12 2 6 In the event of an exposure incident, the following records will be kept in the employee’s medical file:

12 2 6 1 The results of any examination, medical testing, and follow-up procedures

12.2.6.2 A copy of the health care professional’s written opinion to Purity Laboratories

12.2.6.3 A copy of all information provided Purity Laboratories to the health care professional regarding the exposure incident.

12.2.7 All exposure incidents determined to meet OSHA’s Recordkeeping Requirements will be recorded in the OSHA 300 log.

12.2.8 All injuries from contaminated sharps will be recorded in a LF3054 Sharps Injury Log. Information recorded on the Sharps Injury Log will be retained for 5 years.

12.2.9 The Sharps Injury Log will include the following information: date of the injury type and brand of the device involved work area where the incident occurred explanation of how the incident occurred

12.3 Vaccinations:

12.3.1 All personnel that complete the bloodborne pathogen training will have the option to receive the hepatitis B series vaccines Vaccination is not required and will be paid for by the company

12 3 2 An employee may decline to take the vaccination series but must sign a declination statement Any employee who initially declines the vaccination may request and obtain the vaccination at a later date at no cost

12 3 3 Any previously vaccinated employees must provide a vaccination record that includes the vaccination dates

12.3.4 Purity Laboratories will retain copies of employee vaccination records and documentation of refusal of the vaccination. This will be kept in the employee’s medical file.

12.4 Labeling of biohazards:

12.4.1 Sharps disposal containers and other biohazard waste containers must be completely red in color or labeled with a biohazard warning label.

12.4.2 Refrigerators and freezers used for containing and storing blood and OPIM must be labeled with a biohazard warning label.
