midwife which I absolutely loved. The four pillars of my role involve supporting and leading in clinical environment, leadership, research and education.

I am a Professional Midwifery Advocate, which I am very happy to be continuing here. The last several years have been a very difficult time for all with many challenges, but also many celebrations that you are warmly sharing with me on my journey around the Trusts.

It is a privilege to be part of this amazing maternity team and I look forward to hearing your ideas and how I can support you all to continue to provide compassionate, safe care for all.

Liberty team have a new Team Leader due to start in September, Lyanne Roxborough, and an additional 3 team members: Michelle Brown, Anna Starcevic, and Paige Treasure.

We hope to launch in October once the guideline is ratified.

We would like to say well done to all our NQM who are nearing the end of their rotation in community. They have learned so much and we are sad to see them move on. We look forward to welcoming the next cohort and hope they find their rotation to be as enjoyable and valuable as others have. Good byes & good lucks go out to: Elis Feasey, Stacey Reid, Lucy Westcott, Morgan Walters – we have enjoyed watching you grow and develop, wishing you every success as you near the end of your preceptorship!

I’d like to say a big thank you to community teams for supporting our international midwives to integrate into the teams. Sarah Addai, Priscilla Karikari, Joyce Addo & Joana Williams are all thriving in community practice and very much enjoying it.

A very warm welcome to our lovely Cohort 13 of Internationally Educated Midwives. We are very excited to support you on your journey.

IEM

Last year, the Gemini Multiple Pregnancy team at the L&D which was established in 2020 to provide specialist care to families expecting twins, set up a monthly tea and coffee morning held at St Thomas’ Church in Luton for families who are expecting and those who have recently had twins (and more)!

These monthly sessions have created an inclusive, close knit community of individuals going through similar experiences, has cultivated ongoing friendships and provides an opportunity to seek guidance from the Gemini team if needed.

Hazel Cathcart, Head of Midwifery at the L&D and Roxy Vidal, Community Matron, attended the special anniversary event held on 14 July to talk to the families and celebrate this wonderful milestone.

Head of Midwifery, Hazel Cathcart, said: “I’m so proud of our Gemini team who continue to go above and beyond for families in our care. It was an honour to be invited to the celebration, to see in person the product of Debbie and Nicki’s hard work and speak to our service users who only had positive things to say!

Big thank you to Sally Jacobs who have done well with completing our monthly IPC our percentage on most are reaching 100%

In regards to a NBBS being taken and labelled with the incorrect baby sticker. This was flagged up by the national reporting system. The sample had to be repeated we apologises to the mother and explained what had happened. A set of twins that were born one was alive the other was a still birth.

Upon investigation we identified:

Community midwives were given stickers for the still birth baby and not for the baby that was alive

Community midwives labelled the sample with the incorrect sticker

Screening team sent sample to GOSH with incorrect sticker.

The incorrect sticker was for the stillbirth baby. In ALL AREAS we failed to check the NHS number on the stickers.

Please can we all be vigilant and check the NHS number as the identifier as this number is unique to everyone.

KWIP TEAM

The KWiP team have introduced CO Champions to thank and congratulate staff who are complying and supporting the service with the highest number of conducted CO readings and referrals to an advisor (bonus points for partner referrals). This will be awarded to the following on a monthly basis and hopefully announced in our newsletter and on our Marvellous Maternity FB page. We are doing this to celebrate the hard work of our staff here at Luton.

National Breastfeeding Week will be taking place from 18th24th September. This year will continue and build on last year's theme, 'Everyone has a part to play in helping mums to breastfeed'. This will include daily themes and activities showing how families, community, health and education services all have a part to play in supporting mothers to have the feeding experience they hoped for.

A new social media toolkit will be available two weeks ahead of National Breastfeeding Week (no later than Monday 4th September), to support partners engage with and promote the week. We will be back in touch to let you know when this is ready.

Public Health have recently launched Health an e-learning programme:-

‘The programme can be accessed on the NHS Learning Hub: Breastfeeding and Perinatal Mental Health programme and feedback can be shared with EoEHealthandWellbeing@dhsc.gov.uk.’

‘The programme aims to raise awareness of the links between breastfeeding and perinatal mental health, and the factors that can influence this relationship. It also provides tools and guidance to support staff to deliver self-reflective, person-centered care. It is aimed at anyone involved in providing support to families during the 1,001 critical days, including midwives, health visitors, infant feeding support workers, perinatal mental health specialists, psychological professionals and early years workers.’

