12 1 4 All personnel that go through the bloodborne pathogen training will need to obtain documentation that they have completed the course This will be kept in their personnel files
12 1 5 Personnel that have not completed bloodborne pathogen training are not permitted to work with any OPIM, blood or its constituents.

12.1.6 All employees who undergo bloodborne pathogen training are required to be competent in the following areas:
12.1.6.1 Epidemiology, symptoms and mode of transmission of pathogenic diseases.
12.1.6.2 An explanation of activities and tasks that may involve exposure to blood or OPIM
12.1.6.3 How appropriate engineering controls, work practices, and PPE will prevent or reduce exposure
12.1.6.4 The basis for the selection of PPE; the types, use, location, removal, handling, decontamination, and disposal procedures
12.1.6.5 Hepatitis B vaccine information including that the vaccine is provided at no cost, the benefits of being vaccinated and methods of administration
12.1.6.6 Employer responsibilities for post-exposure evaluation and medical follow-up; how and who to contact should an exposure incident occur
12.1.6.7 An explanation of the signs and hazard labels
12.1.6.8 An explanation of the OSHA bloodborne pathogen standard and this ECP along with how to obtain a copy of each document
12 2 Documentation:
12 2 1 All training will be documented on the LF2018 Training Record Form
12 2 2 Training records will be maintained for three years and will include the following information:
12 2 2 1 the date of the training session
12.2.2.2 the contents or a summary of the training session
12.2.2.3 the name(s) and qualifications of person(s) conducting the training session
12.2.2.4 the names of all people attending the training sessions
12.2.3 Medical records for employees are confidential and are kept for the duration of employment plus 30 years.
12.2.4 Employee medical records will be provided upon request of the employee or to anyone having written consent of the employee.
12.2.5 Medical records include the employee’s name, Social Security number, and hepatitis B vaccination status (including dates of hepatitis B vaccination and any medical records relative to the employee’s ability to receive the vaccination).
