WHO WE ARE The Riverside Community Health Foundation (RCHF) is the lead agency of the Riverside Community Diabetes Collaborative (RCDC). RCDC is a group of over 40 diabetes-focused organizations and businesses that work together to strengthen community programs and services for the prevention and management of diabetes in Riverside and surrounding areas.

RCDC is your local resource for diabetes and chronic disease prevention information, events and more.

DIABETES TIPS A new year comes with the opportunity for new resolutions and goal setting. In 2018, make your health a priority and set a goal to better manage your diabetes and overall health.

Start by setting small, realistic goals that are measureable and action-oriented. For example, “I am setting a goal to add one serving of vegetables into my daily meal plan.� Always remember that small goals can have a large impact. EATING HEALTHY FOR LIFE Learn how chronic diseases can be prevented by making healthier food choices. Topics include nutrition label reading, fast food alternatives, portion control, food groups, and healthy substitutions.

A FREE PROGRAM FOR PEOPLE WITH DIABETES AND THEIR CAREGIVERS.

DIABETES MANAGEMENT EDUCATION The Diabetes Wellness Program offers diabetes management education using the Conversation Map® education program.

These interactive, small group sessions are designed to help you discover what you want and need to know about managing your diabetes or that of a loved one. These 2-hour sessions are taught by a licensed healthcare professional.

SOLUTIONS Join us for a FREE Chronic Disease Self-Management Program! This 2-hour workshop series meets once a week, for six weeks. Workshops are facilitated by two trained leaders. Come meet new friends and learn to manage your ongoing health conditions together! *A Self-Management Resource Center Program developed by Dr. Kate Lorig, Virginia González and Diana Laurent.

Parkview Community Hospital (Founder’s Center) Class meets from 10:00 am – 12:30 pm every Wednesday for 6 weeks starting on Wednesday, January 10.

DIABETES WELLNESS PROGRAM *DIABETES COOKING CLASSES

Our Diabetes Cooking Classes are led by a Registered Dietitian and focus on how to prepare healthy meals for someone with diabetes. We offer a Recipe of the Month card at each class so that you can build a cookbook throughout the year. Come learn the importance of food and nutrition in diabetes management and how to prepare a diabetes-friendly recipe.

The Diabetes Mini-Series program is designed to teach you in-depth information about diabetes from healthcare professionals. Join us on February 14th to learn more about diabetes management! Free health screenings start at 5:15 pm. Dinner will be provided!

Diabetes Grocery Store Tour: Join us as we explore smart grocery shopping tips for people with diabetes. We’ll learn about carbohydrates, carbohydrate counting, and how to build a healthy plate for someone with diabetes.