Student supervisor & assessor preparation - study day

This study day is for midwives new to student supervision and for those who wish to become student assessors. The day includes:

 A self-study booklet and attendance at sessions concerning the SSSA, the new documentation, and how to raise concerns with a struggling student.

 Group and individual activities involving coaching, the learning environment, practice-based scenarios and how to give effective feedback.

Student attendance and sickness

A reminder for shift leaders: please mark onto the off duty/staffing boards any students that have not arrived for their shift. It is important that we know who is on site at all times. Students must follow the trust sickness reporting policy, and the bleep holder will pass sickness forms to the team; absences not reported will be marked unauthorised and the student will lose the practice hours. Attendance is monitored and the team will follow up all absences. Students are informed prior to the shift who they are working with – if they do not know please ask them to check their e-roster

Student survey – clinical placements

All students have received a survey asking about their placement experiences and the support they receive. Results will be shared in the next newsletter.

What information does the student receive prior to placement? 

Our 3rd year students are nearing the end of their course, and 14 will be starting as Newly Qualified Midwives in October. Congratulations to you all – you made it!

Power hour – we need you!

Students are encouraged to attend the daily power hour in the sitting room on ward 32 at 12pm. Students must use this time for collaborative, peer assisted learning and reflection; as a supervisor you can set them tasks/subjects to learn about (e.g. which might relate to clinical care they have been involved in during that particular shift). The student should then share their findings when they return to you at 1pm. We are looking for enthusiastic staff members to come and join in with the sessions:

Is there a topic you are passionate about that you would like to share with the students?

Are you a specialist, who would like to do a short teaching session about your speciality? Have you noticed a common area that students seem to lack knowledge?

If so, please get in touch with the team and discuss any ideas you have to help us make this learning opportunity as useful as possible.

Birth Trauma Awareness Week: 16-22 July

Birth trauma differs from one person to the next. Often the trauma is minimised or compared to others experiences as not being as bad therefore the trauma may not be processed or validated leading to reoccurrence of this trauma in the next pregnancy.

The Ocean Team at Bedfordshire Hospitals is here to support pregnant women and birthing people to navigate their way through their pregnancy or following birth trauma or loss. Alongside the midwifery and obstetric teams, the Ocean Midwives provide support with trauma informed birth plans, exposure work, coping strategies, hypnobirthing, accompanying to appointments and providing a safe place to talk about their experiences and empowering them to move forward in this new pregnancy.

Every week as part of an MDT we review cases where mothers have suffered a Stillbirth or Neonatal Death. I have been looking at themes from these meetings and thought it would be useful to send out the themes/reminders on one email.

Firstly I would like to say thank you for your feedback last week in regards to the VTE practice alert that I sent out. I have asked the Audit & Guidelines Midwife to look into this for me, once I have feedback from this, we will make sure it is circulated to you all.

Reminders

Antenatal Care

Please make sure that there is documented evidence that the mother has been given a reduced fetal movement leaflet to the woman that is in a language appropriate for her. Please that this is discussed with her at every antenatal clinic visit.

If the mothers first language is not English, please document in her antenatal notes if you have used language line (with in number) or if an interpreter is present.

Please make sure that if the mother meets the criteria for screening for gestational diabetes that this is documented in the notes or on her window.

Intrapartum Care

The mother’s progress in labour should be monitored on a partogram

Postnatal Care

Please use the Stillbirth ICE profile for taking initial blood tests after giving birth (this includes the Kleihauer blood test).

Appraisals

I will continue to support the senior management team with completing appraisals. If you need your appraisal, please contact your line manager or myself.

Revalidations

Well done to everyone who has completed their revalidation. For September, 45 colleagues have been supported.

Sessional PMAs

I’ve started to support the sessional PMAs to gain confidence after COVID and staff shortages. On the ****** Jessica Giblin joined me for the day, where we did a trolley run with healthy goodies, and the PMA booklet.

The sessional PMAs will be available shortly to support with HSIB interviews, revalidation and to be the ‘listening ear’.

Regional PMA news

Keely and myself met with the East of England Regional Lead PMA at the end of July to edit the PMA booklet for Maternity staff, this is so it can be made available to all East of England PMA Teams to share with their colleagues. The East of England region covers from Norfolk to Watford, Southend to Milton Keynes. Changes made were for Trusts to include their own Trust values, referral into the PMA team and generic wellbeing services.

The latest edition of Notice Fraud has been published. Please visit the intranet to read and learn more about current fraud trends to protects yourself and our patients (Bedford / Luton).

