Post-Exposure Evaluation and Follow-up

11 4 1 Once an employee has received medical attention, the LF3053 Post Bloodborne Pathogen Exposure Incident Evaluation and Follow-Up form needs to be filled out

11.4.2 Any medical documents and test results are to be placed in employee’s medical file.

11.4.3 The exposed employee must give consent if he or she wants to be tested for HIV, HBV and HCV. This will be documented on the LF3053 Post Bloodborne Pathogen Exposure Incident Evaluation and Follow-Up form.

11.4.4 If the employee does not give consent for HIV serological testing during collection of blood for baseline testing, preserve the baseline blood sample for at least 90 days; if the exposed employee elects to have the baseline sample testing during this waiting period, perform testing as soon as feasible.

11.4.5 All post-exposure testing will be done at the expense of Purity.

11.5 Post-Exposure Investigation:

11.5.1 The circumstances involving an exposure incident will be reviewed by the CLIA Supervisor and/or the Quality Department to determine the following:

11.5.1.1 engineering controls in use at the time

11.5.1.2 work practices followed

11 5 1 3 any device or equipment being used 11 5 1 4 protective equipment or clothing being used

11 5 1 5 location of the incident

11 5 1 6 procedure being performed when the incident occurred

11 5 1 7 employee’s training

11 5 2 This investigation will review this Exposure Control Plan to determine if changes need to be made to prevent an exposure incident from happening in the future

11.5.3 The exposure incident will be documented on the LF3051 Exposure Incident Investigation Form.

12.0 Quality Control and Documentation

12.1 Training:

12.1.1 Any employee who will be in direct contact with a bloodborne pathogen or OPIM is required to complete and pass bloodborne pathogen training.

12.1.2 Training is required prior to beginning any work with blood or OPIM.

12.1.3 The training may be administered by any private or public organization as long as proper documentation is completed and the training is administered by a knowledgeable healthcare professional.