Responding to incidents with PSIRF

In the coming weeks, we will be changing how we respond to patient safety incidents in line with new national standards. We will no longer be using the Serious Incident Framework and will instead follow the Patient Safety Incident Response Framework (PSIRF).

Under PSIRF, we consider the system surrounding an incident rather than focusing on individual decision-making. We are looking at how we can get better at involving staff, patients and families in improving patient safety and ensure we respond proportionately to incidents.

Once implemented, Trust will prioritise review of incidents with the greatest opportunity for learning and improvement. Over the next few weeks, we will share our response plan, which outlines our Trust Priorities for investigation along with guidance documentation to explain what you can expect if you are involved in a learning response and how to use the new tools undertake a system-focused investigation.

INSIDE NEWS

Dear all,

The months are motoring on and here we are in the height of summer. I really hope that you will have time to read through the newsletter, as it demonstrates what amazing work is being undertaken by all the maternity and neonatal teams in the East of England, despite all the industrial action. Such a tremendous amount of work has been achieved and this is down to everyone working so hard with enthusiasm and positivity. A great big thank you. It is so exciting to see teams learning from one another through the various forums, workshops and quality improvement initiatives that are now in place. If you think we don’t have a forum for your particular interests then do let us know and we will see what can be done to address this.

Our celebration of excellence in March set the standard for a positive Spring and Summer. With so many nominations for our staff and MNVP’s it was a challenge to choose the winners. The launch of the three year delivery plan for maternity and neonatal services on 30th March has given us a framework in which to base our future work plans and bring maternity and neonatal services closer together. The maternity services insurance scheme Clinical Negligent Scheme for Trusts (CNST) year 5 has also been launched to help support Trusts improve their services and also Saving Babies Lives Care Bundle version 3 is now available.

The newsletter outlines the many quality improvements and safety initiatives that are underway, please do feel free to join any of these work streams, we love having staff particularly from the Trusts involved and sharing to make those improvements in maternity and neonatal services. The perinatal equity and equality learning over the last year again demonstrates how far we have come, but still lots of work to do to ensure women and pregnant people and staff get the best possible outcomes.

Many of you will be familiar with your digital midwives who continue to do a fantastic job at improving our digital services to make it easier for you to do your jobs. We now have some wonderful examples of paper light systems in place. Great news on the workforce front, we continue to have the most midwives ever working in the EOE standing at 2597, the vacancy rate has reduced from 18.3% to 11.2%, the leaver rate for midwives has reduced from 8.1% to 6% and for MSW 17% down to 11.5 %. We still have more work to do as we would like to have no vacancies.

Finally huge thanks to you all, every person plays a part in delivering excellent maternity and neonatal services. I hope that you have some wonderful breaks over the summer.

“The treatment and care I received during my visits or admissions to Ward 31 and Ward 32, was amazing. I could not fault the staff for all care and treatment I received. Especially Norah who I believe works between Delivery Suite and Ward 31. She has no idea how much she eased my anxiety. Natalie (Martin), who was my main midwife on the day I delivered my beautiful daughter, was amazing. The team in theatre were amazing and I wish I could thank them all individually. Especially as I lost 1400mls of blood after delivery. They helped me remain calm, considering what was going on.”

“Just wanted to send some positive feedback for the staff working on Ward 31 and 32 on the 7-8 July, in particular the midwives Keri McGrath and Gemma Smith and student midwives Symone and Erica. I felt my concerns were taken seriously, despite some neutral test results. I felt things were dealt with really thoroughly, I felt held in mind and was updated clearly throughout. It reduced my worry about having to be there a lot.

“Lauren and Charlotte were absolutely fantastic. Very knowledgeable, talked us through difficult looking techniques, and how to improve current feeding. Performed a tongue-tie observation and then discussed the pros and cons of the procedure. Very measured and thorough. Completed the procedure straight away and returned to monitor progress. An invaluable service! This was our number 1 concern post-birth and they put our minds to rest. Incredibly approachable, we would recommend this service to all new mothers.”

“When I visited Luton and Dunstable Hospital for the first time, I had the most wonderful experience. I had not been in the entrance for more than 60 seconds and a gentleman in a volunteer top (who clearly was in the middle of something) asked if I needed any help. He did not just provide me with directions but walked me part of the way.

“Once I arrived at the Maternity ward, a nurse asked if she could help me and then said she would be able to take me where I needed to go. I also noted other staff members smiling at me.

“I have no doubt that every single employee often experiences tiredness and fatigue and therefore I could not let the day pass without saying THANK YOU. Thank you for a wonderful experience and thank you for all you are doing to bring in and sustain new life.”
